    Segmentation of pelvic structures from preoperative images for surgical planning and guidance

    Prostate cancer is one of the most frequently diagnosed malignancies globally and the second leading cause of cancer-related mortality in males in the developed world. In recent decades, many techniques have been proposed for prostate cancer diagnosis and treatment. With the development of imaging technologies such as CT and MRI, image-guided procedures have become increasingly important as a means to improve clinical outcomes. Analysis of the preoperative images and construction of 3D models prior to treatment would help doctors to better localize and visualize the structures of interest, plan the procedure, diagnose disease and guide the surgery or therapy. This requires efficient and robust medical image analysis and segmentation technologies to be developed. The thesis mainly focuses on the development of segmentation techniques in pelvic MRI for image-guided robotic-assisted laparoscopic radical prostatectomy and external-beam radiation therapy. A fully automated multi-atlas framework is proposed for bony pelvis segmentation in MRI, using the guidance of MRI AE-SDM. With the guidance of the AE-SDM, a multi-atlas segmentation algorithm is used to delineate the bony pelvis in a new \ac{MRI} where there is no CT available. The proposed technique outperforms state-of-the-art algorithms for MRI bony pelvis segmentation. With the SDM of pelvis and its segmented surface, an accurate 3D pelvimetry system is designed and implemented to measure a comprehensive set of pelvic geometric parameters for the examination of the relationship between these parameters and the difficulty of robotic-assisted laparoscopic radical prostatectomy. This system can be used in both manual and automated manner with a user-friendly interface. A fully automated and robust multi-atlas based segmentation has also been developed to delineate the prostate in diagnostic MR scans, which have large variation in both intensity and shape of prostate. Two image analysis techniques are proposed, including patch-based label fusion with local appearance-specific atlases and multi-atlas propagation via a manifold graph on a database of both labeled and unlabeled images when limited labeled atlases are available. The proposed techniques can achieve more robust and accurate segmentation results than other multi-atlas based methods. The seminal vesicles are also an interesting structure for therapy planning, particularly for external-beam radiation therapy. As existing methods fail for the very onerous task of segmenting the seminal vesicles, a multi-atlas learning framework via random decision forests with graph cuts refinement has further been proposed to solve this difficult problem. Motivated by the performance of this technique, I further extend the multi-atlas learning to segment the prostate fully automatically using multispectral (T1 and T2-weighted) MR images via hybrid \ac{RF} classifiers and a multi-image graph cuts technique. The proposed method compares favorably to the previously proposed multi-atlas based prostate segmentation. The work in this thesis covers different techniques for pelvic image segmentation in MRI. These techniques have been continually developed and refined, and their application to different specific problems shows ever more promising results.Open Acces

    Understanding Nanoparticle Toxicity to Direct a Safe-by-Design Approach in Cancer Nanomedicine

    Nanomedicine is a rapidly growing field that uses nanomaterials for the diagnosis, treatment and prevention of various diseases, including cancer. Various biocompatible nanoplatforms with diversified capabilities for tumor targeting, imaging, and therapy have materialized to yield individualized therapy. However, due to their unique properties brought about by their small size, safety concerns have emerged as their physicochemical properties can lead to altered pharmacokinetics, with the potential to cross biological barriers. In addition, the intrinsic toxicity of some of the inorganic materials (i.e., heavy metals) and their ability to accumulate and persist in the human body has been a challenge to their translation. Successful clinical translation of these nanoparticles is heavily dependent on their stability, circulation time, access and bioavailability to disease sites, and their safety profile. This review covers preclinical and clinical inorganic-nanoparticle based nanomaterial utilized for cancer imaging and therapeutics. A special emphasis is put on the rational design to develop non-toxic/safe inorganic nanoparticle constructs to increase their viability as translatable nanomedicine for cancer therapies

    Forecasting Longitudinal Changes in Oropharyngeal Tumor Volume, Position, and Morphology during Image-Guided Radiation Therapy

    The purpose of this work was to generate, evaluate, and compare models that predict longitudinal changes in oropharyngeal tumor volume, position, and morphology during radiation therapy. One volume, one position, and two morphology (size, shape, and position) feature vectors were used to describe 35 oropharyngeal gross tumor volumes (GTVs) during radiation therapy. The two morphology feature vectors comprised the coordinates of the GTV centroids and one of two shape descriptors. One shape descriptor was based on radial distances between the GTV centroid and 614 surface landmarks. The other was based on a spherical harmonic decomposition of these distances. For a training set of patients, the changes in feature vectors during treatment were represented by static, linear, mean, and median models along with two models derived from principal component analysis. The error of these models in forecasting the GTV volume, position, and morphology of a test patient was evaluated using leave-one-out cross-validation, and the accuracy of the models were compared with Wilcoxon signed-rank tests. The effect on accuracy of adjusting model parameters at 1, 2, 3, or 5 time points (“adjustment points”) was also evaluated. Including a single adjustment point improved the accuracy in forecasting the volume and position by 12.9% – 30.0% and 27.0% – 34.1%, respectively. For the two morphology feature vectors, a single adjustment point improved accuracy by 27.5 – 33.8% and 28.6% – 33.0%. Additional adjustment points further improved accuracy, but with diminishing effects. Non-static models demonstrated greater accuracy than static models with equal numbers of adjustment points. These improvements were small, except for models of the volume (54.2% – 58.0%) for which they were greater than those of adding an adjustment point. For the other three feature vectors, the effect of including an adjustment point was greater than that of selecting a non-static model. Tumor volume, position, and morphology were predicted at each treatment fraction using models that include information from prior patients and/or prior treatment fractions. The predicted tumor morphology can be compared with patient anatomy or dose distributions, thereby providing a more complete depiction of treatment response, influencing clinical decision making, and opening the possibility of anticipatory re-planning

    Heterogeneous volumetric data mapping and its medical applications

    With the advance of data acquisition techniques, massive solid geometries are being collected routinely in scientific tasks, these complex and unstructured data need to be effectively correlated for various processing and analysis. Volumetric mapping solves bijective low-distortion correspondence between/among 3D geometric data, and can serve as an important preprocessing step in many tasks in compute-aided design and analysis, industrial manufacturing, medical image analysis, to name a few. This dissertation studied two important volumetric mapping problems: the mapping of heterogeneous volumes (with nonuniform inner structures/layers) and the mapping of sequential dynamic volumes. To effectively handle heterogeneous volumes, first, we studied the feature-aligned harmonic volumetric mapping. Compared to previous harmonic mapping, it supports the point, curve, and iso-surface alignment, which are important low-dimensional structures in heterogeneous volumetric data. Second, we proposed a biharmonic model for volumetric mapping. Unlike the conventional harmonic volumetric mapping that only supports positional continuity on the boundary, this new model allows us to have higher order continuity C1C^1 along the boundary surface. This suggests a potential model to solve the volumetric mapping of complex and big geometries through divide-and-conquer. We also studied the medical applications of our volumetric mapping in lung tumor respiratory motion modeling. We were building an effective digital platform for lung tumor radiotherapy based on effective volumetric CT/MRI image matching and analysis. We developed and integrated in this platform a set of geometric/image processing techniques including advanced image segmentation, finite element meshing, volumetric registration and interpolation. The lung organ/tumor and surrounding tissues are treated as a heterogeneous region and a dynamic 4D registration framework is developed for lung tumor motion modeling and tracking. Compared to the previous 3D pairwise registration, our new 4D parameterization model leads to a significantly improved registration accuracy.     Validation of in-house knowledge-based planning model for advance-stage lung cancer patients treated using VMAT radiotherapy

    Objectives: Radiotherapy plan quality may vary considerably depending on planner's experience and time constraints. The variability in treatment plans can be assessed by calculating the difference between achieved and the optimal dose distribution. The achieved treatment plans may still be suboptimal if there is further scope to reduce organs-at- risk doses without compromising target coverage and deliverability. This study aims to develop a knowledge-based planning (KBP) model to reduce variability of volumetric modulated arc therapy (VMAT) lung plans by predicting minimum achievable lung volume-dose metrics. Methods: Dosimetric and geometric data collected from 40 retrospective plans were used to develop KBP models aiming to predict the minimum achievable lung dose metrics via calculating the ratio of the residual lung volume to the total lung volume. Model accuracy was verified by replanning 40 plans. Plan complexity metrics were calculated using locally developed script and their effect on treatment delivery was assessed via measurement. Results: The use of KBP resulted in significant reduction in plan variability in all three studied dosimetric parameters V5, V20 and mean lung dose by 4.9% (p = 0.007, 10.8 to 5.9%), 1.3% (p = 0.038, 4.0 to 2.7%) and 0.9 Gy (p = 0.012, 2.5 to 1.6Gy), respectively. It also increased lung sparing without compromising the overall plan quality. The accuracy of the model was proven as clinically acceptable. Plan complexity increased compared to original plans; however, the implication on delivery errors was clinically insignificant as demonstrated by plan verification measurements. Conclusion: Our in-house model for VMAT lung plans led to a significant reduction in plan variability with concurrent decrease in lung dose. Our study also demonstrated that treatment delivery verifications are important prior to clinical implementation of KBP models. Advances in knowledge: In-house KBP models can predict minimum achievable lung dose-volume constraints for advance-stage lung cancer patients treated with VMAT. The study demonstrates that plan complexity could increase and should be assessed prior to clinical implementation

    Respiratory organ motion in interventional MRI : tracking, guiding and modeling

    Respiratory organ motion is one of the major challenges in interventional MRI, particularly in interventions with therapeutic ultrasound in the abdominal region. High-intensity focused ultrasound found an application in interventional MRI for noninvasive treatments of different abnormalities. In order to guide surgical and treatment interventions, organ motion imaging and modeling is commonly required before a treatment start. Accurate tracking of organ motion during various interventional MRI procedures is prerequisite for a successful outcome and safe therapy. In this thesis, an attempt has been made to develop approaches using focused ultrasound which could be used in future clinically for the treatment of abdominal organs, such as the liver and the kidney. Two distinct methods have been presented with its ex vivo and in vivo treatment results. In the first method, an MR-based pencil-beam navigator has been used to track organ motion and provide the motion information for acoustic focal point steering, while in the second approach a hybrid imaging using both ultrasound and magnetic resonance imaging was combined for advanced guiding capabilities. Organ motion modeling and four-dimensional imaging of organ motion is increasingly required before the surgical interventions. However, due to the current safety limitations and hardware restrictions, the MR acquisition of a time-resolved sequence of volumetric images is not possible with high temporal and spatial resolution. A novel multislice acquisition scheme that is based on a two-dimensional navigator, instead of a commonly used pencil-beam navigator, was devised to acquire the data slices and the corresponding navigator simultaneously using a CAIPIRINHA parallel imaging method. The acquisition duration for four-dimensional dataset sampling is reduced compared to the existing approaches, while the image contrast and quality are improved as well. Tracking respiratory organ motion is required in interventional procedures and during MR imaging of moving organs. An MR-based navigator is commonly used, however, it is usually associated with image artifacts, such as signal voids. Spectrally selective navigators can come in handy in cases where the imaging organ is surrounding with an adipose tissue, because it can provide an indirect measure of organ motion. A novel spectrally selective navigator based on a crossed-pair navigator has been developed. Experiments show the advantages of the application of this novel navigator for the volumetric imaging of the liver in vivo, where this navigator was used to gate the gradient-recalled echo sequence

    Optimization of Decision Making in Personalized Radiation Therapy using Deformable Image Registration

    Cancer has become one of the dominant diseases worldwide, especially in western countries, and radiation therapy is one of the primary treatment options for 50% of all patients diagnosed. Radiation therapy involves the radiation delivery and planning based on radiobiological models derived primarily from clinical trials. Since 2015 improvements in information technologies and data storage allowed new models to be created using the large volumes of treatment data already available and correlate the actually delivered treatment with outcomes. The goals of this thesis are to 1) construct models of patient outcomes after receiving radiation therapy using available treatment and patient parameters and 2) provide a method to determine real accumulated radiation dose including the impact of registration uncertainties. In Chapter 2, a model was developed predicting overall survival for patients with hepatocellular carcinoma or liver metastasis receiving radiation therapy. These models show which patients benefit from curative radiation therapy based on liver function, and the survival benefit of increased radiation dose on survival. In Chapter 3, a method was developed to routinely evaluate deformable image registration (DIR) with computer-generated landmark pairs using the scale-invariant feature transform. The method presented in this chapter created landmark sets for comparing lung 4DCT images and provided the same evaluation of DIR as manual landmark sets. In Chapter 4, an investigation was performed on the impact of DIR error on dose accumulation using landmarked 4DCT images as the ground truth. The study demonstrated the relationship between dose gradient, DIR error and dose accumulation error, and presented a method to determine error bars on the dose accumulation process. In Chapter 5, a method was presented to determine quantitatively when to update a treatment plan during the course of a multi-fraction radiation treatment of head and neck cancer. This method investigated the ability to use only the planned dose with deformable image registration to predict dose changes caused by anatomical deformations. This thesis presents the fundamental elements of a decision support system including patient pre-treatment parameters and the actual delivered dose using DIR while considering registration uncertainties
