    Telehealth Group Interactions in the Hospice Setting: Assessing Technical Quality Across Platforms

    Objective: This study aims to examine the technical quality of videoconferencing used in hospice to engage caregivers as “virtual” members of interdisciplinary team meetings and their impressions of telehealth. Furthermore, it aims to compare the quality of plain old telephone service (POTS) and Web-based videoconferencing and provide recommendations for assessing video quality for telehealth group interactions. Materials and Methods: Data were obtained from an ongoing randomized clinical trial exploring Web-based videoconferencing and a completed prospective study of POTS-based videoconferencing in hospice. For the assessment of the technical quality, an observation form was used. Exit interviews with caregivers assessed impressions with the use of telehealth. A retrospective analysis of video-recorded team meetings was conducted rating attributes essential for the quality of videoconferencing (e.g., video artifacts, sharpness). Results: In total, 200 hospice team meetings were analyzed, including 114 video-recorded team meetings using Web-based videoconferencing and 86 meetings using POTS videophones. A direct comparison between the two modalities indicates the superiority of Web-based video in image quality but less so in audio quality. Transcripts of 19 caregiver interviews were analyzed. Caregivers found the use of videoconferencing to be a positive experience and a useful and essential tool to communicating with the hospice team. Conclusions: This study highlights the potential of telehealth to improve communication in hospice and the need for new tools that capture the quality of video-mediated communication among multiple stakeholders and strategies to improve the ongoing documentation of telehealth group sessions\u27 technical quality

    COVID-19 triggering homecare professionals’ change of attitudes towards e-Welfare

    During 2020, the COVID-19 crisis expanded the use of digital tools in public health and social care. The aim of this qualitative, single-case study was to scrutinize how homecare professionals experienced meaningfulness in their work in the midst of a crisis and with the utilization of the videophone in long-term homecare service provision. The empirical data consisted of 20 thematic interviews carried out among homecare professionals and their managers in the city of Tampere, Finland. The results indicated that the videophone can generate significance, self-realization and broader purposes among homecare professionals, thus providing meaningfulness for work in the midst of a crisis and continuous work-related changes. In addition, a crisis may support change in the meaningfulness of e-welfare in work-related tasks and aid in overcoming reluctance amongst public-sector social care (homecare) professionals towards an e-welfare initiative: the videophone (VideoVisit).Peer reviewe

    Improved quality block-based low bit rate video coding.

    The aim of this research is to develop algorithms for enhancing the subjective quality and coding efficiency of standard block-based video coders. In the past few years, numerous video coding standards based on motion-compensated block-transform structure have been established where block-based motion estimation is used for reducing the correlation between consecutive images and block transform is used for coding the resulting motion-compensated residual images. Due to the use of predictive differential coding and variable length coding techniques, the output data rate exhibits extreme fluctuations. A rate control algorithm is devised for achieving a stable output data rate. This rate control algorithm, which is essentially a bit-rate estimation algorithm, is then employed in a bit-allocation algorithm for improving the visual quality of the coded images, based on some prior knowledge of the images. Block-based hybrid coders achieve high compression ratio mainly due to the employment of a motion estimation and compensation stage in the coding process. The conventional bit-allocation strategy for these coders simply assigns the bits required by the motion vectors and the rest to the residual image. However, at very low bit-rates, this bit-allocation strategy is inadequate as the motion vector bits takes up a considerable portion of the total bit-rate. A rate-constrained selection algorithm is presented where an analysis-by-synthesis approach is used for choosing the best motion vectors in term of resulting bit rate and image quality. This selection algorithm is then implemented for mode selection. A simple algorithm based on the above-mentioned bit-rate estimation algorithm is developed for the latter to reduce the computational complexity. For very low bit-rate applications, it is well-known that block-based coders suffer from blocking artifacts. A coding mode is presented for reducing these annoying artifacts by coding a down-sampled version of the residual image with a smaller quantisation step size.     Decision Feedback Equaliser (DFE) aided wideband Burst-by-Burst (BbB) Adaptive Trellis Coded Modulation (TCM), Turbo Trellis Coded Modulation (TTCM) and Bit-Interleaved Coded Modulation (BICM) assisted H.263-based video transceivers are proposed and characterised in performance terms when communicating over the COST 207 Typical Urban wideband fading channel. Specifically, four different modulation modes, namely 4QAM, 8PSK, 16QAM and 64QAM are invoked and protected by the above-mentioned coded modulation schemes. The TTCM assisted scheme was found to provide the best video performance, although at the cost of the highest complexity. A range of lower-complexity arrangements will also be characterised. Finally, in order to confirm these findings in an important practical environment, we have also investigated the adaptive TTCM scheme in the CDMA-based Universal Mobile Telecommunications System's (UMTS) Terrestrial Radio Access (UTRA) scenario and the good performance of adaptive TTCM scheme recorded when communicating over the COST 207 channels was retained in the UTRA environment

    The potential for congressional use of emergent telecommunications: An exploratory assessment

    A study of the use of newly emerging communications technology for improving the understanding between members of Congress and their constituents was conducted. The study employed a number of specific methodologies such as interdisciplinary systems model building, technology analysis, a sample survey, and semi-structured interviews using sketches of the emergent channels. The following configurations were identified as representative of emergent channel characteristics: (1) the teleconference, (2) the videoconference, (3) the videophone, (4) cable television, (5) cable television polling, and (6) information retrieval. Analysis of the interview data resulted in an overview of the current congressional-constituent communication system and an assessment of the potential for emergent telecommunications, as perceived by congressmen and senior staff from 40 offices in the stratified judgement sample

    Evaluating the Efficacy and Acceptability of Videoconferencing in School-Based Behavioral Consultation

    School-based behavioral consultation is a way of providing psychological services to students through their teachers. This indirect service delivery model has been shown to be effective for improving academic and behavioral concerns. Typically, school-based behavioral consultation is conducted face-to-face (in-vivo) between consultants (psychologists) and consultee’s (teachers); however, current technologies have provided an alternative medium to communicate, via videoconferencing. Videoconferencing has been shown to be an effective alternative to face-to-face communication across many different practice domains and applications. Videoconferencing in schools, particularly for school-based behavioral consultation, may be a viable alternative. The proposed study evaluated the efficacy and acceptability of videoconferencing with teachers as an alternative means of conducting the problem identification interview of school-based behavioral consultation. A school psychology doctoral student interviewed teachers on two occasions (face-to-face and via videoconferencing) to obtain information about student problem behavior. The videoconferencing interviews were conducted across iPads using the videoconferencing software FaceTime. All interviews were video recorded and transcribed into text to code verbalizations, using the Consultation Analysis Record (CAR). Frequencies of verbalizations were totaled on the Consultation Analysis checklist (CAC) into 14 required categories. Data from the CAC was used to evaluate the efficacy of the interviews. Additionally, teachers completed two brief measures, the Fast Form of the Technology Acceptability Model (FF-TAM) and the Distance Communication Comfort Scale (DCCS) pre and post interviews. Data from The FF-TAM and DCCS was used to evaluate the acceptability of videoconferencing. Finally, moderators of the acceptability of videoconferencing were evaluated using a multiple regression analysis
