    The design of organisations, products and processes for strategic flexibility : executive summary

    Technological innovation and globalisation are driving profound economic, political and cultural changes. There is a widespread acknowledgement that organisations need to be more strategically flexible to cope with increased levels of competition and market change. The research reported here has two objectives. The first is that of identifying the causes of strategic flexibility in organisations, and the second being to implement methods of improving strategic flexibility. A model of decision-making behaviour has been developed, which identifies the areas of individual and group decision-making behaviour that affect strategic flexibility. The model has general applicability. A significant cause of strategic inflexibility is a behavioural. dysfunction in individuals that produces a much wider dysfunction in the organisation. The same model also provides the basis for the evaluation and improvement of such behaviours. This has led to the development of processes and tools to reduce the barriers to adopting high quality decision-making behaviour. However, individual behavioural. change, while being an essential foundation, is insufficient on its own to achieve high rates of organisational and technological adaptation at low levels of disruption. The second objective has been to implement a systematic process for integrating all players in a strategically flexible organisation. In the absence of a consistent, systematic process, particularly for organisational and technological innovation, a design model of the business has been originated and developed. This has been shown to be applicable to a wide range of organisational cultures and integrates recent trends in organisational thinking. Individual innovations in processes and tools, which have been central to the development and introduction of the design model, have been implemented in an organisation. These innovations are in the areas of innovation management, portfolio management, product targeting and target agreement, and are described to achieve wider application. The concept of the brand has been shown to be a powerful 'attractor' to develop an organisation's fundamental relationship with its environment in the long, medium and short term. Because the values of a brand represent basic human motivational values, they provide stability for long term planning and can align internal decision-making values, innovation and core competencies to the benefit of the organisation and their workers, their customers and the wider environment. The research work has shown that an organisation can meet the simultaneous requirements of design speed, knowledge reuse, semi-independent decision-making and creativity at the lowest possible level of the organisation. The concepts and tools are therefore valuable in supporting a step-change in the performance of conventional and virtual organisations. The modular partitioning of organisations, products and processes is compatible with the design model of the business, -and the strategies are synergistic. While modularity in a traditional organisation. could lead to decay and loss of strategic flexibility, its integration within the design model framework supports a dynamically unstable, but continuously innovative and long-lived organisation

    Analytical solution of two-layer beam taking into account interlayer slip and shear deformation

    A mathematical model is proposed and its analytical solution derived for the analysis of the geometrically and materially linear two-layer beams with different material and geometric characteristics of an individual layer. The model takes into account the effect of the transverse shear deformation on displacements in each layer. The analytical study is carried out to evaluate the influence of the transverse shear deformation on the static and kinematic quantities. We study a simply supported two-layer planar beam subjected to the uniformly distributed load. Parametric studies have been performed to investigate the influence of shear by varying material and geometric parameters, such as interlayer slip modulus (K), flexural-to-shear moduli ratios (E/G) and span-to-depth ratios (L/h). The comparison of the results for vertical deflections shows that shear deformations are more important for high slip modulus, for ``short'' beams with small L/h ratios, and beams with high E/G ratios. In these cases, the effect of the shear deformations becomes significant and has to be addressed in design. It also becomes apparent that models, which consider the partial interaction between the layers, should be employed if beams have very flexible connections

    Non-linear analysis of two-layer timber beams considering interlayer slip and uplift

    A new mathematical model and its finite element formulation for the non-linear analysis of mechanical behaviour of a two-layer timber planar beam is presented. A modified principle of virtual work is employed in formulating the finite element method. The basic unknowns are strains. The following assumptions are adopted in the mathematical model: materials are taken to be non-linear and can differ from layer to layer; interacting shear and normal contact tractions between layers are derived from the non-linear shear contact traction-slip and the non-linear normal contact traction-uplift characteristics of the connectors; the geometrically linear and materially non-linear Bernoulli's beam theory is assumed for each layer. The formulation is found to be accurate, reliable and computationally effective. The suitability of the theory is validated by the comparison of the numerical solution and the experimental results of full-scale laboratory tests on a simply supported beam. An excellent agreement between measured and calculated results is observed for all load levels. The further objective of the paper is the analysis of the effect of different normal contact traction-uplift constitutive relationships on the kinematic and static quantities in a statically determined and undetermined structure. While the shear contact traction-slip constitutive relationship dictates the deformability of the composite beam and has a substantial influence on most of the static and kinematic quantities of the composite beam, a variable normal contact traction-uplift constitutive relationship is in most cases negligible

    Duration of Treatment for Pseudomonas aeruginosa Bacteremia : a Retrospective Study

    Introduction: There is no consensus regarding optimal duration of antibiotic therapy for Pseudomonas aeruginosa bacteremia. We aimed to evaluate the impact of short antibiotic course. Methods: We present a retrospective multicenter study including patients with P. aeruginosa bacteremia during 2009-2015. We evaluated outcomes of patients treated with short (6-10 days) versus long (11-15 days) antibiotic courses. The primary outcome was a composite of 30-day mortality or bacteremia recurrence and/or persistence. Univariate and inverse probability treatment-weighted (IPTW) adjusted multivariate analysis for the primary outcome was performed. To avoid immortal time bias, the landmark method was used. Results: We included 657 patients; 273 received a short antibiotic course and 384 a long course. There was no significant difference in baseline characteristics of patients. The composite primary outcome occurred in 61/384 patients in the long-treatment group (16%) versus 32/273 in the short-treatment group (12%) (p = 0.131). Mortality accounted for 41/384 (11%) versus 25/273 (9%) of cases, respectively. Length of hospital stay was significantly shorter in the short group [median 13 days, interquartile range (IQR) 9-21 days, versus median 15 days, IQR 11-26 days, p = 0.002]. Ten patients in the long group discontinued antibiotic therapy owing to adverse events, compared with none in the short group. On univariate and multivariate analyses, duration of therapy was not associated with the primary outcome. Conclusions: In this retrospective study, 6-10 days of antibiotic course for P. aeruginosa bacteremia were as effective as longer courses in terms of survival and recurrence. Shorter therapy was associated with reduced length of stay and less drug discontinuation

    Alternative epidemic indicators for COVID-19 in three settings with incomplete death registration systems

    Not all COVID-19 deaths are officially reported, and particularly in low-income and humanitarian settings, the magnitude of reporting gaps remains sparsely characterized. Alternative data sources, including burial site worker reports, satellite imagery of cemeteries, and social media-conducted surveys of infection may offer solutions. By merging these data with independently conducted, representative serological studies within a mathematical modeling framework, we aim to better understand the range of underreporting using examples from three major cities: Addis Ababa (Ethiopia), Aden (Yemen), and Khartoum (Sudan) during 2020. We estimate that 69 to 100%, 0.8 to 8.0%, and 3.0 to 6.0% of COVID-19 deaths were reported in each setting, respectively. In future epidemics, and in settings where vital registration systems are limited, using multiple alternative data sources could provide critically needed, improved estimates of epidemic impact. However, ultimately, these systems are needed to ensure that, in contrast to COVID-19, the impact of future pandemics or other drivers of mortality is reported and understood worldwide

    Age-dependent therapeutic effect of memantine in a mouse model of juvenile Batten disease

    Currently there is no treatment for juvenile Batten disease, a fatal childhood neurodegenerative disorder caused by mutations in the CLN3 gene. The Cln3-knockout (Cln3(Δex1-6)) mouse model recapitulates several features of the human disorder. Cln3(Δex1-6) mice, similarly to juvenile Batten disease patients, have a motor coordination deficit detectable as early as postnatal day 14. Previous studies demonstrated that acute attenuation of α-amino-3-hydroxy-5-methyl-4-isoxazolepropionate (AMPA)-type glutamate receptor activity by the non-competitive AMPA antagonist, EGIS-8332, in both 1- and 6–7-month-old Cln3(Δex1-6) mice results in improvement in motor coordination. Here we show that acute inhibition of N-methyl-D-aspartate (NMDA)-type glutamate receptors by memantine (1 and 5 mg/kg i.p.) had no effect on the impaired motor coordination of one-month-old Cln3(Δex1-6) mice. At a later stage of the disease, in 6–7-month-old Cln3(Δex1-6) mice, memantine induced a delayed but extended (8 days) improvement of motor skills similarly to that observed previously with EGIS-8332 treatment. An age-dependent therapeutic effect of memantine implies that the pathomechanism in juvenile Batten disease changes during disease progression. In contrast to acute treatment, repeated administration of memantine or EGIS-8332 (1 mg/kg, once a week for 4 weeks) to 6-month-old Cln3(Δex1-6) mice had no beneficial effect on motor coordination. Moreover, repeated treatments did not impact microglial activation or the survival of vulnerable neuron populations. Memantine did not affect astrocytosis in the cortex. EGIS-8332, however, decreased astrocytic activation in the somatosensory barrelfield cortex. Acute inhibition of NMDA receptors can induce a prolonged therapeutic effect, identifying NMDA receptors as a new therapeutic target for juvenile Batten disease
