    The Structure and Function of the Retina in Multiple Sclerosis

    Background: Multiple sclerosis (MS) is a complex heterogenous autoimmune inflammatory disease with a prolonged and variable time course. The visual system is frequently implicated, either as the presenting symptom, or, with advancement of the disease. This has been documented in the literature with changes in visual acuity (VA) that are accompanied by functional changes in the optic nerve, measured with the visual evoked potential (VEP) and possible retrograde degeneration involving the retinal ganglion cells in the retina, measured with the pattern reversal electroretinogram (PERG). However, inflammatory episodes may be clinical or subclinical in nature and may go unrecognised. Originating from the same embryological origins, the effect of inflammation in MS on the on the retina is less well known. The research hypothesis was that there is a measurable difference in the function of retinal cells in patients with newly diagnosed multiple sclerosis, suggestive of inflammatory retinopathy compared to healthy controls. The overall aim was to investigate any differences in the electrophysiological function of the visual pathway of patients newly diagnosed with MS compared to healthy controls. Methods: The visual system is explored with clinical (VA), electrophysiology (VEP and electroretinography (ERG – pattern and flash) and structural (OCT) measures, in patients presenting with symptoms suggestive of MS to a specialist service. This prospective case control study investigates the visual pathway at the earliest stage of the disease to look for differences in structure and function between patients and healthy volunteers that might serve as a biomarker in the future. Results: There were a number of variables that were significantly different between the two groups, logistic regression analysis found that VA (p 0.038) and VEP P100 peak-time (p 0.014) from the right eye as significant. Dividing the participants by prolongation of the VEP P100 peak-time as defined in clinical practice, found a number of ERG amplitude variables as well as VA that were consistently different between the groups regardless of symptoms. Conclusion: The study confirms optic nerve involvement in MS with VEP and VA abnormalities consistent with the literature in this cohort. Additionally, VA and some ERG amplitude variables were significantly reduced in participants with MS, when grouped according to VEP P100 peak-time, suggesting inner and outer retinal changes. Further work would be required to confirm these findings. No OCT structural changes were found in any of the analysis that included the macula thickness, ganglion cell layer or retinal nerve fibre layer.     Safe passage for attachment systems:Can attachment security at international schools be measured, and is it at risk?

    Relocations challenge attachment networks. Regardless of whether a person moves or is moved away from, relocation produces separation and loss. When such losses are repeatedly experienced without being adequately processed, a defensive shutting down of the attachment system could result, particularly when such experiences occur during or across the developmental years. At schools with substantial turnover, this possibility could be shaping youth in ways that compromise attachment security and young people’s willingness or ability to develop and maintain deep long-term relationships. Given the well-documented associations between attachment security, social support, and long-term physical and mental health, the hypothesis that mobility could erode attachment and relational health warrants exploration. International schools are logical settings to test such a hypothesis, given their frequently high turnover without confounding factors (e.g. war trauma or refugee experiences). In addition, repeated experiences of separation and loss in international school settings would seem likely to create mental associations for the young people involved regarding how they and others tend to respond to such situations in such settings, raising the possibility that people at such schools, or even the school itself, could collectively be represented as an attachment figure. Questions like these have received scant attention in the literature. They warrant consideration because of their potential to shape young people’s most general convictions regarding attachment, which could, in turn, have implications for young people’s ability to experience meaning in their lives

    Corporate social responsibility and climate change: the case of oil and gas industry of Nigeria

    The thesis contributes to the literature on social accounting, accountability, and reporting by providing insights into the perspectives of multinational and indigenous oil and gas corporations in Nigeria regarding climate change, particularly the link between gas flaring and its impact on the environment and local communities. The use of interpretive research methods and the application of climate justice theory provide a unique theoretical lens to challenge existing policies and practices and engage with stakeholders holistically and transparently. The study highlights the inadequacy of current corporate social and environmental responsibility (CSER) practices in addressing climate change challenges and the need for corporations to adopt an ethics or climate justice approach in their actions and reporting, supported by policy instruments to ensure compliance. Empirical evidence shows that corporations in this industry ride on increasing demand for fossil fuels, lax regulation and monitoring of the industry, vulnerability and powerlessness of local communities to take undue advantage of the communities. However, they use some CSR programmes, remote from real solutions to gas flaring or climate change challenges, to pacify community stakeholders and sustain or improve corporate legitimacy. An intentional commitment by the corporations, including imbibing ethics or climate justice lens, and backed by strict and mandatory policy instruments is essential for addressing gas-flaring-induced climate challenges

    Effects of municipal smoke-free ordinances on secondhand smoke exposure in the Republic of Korea

    ObjectiveTo reduce premature deaths due to secondhand smoke (SHS) exposure among non-smokers, the Republic of Korea (ROK) adopted changes to the National Health Promotion Act, which allowed local governments to enact municipal ordinances to strengthen their authority to designate smoke-free areas and levy penalty fines. In this study, we examined national trends in SHS exposure after the introduction of these municipal ordinances at the city level in 2010.MethodsWe used interrupted time series analysis to assess whether the trends of SHS exposure in the workplace and at home, and the primary cigarette smoking rate changed following the policy adjustment in the national legislation in ROK. Population-standardized data for selected variables were retrieved from a nationally representative survey dataset and used to study the policy action’s effectiveness.ResultsFollowing the change in the legislation, SHS exposure in the workplace reversed course from an increasing (18% per year) trend prior to the introduction of these smoke-free ordinances to a decreasing (−10% per year) trend after adoption and enforcement of these laws (β2 = 0.18, p-value = 0.07; β3 = −0.10, p-value = 0.02). SHS exposure at home (β2 = 0.10, p-value = 0.09; β3 = −0.03, p-value = 0.14) and the primary cigarette smoking rate (β2 = 0.03, p-value = 0.10; β3 = 0.008, p-value = 0.15) showed no significant changes in the sampled period. Although analyses stratified by sex showed that the allowance of municipal ordinances resulted in reduced SHS exposure in the workplace for both males and females, they did not affect the primary cigarette smoking rate as much, especially among females.ConclusionStrengthening the role of local governments by giving them the authority to enact and enforce penalties on SHS exposure violation helped ROK to reduce SHS exposure in the workplace. However, smoking behaviors and related activities seemed to shift to less restrictive areas such as on the streets and in apartment hallways, negating some of the effects due to these ordinances. Future studies should investigate how smoke-free policies beyond public places can further reduce the SHS exposure in ROK
