    Effect of dysmorphic plantar arch on venous hemodynamics of the lower limb

    Background: The role of the plantar venous pump (PVP) on venous return is evident but the effects of the foot morphology have never been characterized properly. Method: 52 healthy volunteers—26 with normal plantar arch (control) and 26 with dysmorphic plantar arch (in two subgroups: 13 flat feet, 13 hollow feet)—were included. Using Doppler ultrasound, we measured the diameter and the peak systolic velocity in the large veins of the lower limb after PVP stimulation by manual compression and bodyweight transfer. Result: The mean peak systolic velocity of the studied veins varied from 12.2 cm/s to 41.7 cm/s in the control group and from 10.9 cm/s to 39.1 cm/s in the dysmorphic plantar group. The foot arch morphology did not affect significantly the venous blood flows, except in the great saphenous vein during manual compression. Conclusion: The plantar morphology did not induce a significant increase of venous blood velocity resulting from PVP stimulation.Peer ReviewedPostprint (author's final draft

    The podobarometrics platforms represent a trustworthy and cuantificable method for the study of the human gait and for the aid to the diagnosis of certain anatomical, functional and biomechanics alterations in the lower limb. However, they do not represent a method, that by itself can be able to establish a pathological clinical diagnosis of the locomotive apparatus and/or the foot. Nevertheless, its use as complement to other techniques of diagnosis and exploration is very useful at the time of carrying out an accurate diagnosis in pathology of the foot.     La braquimetatarsia: presentación de un caso

    Se presenta el caso de una mujer de 24 años de edad que consultó por una metatarsalgia central y una alteración estética del antepié derecho. Se trató mediante un alargamiento progresivo del metatarsiano con un mini fijador externo, a lo que se asoció una osteotomía distal del primer metatarsiano. Se consiguió un correcto alineamiento del metatarsiano, la desaparición de la metatarsalgia y la corrección del valguismo del primer metatarsiano. La paciente quedó satisfecha aunque persistió una ligera limitación funcional pero sin dolor. Conclusiones: Se aconseja este tipo de intervención en los acortamientos de metatarsianos, utilizando para la misma un fijador externo que permite elongar el metatarsiano corto de forma progresiva, teniendo en cuenta las posibles complicaciones de la técnica y la larga duración que el tratamiento requiere.A case of a 24-year-old woman affected of symptomatic IV brachymetatarsia associated to a cosmetic trouble of the right foot is presented. She was treated by means of a progressive lengthening of the IV metatarsal bone with a mini external fixator and a distal osteotomy of the first metatarsal bone. It was obtained a correct alignment of the IV metatarsal, healing of metatarsalgia, and correction of the valgus deformity of the I metatarsal bone. The patient was very satisfied with the cosmetic result, although a slight functional limitation but without pain persisted. Conclusions: Metatarsal lengthening may be indicated in the management of symptommatic brachymetatarsia. We recommend the use of a mini external fixator that allows progressive lengthening of the short bone, considering the possible complications of the technique and the long duration that the treatment needs

    El pie plano-valgo en la infancia. Tratamiento quirúrgico mediante la prótesis de expansión

    —Se revisan los pies planos-valgos del niño y adolescente intervenidos en nuestro Hospital desde 1987 hasta 1992, mediante la implantación de endoprótesis subastragalina según técnica de Gianini. Esta técnica ha sido realizada en un total de 73 casos, con una edad media de 11 años y un seguimiento medio de 3 años. La valoración de los resultados se realizó en base a la correción anatómica, corrección radiológica y movilidad del pie, y fueron clasificados en: excelentes (normalización de los tres aspectos), buenos (mejoría de los 3 aspectos con normalización absoluta de 2 e ellos) y malos (mejoría de uno o ningún aspecto). Los resultados fueron: excelentes, 34%; buenos, 64%, y malos 4%.The authors review a series of children and adolescent with valgus flatfoot which were operated on our Hospital from 1987 to 1992. The operation consisted of the placement of a subtalar prosthesis according to Gianini's technique wich was employed in 73 cases, on an average 11 year-old children and a follow up of 3 year average. The evaluation of the results was done according to the anatomic correction, radiologic correction and the foot mobility, and were classified in: excellent (the tree parameters were standarizated), good (important improvement of them), and poor (only one parameter was improved).     How susceptible are our Achilles Tendons? Sonoanatomical assessment. A cross-sectional study

    Objective: the aim of this study is to observe whether there are ultrasound changes between men and women in the Achilles tendon at rest, at maximum passive force is applied and during walking. Material and methods: it was a cross-sectional study involving 27 healthy young participants recruited as vol-unteers between April to July 2022. A variety of data was recorded: (age, Body Mass Index, sex, smoking, current injury status, allergies, medications, previous surgeries, type of sport, and number of weekly workouts) and ultrasound measurements at rest and at passive force (Cross Sectional Area Achilles Tendon length, tendon thickness, Cross Sectional Area and pennation angle of the soleus muscle to the Achilles Tendon). Results: women demonstrated a statistically significant lower proximal and median thickness both at rest (4.5 vs 5.1 mm with p < 0.001 for proximal thickness; 4.4 vs 5.3 mm with p < 0.001 for median thickness) as well as during maximum eccentric contraction (4.3 vs 4.8 mm with p=<0.001 for proximal thickness; 4.1 vs 4.8 mm with p < 0.001 for median thickness).
