    Investigating the 'latent' deficit hypothesis : age at time of head injury, executive and implicit functions and behavioral insight

    This study investigated the 'latent deficit' hypothesis in two groups of frontotemporal headinjured patients, those injured prior to steep morphological and corresponding functional maturational periods for frontotemporal networks (≤ age 25), and those injured >28 years. The latent deficit hypothesis proposes that early injuries produce enduring cognitive deficits manifest later in the lifespan with graver consequences for behavior than adult injuries, particularly after frontal pathology (Eslinger, Grattan, Damasio & Damasio, 1992). Implicit and executive deficits both contribute to behavioral insight after frontotemporal head injury (Barker, Andrade, Romanowski, Morton & Wasti, 2006). On the basis of morphological and behavioral data, we hypothesised that early injury would confer greater vulnerability to impairment on tasks associated with frontotemporal regions than later injury. Patients completed experimental tasks of implicit cognition, executive function measures and the DEX measure of behavioural insight (Behavioral Assessment of the Dysexecutive Syndrome: Wilson, Alderman, Burgess, Emslie, & Evans, 1996). The Early Injury group were more impaired on implicit cognition tasks compared to controls that Late Injury patients. There were no marked group differences on most executive function measures. Executive ability only contributed to behavioral awareness in the Early Injury Group. Findings showed that age at injury moderates the relationship between executive and implicit cognition and behavioral insight and that early injuries result in longstanding deficits to functions associated with frontotemporal regions partially supporting the latent deficit hypothesis

    Cardiovascular risk factors since childhood and cognitive performance in midlife: The Cardiovascular Risk in Young Finns Study

    According to the World Health Organization, dementia and cognitive deficits are major global health and social care challenges in the 21st century. As the population ages, the need for preventive means against cognitive deficits becomes crucial. The risk of cognitive deficits is influenced by e.g., cardiovascular risk factors, which may begin to exert their influence decades before any detectable cognitive symptoms. This thesis aims to close the existing knowledge gap on the association between the cardiovascular risk factors since childhood and cognitive performance in midlife. The specific aim is to study how 1) physical activity accumulation since childhood; 2) blood pressure, serum lipids, obesity, and their accumulation since childhood; and 3) serum creatinine since adulthood are associated with cognitive performance. This thesis is a part of the prospective population-based Cardiovascular Risk in Young Finns Study that focuses on cardiovascular risk factors from childhood to adulthood with a follow-up time of over 30 years. The baseline study was conducted in 1980, with 3,596 randomly selected boys and girls aged between three and 18 participating in clinical examinations. The follow-up studies were conducted in three- to nine-year intervals. In the follow-up study conducted in 2011, the cognitive performance of 2,026 subjects aged between 34 and 49 was assessed using computerized cognitive testing. In this thesis, low physical activity, high blood pressure, elevated total cholesterol and LDL cholesterol, and overweight and obesity since childhood and low serum creatinine since adulthood were observed to be associated with poorer cognitive performance in midlife. Especially, cardiovascular risk factor accumulation (blood pressure, total cholesterol, and obesity) was observed to be associated with poorer cognitive performance in a dose-responsive manner. If these associations turn out to be causal, the observations can be utilized for aiming to better cognitive health in adulthood by targeting preventive means against cardiovascular risk factors already in childhood and adolescence. Hence, those individuals with a worse cardiovascular risk factor profile since childhood might especially benefit from, for example, adopting healthier lifestyle

    Background: Cardiovascular risk factors, such as high blood pressure, adverse serum lipids, and elevated body mass index in midlife, may harm cognitive performance. It is important to note that longitudinal accumulation of cardiovascular risk factors since childhood may be associated with cognitive performance already since childhood, but the previous evidence is scarce. We studied the associations of cardiovascular risk factors from childhood to midlife, their accumulation, and midlife cognitive performance. Methods: From 1980, a population-based cohort of 3596 children (3-18 years of age) have been repeatedly followed up for 31 years. Blood pressure, serum lipids, and body mass index were assessed in all follow-ups. Cardiovascular risk factor trajectories from childhood to midlife were identified using latent class growth mixture modeling. Cognitive testing was performed in 2026 participants 34 to 49 years of age using a computerized test. The associations of the cardiovascular risk factor trajectories and cognitive performance were studied for individual cardiovascular risk factors and cardiovascular risk factor accumulation. Results: Consistently high systolic blood pressure (beta=-0.262 SD [95% CI, -0.520 to -0.005]) and serum total cholesterol (beta=-0.214 SD [95% CI, -0.365 to -0.064]) were associated with worse midlife episodic memory and associative learning compared with consistently low values. Obesity since childhood was associated with worse visual processing and sustained attention (beta=-0.407 SD [95% CI, -0.708 to -0.105]) compared with normal weight. An inverse association was observed for the cardiovascular risk factor accumulation with episodic memory and associative learning (P for trend=0.008; 3 cardiovascular risk factors: beta=-0.390 SD [95% CI, -0.691 to -0.088]), with visual processing and sustained attention (P for trend Conclusions: Longitudinal elevated systolic blood pressure, high serum total cholesterol, and obesity from childhood to midlife were inversely associated with midlife cognitive performance. It is important to note that the higher the number of cardiovascular risk factors, the worse was the observed cognitive performance. Therefore, launching preventive strategies against cardiovascular risk factors beginning from childhood might benefit primordial promotion of cognitive health in adulthood.Peer reviewe

    Neurofunctional Abnormalities during Sustained Attention in Severe Childhood Abuse

    <div><p>Childhood maltreatment is associated with adverse affective and cognitive consequences including impaired emotion processing, inhibition and attention. However, the majority of functional magnetic resonance imaging (fMRI) studies in childhood maltreatment have examined emotion processing, while very few studies have tested the neurofunctional substrates of cognitive functions and none of attention. This study investigated the association between severe childhood abuse and fMRI brain activation during a parametric sustained attention task with a progressively increasing load of sustained attention in 21 medication-naĂŻve, drug-free young people with a history of childhood abuse controlling for psychiatric comorbidities by including 19 psychiatric controls matched for psychiatric diagnoses, and 27 healthy controls. Behaviorally, the participants exposed to childhood abuse showed increased omission errors in the task which correlated positively trend-wise with the duration of their abuse. Neurofunctionally, the participants with a history of childhood abuse, but not the psychiatric controls, displayed significantly reduced activation relative to the healthy controls during the most challenging attention condition only in typical attention regions including left inferior and dorsolateral prefrontal cortex, insula and temporal areas. We therefore show for the first time that severe childhood abuse is associated with neurofunctional abnormalities in key ventral frontal-temporal sustained attention regions. The findings represent a first step towards the delineation of abuse-related neurofunctional abnormalities in sustained attention, which may help in the development of effective treatments for victims of childhood abuse.</p></div

    This report reviews the effectiveness of early childhood stimulation interventions in developing countries. The report aims to answer the questions: What works in terms of early stimulation for young children in developing countries? For whom and under what conditions do these programs work and why do they work. The report is divided into several sections. Firstly, a brief discussion of the importance of early stimulation for young children in developing countries is provided. Secondly, the methods used to identify and characterize studies are provided and a review of randomized or quasi-experimental trials is presented. Thirdly, a review of the evidence for who benefits most from early interventions is presented followed by a review of program characteristics that affect the success of interventions and an examination of potential mechanisms through which interventions achieve their effects.     Social re-orientation and brain development: An expanded and updated view.

    Social development has been the focus of a great deal of neuroscience based research over the past decade. In this review, we focus on providing a framework for understanding how changes in facets of social development may correspond with changes in brain function. We argue that (1) distinct phases of social behavior emerge based on whether the organizing social force is the mother, peer play, peer integration, or romantic intimacy; (2) each phase is marked by a high degree of affect-driven motivation that elicits a distinct response in subcortical structures; (3) activity generated by these structures interacts with circuits in prefrontal cortex that guide executive functions, and occipital and temporal lobe circuits, which generate specific sensory and perceptual social representations. We propose that the direction, magnitude and duration of interaction among these affective, executive, and perceptual systems may relate to distinct sensitive periods across development that contribute to establishing long-term patterns of brain function and behavior

    Assessing the three attentional networks and vigilance in the adolescence stages

    Attention involves three functionally and neuroanatomically distinct neural networks: alerting, orienting, and executive control. This study aimed to assess the attentional networks and vigilance in adolescents aged between 10 and 19 years using the attentional network test for interaction and vigilance (ANTI-V). One hundred and eighty-two adolescents divided into three groups (early adolescents, middle adolescents, late adolescents) participated in the study. The results indicate that after age 15, adolescents adopt a more conservative response strategy and increase the monitoring of self-errors. All the attentional networks seem to continue to develop during the age range considered in this study (10–19 y). Performance improved from early adolescence to middle adolescence and began to stabilize in late adolescence. Moreover, a low level of vigilance seems to harm alerting and orienting abilities
