    3D reconstruction of cerebral blood flow and vessel morphology from x-ray rotational angiography

    Three-dimensional (3D) information on blood flow and vessel morphology is important when assessing cerebrovascular disease and when monitoring interventions. Rotational angiography is nowadays routinely used to determine the geometry of the cerebral vasculature. To this end, contrast agent is injected into one of the supplying arteries and the x-ray system rotates around the head of the patient while it acquires a sequence of x-ray images. Besides information on the 3D geometry, this sequence also contains information on blood flow, as it is possible to observe how the contrast agent is transported by the blood. The main goal of this thesis is to exploit this information for the quantitative analysis of blood flow. I propose a model-based method, called flow map fitting, which determines the blood flow waveform and the mean volumetric flow rate in the large cerebral arteries. The method uses a model of contrast agent transport to determine the flow parameters from the spatio-temporal progression of the contrast agent concentration, represented by a flow map. Furthermore, it overcomes artefacts due to the rotation (overlapping vessels and foreshortened vessels at some projection angles) of the c-arm using a reliability map. For the flow quantification, small changes to the clinical protocol of rotational angiography are desirable. These, however, hamper the standard 3D reconstruction. Therefore, a new method for the 3D reconstruction of the vessel morphology which is tailored to this application is also presented. To the best of my knowledge, I have presented the first quantitative results for blood flow quantification from rotational angiography. Additionally, the model-based approach overcomes several problems which are known from flow quantification methods for planar angiography. The method was mainly validated on images from different phantom experiments. In most cases, the relative error was between 5% and 10% for the volumetric mean flow rate and between 10% and 15% for the blood flow waveform. Additionally, the applicability of the flow model was shown on clinical images from planar angiographic acquisitions. From this, I conclude that the method has the potential to give quantitative estimates of blood flow parameters during cerebrovascular interventions

    Development of a Surgical Assistance System for Guiding Transcatheter Aortic Valve Implantation

    Development of image-guided interventional systems is growing up rapidly in the recent years. These new systems become an essential part of the modern minimally invasive surgical procedures, especially for the cardiac surgery. Transcatheter aortic valve implantation (TAVI) is a recently developed surgical technique to treat severe aortic valve stenosis in elderly and high-risk patients. The placement of stented aortic valve prosthesis is crucial and typically performed under live 2D fluoroscopy guidance. To assist the placement of the prosthesis during the surgical procedure, a new fluoroscopy-based TAVI assistance system has been developed. The developed assistance system integrates a 3D geometrical aortic mesh model and anatomical valve landmarks with live 2D fluoroscopic images. The 3D aortic mesh model and landmarks are reconstructed from interventional angiographic and fluoroscopic C-arm CT system, and a target area of valve implantation is automatically estimated using these aortic mesh models. Based on template-based tracking approach, the overlay of visualized 3D aortic mesh model, landmarks and target area of implantation onto fluoroscopic images is updated by approximating the aortic root motion from a pigtail catheter motion without contrast agent. A rigid intensity-based registration method is also used to track continuously the aortic root motion in the presence of contrast agent. Moreover, the aortic valve prosthesis is tracked in fluoroscopic images to guide the surgeon to perform the appropriate placement of prosthesis into the estimated target area of implantation. An interactive graphical user interface for the surgeon is developed to initialize the system algorithms, control the visualization view of the guidance results, and correct manually overlay errors if needed. Retrospective experiments were carried out on several patient datasets from the clinical routine of the TAVI in a hybrid operating room. The maximum displacement errors were small for both the dynamic overlay of aortic mesh models and tracking the prosthesis, and within the clinically accepted ranges. High success rates of the developed assistance system were obtained for all tested patient datasets. The results show that the developed surgical assistance system provides a helpful tool for the surgeon by automatically defining the desired placement position of the prosthesis during the surgical procedure of the TAVI.Die Entwicklung bildgeführter interventioneller Systeme wächst rasant in den letzten Jahren. Diese neuen Systeme werden zunehmend ein wesentlicher Bestandteil der technischen Ausstattung bei modernen minimal-invasiven chirurgischen Eingriffen. Diese Entwicklung gilt besonders für die Herzchirurgie. Transkatheter Aortenklappen-Implantation (TAKI) ist eine neue entwickelte Operationstechnik zur Behandlung der schweren Aortenklappen-Stenose bei alten und Hochrisiko-Patienten. Die Platzierung der Aortenklappenprothese ist entscheidend und wird in der Regel unter live-2D-fluoroskopischen Bildgebung durchgeführt. Zur Unterstützung der Platzierung der Prothese während des chirurgischen Eingriffs wurde in dieser Arbeit ein neues Fluoroskopie-basiertes TAKI Assistenzsystem entwickelt. Das entwickelte Assistenzsystem überlagert eine 3D-Geometrie des Aorten-Netzmodells und anatomischen Landmarken auf live-2D-fluoroskopische Bilder. Das 3D-Aorten-Netzmodell und die Landmarken werden auf Basis der interventionellen Angiographie und Fluoroskopie mittels eines C-Arm-CT-Systems rekonstruiert. Unter Verwendung dieser Aorten-Netzmodelle wird das Zielgebiet der Klappen-Implantation automatisch geschätzt. Mit Hilfe eines auf Template Matching basierenden Tracking-Ansatzes wird die Überlagerung des visualisierten 3D-Aorten-Netzmodells, der berechneten Landmarken und der Zielbereich der Implantation auf fluoroskopischen Bildern korrekt überlagert. Eine kompensation der Aortenwurzelbewegung erfolgt durch Bewegungsverfolgung eines Pigtail-Katheters in Bildsequenzen ohne Kontrastmittel. Eine starrere Intensitätsbasierte Registrierungsmethode wurde verwendet, um kontinuierlich die Aortenwurzelbewegung in Bildsequenzen mit Kontrastmittelgabe zu detektieren. Die Aortenklappenprothese wird in die fluoroskopischen Bilder eingeblendet und dient dem Chirurg als Leitfaden für die richtige Platzierung der realen Prothese. Eine interaktive Benutzerschnittstelle für den Chirurg wurde zur Initialisierung der Systemsalgorithmen, zur Steuerung der Visualisierung und für manuelle Korrektur eventueller Überlagerungsfehler entwickelt. Retrospektive Experimente wurden an mehreren Patienten-Datensätze aus der klinischen Routine der TAKI in einem Hybrid-OP durchgeführt. Hohe Erfolgsraten des entwickelten Assistenzsystems wurden für alle getesteten Patienten-Datensätze erzielt. Die Ergebnisse zeigen, dass das entwickelte chirurgische Assistenzsystem ein hilfreiches Werkzeug für den Chirurg bei der Platzierung Position der Prothese während des chirurgischen Eingriffs der TAKI bietet

    VALIDATION OF COMPUTATIONAL FLUID DYNAMIC SIMULATIONS OF MEMBRANE ARTIFICIAL LUNGS WITH X-RAY IMAGING

    The functional performance of membrane oxygenators is directly related to the perfusion dynamics of blood flow through the fiber bundle. Non-uniform flow and design characteristics can limit gas exchange efficiency and influence susceptibility of thrombus development in the fiber membrane. Computational fluid dynamics (CFD) is a powerful tool for predicting properties of the flow field based on prescribed geometrical domains and boundary conditions. Validation of numerical results in membrane oxygenators has been predominantly based on experimental pressure measurements with little emphasis placed on confirmation of the velocity fields due to opacity of the fiber membrane and limitations of optical velocimetric methods. A novel approach was developed using biplane X-ray digital subtraction angiography to visualize flow through a commercial membrane artificial lung at 1–4.5 L/min. Permeability based on the coefficients of the Ergun equation, α and β, were experimentally determined to be 180 and 2.4, respectively, and the equivalent spherical diameter was shown to be approximately equal to the outer fiber diameter. For all flow rates tested, biplane image projections revealed non-uniform radial perfusion through the annular fiber bundle, yet without flow bias due to the axisymmetric position of the outlet. At 1 L/min, approximately 78.2% of the outward velocity component was in the radial (horizontal) plane verses 92.0% at 4.5 L/min. The CFD studies were unable to predict the non-radial component of the outward perfusion. Two-dimensional velocity fields were generated from the radiographs using a cross-correlation tracking algorithm and compared with analogous image planes from the CFD simulations. Velocities in the non-porous regions differed by an average of 11% versus the experimental values, but simulated velocities in the fiber bundle were on average 44% lower than experimental. A corrective factor reduced the average error differences in the porous medium to 6%. Finally, biplane image pairs were reconstructed to show 3-D transient perfusion through the device. The methods developed from this research provide tools for more accurate assessments of fluid flow through membrane oxygenators. By identifying non-invasive techniques to allow direct analysis of numerical and experimental velocity fields, researchers can better evaluate device performance of new prototype designs

    Management of pediatric radiation dose using GE fluoroscopic equipment

    In this article, we present GE Healthcare’s design philosophy and implementation of X-ray imaging systems with dose management for pediatric patients, as embodied in its current radiography and fluoroscopy and interventional cardiovascular X-ray product offerings. First, we present a basic framework of image quality and dose in the context of a cost–benefit trade-off, with the development of the concept of imaging dose efficiency. A set of key metrics of image quality and dose efficiency is presented, including X-ray source efficiency, detector quantum efficiency (DQE), detector dynamic range, and temporal response, with an explanation of the clinical relevance of each. Second, we present design methods for automatically selecting optimal X-ray technique parameters (kVp, mA, pulse width, and spectral filtration) in real time for various clinical applications. These methods are based on an optimization scheme where patient skin dose is minimized for a target desired image contrast-to-noise ratio. Operator display of skin dose and Dose-Area Product (DAP) is covered, as well. Third, system controls and predefined protocols available to the operator are explained in the context of dose management and the need to meet varying clinical procedure imaging demands. For example, fluoroscopic dose rate is adjustable over a range of 20:1 to adapt to different procedure requirements. Fourth, we discuss the impact of image processing techniques upon dose minimization. In particular, two such techniques, dynamic range compression through adaptive multiband spectral filtering and fluoroscopic noise reduction, are explored in some detail. Fifth, we review a list of system dose-reduction features, including automatic spectral filtration, virtual collimation, variable-rate pulsed fluoroscopic, grid and no-grid techniques, and fluoroscopic loop replay with store. In addition, we describe a new feature that automatically minimizes the patient-to-detector distance, along with an estimate of its dose reduction potential. Finally, two recently developed imaging techniques and their potential effect on dose utilization are discussed. Specifically, we discuss the dose benefits of rotational angiography and low frame rate imaging with advanced image processing in lieu of higher-dose digital subtraction

    A fiducial subject pre-alignment system for the Biomedical Imaging and Therapy beamlines at the Canadian Light Source

    Positioning, immobilization, and organ motion are some of the major concerns in all imaging modalities. With synchrotron X-ray imaging, alignment of the region of interest to the beam is usually done inside the experimental hutch. However, because specimen alignment can be time consuming; such a system is wasteful of valuable beam time. For the purposes of the Biomedical Imaging and Therapy (BMIT) beamlines at the Canadian Light Source, we propose an effective and versatile means of positioning a wide range of subjects (human and animal) with a wide range of sizes using a laser-based fiducial system to define the region of interest (ROI) before imaging; i.e., outside the experimental hutch. This system will allow the beam path through a specific region of interest to be modeled outside the imaging hutch in a way that it can be reproduced relative to the fixed X-ray beamline inside the hutch. The model will include an indication of the center of the beam and a rectangular area around the target delineating the limits of the area to be imaged (i.e., encompassing the “region of interest”). The rectangular field of view would be projected on the incoming (entrance) side of the subject as well as the outgoing (exit) side of the subject, and these projections must be coaxial with each other and parallel with the X-ray beam. This method is user friendly, allows mistake to be corrected before experiment and most importantly saves time

    Visual Perception and Cognition in Image-Guided Intervention

    Surgical image visualization and interaction systems can dramatically affect the efficacy and efficiency of surgical training, planning, and interventions. This is even more profound in the case of minimally-invasive surgery where restricted access to the operative field in conjunction with limited field of view necessitate a visualization medium to provide patient-specific information at any given moment. Unfortunately, little research has been devoted to studying human factors associated with medical image displays and the need for a robust, intuitive visualization and interaction interfaces has remained largely unfulfilled to this day. Failure to engineer efficient medical solutions and design intuitive visualization interfaces is argued to be one of the major barriers to the meaningful transfer of innovative technology to the operating room. This thesis was, therefore, motivated by the need to study various cognitive and perceptual aspects of human factors in surgical image visualization systems, to increase the efficiency and effectiveness of medical interfaces, and ultimately to improve patient outcomes. To this end, we chose four different minimally-invasive interventions in the realm of surgical training, planning, training for planning, and navigation: The first chapter involves the use of stereoendoscopes to reduce morbidity in endoscopic third ventriculostomy. The results of this study suggest that, compared with conventional endoscopes, the detection of the basilar artery on the surface of the third ventricle can be facilitated with the use of stereoendoscopes, increasing the safety of targeting in third ventriculostomy procedures. In the second chapter, a contour enhancement technique is described to improve preoperative planning of arteriovenous malformation interventions. The proposed method, particularly when combined with stereopsis, is shown to increase the speed and accuracy of understanding the spatial relationship between vascular structures. In the third chapter, an augmented-reality system is proposed to facilitate the training of planning brain tumour resection. The results of our user study indicate that the proposed system improves subjects\u27 performance, particularly novices\u27, in formulating the optimal point of entry and surgical path independent of the sensorimotor tasks performed. In the last chapter, the role of fully-immersive simulation environments on the surgeons\u27 non-technical skills to perform vertebroplasty procedure is investigated. Our results suggest that while training surgeons may increase their technical skills, the introduction of crisis scenarios significantly disturbs the performance, emphasizing the need of realistic simulation environments as part of training curriculum
