    Rabies mortality and morbidity associated with animal bites in Africa: a case for integrated rabies disease surveillance, prevention and control: a scoping review

    OBJECTIVE: The objective of this scoping review was to map the current situation and available evidence and gaps on rabies morbidity, mortality, integrated rabies surveillance programmes, and existing prevention and control strategies in Africa. METHODS: We conducted a systematic scoping review following the Joanna Briggs methodology and Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews checklist. Medline, Embase, CINAHL (EBSCOHost), Scopus, Web of Science and rabies web conferences were used to search for peer-reviewed publications between January 1946 and May 2020. Two researchers reviewed the studies and extracted data based on author (year) and region, study design and data collection duration, participants/comparators, interventions, control conditions/exposures and outcomes (rabies mortality and morbidity) and key findings/gaps/challenges. The results were reported narratively using Arksey and O'Malley's methodological framework. RESULTS: Electronic search yielded 2775 records, of which 43 studies were included. A total of 543 714 bite victims were censored through the included studies. Most of the victims were less than 15 years of age. The studies included rabies morbidity (21) and mortality (15) fluctuating in space and time across Africa depending on countries' rabies prevention and control practices (16). Others were surveillance (nine studies); surveillance and prevention (five studies); management and control (seven studies); and surveillance, prevention and control (six studies). We found challenges in rabies reporting, existing dog vaccination programmes and post-exposure prophylaxis availability or compliance.     Data are rapidly accumulating from all over the world regarding the efficacy of standardized treatment regimens for drug-sensitive, drug-resistant TB and latent TB infection. While we are facing the menace of multi drug-resistant TB [MDR-TB], extensively drug-resistant tuberculosis [XDR­ TB] has emerged threatening to undermine global efforts at TB control. Hence we have included chapters to cover all aspects of the diagnosis and management of MDR TB. This book will cover all these developments in great detail. With the widespread availability of internet globally various standard web resources available on TB have also been included so that the readers may get the comprehensive and updated guidelines from these resources. The changing clinical presentation of TB, advances in laboratory, imaging diagnostic modalities, therapeutic measures and emergence of MDR TB all suggest a pressing need to have a updated book on TB. Furthermore, while all physicians encounter the TB disease in their clinical practice, there have been a lot of controversies and misconceptions over various issues for the diagnosis and management of TB. Paucity of a well referenced, updated, standard book of TB has prompted us to undertake this venture sharing the clinical experience of global experts of TB. Our book contains chapters on epidemiology, immune-pathology, diagnosis, treatment and latest advances for TB, highlighting the global perspective of tuberculosis. World-wide resurgence of MDR TB indicates that the battle against this foe of mankind will continue in the coming years. TB still remains to be a research priority of paramount importance from medical, social and financial aspects and we have attempted to highlight all the aspects for the treatment of TB. We believe that this book will serve as a practical guide for the diagnosis and management of TB for practicing physicians (especially pulmonologists and internists) and all those who are involved in the management of TB

    Abstract: There are a high number of COVID-19 cases per capita in the world that goes undetected including clinical diseases compatible with COVID-19. While the presence of the COVID-19 in untreated drinking water is possible, it is yet to be detected in the drinking-water supplies. COVID-19 viral fragments have been found in excrete, this call for wastewater monitoring and analysis (wastewater surveillance) of the potential health risk. This raises concern about the potential of the SARS-CoV-2 transmission via the water systems. The economic limits on the medical screening for the SARS-CoV-2 or COVID-19 worldwide are turning to wastewater-based epidemiology as great potential tools for assessing and management of the COVID-19 pandemic. Surveillance and tracking of the pathogens in the wastewater are key to the early warning system and public health strategy monitoring of the COVID-19. Currently, RT-qPCR assays is been developed for SARS-CoV-2 RNA specimen clinical testing and detection in the water system. Convectional wastewater treatment methods and disinfection are expected to eradicate the SAR-CoV-2. Chlorine, UV radiation, ozone, chloramine is been used to inactivate and disinfect the water treatment system against the SARS-CoV-2. Water management and design of the water infrastructure require major changes to accommodate climate change, water cycle, reimaging of digitalization, infrastructure and privacy protection. The water digital revolution, biosensors and nanoscale, contact tracing, knowledge management can accelerate with disruption of the COVID-19 outbreak (water-health-digital nexus)

    SARS-CoV-2 has infected more than 105 million people worldwide. During this pandemic, researchers and clinicians have been working to understand the molecular mechanisms that underpin viral pathogenesis by studying viral–host interactions. Now, with the global rollout of various COVID-19 vaccines—based on the neutralization of the spike protein using different technologies—viral immunology and cell-based immunity are being investigated. Researchers are also studying how various SARS-CoV-2 genetic mutations will impact the efficacy of these COVID-19 vaccines. At the same time, various antiviral drugs have been identified or repurposed that have potential as anti-SARS-CoV-2 treatments. BRICS (Brazil, Russia, India, China, and South Africa) is the acronym used to associate five major emerging national economies. The BRICS countries are known for their significant influence on regional affairs, including being leaders in scientific and clinical research and innovation. This Special Issue includes researchers from BRICS countries, in particular South Africa, involved in the study of SARS-CoV-2 and COVID-19. Original articles, as well as new perspectives or reviews on the matter, were welcomed.     On the importance of primary and community healthcare in relation to global health and environmental threats:Lessons from the COVID-19 crisis

    In the course of the COVID-19 pandemic, it has become clear that primary healthcare systems play a critical role in clinical care, such as patient screening, triage, physical and psychological support and also in promoting good community advice and awareness in coordination with secondary healthcare and preventive care. Because of the role of social and environmental factors in COVID-19 transmission and burden of disease, it is essential to ensure that there is adequate coordination of population-based health services and public health interventions. The COVID-19 pandemic has shown the primary and community healthcare (PCHC) system's weaknesses worldwide. In many instances, PCHC played only a minor role, the emphasis being on hospital and intensive care beds. This was compounded by political failures, in supporting local community resilience. Placing community building, social cohesion and resilience at the forefront of dealing with the COVID-19 crisis can help align solutions that provide a vision of a € planetary health'. This can be achieved by involving local well-being and participation in the face of any pervasive health and environmental crisis, including other epidemics and large-scale ecological crises. This paper proposes that PCHC should take on two critical roles: first, to support local problem-solving efforts and to serve as a partner in innovative approaches to safeguarding community well-being; and second, to understand the local environment and health risks in the context of the global health perspective.
