    目的探讨adaptive LASSO; logistic回归模型在老年人养老意愿影响因素研究中的应用。方法基于厦门市60岁及以上老年人口的多阶段整群抽样调查数据,建立老年人养老意愿影响; 因素的adaptive LASSO; logistic回归模型,通过交叉验证法选择模型中的调和参数lambda;通过与全变量和逐步logistic回归结果的比较,探讨adaptive; LASSO; logistic回归模型的优势。结果共纳入1244名老年人,其养老意愿为家庭养老、社区居家养老和机构养老的比例分别为70.0%、21.1%和8.; 9%。交叉验证法选择的lambda为0.018;此时adaptive LASSO; logistic回归模型纳入的自变量为居住地、年龄、婚姻状况、文化程度、子女数、每月退休金收入、公费医疗和住院情况;; BIC和AIC分别为1931、1888,均低于全变量logistic回归(2077、1923)和逐步logistic回归(2025、1912); 。结论adaptive LASSO logistic回归模型可用于老年人养老意愿影响因素研究。老年人的养老意愿受多个因素影响。Objective This study was aimed to analyze influencing factors on elders; preference for supporting in case to provide evidence for diversified; elderly supporting. Methods This article applied an adaptive LASSO; logistic model to the multi-stage cluster sampling data of the; population aged 60 or older in Xiamen to analyze influencing factors on; elders preference for supporting. Cross validation method was used to; choose lambda for adaptive LASSO logistic model. In addition,we; evaluated the model fitting of adaptive LASSO logistic model by; comparing the BIC and AIC with full logistic model and stepwise logistic; model. Results The cross validation method resulted in lambda = 0.018; for adaptive LASSO logistic model, in which variables retained were; residence, age,marital status, education level,number of children, the; monthly pension income,medical insurance and hospitalization. BIC of; adaptive LASSO logistic model,full logistic model and stepwise logistic; model were 1931, 2077and 2025 respectively. And AIC of the three models; were 1888, 1923 and 1912 respectively. Conclusion Compared to full; logistic model and stepwise logistic model, adaptive LASSO logistic; model was the best fitting model for influencing factors on elders; preference for supporting. Adaptive LASSO logistic model could be used; to analyze influencing factors on elders preference for supporting.; There were multiple factors which influenced elders preference for; supporting.厦门大学大学生创新创业训练计划项

    解剖型锁定钛板与髓内钉治疗胫骨远端骨折的疗效比较

    目的对比应用解剖型锁定钛板与髓内钉治疗胫骨远端骨折的临床疗效与并发症情况。方法自2019年9月至2021年9月,于中山大学孙逸仙纪念医院诊断为胫骨远端AO-A型骨折的手术患者纳入本研究,共32例。其中15例采用钛板螺钉治疗,17例采用髓内钉治疗。比较两组一般手术指标、骨折愈合时间、术后并发症发生情况、手术前后VAS评分及术后踝关节功能。结果患者均获得随访,随访时间10~20个月,平均12个月。髓内钉组手术时间、术中透视次数均长于钛板螺钉组,但术前等待时间、住院天数少于钛板螺钉组,统计学差异显著(P<0.05);两组骨折愈合时间相比,差异无统计学意义(P>0.05);术后6周,两组VAS评分均低于术前,统计学差异显著(P<0.05);髓内钉组与钛板螺钉组相比,手术前后VAS评分差异无统计学意义(P>0.05);术后6月AOFAS评分、术后1年踝关节活动度、并发症发生率髓内钉组优于钛板螺钉组,统计学差异显著(P<0.05);术后1年AOFAS评分两组差异无统计学意义(P>0.05)。结论钛板螺钉与髓内钉均是治疗胫骨远端AO-A型骨折的有效术式,远期临床疗效无明显差异。髓内钉的软组织并发症较少,对踝关节活动度影响更小,康复更快,安全性更高,而钛板螺钉发生术后软组织感染的概率较大。我们建议在临床工作中,应该做好患者的术前评估,在把握适应症的前提下,髓内钉并发症更少,有一定优势

    Effects of Nd element addition on microstructure and mechanical properties of Ti-6Al-4V-2Cr alloy

    本试验采用冷等静压法及粉末冶金法制备钛合金,消除了成分偏析对合金性能的影响。在Ti-6Al-4V-2Cr中加入稀土Nd元素并进行热处理,运用光学; 显微镜、XRD和TEM对其组织和性能进行观察和分析。通过观察发现合金的组织为均匀的alpha +; beta相。运用XRD和TEM观察发现,晶界和晶内均有Nd_2O_3析出物,Nd_2O_3的出现细化了晶粒,提高了形核率,降低了晶粒尺寸。运用精; 密万能试验机进行了力学性能测试,当Nd元素含量为1%时,合金的综合力学性能最好,合金的抗拉强度提高了10.7%,伸长率提高了49.1%。The Ti-6Al-4V-2Cr-xNd alloys were prepared by the cold isostatic; pressing method and powder metallurgy with addition of Nd element,the; effect of composition segregation on properties of the titanium alloy; was eliminated,and microstructure and the properties of the alloy after; heat treatment were studied by means of optical microscope,XRD and TEM.; The results show that the microstructure of the Ti-6Al-4V-2Cr-xNd is; composed of homogeneous alpha + beta phase. Nd_2O_3 precipitates are; observed in the grain boundaries and in the grains by XRD and TEM,and it; can refine the grains,increase the nucleation rate and decrease the; grain size. The optimal mechanical properties of the alloy are obtained; when the Nd content is 1%,and the tensile strength and elongation of the; alloy increase by 10.7% and 49.1%,respectively.军工863项目; 国防973支持项

    Application of the three-dimensional visualization and 3D printing technology for the preoperative evaluation of retroperitoneal tumors

    目的探讨三维重建及3D打印技术在腹膜后肿瘤术前评估中的应用价值。方法回顾性分析2011年3月至2015年9月厦门大学附属成功医院收治的50例腹膜; 后肿瘤病人的临床资料,应用人体器官三维建模软件将病人的CT二维图像重建成三维可视化图像,使用人体器官模型3D打印设备,用两色热塑性塑料打印实物模; 型,通过观察肿瘤与腹腔器官、腹部大血管之间的关系进行术前评估并制定手术方案。实施手术,术中比较重建结果与实际的区别。结果50例病人均成功完成三维; 重建,其中10例病人完成3D打印,三维重建结果及3D打印模型能够立体、清晰和实物化的显示肿瘤解剖学位置及与周围器官、血管的毗邻关系,与术中实际情; 况大致相符。50例病人均顺利施行手术,术中无死亡。结论三维重建及3D打印技术能够对腹膜后肿瘤术前进行精准评估,对指导手术具有一定的应用价值。Objective To explore the application value of three dimensional; visualization and 3D printing technology in the preoperative evaluation; of retroperitoneal tumors. Methods March 2011 to September 2015,the; clinical data of 50 cases of retroperitoneal tumor patients in Xiamen; University Affiliated successful hospital were retrospectively analyzed.; Xiamen strong science and technology R & D human organ 3D modeling; software will reconstruct 2D CT image visualization into a; three-dimensional visualization image,and then it can print physical; model with the company's XMQB-I human organ model 3D printing equipment; and white thermoplastic. At last,preoperative evaluation and surgical; plan can be made through the relationship between the tumor,organs in; abdominal cavity,and abdominal vascular. During the surgery,make a; comparison between the visualization results with the actual condition.; Results 50 cases of patients were successfully completed the; three-dimensional visualization,including 10 cases of patients; completed. Three-dimensional visualization and 3D printing model can; three-dimensionally,clearly show the adjacent relationship between the; anatomical tumour location and surrounding organs and blood; vessels,broadly in line with the actual surgery. 50 surgeries were; successfully performed,no death during perioperative period. Conclusion; Three dimensional visualization and 3D printing technology can make an; accurate preoperative evaluation of retroperitoneal tumors,which has a; certain application value for guiding surgery.国家自然科学基
