    Efficacy of Zaozhu Yinchen Recipe for Treating Non-alcoholic Steatohepatitis and Its Effect on Free Fatty Acid and TNF-α

    目的观察皂术茵陈方治疗非酒精性脂肪性肝炎(non-alcoholic steatohepatitis,NASH)患者的临床疗效,并探讨其对游离脂肪酸(free fatty acid,FFA)及TNF-α的影响。方法采用随机数字表法将120例NASH患者分为治疗组及对照组,每组60例。治疗组予中药皂术茵陈方,每日1剂;对照组予水飞蓟宾葡甲胺片200 mg口服,每日3次,两组均治疗24周。分别于治疗前后通过检测血清ALT、AST活性及TC、TG水平;行腹部CT计算肝脾CT比值;应用肝组织病理检查评价非酒精性脂肪性肝病活动度积分(NAFLD activity score,NAS)及纤维化程度以评价临床疗效;并检测血清FFA及TNF-α含量。结果与本组治疗前比较,两组治疗后血清ALT、AST、TC、TG、FFA、TNF-α水平、肝组织NAS积分及症状体征积分均明显降低,肝纤维化程度明显改善(P<0.05,P<0.01),且治疗组降低更明显(P<0.05)。治疗24周后,治疗组肝纤维化分期总有效率及临床总有效率分别为80.00%(48/60)、85.00%(51/60),明显高于对照组[60.00%(36/60)、73.33%(44/60)],两组比较,差异有统计学意义(P<0.05,P<0.01)。结论皂术茵陈方可改善NASH患者的临床疗效,其作用可能与抑制血清FFA及TNF-α水平有关。Objective To observe the efficacy of Zaozhu Yinchen Recipe(ZZYCR) on non-alcoholic steatohepatitis(NASH) patients, and to explore its effect on serum free fatty acid(FFA) and tumor necrosis factor α(TNF-α).Methods Totally 120 patients with NASH were randomly assigned to the treatment group(60 cases,treated with ZZYCR, one dose per day) and the control group(60 cases, treated with Silibin Meglumine Tablets, 20 mg each time, thrice per day). The therapeutic course for all was 24 weeks. Serum levels of ALT and AST activities, TC and TG levels were detected before and after treatment. Peritoneal CT was performed in all patients, and CT ratios of liver and spleen calculated. NAFLD activity score(NAS) and degree of hepatic fibrosis were assessed using pathological examinations of liver tissue, and efficacy also evaluated. Serum contents of FFA and TNF-αwere also detected. Results Compared with before treatment in the same group, activities of ALT and AST, serum levels of TC, TG, FFA, and TNF-α, NAS, scores of symptoms and signs all obviously decreased, degree of hepatic fibrosis was obviously improved in the two groups(P < 0. 05, P < 0. 01). These changes were more obviously seen in the treatment group(P < 0. 05). After 24-week treatment, the total effective rate and total clinical efficacy were 80. 00%(48/60 cases) and 85. 00%(51/60 cases) in the treatment group, obviously higher than those in the control group [60. 00%(36/60 cases) and 73. 33%(44/60 cases) respectively], with significant difference(P < 0. 05, P < 0. 01).     Clinical Research of Kangshi Kangxian Recipe Treating on Hepatic Fibrosis of Chronic Hepatitis B

    目的观察康氏抗纤方治疗慢性乙型肝炎肝纤维化的临床疗效,探讨其对肝组织N-花生四烯酸氨基乙醇(; arachidonoylethanolamine,AEA) 、2-花生四烯酸甘油( 2-arachidonoylglycerol,2-AG); 、大麻素受体1( cannabinoid receptor 1,CBR1) mRNA、大麻素受体2( cannabinoid receptor; 1,CBR2); mRNA含量变化的影响。方法将110例慢性乙型肝炎肝纤维化患者按随机数字表法分为治疗组和对照组,每组55例。治疗组予中药康氏抗纤方联合恩替卡韦分; 散片治疗,对照组用恩替卡韦分散片单药治疗,两组均治疗48周。判定两组临床疗效,检测血清ALT水平,HBV; DNA、HBsAg、HBsAb定量,肝组织病理、肝硬度变化,肝组织AEA、 2-AG含量和CBR1、CBR2; mRNA表达水平。结果与对照组比较,治疗组的临床总有效率、HBsAg阴转率、肝组织纤维化分期疗效有效率均高于对照组(chi~2 =; 4.453,4.152,6.364,均P < 0.05); 。与本组治疗前比较,治疗后两组患者肝纤维化瞬时弹性测定值、AEA、2-AG含量及CBR1、CBR2 mRNA表达均降低(均P <; 0.05),且治疗组降低更明显(均P < 0.05); 。肝组织病理显示:治疗后治疗组肝小叶结构基本完整,肝细胞轻度水样变性,汇管区纤维组织无增生,局部少量淋巴细胞浸润,炎症及纤维化较对照组明显改善。; 结论康氏抗纤方具有提高慢性乙型肝炎肝纤维化患者的临床总有效率、HBsAg阴转率、肝组织纤维化分期疗效有效率,其作用机制可能与调节内源性大麻素系统; 有关。Objective To observe the clinical effect of Kangshi Kangxian Decoction (; KSKXD) in treating on patients of hepatic fibrosis of chronic hepatitis; B ( CHB),and to explore the mechanisms of KSKXD by regulating the; contents of arachidonoylethanolamine ( AEA),2-arachidonoylglycerol (; 2-AG),cannabinoid receptor 1 ( CBR1 mRNA) and cannabinoid receptor 2 (; CBR2 mRNA) in the liver tissue. Methods Totally 110 patients with; hepatic fibrosis of CHB were assigned to the treatment group and the; control group according to random digit table,55 cases in each group.; The treatment group was treated with Chinese herb KSKXD combined with; Entecavir,and the control group was treated with Entecavir. Both of the; two groups were treated for 48 weeks. The clinical efficacy of the two; groups was determined. The serum ALT levels,HBV DNA, HBsAg and HBsAb; quantification,liver pathological changes,liver stiffness; changes,contents of AEA and 2-AG,mRNA expression of CBR1 and CBR2 in the; liver tissue were detected. Results The total effective rate,HBsAg; negative conversion rate,hepatic fibrosis staging and curative effect; rate of the treatment group were higher than those of the control group; (chi~2 = 4.453,4.152,6.364,allP <0.05). After treatment,the; instantaneous elasticity of liver fibrosis,the contents of AEA and; 2-AG,the mRNA expressions of CBR1 and CBR2 in the liver tissue in the; two groups were decreased compared to those of the same group before; treatment (P < 0.05),and the treatment group had better effect than that; of the control group (P < 0.05). The pathological changes in the; treatment group after treatment indicated the liver lobules; integrity,mild hydropic degeneration of liver cells,periportal fibrous; tissue hyperplasia,local small lymphocytic infiltration,which fibrosis; and inflammation were better than those of the control group.; Conclusions KSKXD was effective in the treatment of hepatic fibrosis of; CHB, which improved total effective rate,HBsAg seroconversion rate and; liver fibrosis stage efficiency.     抗人DR4、DR5单克隆抗体诱导神经胶质瘤细胞凋亡的实验研究

    目的:探讨抗死亡受体DR4、DR5单克隆抗体(mAb)FMU1·4、FMU1·5对3株神经胶质瘤细胞株U343(敏感株)、U138(部分敏感株)、U373(耐受株)的杀伤作用及机制。方法:采用流式细胞术、RT-PCR、免疫细胞化学法、MTT比色法、电泳、DNA倍体分析和Westernblot等方法。结果:DR5在U343高表达;DR4在U373低表达,U343对FMU1·5敏感并呈剂量依赖性、对FMU1·4部分敏感;U138对FMU1·5部分敏感,对FMU1·4耐受;U373对两种抗体耐受。结论:FMU1·4、FMU1·5能不同程度地诱导三株细胞凋亡,其机制与DR4、DR5、细胞色素C和FLIP的表达有关

    Long-term Follow-up Results of 70 Liver Cancer Cases Received Ultrasound Guided Percutaneous PDT

    目的 分析光动力治疗肝癌的临床表现和远期疗效 ,探讨疗效影响因素和治疗适应证 ,为临床推广应用提供参考数据。方法 肝癌患者 70例 ,其中小肝癌 2例 ,大肝癌 6 8例。均经B超、CT定位 ,甲胎蛋白 (AFP)定量 ,病理组织学确诊。治疗前 48h ,患者行血卟啉衍生物 (HpD)皮试 ,阴性者按每公斤体重 5mg静脉给药。治疗时 ,在B超引导下 ,用 18G肝穿针经皮穿刺将石英光纤引导入肝肿瘤内进行光辐照。激光器为氩离子激光泵浦染料激光器系统 ,激光波长 6 30nm ,光纤末端为 1cm长柱状弥散头 ,输出功率 30 0~ 35 0mW ,每一照射点能量累积约 2 2 0J,肿瘤内实行多点照射。治疗后 1周 ,检查血常规、肝功能、AFP及B超 ,一个月后 ,行肝穿活检。多次治疗间歇时间为 1个月。结果  70例肝癌行 170次治疗 ,其中接受 1次治疗 30例 ,多次治疗 40例。 1次治疗组 1年生存率 10 % ,无生存 2年者 ;2次治疗组 1年生存率 5 0 % ,2年以上生存率 8% ;3次以上治疗组 1年生存率 82 % ,2年生存率 5 0 % ,3年生存率 32 % ,其中 3例存活5年以上。全组只有 18%的患者出现治疗后短期内一过性ALT、AST轻度升高 (<5 0U)和TBIL轻度升高 ,余未见明显异常。随访患者 1~ 5年 ,未见肝功能远期受损。病人接受治疗后 ,只有 17%出现低热 3~ 4天 ,30 %的Objective To investigate the long-term follow-up results of ultrasound-guided percutaneous PDT for liver cancer and to evaluate the efficacy and safety of this technique. Methods 70 patients with liver cancers, including 2 small tumors and 68 large tumors, were examined by ultrasound, CT, AFP assay, and diagnosed by histopatholopy of needle biopsy. The patients received injection of hematoporphyrin derivative(HPD) i.v. in a dose of 5 mg/kg body weight 48 h prior to laser irradiation. An argon laser pumped dye laser system was used to give a CW laser beam at 630 nm. The light beam was split and coupled into three optical fibers. An ultrasound-guided percutaneous liver puncture was performed and optical fibers were inserted into the tumors via 18G needles. Efforts were made to ensure that irradiation points were distributed in entire tumor volume and a light output of 300~350 mW for 12 minutes per point was used. The patients were examined with routine blood test, liver functions, AFP assay and USG at one week after treatment. A month later they were reexamined by needle biopsy and received next PDT treatment. Results Total 170 treatments were performed in the 70 patients. Among them 30 cases were treated for only once, 12 cases twice, 12 cases 3 times, and 16 cases 4 times or more. All 70 cases were followed up for 1-5 years. The results showed that multiple treatment is more effective than single treatment. The 1-, 2-and 3-years survival rate was 10%, 0, 0 in the group by single treatment, respectively, 50%, 8% and 8% in the group treated twice, and 82%, 50% and 30% in the group treated by 3 or more time, respectively. Three patients from the last group have already been surviving for longer than 5 years. Slight increase of ALT, AST and total bilirubin occurred in only 18%, and mild fever occurred in 17% of the 70 cases for 3-4 days shortly after PDT treatment. No severe complications due to PDT such as severe hemorrhage after liver puncture, hepatic failure, biliary peritonitis or long-lasting liver dysfunctions were observed in our group. Conclusions Photodynamic therapy is effective and safe for the treatment of liver cancers including those following hepatic resection or those failed after embolic chemotherapy by hepatic artery infusion. However, we would suggest that single PDT treatment is not enough and repeated PDT treatments are recommended in order to obtain a significant therapeutic effect to patients with large liver cancers.福建省“95”重点科技项

    Mechanism of geniposide in improving free fatty acid metabolism in rats with non-alcoholic fatty liver disease

    观察栀子苷改善非酒精性脂肪性肝病的效果,从游离脂肪酸探讨栀子苷调节非酒精性脂肪性肝病的作用机制。健康雄性Wistar大鼠40只,随机分为4组:正常组、模型组、栀子苷组和血脂康组,每组大鼠10只,正常组大鼠给予正常大鼠饲料喂养,其余3组大鼠均采用高脂饲料喂养以诱导非酒精性脂肪性肝病,造模时间为8周,从第5周起至第8周末,栀子苷组和血脂康组分别灌服相应的药物。记录大鼠体重、肝湿重、脂肪质量;用相应方法检测肝组织TG,FFA,FAS,AMPK,ACCase及Malonyl-Co A含量,血清CHO,LDL-C的含量,血清AST,ALT的活性;观察肝脏组织肉眼及病理变化(HE染色法)。结果显示,与正常组相比,模型组大鼠的体重;肝湿重;脂肪重;血清CHO,LDL-C,ALT,AST;肝组织TG,FFA,FAS,ACCase及Malonyl-Co A含量皆显著升高(P<0.01);肝组织AMPK活性显著降低(P<0.01),肝组织肉眼外观及病理切片脂肪变性明显,并出现炎症损伤;与模型组相比,栀子苷组大鼠体重、脂肪质量、肝组织FFA含量、血清ALT,AST活性均显著降低(P<0.01),肝湿重,肝组织TG,FAS,ACCase及Malonyl-Co A含量明显降低(P<0.05),肝组织AMPK活性明显增多(P<0.05),肝脏肉眼外观及病理学表现均有所改善;与模型组相比,血脂康组大鼠的肝湿重、脂肪质量、肝组织TG,FFA和血清LDL-C水平明显降低(P<0.05);与血脂康组相比,栀子苷组大鼠的体重、脂肪质量、肝组织FFA含量均显著降低(P<0.01),其他方面无明显差异。结果表明,栀子苷具有显著的改善高脂饮食诱导的大鼠非酒精性脂肪性肝病的药理效应;其改善大鼠非酒精性脂肪性肝病的游离脂肪酸代谢是通过调节"AMPK-ACCase-Malonyl-Co A-FFA"轴来实现的。To observe the effect of geniposide on non-alcoholic fatty liver disease( NAFLD),and discuss the mechanism of geniposide for NAFLD from the aspect of free fatty acid,forty healthy Wistar male rats were randomly divided into normal group,model group,geniposide and Xuezhikang group. The rats in normal group were fed with normal diets,and the rats in other 3 groups were given with high-fat diet for 8 weeks to induce the NAFLD models. From the week 5 to end of week 8,the rats in geniposide and Xuezhikang group were intervened with corresponding medicines. The body weight,liver wet weight,and fat weight of the rats were recorded.Visual and pathological changes in hepatic tissues were observed with HE staining. The contents of TG,FFA,FAS,AMPK,ACCase and Malonyl-Co A in hepatic tissue,contents of CHO and LDL-C in serum and activities of AST and ALT in serum were detected by using corresponding methods. The results showed that the body weight,liver wet weight,and fat weight of the rats,CHO,LDL-C,ALT and AST levels in serum,TG,FFA,FAS,ACCase and Malonyl-Co A levels in hepatic tissues of the rats in model group were significantly higher than those in normal group( P < 0. 01),while AMPK activity was significantly lower than that of the normal group( P < 0. 01),with obvious visual and pathological steatosis in hepatic tissues,and inflammatory injury occurred in model group. Compared with the model group,body weight of the rat,fat weight,levels of FFA in hepatic tissues,ALT and AST activities in serum,liver wet weight,TG,FAS,ACCase and Malonyl-Co A levels were significantly decreased in geniposide group( P < 0. 01),while the AMPK activity in hepatic tissues was significantly increased( P < 0. 05),with improvement in visual and pathological performance.Compared with the model group,liver wet weight,fat weight,TG and FFA levels in hepatic tissues,and LDL-C level in serum were significantly decreased in Xuezhikang group( P < 0. 05). Compared with Xuezhikang group,the body weight of rat,fat weight and FFA level in hepatic tissues were significantly lower in geniposide group( P < 0. 01),but with no significant difference in other aspects.     石英玻璃微球吸收光谱上的结构共振

    通过CO2激光器熔融不同直径的熔锥光纤以得到相应直径的石英玻璃微球,利用此微球和熔锥光纤,构造了球微腔耦合系统。实验中利用光腰直径为3.1μm的熔锥光纤与直径为143.1μm的石英玻璃微球进行耦合,通过最大分辨力为1pm的可调谐半导体激光器对该耦合系统进行光谱扫描,发现石英玻璃微球的吸收光谱中出现分立的结构共振峰。利用光学微球腔理论讨论了石英玻璃微球吸收光谱中的结构共振,并用米氏散射理论公式对一阶TE模共振峰的位置以及它们的间隔进行了计算,共振峰位置实验结果与理论结果的误差仅为0.03%,表明实验与计算结果相符

    Ultrasound Guided Percutaneous PDT For Advanced Liver Cancer A Report of 30 Cases

    为探讨治疗中晚期肝癌的理想方法,扩大和发展光动力疗法,自1994年6月起,我们应用光动力疗法(PdT)治疗中晚期肝癌30例,其中肝细胞癌28例、腺癌2例。肿瘤直径7~10CM的13例、10CM以上至16CM的17例。伴肺转移5例、骨转移5例、门静脉或腔静脉浸润4例。首次治疗19例、其他方法治疗无效或复发者11例。本研究采用氩激光泵浦染料激光器系统,获得630nM波长的连续波治疗光。光敏剂选用血卟啉衍生物(HPd)。治疗时通过b超引导经皮肝穿,将光纤导入肿瘤组织中布点照射。治疗后未见肝功能明显损伤,未出现肝穿出血、腹膜炎等严重并发症,没有一个月内病情恶化者。有14例患者只作一次治疗,其中5例出院后失访;另9例肿瘤部分缓解率为22%。16例患者接受二、三次治疗,肿瘤部分缓解率达到62%。随访病例中有8例已存活1年以上。patients with advanced liver cancer were treated by interstitial photodynamic therapy (PDT).These included 28 hepatocellular carcinoma and two adenocarcinoma, 19 primary tumors and 11 recurred Following other treatments.The diameter of tumors were 7 10 cm in 13 cases and 10 16 cm in 17 cases.In this study, an argon laser pumped dye laser system was used to give a CW laser beam at 630 nm which was split and coupled into three optical Fibers.The patients received injection of photosensitizer of hematoporphyrin derivative (HpD) i.v.at a dose of 5 mg/kg body weight 48 h prior to laser irradiation.Then the Fibers were inserted into tumor by ultrasonic visualization guided percutaneous liver puncture.The inserted irradiation points were spaced in entire tumor volume and the tumor was irradiated with a light output power of 300 mW For 12 minuts per point.Totally 52 treatments were perFormed in 30 patients.Among them, 14 cases were treated by only one time and 16 cases 2 or 3 times.The Follow up were carried out in 25 cases For 3 18 months.The results show that signiFicant remission was 22% (2/9) in those patients by only one treatment and 62% (10/16) in those by 2 or 3 treatments.The reduction rate of tumor size was over 90% in Five out of six cases aFter 3 treatments.8 patients have been surviving over one year.No obvious change was Found in all patients examined by liver Function test, renal Function test and blood routine examination.The level of AFP depicted a descending tendency aFter PDT.Our Findings indicate that PDT is eFFective and saFe For the treatment of large liver cancers including those recurred Following hepatic resection and those Failed in hepatic artery inFusion embolic chemotherapy.福建省“八五”攻关资

    Developing fast laboratory screening platform for sulfate dioxide in food by surface-enhanced Raman spectroscopy

    基于表面增强拉曼光谱(SERS)的指纹图谱的高能量分辨率,以位于~630; cm~(-1)亚硫酸根的特征峰为定性和定量依据,本文开展了食品中滥用和非法添加二氧化硫的直接检测研究:一种检测流程实现了不同食品基质中二氧化硫的; 高灵敏检测,最低检出浓度达1 mg; kg~(-1)的水平.针对实际样本的传统蒸馏法前处理流程的一些不足以及SERS检测的指纹图谱优势,本文进行了三方面显著改进:; (1)碳酸钙和沸石取代氮气以提高二氧化硫的挥发提取效率; (2)草酸取代盐酸酸化蒸馏溶液,以有效避免挥发性强酸对操作环境的危害;; (3)氢氧化钠溶液代替乙酸铅溶液作为吸收液,以防止二次污染.环境友好、灵敏度高和不受基质干扰等特点使得本方法有望取代经典蒸馏法和比色法,用于食品; 中非法添加二氧化硫类添加剂的快速高效筛查.The direct detection of sulphur dioxide has been realized in various; food matrixes with high sensitivity and high selectivity, on the basis; of the integration of the high energy resolution of the finger-print; spectrum of surface-enhanced Raman spectroscopy and the universal but; easy-on-going pretreatment procedure. The characteristic peak of sulphur; dioxide at ~630 cm~(-1) was applied as the qualitative and quantitative; standard, which displayed a lowest detectable concentration at the 1 mg; kg~(-1) level for the spiked food samples. The key point of the high; sensitivity and selectivity is the effective pretreatment born out of; the standard distillation one, which has been improved in the three; parts. (1) Using CaCO_3 and Zeolite instead of N_2 gas as the bubbling; reagent. (2) Using oxalic acid as the acidic distillation solution to; eliminate the hazards from acid volatilization, such as hydrochloric; acid. (3) Using diluted sodium hydroxide solution as the absorption; reagent instead of lead acetate solution to avoid the secondary; pollution.     目的:探讨AnTI-dr5 MAb诱导佐剂型关节炎(AdJuVAnT ArTHrITIS,AA)大鼠滑膜细胞凋亡作用及其可能机理。方法:注射弗氏完全佐剂建立AA大鼠模型,分离并体外培养大鼠滑膜细胞。MTT检测、dnA倍体分析及流式细胞术分析AnTI-dr5 MAb对大鼠滑膜细胞的凋亡作用,WESTErn blOT检测大鼠滑膜细胞CASPASE-3、bCl-2蛋白的表达,CASPASE抑制剂实验分析AnTI-dr5 MAb对大鼠滑膜细胞凋亡作用的机制。结果:MTT实验表明AnTI-dr5 MAb能抑制大鼠滑膜细胞的增殖,流式细胞术分析这种作用模式为细胞凋亡。AnTI-dr5 MAb处理后的大鼠滑膜细胞中CASPASE-3蛋白水平上升,bCl-2蛋白水平下调。CAS-PASE抑制剂实验证明AnTI-dr5 MAb诱导大鼠滑膜细胞的凋亡是通过CASPASE途径实现的。结论:AnTI-dr5 MAb诱导大鼠滑膜细胞的凋亡,这种作用与滑膜细胞表面CASPASE-3、bCl-2蛋白表达相关。Objective:To study the effects of Anti-DR5 mAb on inducing synovial cells of adjuvant arthritis(AA) rats and its mechanisms.Methods:AA was induced by CFA in rats.The synovial cells of rats were separated and cultured in vitro system.The apoptosis induced by anti-DR5 mAb on synovial cells was measured by MTT analysis,DNA fragmentation and flow cytometry.Caspase-3 and Bcl-2 expression in synovial cells were detected by Western blot.The involvement of the apoptotic pathway was further proved by a caspase inhibition assay.Results:MTT result showed that Anti-DR5 mAb could inhibit synovial cells growth.And flow cytometry suggested that the cell death mode was apoptosis.The protein level of caspase-3 in synovial cells treated with anti-DR5 mAb was raised,while Bcl-2 level declined.When the caspase inhibitor was added to the synovial cells treated with anti-DR5 mAb,it was showed in a dose-dependence inhibition effect,indicating that anti-DR5 mAb inducing apoptosis might be through the caspase pathway.Conclusion:Anti-DR5 mAb could induce synovial cell apoptosis through caspase pathway.And this effect may be related to the protein level of Caspase-3 and Bcl-2.福建省自然科学基金项目(C0710046
