    A qualitative study of parents' experiences using family support services: applying the concept of surface and depth

    UK policy and practice endorses family support for child well-being. Achieving such support requires multi-agency approaches, that consider all aspects of parents’ and children’s lives and which offer practical, social and emotional help. The potential for services to make a positive impact on parents and their families will depend in part on the level and nature of engagement. In this paper a case is made for the application of the two-part ‘surface and depth’ concept for understanding how practitioners engage with families and how they might improve the chances of supporting sustainable differences for parents and families. To illustrate, qualitative data from a review of family centre support provided by a north of England local authority, are presented. The review was commissioned to explore why families often need to re-engage with intensive support services. Data are drawn from interviews with parents (n=18, recruited following a survey of all those registered with the service during April – May 2009) and discussions with family centre support workers (n=4) and following thematic analysis three dominant themes emerged: ‘resources available’, ‘staff approach’ and ‘real life’, were appraised in light of the ‘surface and depth’ concept. Much of the work with parents effectively dealt with pressing needs. This felt gratifying for both parent and worker and supported immediate service engagement. However, each noted that the more complex issues in parents’ lives went unchallenged and thus the sustainability of progress in terms of parenting practice was questionable. A ‘strengths focused’ approach by staff, that understood needs in the context of parents’ ‘real life’ circumstances was important to parent engagement. Thus, longer term benefits from family support requires practitioners to work with parents to problem solve immediate issues whilst also digging deeper to acknowledge and seek to resolve the more complex challenges parents face in their real lives

    Client Privacy and Social Work: A Comparison By Agency Function

    This study examines the effect of agency function or purpose on the handling of client privacy issues in social work agencies. Practitioners working in public and private agencies were compared. The data revealed that, more than those in private agencies, social workers in public agencies: (1) thought that their work would be more affected if they could not rely on outside sources for information about clients; (2) were more often requested to supply information about clients to outside sources; and (3) were more likely to reveal information about clients as a form of ethical dilemma resolution. It is suggested that compliance from clients is the primary social reward sought by practitioners in public agencies and that client information is used as an instrument of power in obtaining it. Practitioners in private agencies, on the other hand, seek different social rewards and tend to be more protective of client privacy

    Chinese Medicine Student Clubs in Taipei, Taiwan

    This thesis focuses on a communal form of transmission of Chinese medicine in contemporary Taiwan: Chinese medicine university student clubs. Offering fundamental Chinese medicine curricula to students and the interested public, the student clubs used to serve as a direct educational steppingstone towards licensed practice. Recent changes in medical education policy, however, made a university degree in Chinese medicine a requirement, thereby pushing informal ways of knowledge transmission into the realm of lay activity. Nevertheless, the clubs remain active and still serve as a community for people interested in Chinese medicine, including those wanting to pursue it professionally. Based on field research conducted in two such university clubs in Taipei in early 2018, this thesis first outlines the challenges and tensions faced and negotiated by those club members with professional ambitions. Not (yet) enrolled in “official” Chinese medicine programs at university but already deeply engaged in learning, they constitute a group of people rarely represented in academic literature, namely those just orienting themselves towards becoming Chinese medicine physicians. These processes of orientation and becoming are shaped by organizational, economic, and epistemological pressures and embedded in transnational movements, imaginaries, and regulatory regimes. Secondly, the thesis examines the function and position of the clubs in the changing landscape of Chinese medical education in Taiwan, as well as in the wider field of transmission of Chinese medicine. I argue that they foster continued interest in Chinese medicine in an environment that has favored biomedicine since the Japanese colonial era and that they, although through paths more winded than before, still contribute to the reproduction of professional Chinese medical expertise. In addition, they provide space for communal forms of healthcare. Lastly, they contribute to the maintenance of everyday healthcare competence in the wider public, or what Arthur Kleinman (1980) has called the “popular sector of healthcare.

    The Use of Implants vs. Endodontics: Practitioner Attitudes in 2007

    The purpose of this study was to determine what factors influence dentists when deciding whether to place an implant or perform endodontic retreatment. Null hypothesis: Dentists today are no more likely to place an implant than to perform endodontic retreatment. A twelve-item questionnaire was mailed to 525 general dentists and specialists who are members of the Richmond Dental Society. Response rate was 61.9%. The questionnaire included items on demographics, practice profile and cases of failing endodontic therapy which participants were asked to consider and chose between endodontic retreatment or implant placement. The relationship between the treatment choices and the characteristics of each dentist was assessed using a chi-square analyses and logistic regression analyses. Generally practitioners preferred retreatment (66%). This preference for retreatment varied between 25.5% and 85.9%. Associations were found between years of experience and implant use. In those practitioners with 10 years or less experience the odds of choosing an implant were higher than practitioners with more than 10 years of experience. Associations were also found between those who place implants vs. those who do not. The odds of choosing an implant verses retreatment were lower in those who did not place implants versus those who did. In conclusion, it appears that clinicians participating in this study in general still consider endodontic retreatment to be a viable treatment option prior to implant placement. However, this varies greatly with years of experience and the use of implants in practice

    Comparing Moral Reasoning across Graduate Occupational and Physical Therapy Students and Practitioners

    Practicing clinicians must use moral reasoning to solve ethical problems and combat moral distress. Development of moral reasoning in occupational therapy (OT) and physical therapy (PT) students has been significantly under researched. The purpose of this study was to analyze the differences in moral reasoning between first year and second year OT and PT students and between students and OT and PT practitioners. Investigators utilized a cross-sectional study design with a convenience sample of University of Indianapolis OT and PT students and a combination of convenience and snowball sampling to recruit licensed OT and PT practitioners. One hundred and fifty-four participants completed the Defining Issues Test - 2 (DIT-2; Rest et al., 1999) survey. Comparisons of N2 scores using t-tests found no differences between groups in moral reasoning schema (one’s preferred way of approaching moral issues, divided into stratified developmental levels). However, Pearson’s Chi-Square analysis for a comparison of all students to all practitioners for moral reasoning patterns (one’s ability to discriminate between types of moral reasoning schemas when presented with a complex moral dilemma) was significant between students (transitional) and practitioners (consolidated), with the greatest difference between second-year students and practitioners. Continual expansion of ethics content, including interjecting clinical experiences into the classroom, within OT and PT graduate programs may promote moral reasoning pattern development with carryover into practice. Clinical experiences provide real-world opportunities necessary to progress students from transitional to consolidated thinking patterns. To improve ethics education, authors recommend active learning strategies and ethics mentorship throughout clinical experiences

    Participation Motivation in Martial Artists in the West Midlands Region of England

    The objectives were to identify the participation motivations and the perceived importance of certain participation factors in martial artists in the West Midlands, England, UK. A 28-item adapted version of the Participation Motivation Questionnaire with additional demographic questions was distributed to 30 martial arts clubs in the West Midlands region. Eight questions that assessed the perceived importance for participation of progression through grades, learning self defence skills, technical ability of instructors, cost of participating, development of confidence, underpinning philosophy and instructional style were included. Seventy-five questionnaires were returned from a total of 11 clubs from across representing practitioners in Tai Chi, Karate, Kung fu, Aikido, Jeet Kune Do, British Free Fighting, Taekwon-Do and Jujitsu. Results indicated that the rank order in terms of participation motives was: 1- Affiliation; 2-Friendship; 3-Fitness; 4-Reward/status; 5-Competition; 6-Situational and 7-Skill development. Participants who trained for more than 4 hours per week placed greater importance on the underpinning philosophy of the martial art. Findings suggest that whilst there is a gender discrepancy in participation level, once engaged, females were equally committed to weekly training. The ‘style’ of the instructor is of paramount importance for enhancing student motivation to participate. High volume practitioners would appear to be fully immersed in the holistic appreciation of the martial art through increased value placed on its underpinning philosophy

Criticisms of case-lawyers appeared in English journals in the late 1820s, then in the United States, usually from those who believed that every lawyer needed to know and understand the unchanging principles of the common law in order to resolve issues not found in the reported cases. After the Civil War, expressions of concern about caselawyers increased with the significant growth in the amount of published law after private companies entered the legal publishing market. By the turn of the twentieth century, it was generally acknowledged the number of cases had made it impossible for attorneys to not focus on locating precedents. In the twentieth century most references to case-lawyers were historical, even as the amount of published law facing lawyers continued to grow

    Dr. O\u27Sullivan traces the process leading to the passage of the 1968 British Abortion Act and the consequences of that act with a view to helping the American medical profession learn from the British experience. This article discusses the effect that legalized abortion has had upon the people, and particularly the physicians, in England. Although New York State, among others, has now legalized abortion, it has not had sufficient experience with it to adequately assess its effects. But the English have had sufficient experience with it, and its effects there indicate that attempts to legalize abortion demand close watching.
