    Fractal tool; Calculating 3D fractal dimension of brain regions for a Dementia classification problem

    Η άνοια είναι ένα σύνδρομο που είναι κοινό στους ηλικιωμένους και ο ρυθμός εμφάνισής του αυξάνεται. Η πλειονότητα των μελετών επικεντρώνεται στην εύρεση βιοδεικτών για διάγνωση, ενώ η πρόληψη και η παρακολούθηση της ανάπτυξης είναι ακόμη ένα αδύνατο έργο. Σήμερα, η έρευνα για την άνοια περιορίζεται στη νευροαπεικόνιση, καθώς είναι μια μη επεμβατική τεχνολογία. Υπάρχει μια πληθώρα νευροαπεικονιστικών εργαλείων τα οποία βελτιστοποιούν την απεικόνιση μιας εισαγόμενης εικόνας μέσω της επεξεργασίας εικόνας ή ακόμη και συνεισφέρουν στη λήψη ιατρικών αποφάσεων μέσω της ανάλυσης της εικόνας. Ωστόσο, η ατροφία του εγκεφάλου στην άνοια δεν έχει ακόμη χαρακτηριστεί σωστά. Η νευροαπεικόνιση στοχεύει κυρίως στην παρακολούθηση της μείωσης του όγκου του εγκεφάλου και λιγότερο σε άλλες δομικές υφές. Τέτοια χαρακτηριστικά είναι η μορφοκλασματική διάσταση και η ύπαρξη κενών στις εγκαφαλικές δομές (lacunarity). Μερικά εργαλεία νευροαπεικόνισης υπολογίζουν τη διάσταση του φράκταλ και του lacunarity για ολόκληρο τον όγκο του εγκεφάλου. Ωστόσο, η λειτουργικότητά τους δεν περιλαμβάνει αυτοματοποιημένη εκτίμηση πολλαπλών εικόνων και, συνεπώς, δημιουργία συνόλων δεδομένων (datasets). Υπολογίζουν απλώς αυτά τα χαρακτηριστικά για συγκεκριμένες δομές, ενώ δεν εκτελούν τα απαραίτητα βήματα επεξεργασίας εικόνας. Δεδομένου ότι τα περισσότερα από αυτά τα εργαλεία εξειδικεύονται σε συγκεκριμένες εργασίες, δεν υπάρχει μια ολιστική μέθοδος που εισάγει πολλαπλά δεδομένα απεικόνισης και εξάγει μετρήσεις για τη μορφοκλασματική διάσταση και το lacunarity. Αυτή η μελέτη παρουσιάζει ένα εργαλείο γενικού σκοπού για την αυτοματοποιημένη επεξεργασία εικόνας, την τμηματοποίηση εικόνας, την εκτίμηση της μορφοκλασματική διάσταση, του lacunarity και άλλων υφών που προέρχονται από τον υπολογισμό της μορφοκλασματικής διάστασης. Τα εξαγόμενα αρχεία είναι σύνολα δεδομένων με τέτοιες μετρήσεις. Έπειτα γίνεται μια ταξινόμηση υγειών και ατόμων με άνοια η οποία επιβεβαιώνει τη χρησιμότητα του λογισμικού. Παρόλο που υπήρχαν περιορισμοί στην απόκτηση δεδομένων, πραγματοποιήθηκε αποτελεσματική ταξινόμηση με μηχανές διανυσματικής υποστήριξης. Για αρκετές περιοχές του εγκεφάλου, η ακρίβεια Fbeta score κυμαινόταν μεταξύ 95% και 100% υπερισχύοντας όλων των άλλων μεθόδων. Ωστόσο, η διάγνωση της άνοιας απαιτεί ένα μοντέλο που διαχωρίζει αποτελεσματικά τις περιοχές του εγκεφάλου για όλες τις κλάσεις. Σε αυτή τη διατριβή, εκπαιδεύτηκαν δύο τέτοια μοντέλα, ένα κάθε ομάδα. The majority of the studies are focused on finding biomarkers for diagnosis, while prevention and monitoring of the development is yet an impossible task. Nowadays, research on dementia is limited to neuroimaging as it is a non-invasive technology. There is a plethora of neuroimaging tools which optimize the virtualization of an imported image through image processing or even contribute medical decision making through image analysis. Still, brain atrophy in Dementia is yet to be characterized properly. Neuroimaging mainly aims on volume decline of brain volume and less on other structural textures. Such features are fractal dimension and lacunarity. Some neuroimaging tools calculate fractal dimension and lacunarity for whole brain volume. However, their functionality does not include automated estimation of multiple images and thus creation of datasets. They just compute these features for given structures while not performing the necessary image processing steps. As most of these tools are specialized in specific tasks, there is not a holistic method that inputs multiple imaging data and exports measurements for fractal dimension, lacunarity. This study presents a general purpose tool for automated image processing, image segmentation, estimation of fractal dimension, lacunarity and other textures that are derived from the calculation of fractal dimension. The exported files are datasets with such measurements. Finally, a classification between healthy and dementia subjects underlines the utility of the software. Even though there were limitations to data acquirement, efficient classification with SVM models has been performed. For several brain regions, the ranging Fbeta score accuracy was 97% - 100 % outperforming all other methods. However, diagnosis of Dementia requires a unique prior model which efficiently segment brain regions for any given class. In this thesis, two models were trained, one for each group. Nevertheless, the final results reveal the necessity of fractal properties as a tool for Dementia classification and monitoring of the Dementia development. Also, the utility of the software can be extended to any structural neuroimaging problem such as detection of cancer

Приведено огляд сучасних інженерних методів автоматичної діагностики хвороби Альцгеймера за зображеннями магнітно-резонансної томографії та позитронно-еміснійної томографії. Наведено алгоритм методу відбору ознак, розроблений з використанням статистичних критеріїв. Розроблено и експериментально досліджено метод на базі математичного апарату нечіткої логіки для автоматизованої діагностики хвороби Альцгеймера.The problem of Alzheimer disease diagnosis is considered. The review of current existing automated methods of Alzheimer disease diagnosis using MRI and PET/SPECT images is given. Advantages and disadvantages are presented. Problem of potential redundancy of Alzheimer disease features, which are used in modern diagnosis systems, is considered. A feature selection algorithm was developed using statistical tests. The new approach based on a fuzzy logic application for the computer-aided diagnosis of Alzheimer’s disease is developed and experimentally investigated.Рассмотрено проблематику диагностики болезни Альцгеймера. Приведен обзор современных инженерных методов автоматизированой диагностики болезни Альцгеймреа по изображениям магнитно-резонансной томографии и позитронно-эмисионной томографии мозга человека. Приведен алгоритм метода отбора признаков, разработанный с применением статистических критериев. Разработан и экспериментально исследован метод на базе математического аппарата нечеткой логики для автоматизированной диагностики болезни Альцгеймера

    Differentiation of Alzheimer's disease dementia, mild cognitive impairment and normal condition using PET-FDG and AV-45 imaging : a machine-learning approach

    Nous avons utilisé l'imagerie TEP avec les traceurs F18-FDG et AV45 en conjonction avec les méthodes de classification du domaine du "Machine Learning". Les images ont été acquises en mode dynamique, une image toutes les 5 minutes. Les données ont été transformées par Analyse en Composantes Principales et Analyse en Composantes Indépendantes. Les images proviennent de trois sources différentes: la base de données ADNI (Alzheimer's Disease Neuroimaging Initiative) et deux protocoles réalisés au sein du centre TEP de l'hôpital Purpan. Pour évaluer la performance de la classification nous avons eu recours à la méthode de validation croisée LOOCV (Leave One Out Cross Validation). Nous donnons une comparaison entre les deux méthodes de classification les plus utilisées, SVM (Support Vector Machine) et les réseaux de neurones artificiels (ANN). La combinaison donnant le meilleur taux de classification semble être SVM et le traceur AV45. Cependant les confusions les plus importantes sont entre les patients MCI et les sujets normaux. Les patients Alzheimer se distinguent relativement mieux puisqu'ils sont retrouvés souvent à plus de 90%. Nous avons évalué la généralisation de telles méthodes de classification en réalisant l'apprentissage sur un ensemble de données et la classification sur un autre ensemble. Nous avons pu atteindre une spécificité de 100% et une sensibilité supérieure à 81%. La méthode SVM semble avoir une meilleure sensibilité que les réseaux de neurones. L'intérêt d'un tel travail est de pouvoir aider à terme au diagnostic de la maladie d'Alzheimer.We used PET imaging with tracers F18-FDG and AV45 in conjunction with the classification methods in the field of "Machine Learning". PET images were acquired in dynamic mode, an image every 5 minutes.The images used come from three different sources: the database ADNI (Alzheimer's Disease Neuro-Imaging Initiative, University of California Los Angeles) and two protocols performed in the PET center of the Purpan Hospital. The classification was applied after processing dynamic images by Principal Component Analysis and Independent Component Analysis. The data were separated into training set and test set. To evaluate the performance of the classification we used the method of cross-validation LOOCV (Leave One Out Cross Validation). We give a comparison between the two most widely used classification methods, SVM (Support Vector Machine) and artificial neural networks (ANN) for both tracers. The combination giving the best classification rate seems to be SVM and AV45 tracer. However the most important confusion is found between MCI patients and normal subjects. Alzheimer's patients differ somewhat better since they are often found in more than 90%. We evaluated the generalization of our methods by making learning from set of data and classification on another set . We reached the specifity score of 100% and sensitivity score of more than 81%. SVM method showed a bettrer sensitivity than Artificial Neural Network method. The value of such work is to help the clinicians in diagnosing Alzheimer's disease

    A Machine Learning Classification Framework for Early Prediction of Alzheimer’s Disease

    People today, in addition to their concerns about getting old and having to go through watching themselves grow weak and wrinkly, are facing an increasing fear of dementia. There are around 47 million people affected by dementia worldwide and the cost associated with providing them health and social care support is estimated to reach 2 trillion by 2030 which is almost equivalent to the 18th largest economy in the world. The most common form of dementia with the highest costs in health and social care is Alzheimer’s disease, which gradually kills neurons and causes patients to lose loving memories, the ability to recognise family members, childhood memories, and even the ability to follow simple instructions. Alzheimer’s disease is irreversible, unstoppable and has no known cure. Besides being a calamity to affected patients, it is a great financial burden on health providers. Health care providers also face a challenge in diagnosing the disease as current methods used to diagnose Alzheimer’s disease rely on manual evaluations of a patient’s medical history and mental examinations such as the Mini-Mental State Examination. These diagnostic methods often give a false diagnosis and were designed to identify Alzheimer’s after stage two when the part of all symptoms are evident. The problem is that clinicians are unable to stop or control the progress of Alzheimer’s disease, because of a lack of knowledge on the patterns that triggered the development of the disease. In this thesis, we explored and investigated Alzheimer’s disease from a computational perspective to uncover different risk factors and present a strategic framework called Early Prediction of Alzheimer’s Disease Framework (EPADf) that would give a future prediction of early-onset Alzheimer’s disease. Following extensive background research that resulted in the formalisation of the framework concept, prediction approaches, and the concept of ranking the risk factors based on clinical instinct, knowledge and experience using mathematical reasoning, we carried out experiments to get further insight and investigate the disease further using machine learning models. In this study, we used machine learning models and conducted two classification experiments for early prediction of Alzheimer’s disease, and one ranking experiment to rank its risk factors by importance. Besides these experiments, we also presented two logical approaches to search for patterns in an Alzheimer’s dataset, and a ranking algorithm to rank Alzheimer’s disease risk factors based on clinical evaluation. For the classification experiments we used five different Machine Learning models; Random Forest (RF), Random Oracle Model (ROM), a hybrid model combined of Levenberg-Marquardt neural network and Random Forest, combined using Fischer discriminate analysis (H2), Linear Neural Networks (LNN), and Multi-layer Perceptron Model (MLP). These models were deployed on a de-identified multivariable patient’s data, provided by the ADNI (Alzheimer’s disease Neuroimaging Initiative), to illustrate the effective use of data analysis to investigate Alzheimer’s disease biological and behavioural risk factors. We found that the continues enhancement of patient’s data and the use of combined machine learning models can provide an early cost-effective prediction of Alzheimer’s disease, and help in extracting insightful information on the risk factors of the disease. Based on this work and findings we have developed the strategic framework (EPADf) which is discussed in more depth in this thesis
