    Cases of Pediatric Pyelonephritis: A Single-Center Retrospective Study from an Extended-Spectrum β-Lactamase-Producing Escherichia coli Endemic Area in Japan

    Background: Extended-spectrum β-lactamase (ESBL)-producing Escherichia coli has been increasingly recognized as the cause of upper urinary tract infection (UTI) in children. We have been using flomoxef at our department since 2017 as the first-line empiric therapy for children diagnosed with UTIs, and we avoid using carbapenems, which are considered the first-line treatment for ESBL-producing E. coli. However, reports on the use of flomoxef for UTIs are limited, especially for pediatric patients. The presence of vesicoureteral reflux at the onset of pyelonephritis is a concern. Severe vesicoureteral reflux can lead to repeated UTI and future deterioration of renal function, but the indication for voiding urethrography, which closely examines the presence of vesicoureteral reflux complications, is controversial. Methods: We retrospectively reviewed the laboratory findings, treatment, and clinical course of 96 pyelonephritis cases experienced at our department over a 7-year period from April 2014 to March 2021. Results: ESBL-producing E. coli were identified as the cause of pyelonephritis in 51% of cases, and this value was significantly higher (88%) in 2017. No significant differences were found in the febrile period or recurrence rate between the flomoxef-initiated group and other antibiotics groups. We also examined clinical indicators to predict vesicoureteral reflux and found no significant differences in ultrasonographic findings of hydronephrosis. Conclusion: In the present series, 51% of all pyelonephritis cases were found to be caused by ESBL-producing E. coli, with a significant increase in recent years. Flomoxef may be a useful alternative to carbapenem for ESBL-producing E. coli and the initial antibiotic of choice for upper UTIs in children. The indication for voiding cystourethrography should be carefully determined.journal articl

    The Optimum Received Signal-Power Distribution for CDMA Packet Communication Systems Employing Successive Interference Cancellation

    In CDMA packet communication systems employing Successive Interference Cancellation(SIC) scheme, the suppression of MAI depends on the received signal-power distribution, in addition to the cross-correlation characteristic of spreading sequences and the number of transmitted packets. Some works have been done about discovering the received signal-power distribution which makes MAI be suppressed effectively. This distribution minimizes the averaged Bit Error Rate(BER). However, in packet communication systems, it is dominant to the performance if packets can be transmitted successfully or not. Even if the averaged BER is high, many packets can be correctly transmitted[7]. It implies that the optimum distribution on averaged BER does not give the best performance of packet transmission. In this paper, the received signal-power distribution which gives the best performance of packet transmission is derived. Such a distribution will make clear the limit of performance improvement using SIC.journal articl

Even at the best efficiency point, the internal flow in the pump of circular casing is asymmetric. The flow goes out from the impeller only at the exhaust area of the casing.     A Three-Parameter Distribution Used for Structural Reliability Evaluation

    It is a fundamental task to determine the distribution of basic random variables in structural reliability evaluation. In the present paper, a three-parameter distribution, directly defined in terms of mean value, deviation and skewness, is suggested. The new distribution can be applied (1) as a candidate distribution in fitting the statistical data of basic variables and generally presenting two-parameter distributions with small skewness, (2) to realize normal transformation and generate random samplings for random variables with unknown cumulative distribution functions in order to include them into structural reliability analysis, and (3) to provide a moment reliability index for the cases where the first-three moments of the performance function can be easily obtained. Some numerical examples are presented, the simplicity, generality and flexibility of the distribution are investigated, the applicability and efficiency of the distribution are demonstrated, and the distributions of some basic random variables are discussed.application/pdfjournal articl
