    Shape-correlated statistical modeling and analysis for respiratory motion estimation

    Respiratory motion challenges image-guided radiation therapy (IGRT) with location uncertainties of important anatomical structures in the thorax. Effective and accurate respiration estimation is crucial to account for the motion effects on the radiation dose to tumors and organs at risk. Moreover, serious image artifacts present in treatment-guidance images such 4D cone-beam CT cause difficulties in identifying spatial variations. Commonly used non-linear dense image matching methods easily fail in regions where artifacts interfere. Learning-based linear motion modeling techniques have the advantage of incorporating prior knowledge for robust motion estimation. In this research shape-correlation deformation statistics (SCDS) capture strong correlations between the shape of the lung and the dense deformation field under breathing. Dimension reduction and linear regression techniques are used to extract the correlation statistics. Based on the assumption that the deformation correlations are consistent between planning and treatment time, patient-specific SCDS trained from a 4D planning image sequence is used to predict the respiratory motion in the patient's artifact-laden 4D treatment image sequence. Furthermore, a prediction-driven atlas formation method is developed to weaken the consistency assumption, by integrating intensity information from the target images and the SCDS predictions into a common optimization framework. The strategy of balancing between the prediction constraints and the intensity-matching forces makes the method less sensitive to variation in the correlation and utilizes intensity information besides the lung boundaries. This strategy thus provides improved motion estimation accuracy and robustness. The SCDS-based methods are shown to be effective in modeling and estimating respiratory motion in lung, with evaluations and comparisons carried out on both simulated images and patient images

    A biomechanical approach for real-time tracking of lung tumors during External Beam Radiation Therapy (EBRT)

    Lung cancer is the most common cause of cancer related death in both men and women. Radiation therapy is widely used for lung cancer treatment. However, this method can be challenging due to respiratory motion. Motion modeling is a popular method for respiratory motion compensation, while biomechanics-based motion models are believed to be more robust and accurate as they are based on the physics of motion. In this study, we aim to develop a biomechanics-based lung tumor tracking algorithm which can be used during External Beam Radiation Therapy (EBRT). An accelerated lung biomechanical model can be used during EBRT only if its boundary conditions (BCs) are defined in a way that they can be updated in real-time. As such, we have developed a lung finite element (FE) model in conjunction with a Neural Networks (NNs) based method for predicting the BCs of the lung model from chest surface motion data. To develop the lung FE model for tumor motion prediction, thoracic 4D CT images of lung cancer patients were processed to capture the lung and diaphragm geometry, trans-pulmonary pressure, and diaphragm motion. Next, the chest surface motion was obtained through tracking the motion of the ribcage in 4D CT images. This was performed to simulate surface motion data that can be acquired using optical tracking systems. Finally, two feedforward NNs were developed, one for estimating the trans-pulmonary pressure and another for estimating the diaphragm motion from chest surface motion data. The algorithm development consists of four steps of: 1) Automatic segmentation of the lungs and diaphragm, 2) diaphragm motion modelling using Principal Component Analysis (PCA), 3) Developing the lung FE model, and 4) Using two NNs to estimate the trans-pulmonary pressure values and diaphragm motion from chest surface motion data. The results indicate that the Dice similarity coefficient between actual and simulated tumor volumes ranges from 0.76±0.04 to 0.91±0.01, which is favorable. As such, real-time lung tumor tracking during EBRT using the proposed algorithm is feasible. Hence, further clinical studies involving lung cancer patients to assess the algorithm performance are justified

    Heterogeneous volumetric data mapping and its medical applications

    With the advance of data acquisition techniques, massive solid geometries are being collected routinely in scientific tasks, these complex and unstructured data need to be effectively correlated for various processing and analysis. Volumetric mapping solves bijective low-distortion correspondence between/among 3D geometric data, and can serve as an important preprocessing step in many tasks in compute-aided design and analysis, industrial manufacturing, medical image analysis, to name a few. This dissertation studied two important volumetric mapping problems: the mapping of heterogeneous volumes (with nonuniform inner structures/layers) and the mapping of sequential dynamic volumes. To effectively handle heterogeneous volumes, first, we studied the feature-aligned harmonic volumetric mapping. Compared to previous harmonic mapping, it supports the point, curve, and iso-surface alignment, which are important low-dimensional structures in heterogeneous volumetric data. Second, we proposed a biharmonic model for volumetric mapping. Unlike the conventional harmonic volumetric mapping that only supports positional continuity on the boundary, this new model allows us to have higher order continuity C1C^1 along the boundary surface. This suggests a potential model to solve the volumetric mapping of complex and big geometries through divide-and-conquer. We also studied the medical applications of our volumetric mapping in lung tumor respiratory motion modeling. We were building an effective digital platform for lung tumor radiotherapy based on effective volumetric CT/MRI image matching and analysis. We developed and integrated in this platform a set of geometric/image processing techniques including advanced image segmentation, finite element meshing, volumetric registration and interpolation. The lung organ/tumor and surrounding tissues are treated as a heterogeneous region and a dynamic 4D registration framework is developed for lung tumor motion modeling and tracking. Compared to the previous 3D pairwise registration, our new 4D parameterization model leads to a significantly improved registration accuracy.     Biomechanical Modeling for Lung Tumor Motion Prediction during Brachytherapy and Radiotherapy

    A novel technique is proposed to develop a biomechanical model for estimating lung’s tumor position as a function of respiration cycle time. Continuous tumor motion is a major challenge in lung cancer treatment techniques where the tumor needs to be targeted; e.g. in external beam radiotherapy and brachytherapy. If not accounted for, this motion leads to areas of radiation over and/or under dosage for normal tissue and tumors. In this thesis, biomechanical models were developed for lung tumor motion predication in two distinct cases of lung brachytherapy and lung external beam radiotherapy. The lung and other relevant surrounding organs geometries, loading, boundary conditions and mechanical properties were considered and incorporated properly for each case. While using material model with constant incompressibility is sufficient to model the lung tissue in the brachytherapy case, in external beam radiation therapy the tissue incompressibility varies significantly due to normal breathing. One of the main issues tackled in this research is characterizing lung tissue incompressibility variations and measuring its corresponding parameters as a function of respiration cycle time. Results obtained from an ex-vivo porcine deflated lung indicated feasibility and reliability of using the developed biomechanical model to predict tumor motion during brachytherapy. For external beam radiotherapy, in-silico studies indicated very significant impact of considering the lung tissue incompressibility on the accuracy of predicting tumor motion. Furthermore, ex-vivo porcine lung experiments demonstrated the capability and reliability of the proposed approach for predicting tumor motion as a function of cyclic time. As such, the proposed models have a good potential to be incorporated effectively in computer assisted lung radiotherapy treatment systems

    Improving the Accuracy of CT-derived Attenuation Correction in Respiratory-Gated PET/CT Imaging

    The effect of respiratory motion on attenuation correction in Fludeoxyglucose (18F) positron emission tomography (FDG-PET) was investigated. Improvements to the accuracy of computed tomography (CT) derived attenuation correction were obtained through the alignment of the attenuation map to each emission image in a respiratory gated PET scan. Attenuation misalignment leads to artefacts in the reconstructed PET image and several methods were devised for evaluating the attenuation inaccuracies caused by this. These methods of evaluation were extended to finding the frame in the respiratory gated PET which best matched the CT. This frame was then used as a reference frame in mono-modality compensation for misalignment. Attenuation correction was found to affect the quantification of tumour volumes; thus a regional analysis was used to evaluate the impact of mismatch and the benefits of compensating for misalignment. Deformable image registration was used to compensate for misalignment, however, there were inaccuracies caused by the poor signal-to-noise ratio (SNR) in PET images. Two models were developed that were robust to a poor SNR allowing for the estimation of deformation from very noisy images. Firstly, a cross population model was developed by statistically analysing the respiratory motion in 10 4DCT scans. Secondly, a 1D model of respiration was developed based on the physiological function of respiration. The 1D approach correctly modelled the expansion and contraction of the lungs and the differences in the compressibility of lungs and surrounding tissues. Several additional models were considered but were ruled out based on their poor goodness of fit to 4DCT scans. Approaches to evaluating the developed models were also used to assist with optimising for the most accurate attenuation correction. It was found that the multimodality registration of the CT image to the PET image was the most accurate approach to compensating for attenuation correction mismatch. Mono-modality image registration was found to be the least accurate approach, however, incorporating a motion model improved the accuracy of image registration. The significance of these findings is twofold. Firstly, it was found that motion models are required to improve the accuracy in compensating for attenuation correction mismatch and secondly, a validation method was found for comparing approaches to compensating for attenuation mismatch

    dissertationThe statistical study of anatomy is one of the primary focuses of medical image analysis. It is well-established that the appropriate mathematical settings for such analyses are Riemannian manifolds and Lie group actions. Statistically defined atlases, in which a mean anatomical image is computed from a collection of static three-dimensional (3D) scans, have become commonplace. Within the past few decades, these efforts, which constitute the field of computational anatomy, have seen great success in enabling quantitative analysis. However, most of the analysis within computational anatomy has focused on collections of static images in population studies. The recent emergence of large-scale longitudinal imaging studies and four-dimensional (4D) imaging technology presents new opportunities for studying dynamic anatomical processes such as motion, growth, and degeneration. In order to make use of this new data, it is imperative that computational anatomy be extended with methods for the statistical analysis of longitudinal and dynamic medical imaging. In this dissertation, the deformable template framework is used for the development of 4D statistical shape analysis, with applications in motion analysis for individualized medicine and the study of growth and disease progression. A new method for estimating organ motion directly from raw imaging data is introduced and tested extensively. Polynomial regression, the staple of curve regression in Euclidean spaces, is extended to the setting of Riemannian manifolds. This polynomial regression framework enables rigorous statistical analysis of longitudinal imaging data. Finally, a new diffeomorphic model of irrotational shape change is presented. This new model presents striking practical advantages over standard diffeomorphic methods, while the study of this new space promises to illuminate aspects of the structure of the diffeomorphism group
