    Comparison of Relative Efficiency of Hospitals Affiliated to Babol and Mazandaran Universities of Medical Sciences before and after Health Development Plan with Data Envelopment Analysis Method

    BACKGROUND AND OBJECTIVE: Considering to the efficiency of hospitals as the largest and most costly unit of the health system, is important. The purpose of this study was to compare the efficacy of hospitals affiliated to the Babol and Mazandaran universities of medical sciences before and after the health development plan. METHODS: This practical retrospective-comparative study was conducted before and after the Health Development Plan on 21 hospitals affiliated to the Health Development Plan of Babol and Mazandaran Universities of Medical Sciences. The data were collected using the form number 1-201 and 2-201 of the university's medical council. In order to calculate the relative efficiency and ranking of hospitals using the data envelopment analysis method, input parameters and output indices were measured by CCR model. RESULTS: In 2012 and 2013 before the Health Development Plan, 61.9% of the hospitals were effective and in 2015, after the Health Development Plan, 71.45% of them were effective. The plan contributes to an increase of 10% of the number of efficient units. Also, the total efficiency ratio before the plan was equal to 34.5 and in 2015, it was 45. Thus, an increase of 25% was observed in the improvement of efficiency ratio in the year following the Health Development Plan. CONCLUSION: The results of the study showed that the efficiency and ranking of hospitals affiliated to the Babol and Mazandaran universities of medical sciences have increased after the Health Development Plan

    Comparing the therapeutic effect of local corticosteroid injections and laser in treating coccyx pains (sacrum)

    Coccydynia (pain in the coccyx or tailbone) is a fairly common pain in the bottom of backbone that spreads towards sacrum and lower legs. Supportive treatments, such as corticosteroids, use of muscle relaxants and correction of body position can help relieve the pain. In recent years, the use of laser is introduced as an effective method to reduce backbone pain. The purpose of this study is to compare the effects of corticosteroids and laser in treating tailbone pains. 61 patients with tailbone pain referred to the orthopedic clinic of Rasul Akram Hospital were entered into the study and randomly divided into two groups of laser and local corticosteroid injection. The mean difference in pain score was compared in both groups at the beginning of the study, end of the second week and end of the second month to determine the response to treatment. Data were analyzed using SPSS software. Of the patients enrolled, 24 were males (37.6) and 37 were females. The mean age of subjects studied was 36.16 years old. The mean difference in pain scores at the beginning of the study and at the end of the second month in the group treated by laser and corticosteroid was 2.54 and 3.8, respectively, that this difference was also statistically significant (P: 0.019) Effect of corticosteroid injection for pain relief was better than laser. The study should be done in a longer timeframe

    Exploring Consumer and Patient Knowledge, Behavior, and Attitude Toward Medicinal and Lifestyle Products Purchased From the Internet: A Web-Based Survey.

    BACKGROUND: In recent years, lifestyle products have emerged to help improve people's physical and mental performance. The Internet plays a major role in the spread of these products. However, the literature has reported issues regarding the authenticity of medicines purchased from the Internet and the impact of counterfeit medicines on public health. Little or no data are available on the authenticity of lifestyle products and actual toxicity associated with their use and misuse. OBJECTIVE: Our aim was to investigate consumer and patient attitudes toward the purchase of lifestyle products from the Internet, their knowledge of product authenticity and toxicity, and their experiences with counterfeit lifestyle products. METHODS: A Web-based study was performed between May 2014 and May 2015. Uniform collection of data was performed through an anonymous online questionnaire. Participants were invited worldwide via email, social media, or personal communication to complete the online questionnaire. A total of 320 participants completed the questionnaire. RESULTS: The results of the questionnaire showed that 208 (65.0%) participants purchased lifestyle products from the Internet mainly due to convenience and reduced cost. More than half (55.6%, 178/320) of participants purchased cosmetic products, whereas only a minority purchased medicinal products. Yet, 62.8% (201/320) of participants were aware of the presence of counterfeit lifestyle products from the Internet, and 11.9% (38/320) experienced counterfeit products. In only 0.9% (3/320) of those cases were counterfeit lifestyle products reported to authorities. Moreover, 7.2% (23/320) of the participants experienced adverse effects due to counterfeit lifestyle products. CONCLUSIONS: In summary, patients experienced counterfeit lifestyle products that resulted in adverse effects on their health. Although certain adverse effects were reported in this study, counterfeit products were underreported to authorities. Further public awareness campaigns and patient education are needed

    ILSF, A THIRD GENERATION LIGHT SOURCE LABORATORY IN IRAN

    Abstract The Iranian Light Source Facility (ILSF) project is a first large scale accelerator facility which is currently under planning in Iran. On the basis of the present design, circumference of the 3 GeV storage ring is 297.6 m. Beam current and natural beam emittance are 400 mA and 3.278 nm.rad respectively. The facility will be built on a land of 50 hectares area in the city of Qazvin, located 150 km West of Tehran. The city is surrounded by many universities, research centers and industrial companies. The design and construction of prototype items such as radio frequency solid state amplifier, dipole magnets, highly stable magnet power supplies and girders have already begun. Site selection studies, including geotechnical and seismological measurements are being performed. Conceptual Design Report, CDR, as the first milestone of the project was published in October 2012

    COMPARE CPM-RMI Trial: Intramyocardial transplantation of autologous bone marrow-derived CD133+ Cells and MNCs during CABG in patients with recent MI: A Phase II/III, multicenter, placebo-controlled, randomized, double-blind clinical trial

    Objective: The regenerative potential of bone marrow-derived mononuclear cells (MNCs) and CD133+ stem cells in the heart varies in terms of their pro-angiogenic effects. This phase II/III, multicenter and double-blind trial is designed to compare the functional effects of intramyocardial autologous transplantation of both cell types and placebo in patients with recent myocardial infarction (RMI) post-coronary artery bypass graft. Materials and Methods: This was a phase II/III, randomized, double-blind, placebo-controlled trial COMPARE CPM-RMI (CD133, Placebo, MNCs - recent myocardial infarction) conducted in accordance with the Declaration of Helsinki that assessed the safety and efficacy of CD133 and MNCs compared to placebo in patients with RMI. We randomly assigned 77 eligible RMI patients selected from 5 hospitals to receive CD133+ cells, MNC, or a placebo. Patients underwent gated single photon emission computed tomography assessments at 6 and 18 months post-intramyocardial transplantation. We tested the normally distributed efficacy outcomes with a mixed analysis of variance model that used the entire data set of baseline and between-group comparisons as well as within subject (time) and group�time interaction terms. Results: There were no related serious adverse events reported. The intramyocardial transplantation of both cell types increased left ventricular ejection fraction by 9 95% confidence intervals (CI): 2.14% to 15.78%, P=0.01 and improved decreased systolic wall thickening by -3.7 (95% CI: -7.07 to -0.42, P=0.03). The CD133 group showed significantly decreased non-viable segments by 75% (P=0.001) compared to the placebo and 60% (P=0.01) compared to the MNC group. We observed this improvement at both the 6- and 18-month time points. Conclusion: Intramyocardial injections of CD133+ cells or MNCs appeared to be safe and efficient with superiority of CD133+ cells for patients with RMI. Although the sample size precluded a definitive statement about clinical outcomes, these results have provided the basis for larger studies to confirm definitive evidence about the efficacy of these cell types (Registration Number: NCT01167751). © 2018 Royan Institute (ACECR).     Clinical efficacy of convalescent plasma for treatment of COVID-19 infections: Results of a multicenter clinical study

    Since Dec. 2019 the new coronavirus (SARS-CoV-2) has infected millions and claimed life of several hundred thousand worldwide. However, so far no approved vaccine or drug therapy is available for treatment of virus infection. Convalescent plasma has been considered a potential modality for COVID-19 infection. One hundred eighty-nine COVID-19 positive patients including 115 patients in plasma therapy group and 74 patients in control group, registered in the hospitals with confirmed COVID-19 infection, entered this multi-center clinical study. Comparison of outcomes including all-cause mortality, total hospitalization days and patients� need for intubation between the two patient groups shows that total of 98 (98.2 ) of patients who received convalescent plasma were discharged from hospital which is substantially higher compared to 56 (78.7 ) patients in control group. Length of hospitalization days was significantly lower (9.54 days) in convalescent plasma group compared with that of control group (12.88 days). Only 8 patients (7) in convalescent plasma group required intubation while that was 20 in control group. This clinical study provides strong evidence to support the efficacy of convalescent plasma therapy in COVID-19 patients and recommends this treatment for management of these patients. Clinical efficacy, immediate availability and potential cost effectiveness could be considered as main advantages of convalescent plasma therapy. © 2020 Elsevier Lt
