    Acetabular fracture: Long-term follow-up and factors associated with secondary implantation of total hip arthroplasty

    SummaryHypothesisThe present study sought to determine long-term outcome in acetabular fracture and the factors associated with secondary implantation of a total hip arthroplasty and/or with poor functional results.Material and methodsSeventy-two patients admitted between 2000 and 2005 were followed up for a maximum 11years (mean, 6.8years): 16 females, 56 males; mean age at injury, 41.6years (median, 40years). There were 45 simple acetabular fractures, 27 complex fractures and 27 dislocations. Late complications were: osteoarthritis (n=29), osteonecrosis of the femoral head (ONFH: n=8) and heterotopic ossification (n=2).Results and discussionTwenty-five total hip arthroplasties (THA) were performed, with a mean time to surgery of 3.7years. Associated factors for THA were: VAS (P<0.0001), PMA (P<0.0001), osteoarthritis (P<0.0001), ONFH (P<0.0002), initial dislocation (P=0.0002), no functional treatment (P=0.0014), surgical treatment (P=0.0065), initial traction (P=0.0068), anterior and posterior congruency defect (P=0.0072 and P<0.0001), and initial intra-articular foreign body (P=0.045). Factors associated with poor or bad functional results were the same, plus: etiology (P=0.0021), BMI (P=0.03) and posterior wall fracture (P=0.0325).Level of evidence4; retrospective study

Preliminary results for a thulium-doped double-clad fibre indicate that the pump absorption coefficient attainable via this approach is comparable to an equivalent octagonal fibre

    Quantitative reconstruction of climatic variations during the Bronze and early Iron ages based on pollen and lake-level data in the NW Alps, France

    International audienceVegetation and lake-level data from the archaeological site of Tresserve, on the eastern shore of Lake Le Bourget (Savoie, France), are used to provide quantitative estimates of climatic variables over the period 4000-2300 cal BP in the northern French Pre-Alps, and to examine the possible impact of climatic changes on societies of the Bronze and early Iron Ages. The results obtained indicate that phases of higher lake level at 3500-3100 and 2750-2350 cal BP coincided with major climate reversals in the North Atlantic area. In west-central Europe, they were marked by cooler and wetter conditions. These two successive events may have affected ancient agricultural communities in west-central Europe by provoking harvest failures, more particularly due to increasing precipitation during the growing season. However, archaeological data in the region of Franche-Comté (Jura Mountains, eastern France) show a general expansion of population density from the middle Bronze Age to the early Iron Age. This suggests a relative emancipation of proto-historic societies from climatic conditions, probably in relation to the spread of new modes of social and economic organisation

    Medial Hypoxia and Adventitial Vasa Vasorum Remodeling in Human Ascending Aortic Aneurysm

    Human ascending aortic aneurysms characteristically exhibit cystic medial degeneration of the aortic wall encompassing elastin degeneration, proteoglycan accumulation and smooth muscle cell loss. Most studies have focused on the aortic media and there is a limited understanding of the importance of the adventitial layer in the setting of human aneurysmal disease. We recently demonstrated that the adventitial ECM contains key angiogenic factors that are downregulated in aneurysmal aortic specimens. In this study, we investigated the adventitial microvascular network (vasa vasorum) of aneurysmal aortic specimens of different etiology and hypothesized that the vasa vasorum is disrupted in patients with ascending aortic aneurysm. Morphometric analyses of hematoxylin and eosin-stained human aortic cross-sections revealed evidence of vasa vasorum remodeling in aneurysmal specimens, including reduced density of vessels, increased lumen area and thickening of smooth muscle actin-positive layers. These alterations were inconsistently observed in specimens of bicuspid aortic valve (BAV)-associated aortopathy, while vasa vasorum remodeling was typically observed in aneurysms arising in patients with the morphologically normal tricuspid aortic valve (TAV). Gene expression of hypoxia-inducible factor 1α and its downstream targets, metallothionein 1A and the pro-angiogenic factor vascular endothelial growth factor, were down-regulated in the adventitia of aneurysmal specimens when compared with non-aneurysmal specimens, while the level of the anti-angiogenic factor thrombospondin-1 was elevated. Immunodetection of glucose transporter 1 (GLUT1), a marker of chronic tissue hypoxia, was minimal in non-aneurysmal medial specimens, and locally accumulated within regions of elastin degeneration, particularly in TAV-associated aneurysms. Quantification of GLUT1 revealed elevated levels in the aortic media of TAV-associated aneurysms when compared to non-aneurysmal counterparts. We detected evidence of chronic inflammation as infiltration of lymphoplasmacytic cells in aneurysmal specimens, with a higher prevalence of lymphoplasmacytic infiltrates in aneurysmal specimens from patients with TAV compared to that of patients with BAV. These data highlight differences in vasa vasorum remodeling and associated medial chronic hypoxia markers between aneurysms of different etiology. These aberrations could contribute to malnourishment of the aortic media and could conceivably participate in the pathogenesis of thoracic aortic aneurysm

    Rate Dependent Performance Related to Crystal Structure Evolution of Na0.67Mn0.8Mg0.2O2 in a Sodium-Ion Battery

    Sodium-ion batteries are considered as a favorable alternative to the widely used lithium-ion batteries for applications such as grid-scale energy storage. However, to meet the energy density and reliability that is necessary, electrodes that are structurally stable and well characterized during electrochemical cycling need to be developed. Here, we report on how the applied discharge current rate influences the structural evolution of Na0.67Mn0.8Mg0.2O2 electrode materials. A combination of ex situ and in situ X-ray diffraction (XRD) data were used to probe the structural transitions at the discharged state and during charge/discharge. Ex situ data shows a two-phase electrode at the discharged state comprised of phases that adopt Cmcm and P63/mmc symmetries at the 100 mA/g rate but a predominantly P63/mmc electrode at 200 and 400 mA/g rates. In situ synchrotron XRD data at 100 mA/g shows a solely P63/mmc electrode when 12 mA/g charge and 100 mA/g discharge is used even though ex situ XRD data shows the p..

    Stark Effect of Interactive Electron-hole pairs in Spherical Semiconductor Quantum Dots

    We present a theoretical variational approach, based on the effective mass approximation (EMA), to study the quantum-confinement Stark effects for spherical semiconducting quantum dots in the strong confinement regime of interactive electron-hole pair and limiting weak electric field. The respective roles of the Coulomb potential and the polarization energy are investigated in details. Under reasonable physical assumptions, analytical calculations can be performed. They clearly indicate that the Stark shift is a quadratic function of the electric field amplitude in the regime of study. The resulting numerical values are found to be in good agreement with experimental data over a significant domain of validity
