    The Health System and the Russian Orthodox Church: Prospects for Development

    The article examines the participation and assistance of the Orthodox Church in solving problems that allowed to give a scientific justification for the cooperation of health care and Orthodox religious institutions, to determine their role in the historical context and structure of modern healthcare in Russia. The article presents an algorithm for organizing sisters of mercy, their system of upbringing. Particular attention is given to the possibility of teaching the course "Foundations of Orthodox Culture" in secular educational institutions. Research materials can serve as a basis for the formation of morally sound positions of medical workers and the population on the main problems of medical activity. Based on the study, the authors published a series of articles in which the experience of the work of the Orthodox Church in the charitable sphere has been summarized. This experience can be used to create new charitable institutions, including those who provided medical assistance. In preparing the article, the authors used concrete historical, civilizational, formational and social methods of research that allowed us to uncover facts, phenomena and processes in the interconnection and unity of the past, present and future

    The variability of birth weight

    The material for this study was obtained from the maternity records relating to single births occurring in Malta throughout 1965. In this year there were 5,719 total births; of these the sample studied covers 2,517 births (44.11 percent). The records were personally examined by one of the authors (Cremona, 1967) from St. Luke's Hospital Malta, from Maltese private Hospitals, and from the District Nursing Association. In Malta the mean birth weight for male babies is 3446g, and for female babies 3358g. The mean birth weight tends to increase with the mother's parity, and this trend continues even with the higher parities. The association between birth weight and rising maternal age remains uncertain. It is also probable that birth weight has a geographical determinant. Birth weight is a multifactorial product and cannot be very reliable as a clinical parameter. A new terminology is necessary to distinguish between three different groups of babies with low birth weights.peer-reviewe

    Nighttime is the worst time: Parental fear of hypoglycemia in young children with type 1 diabetes

    BACKGROUND: Fear of hypoglycemia is common in parents of young children with type 1 diabetes (T1D), but little is known about the specific fears that parents most often experience. Hypoglycemia fear has been associated with poorer glycemic control in older children, though not yet studied in a large cohort of very young children. MATERIALS AND METHODS: Parents of 549 children <7 years (mean 5.2 ± 1.2 years [19% <3 years]) with a mean diabetes duration of 2.4 ± 1.0 years (range 1-6 years) and mean HbA1c 8.2% ± 1.1% (66 ± 12 mmol/mol) registered in the T1D Exchange completed the worry scale of the Hypoglycemia Fear Survey modified for parents (HFS-P). RESULTS: Mean parental fear of hypoglycemia worry score was 36.1 ± 23.1 (possible range 0-100), with most frequent worries related to the child having a low while asleep and the child not recognizing a low. The mean worry score was not associated with the child's age, glycemic control, or recent severe hypoglycemic event. Parental worries about lows while sleeping were significantly higher in pump users than non-users (61% vs. 45%; P < .001), and tended to be higher in CGM users than non-users (62% vs 51%; P = .02). CONCLUSIONS: The greatest worries of parents of young children with T1D were related to hypoglycemia during sleep and other times/circumstances during which it would be difficult to detect hypoglycemia. Using advanced diabetes technologies may be an effort to temper fears about hypoglycemia during sleep, though the directionality of this relationship is undetermined. Additional studies can clarify this association and leverage use of diabetes technologies to improve glycemic control

    Status of the “Mangrove tunicate” Ecteinascidia turbinata (Ascidiacea: Perophoridae) in the Mediterranean Sea

    The ascidian Ecteinascidia turbinata is reported from Maltese waters for the first time. Mature colonies were recorded on artificial substrata at two different sites (and on natural substrata at one of these), 4 km apart, during the summer months. The appearance of this ascidian is expected to be seasonal as the winter temperature in Malta may fall below that required for the maintenance of zooid growth. A second species, E. moorei, which was described in 1890 is here confirmed to be the same as E. turbinata, meaning that the species has existed in the Mediterranean since at least ~1880. The possibility that the Mediterranean population is a relic one from warmer periods cannot yet be excluded, so it is best described as being cryptogenic. The species appears to be spreading slowly, perhaps as a result of the rise in surface sea temperature. The Maltese sites offer an opportunity to monitor the species as they are accessible dive sites. This will allow assessment of whether this species remains restricted to these sites, or if it spreads perhaps to impact other species

    Inter- and intracontinental migrations and local differentiation have shaped the contemporary epidemiological landscape of canine parvovirus in South America

    Canine parvovirus (CPV) is a fast-evolving single-stranded DNA virus that causes one of the most significant infectious diseases of dogs. Although the virus dispersed over long distances in the past, current populations are considered to be spatially confined and with only a few instances of migration between specific localities. It is unclear whether these dynamics occur in South America where global studies have not been performed. The aim of this study is to analyze the patterns of genetic variability in South American CPV populations and explore their evolutionary relationships with global strains. Genomic sequences of sixty-three strains from South America and Europe were generated and analyzed using a phylodynamic approach. All the obtained strains belong to the CPV-2a lineage and associate with global strains in four monophyletic groups or clades. European and South American strains from all the countries here analyzed are representative of a widely distributed clade (Eur-I) that emerged in Southern Europe during 1990–98 to later spread to South America in the early 2000s. The emergence and spread of the Eur-I clade were correlated with a significant rise in the CPV effective population size in Europe and South America. The Asia-I clade includes strains from Asia and Uruguay. This clade originated in Asia during the late 1980s and evolved locally before spreading to South America during 2009–10. The third clade (Eur-II) comprises strains from Italy, Brazil, and Ecuador. This clade appears in South America as a consequence of an early introduction from Italy to Ecuador in the middle 1980s and has experienced extensive local genetic differentiation. Some strains from Argentina, Uruguay, and Brazil constitute an exclusive South American clade (SA-I) that emerged in Argentina in the 1990s. These results indicate that the current epidemiological scenario is a consequence of inter- and intracontinental migrations of strains with different geographic and temporal origins that set the conditions for competition and local differentiation of CPV populations. The coexistence and interaction of highly divergent strains are the main responsible for the drastic epidemiological changes observed in South America in the last two decades. This highlights the threat of invasion from external sources and the importance of whole-genome resolution to robustly infer the origin and spread of new CPV variants. From a taxonomic standpoint, the findings herein show that the classification system that uses a single amino acid to identify variants (2a, 2b, and 2c) within the CPV-2a lineage does not reflect phylogenetic relationships and is not suitable to analyze CPV evolution. In this regard, the identification of clades or sublineages within circulating CPV strains is the first step towards a genetic and evolutionary classification of the virus
