    International health IT benchmarking:learning from cross-country comparisons

    Objective, to pilot benchmark measures of health information and communication technology (ICT) availability and use to facilitate cross-country learning. Materials and Methods, a prior Organization for Economic Cooperation and Development-led effort involving 30 countries selected and defined functionality-based measures for availability and use of electronic health records, health information exchange, personal health records, and telehealth. In this pilot, an Organization for Economic Cooperation and Development Working Group compiled results for 38 countries for a subset of measures with broad coverage using new and/or adapted country-specific or multinational surveys and other sources from 2012 to 2015. We also synthesized country learnings to inform future benchmarking. Results, while electronic records are widely used to store and manage patient information at the point of care- all but 2 pilot countries reported use by at least half of primary care physicians; many had rates above 75%- patient information exchange across organizations/settings is less common. Large variations in the availability and use of telehealth and personal health records also exist. Discussion, pilot participation demonstrated interest in cross-national benchmarking. Using the most comparable measures available to date, it showed substantial diversity in health ICT availability and use in all domains. The project also identified methodological considerations (e.g., structural and health systems issues that can affect measurement) important for future comparisons. Conclusion, while health policies and priorities differ, many nations aim to increase access, quality, and/or efficiency of care through effective ICT use. By identifying variations and describing key contextual factors, benchmarking offers the potential to facilitate cross-national learning and accelerate the progress of individual countries

    The Impact of ICT on Health Promotion: A Randomized Experiment with Diabetic Patients

    This paper summarizes randomized experiment to study the effects of an Internetbased intervention on type 2 diabetes patients in Montevideo, Uruguay. The intervention consisted of a specially designed website and an electronic social network where participants were able to navigate freely, download materials, and interact with other diabetics and with specialists. No significant impact was found on participants` knowledge, behavior, or health outcomes. It was also found that only a minority of patients logged on to the website, and most were only reached by email and mobile text (SMS).     eHealth Conversations : using information management, dialogue, and knowledge exchange to move toward universal

    The publication of eHealth Conversations, developed with the support of the Spanish Agency for International Development Cooperation (AECID), represents a major step forward for the PAHO/WHO Strategy, since it explores ways of implementing regional mechanisms with free and equitable access to information and knowledge sharing. These initiatives aim to advance the goals of more informed, equitable, competitive, and democratic societies, where access to health information is considered a basic right. This publication is one of the instruments used by PAHO/WHO to develop the initiatives outlined in the Strategy, which coincides with the global eHealth strategy. One of the fundamental needs for the improvement of eHealth is the dissemination of information, and PAHO/WHO is assuming a leading role in this effort. The development of this new electronic publication is a key step in disseminating information that will be useful for decision makers on applying these technologies for the health of the Americas. This electronic book is one of the products of PAHO/WHO’s project: “eHealth Conversations: Using Information Management, Dialogue, and Knowledge Exchange to Move Toward Universal Access to Health.” Participants in these conversations included experts on electronic health and other specialties. Through virtual dialogues, the experts contributed with knowledge and reflections on the present and the future of eHealth in the Americas, analyzed the situation, and made recommendations for the implementation of electronic health initiatives. These recommendations are not only intended for PAHO/ WHO, but also for governments and the private sector. The aim of the project is to guarantee the convergence of local, national, and regional initiatives regarding the adoption and application of ICTs for public health, with special attention on critical issues in this field.     The state of broadband 2015: Broadband as a foundation for sustainable development

    Every year, the ITU/UNESCO Broadband Commission for Digital Development ‘State of Broadband’ report takes the pulse of the global broadband industry and explores progress in broadband connectivity. This year’s report finds mixed messages about the growth of ICTs and the global state of broadband. Although strong growth rates continue for mobile broadband and Facebook usage, and mobile cellular subscriptions exceeded 7 billion for the first time during 2015, growth in both mobile cellular subscriptions and Internet usage has slowed sharply. The UN Broadband Commission’s 2011 targets have not been achieved by the target date of 2015 and seem unlikely to be achieved before 2020. Likewise, the milestone of four billion Internet users is unlikely to be surpassed before 2020. The growth in Facebook subscribers is now outpacing growth in the Internet. Internet growth By end 2015, some 3.2 billion people will be online, equating to over 43.4% of the total world population, and up from 2.9 billion a year earlier (almost 40.6% of the population). In the developing world, Internet penetration will surpass 35.3% by the end of 2015; penetration will still be under 10% at 9.5% in the UN-designated Least Developed Countries, however. Even though Internet penetration is approaching saturation in the developed world, with 82.2% of the population online, the global target of 60% set by the Broadband Commission in 2011, to be achieved by 2015, is unlikely to be achieved before 2021 at the earliest. Internet user penetration in the developing world is unlikely to achieve the Broadband Commission target of 50% before 2020. By the end of 2015, there will still be 57% of the world’s population – or four billion people – still offline. Household Internet access in developed countries is close to saturation, with more than 81.3% of households connected. The proportion of households in the developing world with Internet access has increased from 31.5% at the end of 2014 to over 34.1% a year later – still well short of the Broadband Commission target of 40% by 2015. Household connectivity figures mask strong disparities – fewer than 7% of households in LDCs have access, while in sub-Saharan Africa only 1 in 9 households is connected. According to Point Topic, Asia has the largest total number of broadband-connected homes, with nearly as many in total as Europe and the Americas combined. The gender gap in Internet users is proving stubbornly persistent, with an estimated 200 million more men online than women as recently as 2013; one major problem is that sex-disaggregated data are not yet widely reported by national governments and statistics agencies. Mobile growth The mobile industry is growing strongly, but unevenly. ITU forecasts that the milestone of seven billion mobile cellular subscriptions will be exceeded by end 2015, equivalent to a global penetration rate of 97 subscriptions per 100 people. ITU also estimates that there will be a total of almost 3.5 billion mobile broadband subscriptions by end 2015. Industry analysts predict 6.5 billion mobile broadband (3G/4G/5G) subscriptions by 2019, making mobile broadband the fastest growing ICT service in history. Asia-Pacific now accounts for half of all mobile broadband subscriptions, up from just under 45% at the end of 2014. In January 2015, China Mobile became the world’s largest mobile operator by number of subscribers. The rapid expansion of Asia-Pacific is squeezing other world regions in terms of their mobile broadband market share – Europe and the Americas saw declining proportional shares of mobile broadband subscriptions from the end of 2014 to the end of 2015, despite absolute increases in subscription numbers. Smartphones now dominate the mobile device market, and will continue to do so for the foreseeable future. Ericsson forecasts that the number of smartphones in service could exceed ‘basic’ phones by 2016. While developed markets become saturated in terms of total mobile penetration, analysts still see plenty of room for growth, with only an estimated one third of all mobile subscriptions currently associated with a smartphone. In hindsight, the year 2014 is likely to prove a tipping point as the year in which growth in ‘3G’ services began to slow, as growth in ‘4G’ services accelerated. Continued in report. &nbsp

    The global Information technology report 2015: ICTs for inclusive growth

    Information and communication technologies (ICTs) are transforming our economies and societies. Since 2001, The Global Information and Technology Report series and the Networked Readiness Index (NRI) have been taking the pulse of the ICT revolution worldwide. The NRI identifies the capacity of countries to leveraging ICT, by assessing the overall political and business environment, the level of ICT readiness and usage of ICT among the population, businesses and government, as well as the overall impacts of ICTs on the economy and society at large. The 2015 results, which covers 143 economies, confirms the dominance of advanced economies and the persistence of the multiple-faceted digital divides not only across but also within economies. They reveal the pervasive digital poverty that deprives the neediest from the opportunities offered by ICTs. Beyond this diagnosis, under the theme “ICTs for Inclusive Growth”, the 2015 edition of the report provides solutions from leading experts and practitioners to alleviate digital poverty and make the ICT revolution a global reality.&nbsp

    The Contribution of Digital Technologies to Service Delivery: An Evidence Review

    The explosion in digital connectivity, globalisation and the rapid growth in digital technologies over the last two decades has revolutionised the way that businesses perform and compete globally. Governments around the world have been put under strong pressure to transform themselves into electronic governments, in recognition of the efficiencies brought about by the appropriate use of information communication technologies (ICTs) in businesses and the need for development. The aim has been to maximise the state’s capacity to serve its stakeholders: namely citizens, business, employees and other government and non-government agencies. E-government or digital government has been a significant feature of public sector reform in recent years in both developed and developing countries with a substantial amount of resources dedicated to the development of necessary systems and infrastructure. Yet the transformational potential of digital for development risks not being replicated in the real world. Large-scale and sustainable use of ICTs for education is not yet being realised in developing countries, despite the fact that digital technologies have the potential to reduce costs and strengthen education systems. In the field of health care, mHealth systems are reaching significant scale in many developing countries but there is still a lack of concrete evidence with which to fully assess the economic impact of these technologies. This report explores and assesses the evidence for the impact and use of digital technologies in development, identifying cross-cutting themes that are important for use, implementation and scale-up.
