    Lifestyle and cardiovascular mortality in menopausal women: a population-based cohort study

    Introduction and objectives: There are models for cardiovascular risk prediction in the general population, but the prediction of risk in postmenopausal women has not been specifically studied. This study aimed to determine the association of lifestyle habits and chronic diseases with cardiovascular risk in menopausal women, as well as to build a risk scale. Methods: Retrospective population-based cohort study using data from the 2011 National Health Survey of Spain as a data source, Women 50 years were included. The characteristics that best defined the life habits of the study women were collected, as well as their health status and self-reported medical history at the time of the survey. Follow-up data on all-cause mortality were obtained from participants from 2011 to 2017. Results: A total of 5953 women 50 years of age were included, with a mean age of 66.4 11.4 years. The incidence of cardiovascular mortality in the follow-up period was 4%. Vegetable consumption less than 1 time/week (HR, 1.758), smoking (HR, 1.816) or excess hours of sleep ( 9 h/day, HR, 1.809), or o have main daily activity sitting most of the time (HR, 2.757) were related to cardiovascular mortality. The predictive model presents an honest C-index in test sample of 0.8407 (95%CI, 0.8025-0.8789). Conclusions: Life habits such as the consumption of vegetables, daily main activity, sleeping hours or smoking are risk factors for cardiovascular mortality of great relevance among menopausal women.     Energy efficiency considerations in integrated IT and optical network resilient infrastructures

    The European Integrated Project GEYSERS - Generalised Architecture for Dynamic Infrastructure Services - is concentrating on infrastructures incorporating integrated optical network and IT resources in support of the Future Internet with special emphasis on cloud computing. More specifically GEYSERS proposes the concept of Virtual Infrastructures over one or more interconnected Physical Infrastructures comprising both network and IT resources. Taking into consideration the energy consumption levels associated with the ICT today and the expansion of the Internet in size and complexity, that incurring increased energy consumption of both IT and network resources, energy efficient infrastructure design becomes critical. To address this need, in the framework of GEYSERS, we propose energy efficient design of infrastructures incorporating integrated optical network and IT resources, supporting resilient end-to-end services. Our modeling results quantify significant energy savings of the proposed solution by jointly optimizing the allocation of both network and IT resources

    QED Logarithms in the Electroweak Corrections to the Muon Anomalous Magnetic Moment

    We employ an effective Lagrangian approach to derive the leading-logarithm two-loop electroweak contributions to the muon anomalous magnetic moment, a_mu. We show that these corrections can be obtained using known results on the anomalous dimensions of composite operators. We confirm the result of Czarnecki et al. for the bosonic part and present the complete sin^2 \theta_W dependence of the fermionic contribution. The approach is then used to compute the leading-logarithm three-loop electroweak contribution to a_mu. Finally we derive, in a fairly model-independent way, the QED improvement of new-physics contributions to a_mu and to the electric dipole moment (EDM) of the electron. We find that the QED corrections reduce the effect of new physics at the electroweak scale by 6% (for a_mu) and by 11% (for the electron EDM).Comment: 13 page

    14 páginas, 5 figuras, 10 tablas.A mechanistic lactation model, based on a theory of mammary cell proliferation and cell death, was studied and compared to the equation of Wood (1967). Lactation curves of British Holstein Friesian cows (176 curves), Spanish Churra sheep (40 curves) and Spanish Murciano-Granadina goats (30 curves) were used for model evaluation. Both models were fitted in their original form using non-linear least squares estimation. The parameters were compared among species and among parity groups within species. In general, both models provided highly significant fits to lactation data and described the data accurately. The mechanistic model performed well against Wood's 1967 equation (hereafter referred to as Wood's equation), resulting in smaller residual mean square values in more than two-thirds of the datasets investigated, and producing parameter estimates that allowed appropriate comparisons and noticeable trends attributed to shape. Using Akaike or Bayesian information criteria, goodness-of-fit with the mechanistic model was superior to that with Wood's equation for 1 Lie cow lactation curves, with no significant differences between models when fitted to goat or sheep lactation curves. The rate parameters of the mechanistic model, representing specific proliferation rate of mammary secretory cells at parturition, decay associated with reduction in cell proliferation capacity with time and specific death rate of mammary secretory cells, were smaller for primiparous than for multiparous cows. Greater lactation persistency of cows compared to goats and sheep, and decrease in persistency with parity, were shown to be represented by different values of the specific secretory cell death rate parameter in the mechanistic model. The plausible biological interpretation and fitting properties of the mechanistic model enable it to be used in complex models of whole-cow digestion and metabolism and as a tool in selection programmes and by dairy producers for management decisions.Canada Research Chairs ProgramPeer reviewe

    Quantum coherent control of highly multipartite continuous-variable entangled states by tailoring parametric interactions

    The generation of continuous-variable multipartite entangled states is important for several protocols of quantum information processing and communication, such as one-way quantum computation or controlled dense coding. In this article we theoretically show that multimode optical parametric oscillators can produce a great variety of such states by an appropriate control of the parametric interaction, what we accomplish by tailoring either the spatio-temporal shape of the pump, or the geometry of the nonlinear medium. Specific examples involving currently available optical parametric oscillators are given, hence showing that our ideas are within reach of present technology.Comment: 14 pages, 5 figure

    Insights into the degradation capacities of Amycolatopsis tucumanensis DSM 45259 guided by microarray data

    The analysis of catabolic capacities of microorganisms is currently often achieved by cultivation approaches and by the analysis of genomic or metagenomic datasets. Recently, a microarray system designed from curated key aromatic catabolic gene families and key alkane degradation genes was designed. The collection of genes in the microarray can be exploited to indicate whether a given microbe or microbial community is likely to be functionally connected with certain degradative phenotypes, without previous knowledge of genome data. Herein, this microarray was applied to capture new insights into the catabolic capacities of copper-resistant actinomycete Amycolatopsis tucumanensis DSM 45259. The array data support the presumptive ability of the DSM 45259 strain to utilize single alkanes (n-decane and n-tetradecane) and aromatics such as benzoate, phthalate and phenol as sole carbon sources, which was experimentally validated by cultivation and mass spectrometry. Interestingly, while in strain DSM 45259 alkB gene encoding an alkane hydroxylase is most likely highly similar to that found in other actinomycetes, the genes encoding benzoate 1,2-dioxygenase, phthalate 4,5-dioxygenase and phenol hydroxylase were homologous to proteobacterial genes. This suggests that strain DSM 45259 contains catabolic genes distantly related to those found in other actinomycetes. Together, this study not only provided new insight into the catabolic abilities of strain DSM 45259, but also suggests that this strain contains genes uncommon within actinomycetes.Fil: Bourguignon, Natalia.     Optimizing the clinical utility of PCA3 to diagnose prostate cancer in initial prostate biopsy

    Background: PCA3 has been included in a nomogram outperforming previous clinical models for the prediction of any prostate cancer (PCa) and high grade PCa (HGPCa) at the initial prostate biopsy (IBx). Our objective is to validate such IBx-specific PCA3-based nomogram. We also aim to optimize the use of this nomogram in clinical practice through the definition of risk groups. Methods: Independent external validation. Clinical and biopsy data from a contemporary cohort of 401 men with the same inclusion criteria to those used to build up the reference’s nomogram in IBx. The predictive value of the nomogram was assessed by means of calibration curves and discrimination ability through the area under the curve (AUC). Clinical utility of the nomogram was analyzed by choosing thresholds points that minimize the overlapping between probability density functions (PDF) in PCa and no PCa and HGPCa and no HGPCa groups, and net benefit was assessed by decision curves. Results: We detect 28 % of PCa and 11 % of HGPCa in IBx, contrasting to the 46 and 20 % at the reference series. Due to this, there is an overestimation of the nomogram probabilities shown in the calibration curve for PCa. The AUC values are 0.736 for PCa (C.I.95 %:0.68–0.79) and 0.786 for HGPCa (C.I.95 %:0.71–0.87) showing an adequate discrimination ability. PDF show differences in the distributions of nomogram probabilities in PCa and not PCa patient groups. A minimization of the overlapping between these curves confirms the threshold probability of harboring PCa >30 % proposed by Hansen is useful to indicate a IBx, but a cut-off > 40 % could be better in series of opportunistic screening like ours. Similar results appear in HGPCa analysis. The decision curve also shows a net benefit of 6.31 % for the threshold probability of 40 %. Conclusions: PCA3 is an useful tool to select patients for IBx. Patients with a calculated probability of having PCa over 40 % should be counseled to undergo an IBx if opportunistic screening is required

    Estudio piloto de metilfenidato y entrenamiento a padres en el tratamiento de niños con trastorno por déficit de atención hiperactividad [A pilot study of methylphenidate and parent training in the treatment of children with attention-deficit hiperactivity disorder]

    Objetivo. Este estudio comunica la eficacia de un programa de entrenamiento a padres y un ensayo clínico con medicación estimulante (metilfenidato), en la disminución de síntomas de trastorno por déficit de atención-hiperactividad (TDAH) en una muestra de niños venezolanos.Pacientes y métodos. 24 niños, con edades entre 6 y 10 años, con diagnóstico de TDAH, identificados en unas jornadas de cribado de TDAH, se asignaron aleatoriamente a los dos grupos de tratamiento: entrenamiento a padres y medicación estimulante (metilfenidato).Resultados. Ambos grupos mostraron reducción de síntomas al finalizar la fase de intervención terapéutica en diferentes grados. Estas diferencias fueron estadísticamente significativas para las valoraciones de los padres en ambos grupos, en los síntomas de falta de atención, hiperactividad e impulsividad. Las valoraciones de los profesores mostraron disminución de síntomas significativos en el grupo de medicación para las medidas de hiperactividad e impulsividad, pero no para las de falta de atención, mientras que para el grupo de entrenamiento a padres sólo hubo diferencias significativas en el índice de TDAH. No hubo diferencia en cuanto a la efectividad de ambas intervenciones.Conclusiones. Al comparar ambos programas de tratamiento, se observa que tanto el entrenamiento a padres como la medicación psicoestimulante son tratamientos efectivos para la disminución de los síntomas del TDAH. A pesar de que no existen diferencias entre la efectividad de ambas intervenciones, se observa una tendencia a que la medicación sea más efectiva. - OBJECTIVE. This study gives information about a parent training program and a clinical trial with a stimulant drug (methylphenidate) to reduce the symptoms of attention deficit hiperactivity disorder (ADHD) in a group of children in Venezuela.PATIENTS AND METHODS. 24 children, aged between 6 and 10 years and diagnosed as having ADHD, identified in ADHD screening days, were randomly assigned to two groups of treatment: parent training and a stimulant drug (methylphenidate).RESULTS. Both groups showed an improvement in their symptoms, to different degrees, after treatment. The differences were statistically significant, as evaluated by their parents, regarding the symptoms of inattention, hyperactivity and impulsivity. The teachers observed a significant improvement for the medication group with respect to the symptoms of hyperactivity and impulsivity, but no change in the inattention, whilst in the parent training group there was only significant difference in the ADHD index. There was no difference in the effectiveness of the two types of treatment.CONCLUSIONS. When the two programs of treatment were compared it was observed that both parent training and psychostimulant medication were effective in reducing the symptoms of ADHD. Although there was no difference in the effectiveness of the two programs, there was a tendency for medication to be more effectiv

    Pharmacokinetic / pharmacodynamic relationships of liposomal amphotericin B and miltefosine in experimental visceral leishmaniasis.

    BACKGROUND: There is a continued need to develop effective and safe treatments for visceral leishmaniasis (VL). Preclinical studies on pharmacokinetics and pharmacodynamics of anti-infective agents, such as anti-bacterials and anti-fungals, have provided valuable information in the development and dosing of these agents. The aim of this study was to characterise the pharmacokinetic and pharmacodynamic properties of the anti-leishmanial drugs AmBisome and miltefosine in a preclinical disease model of VL. METHODOLOGY / PRINCIPAL FINDINGS: BALB/c mice were infected with L. donovani (MHOM/ET/67/HU3) amastigotes. Groups of mice were treated with miltefosine (orally, multi-dose regimen) or AmBisome (intravenously, single dose regimen) or left untreated as control groups. At set time points groups of mice were killed and plasma, livers and spleens harvested. For pharmacodynamics the hepatic parasite burden was determined microscopically from tissue impression smears. For pharmacokinetics drug concentrations were measured in plasma and whole tissue homogenates by LC-MS. Unbound drug concentrations were determined by rapid equilibrium dialysis. Doses exerting maximum anti-leishmanial effects were 40 mg/kg for AmBisome and 150 mg/kg (cumulatively) for miltefosine. AmBisome displayed a wider therapeutic range than miltefosine. Dose fractionation at a total dose of 2.5 mg/kg pointed towards concentration-dependent anti-leishmanial activity of AmBisome, favouring the administration of large doses infrequently. Protein binding was >99% for miltefosine and amphotericin B in plasma and tissue homogenates. CONCLUSION / SIGNIFICANCE: Using a PK/PD approach we propose optimal dosing strategies for AmBisome. Additionally, we describe pharmacokinetic and pharmacodynamic properties of miltefosine and compare our findings in a preclinical disease model to available knowledge from studies in humans.
