Risk Factors for Frequent Readmissions and Barriers to Transplantation in Patients with Cirrhosis
Background: Hospital readmission rate is receiving increasing regulatory scrutiny. Patients with cirrhosis have high hospital readmissions rates but the relationship between frequent readmissions and barriers to transplantation remains unexplored. The goal of this study was to determine risk factors for frequent readmissions among patients with cirrhosis and identify barriers to transplantation in this population. Methods: We retrospectively reviewed medical records of 587 patients with a confirmed diagnosis of cirrhosis admitted to a large tertiary care center between May 1, 2008 and May 1, 2009. Demographics, clinical factors, and outcomes were recorded. Multivariate logistic regression was performed to identify risk factors for high readmission rates. Transplant-related factors were assessed for patients in the high readmission group. Results: The 587 patients included in the study had 1557 admissions during the study period. A subset of 87 (15%) patients with 5 or more admissions accounted for 672 (43%) admissions. The factors associated with frequent admissions were non-white race (OR = 2.45, p = 0.01), diabetes (OR = 2.04, p = 0.01), higher Model for End-Stage Liver Disease (MELD) score (OR = 35.10, p30) and younger age (OR = 0.98, p = 0.02). Among the 87 patients with ≥5 admissions, only 14 (16%) underwent liver transplantation during the study period. Substance abuse, medical co-morbidities, and low (<15) MELD scores were barriers to transplantation in this group. Conclusions: A small group of patients with cirrhosis account for a disproportionately high number of hospital admissions. Interventions targeting this high-risk group may decrease frequent hospital readmissions and increase access to transplantation. © 2013 Ganesh et al
IMPACT OF IRRIGATION WATER MANAGEMENT AND SILICON SPRAYING ON AVOIDING COLD INJURY AND IMPROVEMENT YIELD OF KITT MANGOS
The present study was carried out during 2015 and 2016 seasons on Kitt mongo trees to evaluate the effect of three irrigation water management (4000 m3 of irrigation water were applied to the four physiological stages of mango tree growth as 15 & 20 & 40 & 25 % , 30 & 15 & 35& 20 % and 35 & 20 & 30& 15 % m3 / tree/ year, respectively) as well as four concentrations of potassium silicate on avoiding cold injury and improvement yield of kitt mango . Irrigation Mango tree by the second program water management (30, 15, 35 , 20) recorded The highest values of vegetative growth in terms of number of shoot /tree and lower values of leaves percentage affected with cold injury, Also, an evident increase in the content leaves of N & K elements , C/N ratio and final fruit set % total yield by divided irrigation water at the four physiological stages of mango trees (30, 15, 35, 20) was recorded compared with the other two water managements. No significant differences between the second and third irrigation water management programs (30%, 15%, 35%, 20% and 35%, 20%, 30%, 15%) on values of leaf area, P% and total carbohydrates as well as on TSS, acidity and L-ascorbic acid content of fruits . Spraying trees with potassium silicate in three times during the period from October to January at the rate of 1000 ppm give the highest values of quality characteristics at mango trees and their fruits. While sprayed with potassium silicate at 2000 ppm achieve moderate values of the mentioned characteristics It could be recommended from the obtained data that irrigation water management at the four physiological stages (30, 15, 35, 20) of Kitt mango trees and spraying 1000 ppm potassium silicate in dormancy period is considered a good treatment to avoid cold damage on leaves and fruits as well as in giving the highest yield and quality of kitt fruit
Physicians Infrequently Adhere to Hepatitis Vaccination Guidelines for Chronic Liver Disease
Background and Goals:Hepatitis A (HAV) and hepatitis B (HBV) vaccination in patients with chronic liver disease is an accepted standard of care. We determined HAV and HBV vaccination rates in a tertiary care referral hepatology clinic and the impact of electronic health record (EHR)-based reminders on adherence to vaccination guidelines.Methods:We reviewed the records of 705 patients with chronic liver disease referred to our liver clinic in 2008 with at least two follow-up visits during the subsequent year. Demographics, referral source, etiology, and hepatitis serology were recorded. We determined whether eligible patients were offered vaccination and whether patients received vaccination. Barriers to vaccination were determined by a follow-up telephone interview.Results:HAV and HBV serologic testing prior to referral and at the liver clinic were performed in 14.5% and 17.7%; and 76.7% and 74% patients, respectively. Hepatologists recommended vaccination for HAV in 63% and for HBV in 59.7% of eligible patients. Patient demographics or disease etiology did not influence recommendation rates. Significant variability was observed in vaccination recommendation amongst individual providers (30-98.6%), which did not correlate with the number of patients seen by each physician. Vaccination recommendation rates were not different for Medicare patients with hepatitis C infection for whom a vaccination reminder was automatically generated by the EHR. Most patients who failed to get vaccination after recommendation offered no specific reason for noncompliance; insurance was a barrier in a minority.Conclusions:Hepatitis vaccination rates were suboptimal even in an academic, sub-speciality setting, with wide-variability in provider adherence to vaccination guidelines. © 2013 Thudi et al
Femtosecond nonlinear ultrasonics in gold probed with ultrashort surface plasmons
Fundamental interactions induced by lattice vibrations on ultrafast time
scales become increasingly important for modern nanoscience and technology.
Experimental access to the physical properties of acoustic phonons in the THz
frequency range and over the entire Brillouin zone is crucial for understanding
electric and thermal transport in solids and their compounds. Here, we report
on the generation and nonlinear propagation of giant (1 percent) acoustic
strain pulses in hybrid gold/cobalt bilayer structures probed with ultrafast
surface plasmon interferometry. This new technique allows for unambiguous
characterization of arbitrary ultrafast acoustic transients. The giant acoustic
pulses experience substantial nonlinear reshaping already after a propagation
distance of 100 nm in a crystalline gold layer. Excellent agreement with the
Korteveg-de Vries model points to future quantitative nonlinear femtosecond
THz-ultrasonics at the nano-scale in metals at room temperature
Statistical optimization for lipase production from solid waste of vegetable oil industry
<p>The production of biofuel using thermostable bacterial lipase from hot spring bacteria out of low-cost agricultural residue olive oil cake is reported in the present paper. Using a lipase enzyme from <i>Bacillus licheniformis</i>, a 66.5% yield of methyl esters was obtained. Optimum parameters were determined, with maximum production of lipase at a pH of 8.2, temperature 50.8°C, moisture content of 55.7%, and biosurfactant content of 1.693 mg. The contour plots and 3D surface responses depict the significant interaction of pH and moisture content with biosurfactant during lipase production. Chromatographic analysis of the lipase transesterification product was methyl esters, from kitchen waste oil under optimized conditions, generated methyl palmitate, methyl stearate, methyl oleate, and methyl linoleate.</p
Postpartum Mastitis and Community-acquired Methicillin-resistant Staphylococcus aureus
This single-center, case-control study documents a relative increase in methicillin resistance among 48 cases of Staphylococcus aureus–associated postpartum mastitis during 1998–2005. Of 21 cases with methicillin resistance, 17 (81%) occurred in 2005. Twenty (95%) isolates contained the Staphylococcus cassette chromosome mec type IV gene; this suggests that the increase is due to community-acquired methicillin-resistant Staphylococcus aureus
Operational experience, improvements, and performance of the CDF Run II silicon vertex detector
The Collider Detector at Fermilab (CDF) pursues a broad physics program at
Fermilab's Tevatron collider. Between Run II commissioning in early 2001 and
the end of operations in September 2011, the Tevatron delivered 12 fb-1 of
integrated luminosity of p-pbar collisions at sqrt(s)=1.96 TeV. Many physics
analyses undertaken by CDF require heavy flavor tagging with large charged
particle tracking acceptance. To realize these goals, in 2001 CDF installed
eight layers of silicon microstrip detectors around its interaction region.
These detectors were designed for 2--5 years of operation, radiation doses up
to 2 Mrad (0.02 Gy), and were expected to be replaced in 2004. The sensors were
not replaced, and the Tevatron run was extended for several years beyond its
design, exposing the sensors and electronics to much higher radiation doses
than anticipated. In this paper we describe the operational challenges
encountered over the past 10 years of running the CDF silicon detectors, the
preventive measures undertaken, and the improvements made along the way to
ensure their optimal performance for collecting high quality physics data. In
addition, we describe the quantities and methods used to monitor radiation
damage in the sensors for optimal performance and summarize the detector
performance quantities important to CDF's physics program, including vertex
resolution, heavy flavor tagging, and silicon vertex trigger performance.Comment: Preprint accepted for publication in Nuclear Instruments and Methods
