(The) effect of propofol on cytokine release in patients who undergo colo-rectal cancer surgery

의과학사업단/석사[한글]목적: Propofol이 수술 중 cytokine 반응에 대한 영향은 친 염증 또는 세포 매개 면역 기능 항진에 대한 연구와 반 염증 또는 세포 매개 면역 기능 억제에 대한 연구 보고가 있다. 수술 중 cytokine 반응에 대한 propofol의 영향을 알아보기 위해 본 저자는 interleukin-10과 interleukin-12를 측정하여 enflurane 흡입 마취제와 비교하고자 하였다. 재료 및 방법: 본 연구는 미국 마취과학회 신체 등급(American Society of Anesthesiologists:ASA) I 또는 II에 해당되는 대장 직장암 수술을 받는 30 명 환자를 대상으로 propofol군과 enflurane군으로 15 명 씩 무작위로 나누었다. Propofol 군은 정주로 마취 유도 및 유지를 하였다. 술 후 통증 관리는 회복실에서 부하량으로 pethidine HCl 25 mg을 1 회 내지 2 회 정도 투여하여 visual analogue scale 점수 5 이하로 통증 조절 후 통증 자가 조절 장치(patient controlled analgesia device: PCA)를 연결하였다. Enflurane 군은 thiopental로 마취 유도하였고 마취 유지는 enflurane 흡입으로 조절하였다. 술 후 통증관리는 propofol 군과 동일하게 하였다. 채혈은 수술장 도착 시, 피부 절개 1시간 후, 4 시간 후, 8 시간 후, 24 시간 후에 시행하였다. 채혈 당시 환자의 혈압, 맥박수를 측정하였고, 수술 중 투여된 수액 및 수혈량, 출혈량, 마취 시간, 수술 시간을 기록하였다. 결과: 수술 시작 1 시간 후 측정한 interleukin-10은 양군 모두에서 수술 전보다 의미 있게 상승 후 다시 회복되었고, 양군 간의 의미 있는 차이는 보이지 않았다. Interleukin-12는 양 군 모두 각 군 내에서는 의미 있는 변화가 없었고 양 군간 비교에서도 의미 있는 차이를 보이지 않았다. 결론: 수술과 연관된 반 염증성 cytokine 반응을 확인할 수 있었으나 propofol 정주마취가 enflurane 흡입마취에 비해 친 또는 반 염증 반응 중 어느 한 쪽으로 반응을 더 향상시킨다는 기존의 주장들을 지지하지는 못 하였다. 핵심되는 말: propofol, 친염증 반응, 반염증 반응, cytokine, 대장-직장암 수술 [영문]Purpose: The role of propofol on the inflammatory cytokine response during the surgery has been contradictory. This study was conducted to evaluate the effect of propofol on cytokine responses compared to that of enflurane. Pro- and anti-inflammatory cytokines, interleukin (IL)-10 and IL-12, were measured. Methods: Total 30 patients, who underwent colo-rectal cancer surgery, were randomly assigned into two groups in double-blind fashion. One group (n = 15) was anesthetized with enflurane vapor anesthetic, whereas the other group (n = 15) with propofol intravenous infusion. At predetermined time intervals, the changes of IL-10 and IL-12 were measured. In addition, vital signs, amount of fluid infused during operation, estimated blood loss, total amount of pethidine given within 24 hours after operation were also measured. Results: There were no statistically significant differences in the changes of IL-10 and IL-12 concentrations at all intervals when compared between two groups, although the IL-10 was significantly increased at 1 hour after incision within each group. No other measured variables showed any meaningful differences. Conclusion: This study has demonstrated that the propofol anesthesia did not prevent the surgical stress provoked cytokine responses.
