    Self-monitoring Practices, Attitudes, and Needs of Individuals with Bipolar Disorder: Implications for the Design of Technologies to Manage Mental Health

    Objective To understand self-monitoring strategies used independently of clinical treatment by individuals with bipolar disorder (BD), in order to recommend technology design principles to support mental health management. Materials and Methods Participants with BD (N = 552) were recruited through the Depression and Bipolar Support Alliance, the International Bipolar Foundation, and WeSearchTogether.org to complete a survey of closed- and open-ended questions. In this study, we focus on descriptive results and qualitative analyses. Results Individuals reported primarily self-monitoring items related to their bipolar disorder (mood, sleep, finances, exercise, and social interactions), with an increasing trend towards the use of digital tracking methods observed. Most participants reported having positive experiences with technology-based tracking because it enables self-reflection and agency regarding health management and also enhances lines of communication with treatment teams. Reported challenges stem from poor usability or difficulty interpreting self-tracked data. Discussion Two major implications for technology-based self-monitoring emerged from our results. First, technologies can be designed to be more condition-oriented, intuitive, and proactive. Second, more automated forms of digital symptom tracking and intervention are desired, and our results suggest the feasibility of detecting and predicting emotional states from patterns of technology usage. However, we also uncovered tension points, namely that technology designed to support mental health can also be a disruptor. Conclusion This study provides increased understanding of self-monitoring practices, attitudes, and needs of individuals with bipolar disorder. This knowledge bears implications for clinical researchers and practitioners seeking insight into how individuals independently self-manage their condition as well as for researchers designing monitoring technologies to support mental health management

    Analysis of user-generated content from online social communities to characterize and predict depression degree

    The identification of a mental disorder at its early stages is a challenging task because it requires clinical interventions that may not be feasible in many cases. Social media such as online communities and blog posts have shown some promising features to help detect and characterise mental disorder at an early stage. In this work, we make use of user-generated content to identify depression and further characterise its degree of severity. We used the user-generated post contents and its associated mood tag to understand and differentiate the linguistic style and sentiments of the user content. We applied machine learning and statistical analysis methods to discriminate the depressive posts and communities from non-depressive ones. The depression degree of a depressed post is identified using variations of valence values based on the mood tag. The proposed methodology achieved 90%, 95% and 92% accuracy for the classification of depressive posts, depressive communities and depression degree, respectively. </jats:p

    The Crossroads of Digital Phenotyping

    The term ‘Digital Phenotyping’ has started to appear with increasing regularity in medical research, especially within psychiatry. This aims to bring together digital traces (e.g., from smartphones), medical data (e.g., electronic health records), and lived experiences (e.g., daily activity, location, social contact), to better monitor, intervene, and diagnose various psychiatric conditions. However, is this notion any different from digital traces or the quantified self? While digital phenotyping has the potential to transform and revolutionize medicine as we know it; there are a number of challenges that must be addressed if research is to blossom. At present, these issues include; (1) methodological issues, for example, the lack of clear theoretical links between digital markers (e.g., battery life, interactions with smartphones) and condition relapses, (2) the current tools being employed, where they typically have a number of security or privacy issues, and are invasive by nature, (3) analytical methods and approaches, where I question whether research should start in larger-scale epidemiological scale or in smaller (and potentially highly vulnerable) patient populations as is the current norm, (4) the current lack of security and privacy regulation adherence of apps used, and finally, (5) how do such technologies become integrated into various healthcare systems? This aims to provide deep insight into how the Digital Phenotyping could provide huge promise if we critically reflect now and gather clinical insights with a number of other disciplines such as epidemiology, computer- and the social sciences to move forward

    Detecting Mental Distresses Using Social Behavior Analysis in the Context of COVID-19: A Survey

    Online social media provides a channel for monitoring people\u27s social behaviors from which to infer and detect their mental distresses. During the COVID-19 pandemic, online social networks were increasingly used to express opinions, views, and moods due to the restrictions on physical activities and in-person meetings, leading to a significant amount of diverse user-generated social media content. This offers a unique opportunity to examine how COVID-19 changed global behaviors regarding its ramifications on mental well-being. In this article, we surveyed the literature on social media analysis for the detection of mental distress, with a special emphasis on the studies published since the COVID-19 outbreak. We analyze relevant research and its characteristics and propose new approaches to organizing the large amount of studies arising from this emerging research area, thus drawing new views, insights, and knowledge for interested communities. Specifically, we first classify the studies in terms of feature extraction types, language usage patterns, aesthetic preferences, and online behaviors. We then explored various methods (including machine learning and deep learning techniques) for detecting mental health problems. Building upon the in-depth review, we present our findings and discuss future research directions and niche areas in detecting mental health problems using social media data. We also elaborate on the challenges of this fast-growing research area, such as technical issues in deploying such systems at scale as well as privacy and ethical concerns

    Utilizing Consumer Health Posts for Pharmacovigilance: Identifying Underlying Factors Associated with Patients’ Attitudes Towards Antidepressants

    Non-adherence to antidepressants is a major obstacle to antidepressants therapeutic benefits, resulting in increased risk of relapse, emergency visits, and significant burden on individuals and the healthcare system. Several studies showed that non-adherence is weakly associated with personal and clinical variables, but strongly associated with patients’ beliefs and attitudes towards medications. The traditional methods for identifying the key dimensions of patients’ attitudes towards antidepressants are associated with some methodological limitations, such as concern about confidentiality of personal information. In this study, attempts have been made to address the limitations by utilizing patients’ self report experiences in online healthcare forums to identify underlying factors affecting patients attitudes towards antidepressants. The data source of the study was a healthcare forum called “askapatients.com”. 892 patients’ reviews were randomly collected from the forum for the four most commonly prescribed antidepressants including Sertraline (Zoloft) and Escitalopram (Lexapro) from SSRI class, and Venlafaxine (Effexor) and duloxetine (Cymbalta) from SNRI class. Methodology of this study is composed of two main phases: I) generating structured data from unstructured patients’ drug reviews and testing hypotheses concerning attitude, II) identification and normalization of Adverse Drug Reactions (ADRs), Withdrawal Symptoms (WDs) and Drug Indications (DIs) from the posts, and mapping them to both The UMLS and SNOMED CT concepts. Phase II also includes testing the association between ADRs and attitude. The result of the first phase of this study showed that “experience of adverse drug reactions”, “perceived distress received from ADRs”, “lack of knowledge about medication’s mechanism”, “withdrawal experience”, “duration of usage”, and “drug effectiveness” are strongly associated with patients attitudes. However, demographic variables including “age” and “gender” are not associated with attitude. Analysis of the data in second phase of the study showed that from 6,534 identified entities, 73% are ADRs, 12% are WDs, and 15 % are drug indications. In addition, psychological and cognitive expressions have higher variability than physiological expressions. All three types of entities were mapped to 811 UMLS and SNOMED CT concepts. Testing the association between ADRs and attitude showed that from twenty-one physiological ADRs specified in the ASEC questionnaire, “dry mouth”, “increased appetite”, “disorientation”, “yawning”, “weight gain”, and “problem with sexual dysfunction” are associated with attitude. A set of psychological and cognitive ADRs, such as “emotional indifference” and “memory problem were also tested that showed significance association between these types of ADRs and attitude. The findings of this study have important implications for designing clinical interventions aiming to improve patients\u27 adherence towards antidepressants. In addition, the dataset generated in this study has significant implications for improving performance of text-mining algorithms aiming to identify health related information from consumer health posts. Moreover, the dataset can be used for generating and testing hypotheses related to ADRs associated with psychiatric mediations, and identifying factors associated with discontinuation of antidepressants.     Mining social media data for biomedical signals and health-related behavior

    Social media data has been increasingly used to study biomedical and health-related phenomena. From cohort level discussions of a condition to planetary level analyses of sentiment, social media has provided scientists with unprecedented amounts of data to study human behavior and response associated with a variety of health conditions and medical treatments. Here we review recent work in mining social media for biomedical, epidemiological, and social phenomena information relevant to the multilevel complexity of human health. We pay particular attention to topics where social media data analysis has shown the most progress, including pharmacovigilance, sentiment analysis especially for mental health, and other areas. This work demonstrates that psychological distress, alcohol intake, sleep problems and overweight/obesity are recurrent and ongoing concerns among many FIFO workers and their partners, and their important impact on productivity loss, and provides evidence of important daily variability in potentially modifiable aspects of FIFO work that could be targeted in future intervention
