    Fractures De La diaphyse humérale Une étude rétrospective comparative de technique HACKETHAL Vs plaque vissée : A propos de 19 cas

    Introduction: They represent 2% of all fractures. The main debate around recent fractures focuses on the therapeutic indications for all types of surgical stabilization have sometimes unconditional and strident defenders. Material and Methods: A series of 19 osteosynthesis was reviewed: 4 with other traumas, 1 case of skin incision and two initial radial nerve palsy. The osteosynthesis by plate was performed in 7 patients and by bundle pinning in 12 patients. The outcome evaluation was made with the listing for Stewart & Hundley amended. Results: For patients treated with plate: 2 postoperative radial nerve palsy. There were 1 non- union and no case of sepsis.71, 4% of patients had good and excellent results. For patients treated by bundle pinning: no radial nerve palsy, 1 case of non-union, and no cases of sepsis, under-acromial conflict, or of pin protrusion in the top. Good and very good results concerned 91, 7% of patients. Conclusion: The choice of therapeutic methods is difficult. There is no method that could lead to unanimous. RĂ©sultats Pour les patients traitĂ©s par plaque : Deux paralysies radiales post-opĂ©ratoires, pas de pseudarthrose, pas de cas de sepsis. Les bons et trĂšs bons rĂ©sultats concernent 71,4% des patients. Pour les patients traitĂ©s par embrochage : pas de paralysie radiale ; 1 cas de pseudarthrose, pas de cas de sepsis, aucun cas de conflit sous acromiale ou de protrusion du clou en haut. Les bons et trĂšs bon rĂ©sultats concernent 91,7% des patients. Conclusion Le choix entre les mĂ©thodes thĂ©rapeutiques est difficile. Il n'existe pas de mĂ©thode pouvant entraĂźner une adhĂ©sion unanime. L'embrochage fasciculĂ© pourrait ĂȘtre cette mĂ©thode, mais le risque de raideur de l’épaule en fait une technique redoutĂ©e. L'ostĂ©osynthĂšse par plaque aussi pourrait l’ĂȘtre mais son principal reproche est l’atteinte neurologique

    Staphylococcal entertotoxins of the enterotoxin gene cluster (egcSEs) induce nitrous oxide- and cytokine dependent tumor cell apoptosis in a broad panel of human tumor cells

    International audienceThe egcSEs comprise five genetically linked staphylococcal enterotoxins, SEG, SEI, SElM, SElN, and SElO and two pseudotoxins which constitute an operon present in up to 80% of Staphylococcus aureus isolates. A preparation containing these proteins was recently used to treat advanced lung cancer with pleural effusion. We investigated the hypothesis that egcSEs induce nitrous oxide (NO) and associated cytokine production and that these agents may be involved in tumoricidal effects against a broad panel of clinically relevant human tumor cells. Preliminary studies showed that egcSEs and SEA activated T cells (range: 11-25%) in a concentration dependent manner. Peripheral blood mononuclear cells (PBMCs) stimulated with equimolar quantities of egcSEs expressed NO synthase and generated robust levels of nitrite (range: 200-250 ÎŒM), a breakdown product of NO; this reaction was inhibited by NG-monomethyl-L-arginine (L-NMMA) (0.3 mM), an NO synthase antagonist. Cell free supernatants (CSFs) of all egcSE-stimulated PBMCs were also equally effective in inducing concentration dependent tumor cell apoptosis in a broad panel of human tumor cells. The latter effect was due in part to the generation of NO and TNF-α since it was significantly abolished by L-NMMA, anti-TNF-α antibodies, respectively, and a combination thereof. A hierarchy of tumor cell sensitivity to these CFSs was as follows: lung carcinoma > osteogenic sarcoma > melanoma > breast carcinoma >neuroblastoma. Notably, SEG induced robust activation of NO/TNFα-dependent tumor cell apoptosis comparable to the other egcSEs and SEA despite TNF-α and IFN-Îł levels that were 2 and 8 fold lower, respectively, than the other egcSEs and SEA. Thus, egcSEs produced by S. aureus induce NO synthase and the increased NO formation together with TNF-α appear to contribute to egcSE-mediated apoptosis against a broad panel of human tumor cells

    Inverse spectral problems for energy-dependent Sturm-Liouville equations

    We study the inverse spectral problem of reconstructing energy-dependent Sturm-Liouville equations from their Dirichlet spectra and sequences of the norming constants. For the class of problems under consideration, we give a complete description of the corresponding spectral data, suggest a reconstruction algorithm, and establish uniqueness of reconstruction. The approach is based on connection between spectral problems for energy-dependent Sturm-Liouville equations and for Dirac operators of special form.Comment: AMS-LaTeX, 28 page

    Spectral Asymptotics for Perturbed Spherical Schr\"odinger Operators and Applications to Quantum Scattering

    We find the high energy asymptotics for the singular Weyl--Titchmarsh m-functions and the associated spectral measures of perturbed spherical Schr\"odinger operators (also known as Bessel operators). We apply this result to establish an improved local Borg-Marchenko theorem for Bessel operators as well as uniqueness theorems for the radial quantum scattering problem with nontrivial angular momentum.Comment: 20 page

    Finite element analysis of the behaviour of a crack in the orthopedic cement

    In this paper, the finite element method is used to analyse the crack behaviour in the orthopedic cement of the total hip replacement by computing the stress intensity factors (SIFs) arround the crack tip. In this work, three cases are studied: crack emanating from a cavity, interaction effect of the crack emanating from a cavity with another cavity and the interaction effect of two cracks emanatingfrom two cavities. The stress intensity factors under mixed mode problems at the crack tip are computed for three zones of prosthesis: proximal, median and distal. The obtained results show that the crack initiated from a micro-cavity in the distal zone of cement can be propagated at the same time by opening and shearing of its lips. It is contrary to that initiated in the proximal zone which cannot be propagated. The mechanical behaviour of cracks in the medial zone depends of the crack initiation position
