FRAM for systemic accident analysis: a matrix representation of functional resonance
Due to the inherent complexity of nowadays Air Traffic Management (ATM) system, standard methods looking at an event as a linear sequence of failures might become inappropriate. For this purpose, adopting a systemic perspective, the Functional Resonance Analysis Method (FRAM) originally developed by Hollnagel, helps identifying non-linear combinations of events and interrelationships.
This paper aims to enhance the strength of FRAM-based accident analyses, discussing the Resilience Analysis Matrix (RAM), a user-friendly tool that supports the analyst during the analysis, in order to reduce the complexity of representation of FRAM. The RAM offers a two dimensional representation which highlights systematically connections among couplings, and thus even highly connected group of couplings. As an illustrative case study, this paper develops a systemic accident analysis for the runway incursion happened in February 1991 at LAX airport, involving SkyWest Flight 5569 and USAir Flight 1493. FRAM confirms itself a powerful method to characterize the variability of the operational scenario, identifying the dynamic couplings with a critical role during the event and helping discussing the systemic effects of variability at different level of analysis
Timed Fault Tree Models of the China Yongwen Railway Accident
Safety is an essential requirement for railway transportation. There are many methods that have been developed to predict, prevent and mitigate accidents in this context. All of these methods have their own purpose and limitations. This paper presents a new useful analysis technique: timed fault tree analysis. This method extends traditional fault tree analysis with temporal events and fault characteristics. Timed Fault Trees (TFTs) can determine which faults need to be eliminated urgently, and it can also provide a safe time window to repair them. They can also be used to determine the time taken for railway maintenance requirements, and thereby improve maintenance efficiency, and reduce risks. In this paper, we present the features and functionality of a railway transportation system based on timed fault tree models. We demonstrate the applicability of our framework via a case study of the China Yongwen line railway accident
Optimal control of the heave motion of marine cable subsea-unit systems
One of the key problems associated with subsea operations involving tethered subsea units is the motions of support vessels on the ocean surface which can be transmitted to the subsea unit through the cable and increase the tension. In this paper, a theoretical approach for heave compensation is developed. After proper modelling of each element of the system, which includes the cable/subsea-unit, the onboard winch, control theory is applied to design an optimal control law. Numerical simulations are carried out, and it is found that the proposed active control scheme appears to be a promising solution to the problem of heave compensation
An Assurance Framework for Independent Co-assurance of Safety and Security
Integrated safety and security assurance for complex systems is difficult for
many technical and socio-technical reasons such as mismatched processes,
inadequate information, differing use of language and philosophies, etc.. Many
co-assurance techniques rely on disregarding some of these challenges in order
to present a unified methodology. Even with this simplification, no methodology
has been widely adopted primarily because this approach is unrealistic when met
with the complexity of real-world system development.
This paper presents an alternate approach by providing a Safety-Security
Assurance Framework (SSAF) based on a core set of assurance principles. This is
done so that safety and security can be co-assured independently, as opposed to
unified co-assurance which has been shown to have significant drawbacks. This
also allows for separate processes and expertise from practitioners in each
domain. With this structure, the focus is shifted from simplified unification
to integration through exchanging the correct information at the right time
using synchronisation activities
Integrated survey for the reconstruction of the Papal Basilica and the Sacred Convent of St. Francis in Assisi, Italy
The Papal Basilica and the Sacred Convent of Saint Francis in Assisi in Italy are characterized by unique and composite particularities that need an exhaustive knowledge of the sites themselves to guarantee visitor's security and safety, considering all the people and personnel normally present in the site, visitors with disabilities and finally the needs for cultural heritage preservation and protection. This aim can be reached using integrated systems and innovative technologies, such as Internet of Everything (IoE), which can connect people, things (smart sensors, devices and actuators; mobile terminals; wearable devices; etc.), data/information/knowledge and processes to reach the wanted objectives. The IoE system must implement and support an Integrated Multidisciplinary Model for Security and Safety Management (IMMSSM) for the specific context, using a multidisciplinary approach. The purpose of the paper is to illustrate the integrated survey for the reconstruction of the considered site that was necessary to obtain all the necessary information to start to set up the considered IMMSSM and the related IoE based technological system
Meta-models for structural reliability and uncertainty quantification
A meta-model (or a surrogate model) is the modern name for what was
traditionally called a response surface. It is intended to mimic the behaviour
of a computational model M (e.g. a finite element model in mechanics) while
being inexpensive to evaluate, in contrast to the original model which may take
hours or even days of computer processing time. In this paper various types of
meta-models that have been used in the last decade in the context of structural
reliability are reviewed. More specifically classical polynomial response
surfaces, polynomial chaos expansions and kriging are addressed. It is shown
how the need for error estimates and adaptivity in their construction has
brought this type of approaches to a high level of efficiency. A new technique
that solves the problem of the potential biasedness in the estimation of a

Continuous maintenance and the future – Foundations and technological challenges
High value and long life products require continuous maintenance throughout their life cycle to achieve required performance with optimum through-life cost. This paper presents foundations and technologies required to offer the maintenance service. Component and system level degradation science, assessment and modelling along with life cycle ‘big data’ analytics are the two most important knowledge and skill base required for the continuous maintenance. Advanced computing and visualisation technologies will improve efficiency of the maintenance and reduce through-life cost of the product. Introducing the STAMP method in road tunnel safety assessment
After the tremendous accidents in European road tunnels over the past decade, many risk assessment methods have been proposed worldwide, most of them based on Quantitative Risk Assessment (QRA). Although QRAs are helpful to address physical aspects and facilities of tunnels, current approaches in the road tunnel field have limitations to model organizational aspects, software behavior and the adaptation of the tunnel system over time. This paper reviews the aforementioned limitations and highlights the need to enhance the safety assessment process of these critical infrastructures with a complementary approach that links the organizational factors to the operational and technical issues, analyze software behavior and models the dynamics of the tunnel system. To achieve this objective, this paper examines the scope for introducing a safety assessment method which is based on the systems thinking paradigm and draws upon the STAMP model. The method proposed is demonstrated through a case study of a tunnel ventilation system and the results show that it has the potential to identify scenarios that encompass both the technical system and the organizational structure. However, since the method does not provide quantitative estimations of risk, it is recommended to be used as a complementary approach to the traditional risk assessments rather than as an alternative. Screening analysis and selection of emission reduction concepts for intermittent combustion aircraft engines
An analysis was conducted to screen, evaluate, and select three engine exhaust emission reduction concepts from a group of 14 candidate alternatives. A comprehensive literature search was conducted to survey the emission reduction technology state-of-the-art and establish contact with firms working on intermittent combustion engine development and pollution reduction problems. Concept development, advantages, disadvantages, and expected emission reduction responses are stated. A set of cost effectiveness criteria was developed, appraised for relative importance, and traded off against each concept so that its merit could be determined. A decision model was used to aid the evaluators in managing the criteria, making consistent judgements, calculating merit scores, and ranking the concepts. An Improved Fuel Injection System, Improved Cooling Combustion Chamber, and a Variable Timing Ignition System were recommended to NASA for approval and further concept development. An alternate concept, Air Injection, was also recommended

Advances in food safety have yielded promising results leading to the development of intelligent packaging (IP). By these containers, it is possible to monitor and provide information of the condition of food, packaging, or the environment. This article describes the role of the different concepts of intelligent packaging.
