    Assessing temporal modulation sensitivity using electrically evoked auditory steady state responses

    AbstractTemporal cues are important for cochlear implant (CI) users when listening to speech. Users with greater sensitivity to temporal modulations show better speech recognition and modifications to stimulation parameters based on modulation sensitivity have resulted in improved speech understanding. Behavioural measures of temporal sensitivity require cooperative participants and a large amount of time. These limitations have motivated the desire for an objective measure with which to appraise temporal sensitivity for CI users.Electrically evoked auditory steady state responses (EASSRs) are neural responses to periodic electrical stimulation that have been used to predict threshold (T) levels. In this study we evaluate the use of EASSRs as a tool for assessing temporal modulation sensitivity.Modulation sensitivity was assessed behaviourally using modulation detection thresholds (MDTs) for a 20 Hz rate. On the same stimulation sites, EASSRS were measured using sinusoidally amplitude modulated pulse trains at 4 and 40 Hz. Measurements were taken using a bipolar configuration on 12 electrode pairs over 5 participants. Results showed that EASSR amplitudes and signal-to-noise ratios (SNRs) were significantly related to the MDTs. Larger EASSRs corresponded with sites of improved modulation sensitivity. This relation was driven by across-subject variation. This result indicates that EASSRs may be used as an objective measure of site-specific temporal sensitivity for CI users

    Characterization of cochlear implant artifacts in electrically evoked auditory steady-state responses

    AbstractObjectiveElectrically evoked auditory steady-state responses (EASSRs) are neural potentials measured in the electroencephalogram (EEG) in response to periodic pulse trains presented, for example, through a cochlear implant (CI). EASSRs could potentially be used for objective CI fitting. However, EEG signals are contaminated with electrical CI artifacts. In this paper, we characterized the CI artifacts for monopolar mode stimulation and evaluated at which pulse rate, linear interpolation over the signal part contaminated with CI artifact is successful.MethodsCI artifacts were characterized by means of their amplitude growth functions and duration.ResultsCI artifact durations were between 0.7 and 1.7ms, at contralateral recording electrodes. At ipsilateral recording electrodes, CI artifact durations are range from 0.7 to larger than 2ms.ConclusionAt contralateral recording electrodes, the artifact was shorter than the interpulse interval across subjects for 500pps, which was not always the case for 900pps.SignificanceCI artifact-free EASSRs are crucial for reliable CI fitting and neuroscience research. The CI artifact has been characterized and linear interpolation allows to remove it at contralateral recording electrodes for stimulation at 500pps

    The Electrically Evoked Compound Action Potential: From Laboratory to Clinic

    The electrically evoked compound action potential (eCAP) represents the synchronous firing of a population of electrically stimulated auditory nerve fibers. It can be directly recorded on a surgically exposed nerve trunk in animals or from an intra-cochlear electrode of a cochlear implant. In the past two decades, the eCAP has been widely recorded in both animals and clinical patient populations using different testing paradigms. This paper provides an overview of recording methodologies and response characteristics of the eCAP, as well as its potential applications in research and clinical situations. Relevant studies are reviewed and implications for clinicians are discussed

    Role of Cortical Auditory Evoked Potentials in Reducing the Age at Hearing Aid Fitting in Children With Hearing Loss Identified by Newborn Hearing Screening.

    Recording of free-field cortical auditory evoked potential (CAEP) responses to speech tokens was introduced into the audiology management for infants with a permanent childhood hearing impairment (PCHI) during 2011-2015 at a U.K. service. Children with bilateral PCHI were studied from two sequential cohorts. Thirty-four children had followed an audiology pathway prior to CAEP introduction, and 44 children followed a pathway after the introduction of CAEP and were tested with unaided and aided CAEP responses. Data analysis explored the age of diagnosis, hearing aid fitting, and referral for cochlear implant (CI) assessment for each of these groups. CAEP offered a novel educative process for the parents and audiologists supporting decision-making for hearing aid fitting and CI referral. Delays in hearing aid fitting and CI referral were categorized as being due to the audiologist's recommendation or parental choice. Results showed that the median age of hearing aid fitting prior to CAEP introduction was 9.2 months. After the inclusion of CAEP recording in the infant pathways, it was 3.9 months. This reduction was attributable to earlier fitting of hearing aids for children with mild and moderate hearing losses, for which the median age fell from 19 to 5 months. Children with profound hearing loss were referred for CI assessment at a significantly earlier age following the introduction of CAEP. Although there has also been a national trend for earlier hearing aid fitting in children, the current study demonstrates that the inclusion of CAEP recording in the pathway facilitated earlier hearing aid fitting for milder impairments

    The development of cortical processing of speech differs between children with cochlear implants and normal hearing and changes with parental singing

    Objective: The aim of the present study was to investigate speech processing development in children with normal hearing (NH) and cochlear implants (CI) groups using a multifeature event-related potential (ERP) paradigm. Singing is associated to enhanced attention and speech perception. Therefore, its connection to ERPs was investigated in the CI group. Methods: The paradigm included five change types in a pseudoword: two easy- (duration, gap) and three difficult-to-detect (vowel, pitch, intensity) with CIs. The positive mismatch responses (pMMR), mismatch negativity (MMN), P3a and late differentiating negativity (LDN) responses of preschoolers (below 6 years 9 months) and schoolchildren (above 6 years 9 months) with NH or CIs at two time points (T1, T2) were investigated with Linear Mixed Modeling (LMM). For the CI group, the association of singing at home and ERP development was modeled with LMM. Results: Overall, responses elicited by the easy- and difficult to detect changes differed between the CI and NH groups. Compared to the NH group, the CI group had smaller MMNs to vowel duration changes and gaps, larger P3a responses to gaps, and larger pMMRs and smaller LDNs to vowel identity changes. Preschoolers had smaller P3a responses and larger LDNs to gaps, and larger pMMRs to vowel identity changes than schoolchildren. In addition, the pMMRs to gaps increased from T1 to T2 in preschoolers. More parental singing in the CI group was associated with increasing pMMR and less parental singing with decreasing P3a amplitudes from T1 to T2. Conclusion: The multifeature paradigm is suitable for assessing cortical speech processing development in children. In children with CIs, cortical discrimination is often reflected in pMMR and P3a responses, and in MMN and LDN responses in children with NH.     Gochlear implants from model to patients

    Cochlear implants (CI) are by now an accepted form of rehabilitation for profoundly deaf patients. CI users regain part of their hearing by direct electrical stimulation of the auditory nerve. With modern cochlear implants most users are able to achieve open-set speech understanding and are able to use the telephone. There are, however, still a lot of unanswered questions regarding the optimal design, stimulation paradigms, fitting methods and objective measurements. With the development of a realistic computer model of the implanted cochlea, as described in this thesis, these questions are analyzed from a fundamental perspective. This realistic model enables the analysis of clinical devices and gives insight in discrepancies between human and animal results. Insights gained from the model are used to improve clinical practice. Based on the model outcomes presented the characteristics of an improved electrode design were defined, and finally tested in a temporal bone study.UBL - phd migration 201
