    What does your neighbourhood say about you? : a study of life expectancy in 1.3 million Swiss neighbourhoods

    BACKGROUND: Switzerland had the highest life expectancy at 82.8 years among the Organisation for Economic Co-operation and Development (OECD) countries in 2011. Geographical variation of life expectancy and its relation to the socioeconomic position of neighbourhoods are, however, not well understood. METHODS: We analysed the Swiss National Cohort, which linked the 2000 census with mortality records 2000-2008 to estimate life expectancy across neighbourhoods. A neighbourhood index of socioeconomic position (SEP) based on the median rent, education and occupation of household heads and crowding was calculated for 1.3 million overlapping neighbourhoods of 50 households. We used skew-normal regression models, including the index and additionally marital status, education, nationality, religion and occupation to calculate crude and adjusted estimates of life expectancy at age 30 years. RESULTS: Based on over 4.5 million individuals and over 400,000 deaths, estimates of life expectancy at age 30 in neighbourhoods ranged from 46.9 to 54.2 years in men and from 53.5 to 57.2 years in women. The correlation between life expectancy and neighbourhood SEP was strong (r=0.95 in men and r=0.94 women, both p values <0.0001). In a comparison of the lowest with the highest percentile of neighbourhood SEP, the crude difference in life expectancy from skew-normal regression was 4.5 years in men and 2.5 years in women. The corresponding adjusted differences were 2.8 and 1.9 years, respectively (all p values <0.0001). CONCLUSIONS: Although life expectancy is high in Switzerland, there is substantial geographical variation and life expectancy is strongly associated with the social standing of neighbourhoods

    Luttinger model approach to interacting one-dimensional fermions in a harmonic trap

    A model of interacting one--dimensional fermions confined to a harmonic trap is proposed. The model is treated analytically to all orders of the coupling constant by a method analogous to that used for the Luttinger model. As a first application, the particle density is evaluated and the behavior of Friedel oscillations under the influence of interactions is studied. It is found that attractive interactions tend to suppress the Friedel oscillations while strong repulsive interactions enhance the Friedel oscillations significantly.     Treatment of backscattering in a gas of interacting fermions confined to a one-dimensional harmonic atom trap

    An asymptotically exact many body theory for spin polarized interacting fermions in a one-dimensional harmonic atom trap is developed using the bosonization method and including backward scattering. In contrast to the Luttinger model, backscattering in the trap generates one-particle potentials which must be diagonalized simultaneously with the two-body interactions. Inclusion of backscattering becomes necessary because backscattering is the dominant interaction process between confined identical one-dimensional fermions. The bosonization method is applied to the calculation of one-particle matrix elements at zero temperature. A detailed discussion of the validity of the results from bosonization is given, including a comparison with direct numerical diagonalization in fermionic Hilbert space. A model for the interaction coefficients is developed along the lines of the Luttinger model with only one coupling constant KK. With these results, particle densities, the Wigner function, and the central pair correlation function are calculated and displayed for large fermion numbers. It is shown how interactions modify these quantities. The anomalous dimension of the pair correlation function in the center of the trap is also discussed and found to be in accord with the Luttinger model.Comment: 19 pages, 5 figures, journal-ref adde

    Spin- and charge-density oscillations in spin chains and quantum wires

    We analyze the spin- and charge-density oscillations near impurities in spin chains and quantum wires. These so-called Friedel oscillations give detailed information about the impurity and also about the interactions in the system. The temperature dependence of these oscillations explicitly shows the renormalization of backscattering and conductivity, which we analyze for a number of different impurity models. We are also able to analyze screening effects in one dimension.     A CHEOPS-enhanced view of the HD 3167 system

W. acknowledge support from STFC consolidated grant numbers ST/R000824/1 and ST/V000861/1, and UKSA grant number ST/R003203/1.Much remains to be understood about the nature of exoplanets smaller than Neptune, most of which have been discovered in compact multi-planet systems. With its inner ultra-short period planet b aligned with the star and two larger outer planets d-c on polar orbits, the multi-planet system HD 3167 features a peculiar architecture and offers the possibility to investigate both dynamical and atmospheric evolution processes. To this purpose we combined multiple datasets of transit photometry and radial velocimetry (RV) to revise the properties of the system and inform models of its planets. This effort was spearheaded by CHEOPS observations of HD 3167b, which appear inconsistent with a purely rocky composition despite its extreme irradiation. Overall the precision on the planetary orbital periods are improved by an order of magnitude, and the uncertainties on the densities of the transiting planets b and c are decreased by a factor of 3. Internal structure and atmospheric simulations draw a contrasting picture between HD 3167d, likely a rocky super-Earth that lost its atmosphere through photo-evaporation, and HD 3167c, a mini-Neptune that kept a substantial primordial gaseous envelope. We detect a fourth, more massive planet on a larger orbit, likely coplanar with HD 3167d-c. Dynamical simulations indeed show that the outer planetary system d-c-e was tilted, as a whole, early in the system history, when HD 3167b was still dominated by the star influence and maintained its aligned orbit. RV data and direct imaging rule out that the companion that could be responsible for the present-day architecture is still bound to the HD 3167 system. Similar global studies of multi-planet systems will tell how many share the peculiar properties of the HD 3167 system, which remains a target of choice for follow-up observations and simulations. © 2022 EDP Sciences.     TOI-1055 b: Neptunian planet characterised with HARPS, TESS, and CHEOPS

    Context. TOI-1055 is a Sun-like star known to host a transiting Neptune-sized planet on a 17.5-day orbit (TOI-1055 b). Radial velocity (RV) analyses carried out by two independent groups using nearly the same set of HARPS spectra have provided measurements of planetary masses that differ by ∼2σ. Aims. Our aim in this work is to solve the inconsistency in the published planetary masses by significantly extending the set of HARPS RV measurements and employing a new analysis tool that is able to account and correct for stellar activity. Our further aim was to improve the precision on measurements of the planetary radius by observing two transits of the planet with the CHEOPS space telescope. Methods. We fit a skew normal function to each cross correlation function extracted from the HARPS spectra to obtain RV measurements and hyperparameters to be used for the detrending. We evaluated the correlation changes of the hyperparameters along the RV time series using the breakpoint technique. We performed a joint photometric and RV analysis using a Markov chain Monte Carlo scheme to simultaneously detrend the light curves and the RV time series. Results. We firmly detected the Keplerian signal of TOI-1055 b, deriving a planetary mass of Mb = 20.4-2.5+2.6 MO (∼12%). This value is in agreement with one of the two estimates in the literature, but it is significantly more precise. Thanks to the TESS transit light curves combined with exquisite CHEOPS photometry, we also derived a planetary radius of Rb = 3.490-0.064+0.070 RO (∼1.9%). Our mass and radius measurements imply a mean density of ρb = 2.65-0.35+0.37 g cm-3 (∼14%). We further inferred the planetary structure and found that TOI-1055 b is very likely to host a substantial gas envelope with a mass of 0.41-0.20+0.34 MO and a thickness of 1.05-0.29+0.30 RO. Conclusions. Our RV extraction combined with the breakpoint technique has played a key role in the optimal removal of stellar activity from the HARPS time series, enabling us to solve the tension in the planetary mass values published so far for TOI-1055 b

    Non-AIDS defining cancers in the D:A:D Study-time trends and predictors of survival : a cohort study

    BACKGROUND:Non-AIDS defining cancers (NADC) are an important cause of morbidity and mortality in HIV-positive individuals. Using data from a large international cohort of HIV-positive individuals, we described the incidence of NADC from 2004-2010, and described subsequent mortality and predictors of these.METHODS:Individuals were followed from 1st January 2004/enrolment in study, until the earliest of a new NADC, 1st February 2010, death or six months after the patient's last visit. Incidence rates were estimated for each year of follow-up, overall and stratified by gender, age and mode of HIV acquisition. Cumulative risk of mortality following NADC diagnosis was summarised using Kaplan-Meier methods, with follow-up for these analyses from the date of NADC diagnosis until the patient's death, 1st February 2010 or 6 months after the patient's last visit. Factors associated with mortality following NADC diagnosis were identified using multivariable Cox proportional hazards regression.RESULTS:Over 176,775 person-years (PY), 880 (2.1%) patients developed a new NADC (incidence: 4.98/1000PY [95% confidence interval 4.65, 5.31]). Over a third of these patients (327, 37.2%) had died by 1st February 2010. Time trends for lung cancer, anal cancer and Hodgkin's lymphoma were broadly consistent. Kaplan-Meier cumulative mortality estimates at 1, 3 and 5 years after NADC diagnosis were 28.2% [95% CI 25.1-31.2], 42.0% [38.2-45.8] and 47.3% [42.4-52.2], respectively. Significant predictors of poorer survival after diagnosis of NADC were lung cancer (compared to other cancer types), male gender, non-white ethnicity, and smoking status. Later year of diagnosis and higher CD4 count at NADC diagnosis were associated with improved survival. The incidence of NADC remained stable over the period 2004-2010 in this large observational cohort.CONCLUSIONS:The prognosis after diagnosis of NADC, in particular lung cancer and disseminated cancer, is poor but has improved somewhat over time. Modifiable risk factors, such as smoking and low CD4 counts, were associated with mortality following a diagnosis of NADC

    Home Visits to Prevent Nursing Home Admission and Functional Decline in Elderly People: Systematic Review and Meta-regression Analysis

    CONTEXT: The effects of home visitation programs to prevent functional decline in elderly persons have been inconsistent, and the value of these programs is controversial. OBJECTIVE: To evaluate the effect of preventive home visits on functional status, nursing home admission, and mortality. DATA SOURCES: Studies published in English, French, German, Italian, or Spanish reporting randomized trials of the effects of preventive in-home visits in older people (mean age >70 years) living in the community were identified through searches of MEDLINE, PSYCHINFO, and EMBASE (January 1985-November 2001). We also searched the Cochrane Controlled Trials Register, checked reference lists of earlier reviews and book chapters, searched conference proceedings and specialty journals, and contacted experts. STUDY SELECTION: We screened 1349 abstracts and excluded those that did not test in-home interventions or in which the mean age of the study population was younger than 70 years. After further exclusions, 17 articles describing 18 trials were analyzed. DATA EXTRACTION: Two reviewers independently screened abstracts. Discrepancies were resolved by consensus with a third reviewer. For each included trial, we extracted data on the study population and the characteristics of the intervention. Two of us extracted information on 3 end points: nursing home admissions, mortality, and functional status. One of us assessed trial quality, including an examination of the method of randomization, blinding of caregivers and research staff ascertaining outcomes, and proportion of patients included in analyses of the 3 end points. DATA SYNTHESIS: The 18 trials included 13 447 individuals aged 65 years and older. The effect on nursing home admissions depended on the number of visits performed during follow-up. The pooled relative risk (RR) was 0.66 (95% confidence interval [CI], 0.48-0.92) for trials in the upper tertile (>9 visits) but was 1.05 (95% CI, 0.85-1.30) in the lower tertile (0-4 visits). Functional decline was reduced in trials that used multidimensional assessment with follow-up (RR, 0.76; 95% CI, 0.64-0.91) but not in other trials (RR, 1.01; 95% CI, 0.92-1.11). Functional decline was reduced (RR, 0.78; 95% CI, 0.64-0.95) in trials with a control group mortality rate in the lower tertile (3.4%-5.8%) but not (RR, 0.98; 95% CI, 0.84-1.13) in those with a control-group mortality rate in the upper tertile (8.3%-10.7%). A beneficial effect on mortality was evident in younger study populations (RR, 0.76; 95% CI, 0.65-0.88 for ages 72.7-77.5 years) but not in older study populations (RR, 1.09; 95% CI, 0.92-1.28 for ages 80.2-81.6 years). CONCLUSION: Preventive home visitation programs appear to be effective, provided the interventions are based on multidimensional geriatric assessment and include multiple follow-up home visits and target persons at lower risk for death. Benefits on survival were seen in young-old rather than old-old populations
