    Stereotactically guided breast biopsy: a review

    The aims of this review are to compare and contrast the available stereotactic equipment, and to describe the variety of needle types used and their affect on pathological results and subsequent patient management. Initial stereotactic devices were “added-on” to analogue mammography units and have been replaced by prone or ducubitus equipment using digital image acquisition. Biopsies use either 14-G core biopsy (CB) needles or vacuum-assisted biopsies (VAB). Vacuum-assisted biopsy systems consistently out-perform 14-G CB with reduced need for diagnostic or multi-treatment surgery. The false-negative rate is 8% for 14-G CB compared with 0.7% for VAB. There is a risk of underestimating the disease present for lesions of uncertain malignant potential (Cat B3) and suspicious of malignancy (Cat B4) results with 25% of patients with a B3 biopsy found to have cancer at subsequent surgery and 66% of those with a B4 biopsy. A CB diagnosis of in situ malignancy is upgraded to invasive disease at surgery in 15-36% of patients undergoing CB and of the order of 10% with VAB. A high degree of diagnostic accuracy and hence safe patient care can only be achieved by meticulous attention to technique and multi-disciplinary cooperation

    Mitotic spindles are self-organizing protein machines that harness teams of multiple force generators to drive chromosome segregation. Kinesins are key members of these force-generating teams. Different kinesins walk directionally along dynamic microtubules, anchor, crosslink, align and sort microtubules into polarized bundles, and influence microtubule dynamics by interacting with microtubule tips. The mechanochemical mechanisms of these kinesins are specialized to enable each type to make a specific contribution to spindle self-organization and chromosome segregation

    Standard of civilization, nomadism and territoriality in nineteenth-century international society

    In this chapter, the encounter between the Russian Empire and the nomads of the Eurasian steppe in the nineteenth century is analyzed using the theoretical framework of the standard of civilization. The creation of the Westphalian state-model in Europe in the seventeenth century, linked to the later emergence of the notion of the standard of civilization led to the ‘othering’ of the nomads of the Eurasian steppe as barbarians, as a threat to the borders of civilized Europe. The chapter presents also an argument to define ‘territoriality’ as not only an institution of international society of the time but also as a distinctive quality and requirement for being considered ‘civilized’. In this analytical framework, the nomads become the ‘other’, the ‘alien’, the ‘menace’, onto which projections of rationality and modernity were cast in order to prevent threats to Russia’s European and civilized identity. The chapter sheds light on the encounter between ‘fixed’ and ‘mobile’ units in the course of expansion of international society; contextualizes the role played by nomadic tribes in resisting the application of Westphalian spatial categories in the Eurasian space; and scrutinizes what the role of nomads was in constructing a European, civilized identity.PostprintPeer reviewe

    Determinants of the urinary and serum metabolome in children from six European populations

    Background Environment and diet in early life can affect development and health throughout the life course. Metabolic phenotyping of urine and serum represents a complementary systems-wide approach to elucidate environment–health interactions. However, large-scale metabolome studies in children combining analyses of these biological fluids are lacking. Here, we sought to characterise the major determinants of the child metabolome and to define metabolite associations with age, sex, BMI and dietary habits in European children, by exploiting a unique biobank established as part of the Human Early-Life Exposome project (http://www.projecthelix.eu). Methods Metabolic phenotypes of matched urine and serum samples from 1192 children (aged 6–11) recruited from birth cohorts in six European countries were measured using high-throughput 1H nuclear magnetic resonance (NMR) spectroscopy and a targeted LC-MS/MS metabolomic assay (Biocrates AbsoluteIDQ p180 kit). Results We identified both urinary and serum creatinine to be positively associated with age. Metabolic associations to BMI z-score included a novel association with urinary 4-deoxyerythronic acid in addition to valine, serum carnitine, short-chain acylcarnitines (C3, C5), glutamate, BCAAs, lysophosphatidylcholines (lysoPC a C14:0, lysoPC a C16:1, lysoPC a C18:1, lysoPC a C18:2) and sphingolipids (SM C16:0, SM C16:1, SM C18:1). Dietary-metabolite associations included urinary creatine and serum phosphatidylcholines (4) with meat intake, serum phosphatidylcholines (12) with fish, urinary hippurate with vegetables, and urinary proline betaine and hippurate with fruit intake. Population-specific variance (age, sex, BMI, ethnicity, dietary and country of origin) was better captured in the serum than in the urine profile; these factors explained a median of 9.0% variance amongst serum metabolites versus a median of 5.1% amongst urinary metabolites. Metabolic pathway correlations were identified, and concentrations of corresponding metabolites were significantly correlated (r > 0.18) between urine and serum. Conclusions We have established a pan-European reference metabolome for urine and serum of healthy children and gathered critical resources not previously available for future investigations into the influence of the metabolome on child health. The six European cohort populations studied share common metabolic associations with age, sex, BMI z-score and main dietary habits.     Ecological divergence of Chaetopteryx rugulosa species complex (Insecta, Trichoptera) linked to climatic niche diversification

    Climate is often considered to be an important, but indirect driver of speciation. Indeed, environmental factors may contribute to the formation of biodiversity, but to date this crucial relationship remains largely unexplored. Here we investigate the possible role of climate, geological factors, and biogeographical processes in the formation of a freshwater insect species group, the Chaetopteryx rugulosa species complex (Trichoptera) in the Western Balkans. We used multi-locus DNA sequence data to establish a dated phylogenetic hypothesis for the group. The comparison of the dated phylogeny with the geological history of the Western Balkans shows that lineage formation coincided with major past Earth surface and climatic events in the region. By reconstructing present-day habitat conditions (climate, bedrock geology), we show that the lineages of C. rugulosa species complex have distinct climatic but not bedrock geological niches. Without exception, all splits associated with Pliocene/Pleistocene transition led to independent, parallel split into ‘warm’ and ‘cold’ sister lineages. This indicates a non-random diversification on the C. rugulosa species complex associated with late Pliocene climate in the region. We interpreted the results as the diversification of the species complex were mainly driven by ecological diversification linked to past climate change, along with geographical isolation

    O ciclamato é usado como adoçante artificial não calórico em diversos alimentos e bebidas, sendo 30 vezes mais doce que a sacarose sem o sabor amargo da sacarina. Aparece na composição dos produtos como ciclamato de sódio, ciclamato de cálcio e ácido ciclâmico. O ciclamato e a ciclohexilamina, seu principal metabólito, atravessam a barreira placentária em humanos e desse modo podem ser expostos ao feto. O rim de ratos pode ser afetado por elevadas doses de ciclamato de sódio. Estudos sobre efeitos do ciclamato de sódio na espécie humana são necessários, pois, além de poder substituir a sacarose - prejudicial em casos de diabetes ou quando o controle e a redução do peso corporal são essenciais para a saúde dos pacientes - não propicia desenvolvimento de cárie dentária.<br>Cyclamate is used as an artificial non-caloric sweetener in a variety of foods and beverages, being 30 times as sweet as sugar without the bitter after-taste of saccharin. It is present in the formula of products such as sodium and calcium cyclamates and cyclamic acid. Cyclamate and cyclohexylamine, its principal metabolite, can cross the human placenta exposing the fetus. It has been demonstrated in rats that the kidney may be adversely affected by high doses of cyclamate. Studies on the effects of sodium cyclamate in the human species are necessary, because in addition to replacing saccharose - harmful in individuals with diabetes or patients in whom weight reduction and control are essential for health - it does not cause dental caries
