    The Evolution of the NASA Commercial Crew Program Mission Assurance Process

    In 2010, the National Aeronautics and Space Administration (NASA) established the Commercial Crew Program (CCP) in order to provide human access to the International Space Station and low Earth orbit via the commercial (non-governmental) sector. A particular challenge to NASA has been how to determine that the Commercial Provider's transportation system complies with programmatic safety requirements. The process used in this determination is the Safety Technical Review Board which reviews and approves provider submitted hazard reports. One significant product of the review is a set of hazard control verifications. In past NASA programs, 100% of these safety critical verifications were typically confirmed by NASA. The traditional Safety and Mission Assurance (S&MA) model does not support the nature of the CCP. To that end, NASA S&MA is implementing a Risk Based Assurance process to determine which hazard control verifications require NASA authentication. Additionally, a Shared Assurance Model is also being developed to efficiently use the available resources to execute the verifications

    Designing Food Safety Management and Halal Assurance Systems in Mozzarella Cheese Production for Small-Medium Food Industry

    Indonesia's small and medium-sized enterprises (SMEs) are having difficulty implementing a food safety management and halal assurance system. This article aims to design a food safety and halal assurance system for Dairy Farm SMEs. This research designed a food system by identifying the application of Good Manufacturing Practices (GMP) and the HACCP to Dairy Farm SMEs based on the survey, in-depth interviews, and document standard review. The food safety system was implemented using HACCP, and six Critical Control Point (CCP) processes were identified, including milking (raw material), storage, pasteurization, curd filtering, and cheese packaging. The halal assurance system is implemented at Dairy Farm SMEs by identifying and improving the company's business processes and the mozzarella cheese production process. In addition, a Standard Operating Procedure (SOP) was developed, including a food safety system and a halal assurance system. The research results can be used wisely by Dairy Farm SMEs to assist in obtaining recommendations from the Food and Drug Supervisory Agency and halal certification

    Food safety and quality are keys to companies' business survival and great effort and resources are devoted to them. The food production chain, from the farms and feed mills to the finished products leaving the processing plants, is subject to independent examination and auditing either under the sector's own assurance schemes under official regulatory inspection and testing programmes with published results. For farmers and the agro-food industry, this means new market opportunities – and continual change. Food safety is an on-going challenge, demanding the best control systems and day-to-day vigilance on farms, in processing plants and throughout the distribution system. In order to enable consumers to make the right choice when buying their food and in order to build up markets for quality products, labelling has to provide all relevant information about the production process. Besides complete information about its ingredients, food labels should bear information about its place of origin and the way in which it was produced

    A methodology for provably stable behaviour-based intelligent control

    This paper presents a design methodology for a class of behaviour-based control systems, arguing its potential for application to safety critical systems. We propose a formal basis for subsumption architecture design based on two extensions to Lyapunov stability theory, the Second Order Stability Theorems, and interpretations of system safety and liveness in Lyapunov stability terms. The subsumption of the new theorems by the classical stability theorems serves as a model of dynamical subsumption, forming the basis of the design methodology. Behaviour-based control also offers the potential for using simple computational mechanisms, which will simplify the safety assurance process. © 2005 Elsevier B.V. All rights reserved

    Hubble Space Telescope: SRM/QA observations and lessons learned

    The Hubble Space Telescope (HST) Optical Systems Board of Investigation was established on July 2, 1990 to review, analyze, and evaluate the facts and circumstances regarding the manufacture, development, and testing of the HST Optical Telescope Assembly (OTA). Specifically, the board was tasked to ascertain what caused the spherical aberration and how it escaped notice until on-orbit operation. The error that caused the on-orbit spherical aberration in the primary mirror was traced to the assembly process of the Reflective Null Corrector, one of the three Null Correctors developed as special test equipment (STE) to measure and test the primary mirror. Therefore, the safety, reliability, maintainability, and quality assurance (SRM&QA) investigation covers the events and the overall product assurance environment during the manufacturing phase of the primary mirror and Null Correctors (from 1978 through 1981). The SRM&QA issues that were identified during the HST investigation are summarized. The crucial product assurance requirements (including nonconformance processing) for the HST are examined. The history of Quality Assurance (QA) practices at Perkin-Elmer (P-E) for the period under investigation are reviewed. The importance of the information management function is discussed relative to data retention/control issues. Metrology and other critical technical issues also are discussed. The SRM&QA lessons learned from the investigation are presented along with specific recommendations. Appendix A provides the MSFC SRM&QA report. Appendix B provides supplemental reference materials. Appendix C presents the findings of the independent optical consultants, Optical Research Associates (ORA). Appendix D provides further details of the fault-tree analysis portion of the investigation process

Topics to be covered include a short history of the CCP; the development of the Programmatic mission assurance requirements; the current safety review process; a description of the RBA process and its products and ending with a description of the Shared Assurance Model

    Considerations about food safety management system's auditing

    A food safety assurance program often used by the food processing industry is the Hazard Analysis Critical Control Point (HACCP) system with the goal to identify the major risks in the food chain to identify' the critical control points. Most agree that comprehensive risk assessment is increasingly important for success (or even survival) of an entity, but how to go about it systematically is open to debate. Through audit, regulators can look at records of what has been happening within the food business, a day-to-day history of compliance, and not just the "snapshot in time" of current inspection methods. This method gives regulators and the community increased confidence that food safety is being maintained by food businesses on a continuing basis. To help the implementation and improvement of an adequate food safety management system the paper synthetize the role of the audit process in the risk management, propose some criteria's which can be used in the preparation of the audit process and illustrate the application of the proposed criteria's and principles with a food safety audit checklist
