    Erken dönem mikozis fungoides lezyonlarının histopatolojik tanısının konulması ayırıcı tanı zorlukları ve çok benzer morfolojik görünümler nedeni ile oldukça zordur. Tanı koymayı kolaylaştıracak ve hastaların yıllar süren takiplerinde yol göstermeye yardımcı olacak immünhistokimyasal belirteçler araştırılmaktadır. Çalışmamızda GÜTF Tıbbi Patoloji AD de 2008-2016 yılları arasında tanı almış 119 MF hastasının hazır parafin blokları ve boyalı lamları üzerinden yapılan incelemelerle hastaların tanısını koymaya yardımcı olacak TOX antikoru ve prognoz öngörüsünde bulunabilecek CD30, PD-1 ve CXCR-4 antikorları çalışılmıştır. Elde edilen sonuçlara göre CD30 ile nüks ve sağ kalım arasında istatistiksel olarak anlamlı fark mevcut değildir. PD-1 ile %10 un üzerinde boyanma gösteren biyopsiye sahip hastalarda, PD-1 negatif olgulara göre nüks açısından anlamlı fark saptanmamakla birlikte; PD-1 ekspresyonu olan olgularda sağ kalımın negatif olgulara göre daha düşük olduğu görülmüştür. CXCR-4 ve TOX antikorları ile biyopsilerde immünhistokimyasal olarak sağlıklı boyanma elde edilememesi nedeni ile çalışma dışı bırakılmıştır. 119 olgudan oluşan bu çalışmada immünhistokimyasal olarak CD30 ekspresyonunun nüks ve sağ kalıma etkisi gösterilememekle birlikte, CD30 ve PD-1 in klinik olarak agresif seyreden MF olgularında klinik takibe yol gösterebileceği düşünülmektedir.Due to histologic appearances that can be very similar with benign inflammatory skin diseases the diagnosis of early stage mycosis fungoides can be very painful. There has been always a quest for immunohistochemical markers that would help to diagnose and to foresee the long-term outcome of the patient. In our study we applied the CD30, TOX, PD-1, and CXCR-4 immunohistochemical markers to paraffin tissue blocks of 119 patients that were collected at the host institute between the years 2008 and 2016. The results showed that there was no statistically significant difference between CD30 positivity over 10% and a tendency for recurrence and survival. PD-1 expression over 10% had no statistically significant difference for tendency of recurrence but on the other hand statistically significant results for poor survival has been shown. It was unable to stain properly the CXCR-4 and TOX antibodies immunohistochemically so they were excluded from the research. Although this research which has 119 patients was unable to show the effects of CD30 expression for recurrence and survival; CD30 and PD-1 are markers that can make guidance for the clinical follow-up of aggressive MF cases

    Recurrent Sertoli-Leydig cell tumor of ovary in 8-year-old girl

    Sertoli-Leydig cell tumors are rare sex cord-stromal neoplasms that account for <0.2% of ovarian tumors. These tumors with a retiform pattern pose difficult diagnostic problems, with the majority of being misinterpreted as serous papillary cystadenocarcinoma and endodermal sinus tumor. We report an 8-year-old female patient presented to our institution with a huge mass and pain in the lower abdomen and recurrence in the 10th months following the first operation. Only four cases of Sertoli-Leydig cell tumors have been reported under age of the eight years in the literature so far. It is difficult to define the stage and the morphology of Sertoli-Leydig cell tumors with retiform pattern in children and chemotherapy or radiotherapy administration is contraversial. However, fertility sparing surgeries should be considered as a first treatment choice on the time of the diagnosis and the recurrence

    Extended left hepatectomy for intrahepatic cholangiocarcinoma: hepatic vein reconstruction with in-situ hypothermic perfusion and extracorporeal membrane oxygenation

    Abstract Background Liver resection for intrahepatic cholangiocarcinoma (ICC) with invasion of the inferior vena cava (IVC) and hepatic veins (HV) is a challenging procedure. Case presentation We report a case of a 63-year-old woman with a 6-cm, centrally located liver mass. Her biochemistry results were normal except for a Ca19–9 level of 1199 U/ml. The liver biopsy was consistent with ICC and 60% macrosteatosis. Abdominal CT scans revealed a large central mass invading the left HV, middle HV and right HV, infringing on their junction with the vena cava. An operation was planned using a 3-dimensional (3D) computer simulation model using dedicated software. We also describe a novel veno-portal-venous extracorporeal membrane oxygenation (VPV-ECMO) support with in-situ hypothermic perfusion (IHP) during this procedure. We aimed to perform an extended left hepatectomy and reconstruct 3 right HV orifices with an interposition jump graft to the IVC with total vascular exclusion (TVE) and IHP A supplemental video describing the preoperative planning, the operative procedure with the postoperative follow-up in detail is presented. After the patient was discharged, she developed a hepatic venous outflow obstruction 3 months postoperatively, which was effectively managed with hepatic venous stenting by interventional radiology. She is currently symptom free and without tumour recurrence at the 1-year follow-up. Conclusions This report demonstrates that extended left hepatectomy for IHC with IHP and VPV-ECMO is safe and feasible under the supervision of a highly experienced team

    The Histopathological Effect of Aloe Vera on the Wound Healing Process in a Surgically Created Tubularized Incised Plate Urethroplasty Model on Rats

    Background Hypospadias is a common congenital anomaly which is determined as an abnormal urethral opening on the ventral face of penis. The purpose of this rat model study was to research the effect of topically applied Aloe vera after a tubularized incised plate urethroplasty (TIPU). Methods The TIPU model was applied to male Wistar albino rats. A total of 30 rats were randomly grouped into 3 groups of 10. Group I was assigned as the control group, treated with 0.9% saline only twice a day for 15 days. Group II received topical Aloe vera gel once a day and Group III received Aloe vera gel twice a day. Spongiofibrosis was graded as 0: none, 1+:≤10% tissues involved, 2+:10%-49% tissues involved, 3+: ≥ 50% tissues involved. Results A higher degree of fibrosis and inflammation was determined in the Group I subjects than in Groups II and III. Fibrosis of grade 3+ was observed in 33% of the control group and not in any of the two Aloe groups (p = 0.043). Inflammation of grade 3+ was seen in 66.7% of the control group, in 10% of Group II, and in 33% of Group III (p = 0.02). Conclusions The topical application of Aloe vera to a surgically created tubularized incised plate urethroplasty model decreased inflammation and fibrosis that may affect the success rates of this operation

    Impact of the International Nosocomial Infection Control Consortium (INICC) Multidimensional Hand Hygiene Approach, over 8 years, in 11 cities of Turkey

    AIMS: To evaluate the effectiveness of the International Nosocomial Infection Control Consortium (INICC) Multidimensional Hand Hygiene Approach in Turkey and analyse predictors of poor hand hygiene compliance. DESIGN: An observational, prospective, interventional, before-and-after study was conducted from August 2003 to August 2011 in 12 intensive care units (ICU) of 12 hospitals in 11 cities. The study was divided into a baseline and a follow-up period and included random 30-minute observations for hand hygiene compliance in ICU. The hand hygiene approach included administrative support, supplies availability, education and training, reminders in the workplace, process surveillance, and performance feedback. RESULTS: We observed 21,145 opportunities for hand hygiene. Overall hand hygiene compliance increased from 28.8% to 91% (95% CI 87.6–93.0, p 0.0001). Multivariate and univariate analyses showed that several variables were significantly associated with poor hand hygiene compliance: males vs. females (39% vs. 48%; 95% CI 0.79–0.84, p 0.0001), ancillary staff vs. physicians (35% vs. 46%, 95% CI 0.73–0.78, p 0.0001), and adult vs. pediatric ICUs (42% vs. 74%, 95% CI 0.54–0.60, p 0.0001). CONCLUSIONS: Adherence to hand hygiene was significantly increased with the INICC Hand Hygiene Approach. Specific programmes should be directed to improve hand hygiene in variables found to be predictors of poor hand hygiene compliance

    Withdrawal of Staphylococcus aureus from intensive care units in Turkey

    WOS: 000326241700021PubMed ID: 23663858Background: In the past, Staphylococcus aureus infections have displayed various patterns of epidemiologic curves in hospitals, particularly in intensive care units (ICUs). This study aimed to characterize the current trend in a nationwide survey of ICUs in Turkey. Methods: A total of 88 ICUs from 36 Turkish tertiary hospitals were included in this retrospective study, which was performed during the first 3 months of both 2008 (period [P] 1) and 2011 (P2). A P value <=.01 was considered significant. Results: Although overall rates of hospital-acquired infection (HAI) and device-associated infection densities were similar in P1 and P2, the densities of HAIs due to S aureus and methicillin-resistant S aureus (MRSA) were significantly lower in P2 (P < .0001). However, the proportion of HAIs due to Acinetobacter was significantly higher in P2 (P < .0001). Conclusions: The incidence of S aureus infections is declining rapidly in Turkish ICUs, with potential impacts on empirical treatment strategies in these ICUs.
