Requirements for establishing a care management system in Shanghai, China : from the process of service utilization for the elderly

本研究では，中国上海市において介護保険制度が試行されて以降，高齢者がどのようなプロセスで社区居宅養老サービスを利用しているかについて明らかにした。そのうえで，断片化したケアの諸事例を分析することを通して，既存の供給体制における課題を検討し，それらの課題に対応するケアマネジメント体制を構築するための要件を提示した。その結果，社区居宅養老サービス供給体制にケアマネジメント体制を導入し，アセスメント，サービスの調整，モニタリングという3つの機能の整備が必要であることが示された。それを踏まえ，中国上海市においてケアマネジメント体制を構築するための要件について，アセスメント機能を明確にしたうえで，コーディネートの場を中核としたサービスデリバリーシステムの枠組みを提示した。This study revealed the process by which the elderly utilize community-based elderly care services in Shanghai, China, since the introduction of the long-term care insurance system. Through analyzing the cases of fragmented care, the study considered the challenges in the existing supply system and presented requirements for establishing a care management system that addresses those challenges. As a result, it was shown that a care management system is necessary in the community-based elderly care service supply system, with three functions of assessment, service coordination, and monitoring needing to be established. Reduction of Mg segregation in a metalorganic vapor phase epitaxial grown GaN layer by a low-temperature AlN interlayer

The redistribution behavior of Mg in a sequentially regrown GaN epilayer on a p-type doped GaN template was studied. All samples in this study were regrown by metalorganic vapor phase epitaxy on the sapphire substrates. A high density and a slow tail of Mg concentration were observed in a nominally undoped layer due to the surface segregation. We found that the insertion of a low-temperature (LT) AlN interlayer was effective to suppress the Mg redistribution in the GaN regrown layer. Analyzing the temperature dependence of the surface segregation, the activation energy of the Mg segregation was estimated to be 0.63 eV in GaN and 2.47 eV in a LT-AlN layer, respectively.journal articl

Trends and issues in research on integrated care for the elderly in China

本研究では，中国における整合照料に焦点をあて，先行研究の動向について整理したうえで，中国において整合照料を導入する際の課題について検討した。その結果，中国における整合照料は主に，【理念の共有】【協力体制設計の促進】【政策の制定】【新しい情報技術の活用】【アセスメントとモニタリング体制の導入】【資金の確保】という6つの面から論じられていた。また，政府のガバナンス機能に焦点が置かれ，政府主導のもと協力体制を整備していくことが主張されていた。これらの研究動向から，サービスを利用者につなぐ媒介としてのICT等，新しい情報技術の限界や，整合照料の導入におけるメゾ・ミクロレベルの視点の欠如によりもたらされる，整合照料の理念と体制・システム設計の間のずれの問題について検討した。This study focuses on integrated care in China, summarizes previous Chinese research on the subject, and discusses trends in the field. Then, this study examines issues in the future networking of home services for the elderly in China. As a result, it was found that the six main aspects are discussed: [sharing of ideas] [promotion of collaborative system design] [enactment of policies] [utilization of new information technology] [introduction of assessment and monitoring systems] and [financial resources]. And the previous Chinese research focuses on the government's governance function and argues for the development of a government-led cooperation system. Current status and challenges of network building in the supply system of community-based elderly care services in China : investigated through interviews with supporters in Shanghai

本研究は，現在の中国上海市における社区居宅養老サービスをめぐるネットワークをサービス関係組織間の連携の視点でとらえ，既存のサービス供給システムを踏まえ，ネットワークにおける社区居宅養老サービス関係組織間の連携・協働の実態と課題を明らかにした。その結果，既存のネットワークづくり実践における課題について，【政策立案者と現場の支援者の立場の対立】【サービスの入札体制の公平性問題】【潜在化する支援困難事例】，【社会資源の調整や連携における課題】という4つの概念的カテゴリーが抽出された。それらの課題を解決するため，①支援現場の状況を十分に反映させる場を構築すること，②独立したネットワーク間の狭間を埋めること，③地域全体に対応可能な重層的なネットワークづくりの仕組みを構築すること，という3点を提示した。This research examines the current network surrounding community-based elderly care services in Shanghai, China, from the perspective of collaboration among organizations related to social home elderly care services. And based on the existing service supply system, this research clarified the current status and issues of cooperation and cooperation among organizations related to community-based elderly care services in the network. The results reveal five conceptual categories of challenges: (1) conflict between policymakers and on-the-ground supporters, (2) fairness in the bidding system for services, (3) potential support difficulties that may not be immediately visible, and (4) problems related to coordination and collaboration of social resources. To address these challenges, the study suggests three approaches: ① establishment of a platform that fully reflects the situation on the ground, ② fill the gaps between independent networks, and ③ creation of a multi-layered network-building mechanism that can respond to the needs of the entire community.論文(Article)application/pdfdepartmental bulletin pape
