    Diffuse pulmonary hemosiderosis after exposure to pesticides - A case report

    This report describes the clinical, radiological, microscopical and ligandohistochemical findings in a 17-year-old woman who suffered from an acute onset of pulmonary hemosiderosis after inhalation of pesticides used for the cultivation of strawberries. She complained of headache, dyspnea, rhinitis, weakness and recurrent severe hemoptysis. Chest radiographs revealed bilateral patchy infiltrates, predominantly in the lower parts of both lungs. The consecutive severe anemia was treated by multiple blood transfusions which were repeated every 4-5 days. Open lung biopsies displayed signs of diffuse hemorrhage with hemosiderin-loaded macrophages, some hyaline membranes, focal fibroid deposits with intermingled histiocytes, mild interstitial fibrosis and focal intra-alveolar calcified bodies surrounded by foreign body giant cells. Analysis of endogenous lectins failed to demonstrate expression of binding capacities for maltose, fucose, mannose; lactose and sialic acid. Neither binding capacities for the macrophage-migration-inhibitory factor nor its presence, as analyzed by labeled sarcolectin, could be detected histochemically. The light microscopical findings are consistent with a longer-lasting diffuse pulmonary hemosiderosis; the presence of calcified bodies and foreign body giant cells (including the ligandohistochemical data) argues for a causal role of inhaled substances. The patient's clinical course improved after cyclophosphamide treatment, which restored her ability to work and released her from the need for recurrent blood transfusions

    Effects of carbon source, carbon concentration and culture conditions on in vitro rooting of Pinus pinea L. microshoots

    In stone pine (Pinus pinea L.), clonal propagation via adventitious shoot formation from cotyledons has been reported before but rooting of these shoots is poor. The number of rooted shoots had low frequency limiting the used of micropropagation protocols as a choice for mass propagation of superior genotypes. Therefore the main objective of the present work was to increase the number and quality of roots per shoot in order to ensure the survival and growth of a great number of plants in the acclimation phase. To achieve this, different combinations of carbon source (sucrose or glucose) at different concentrations, under different environmental conditions (temperature and light), were tested in the induction and expression phases of the adventitious root formation. Shoots of different clones obtained via organogenesis have been employed for the experiments. Observations were made on the rooting percentage, root length and number of roots per shoot. The results showed a general increased of the number of roots per shoot and an earlier root formation when glucose was used as a carbon source. However, there were no differences in the percentage of rooting between the carbons sources tested. The best results were obtained using 0.117 M of glucose and dark treatment combined with 19°C during the induction phase of the rhizogenic process. Light and low sugar concentration proved to be beneficial for the expression phase, increasing the root length. A remarkable interclonal difference in the ability to form roots was observed. It was possible to obtain a rooting percentage of more than 75% in several of the tested clones

    Biologically active substances of plant origin represent an essential branch of modern cardiovascular pharmacotherapy. Furthermore, drugs of plant origin have the advantage of weaker adverse effects and lower prices than synthetic drugs. Pharmacological studies and traditional medical literature point to the anti-ischemic and hypotensive effects of the Crocus sativus L. (Iridacea). The major goals of the present study were: (1) to determine the negative dormotrophic properties of a hydroalcoholic extract of saffron on an isolated AV node and (2) to establish the role of nitric oxide in the mediating effects of saffron on the electrophysiological properties of the AV node. This was an experimental study. Selective stimulation protocols were used to independently quantify AV nodal recovery, facilitation and fatigue. We used isolated perfused rabbit AV node preparation, in three groups (N=32); in each group, we assessed the plant's effect in comparison with the control. In the pilot study, we used different concentrations (A=9 x 10-2 mg/L, B=19 x 10-2 mg/L and C=27 x 10-2 mg/L) to select the optimum concentration (19 x 10-2 mg/L) of the hydroalcoholic extract of saffron. Saffron has a depressant effect on basic and rate-dependent properties of the AV node. We observed an increasing AVCT (38.8 ±4 to 41.7 ±4 msec) and FRP (157.6 ±3 to 163.7 ±4 msec). Also saffron increased the amount of facilitation and the magnitude of fatigue (5.9 ±0.3 to 11.1 ±1 msec). The NOS inhibitor (L-NAME) has a preventative effect on the depressant effect of saffron on AVCT and FRP

    The spectroscopic detection of drugs of abuse in fingerprints after development with powders and recovery with adhesive lifters

    The application of powders to fingerprints has long been established as an effective and reliable method for developing latent fingerprints. Fingerprints developed in situ at a crime scene routinely undergo lifting with specialist tapes and are then stored in evidence bags to allow secure transit and also to preserve the chain of evidence. In a previous study we have shown that exogenous material within a fingerprint can be detected using Raman spectroscopy following development with powders and lifting with adhesive tapes. Other reports have detailed the use of Raman spectroscopy to the detection of drugs of abuse in latent fingerprints including cyanoacrylate-fumed fingerprints. This study involves the application of Raman spectroscopy for the analysis of drugs of abuse in latent fingerprints for fingerprints that had been treated with powders and also subsequently lifted with adhesive tapes. Samples of seized ecstasy, cocaine, ketamine and amphetamine were supplied by East Sussex Police and by the TICTAC unit at St. Georges Hospital Tooting. Contaminated fingerprintswere deposited on clean glass slides. The application of aluminium or iron based powders to contaminated fingerprints did not interfere with theRamanspectra obtained for the contaminants. Contaminated fingerprints developed with powders and then lifted with lifting tapes were also examined. The combination of these two techniques did not interfere with the successful analysis. The lifting processwas repeated using hinge lifters. As the hinge lifters exhibited strong Raman bands the spectroscopic analysiswas more complex and an increase in the number of exposures to the detector allowed for improved clarification. Spectral subtraction was performed to remove peaks due to the hinge lifters using OMNIC software. Raman spectra of developed and lifted fingerprints recorded through evidence bags were obtained and it was found that the detection process was not compromised. Although the application of powders did not interfere with the detection process the time taken to locate the contaminant was increased due to the physical presence of more material within the fingerprint

    Severe drug-induced liver injury associated with prolonged use of Linezolid

    This study aims to describe a patient developing concomitant severe liver failure and lactic acidosis after long-term treatment with linezolid. A 55-year-old Caucasian woman developed concomitant severe liver failure and lactic acidosis after a treatment with linezolid for 50 days because of infected hip prosthesis. Other causes of liver failure and lactic acidosis were excluded by extensive diagnostic workup. A liver biopsy showed microvesicular steatosis. As linezolid toxicity was considered to be the cause of the lactic acidosis and the severe hepatic failure, the antibiotic was withdrawn. After 4 days of supportive therapy and hemodialysis, the serum lactate level returned within normal limits. The prothrombin time ratio and thrombocytes recovered within 2 weeks. Bilirubin levels normalized within 14 weeks. Since no other cause could be identified, liver injury was considered to be drug-related. Resolution of the hepatotoxicity occurred after discontinuation of linezolid, supportive treatment measures, and hemodialysis. Both lactic acidosis and microvesicular steatosis after the use of linezolid are related to mitochondrial dysfunction. The Council for International Organizations of Medical Sciences/Roussel Ucalf Causality Assessment Method scale revealed that the adverse drug event was probable. Prolonged exposure to linezolid may induce severe hepatotoxicity. Clinicians should be aware of this possible adverse effect especially in case of long-term treatment

    The Modern Approach To The Complex Therapy Of Demodicosis

    Demodicosis is one of the most common diseases of the skin. Despite the large number of scientific publications on this issue the question of the etiology and pathogenesis of this disease still remains unresolved and the development of more effective methods of treatment and prevention has not only medical but also social significance as patients preserving the working ability have actually long enough to be on outpatient and even inpatient treatment. In addition the long existence of the rash on the face that is the "business card" leads to the secondary sometimes severe neurotic disorders that results in reducing abilities, isolation, unwillingness to be in a team, family, etc.We investigated the effect of complex therapy which had been conducted by means of antiparasitic, immunomodulatory, anti-inflammatory and desensitizing drugs in patients with demodicosis, as well as mandatory adjustment of changes in the hepato- panсreato-duodenal system. We examined 109 patients with demodicosis from 18 to 39 years old and 10 healthy individuals of the corresponding age and sex. It was established that the effectiveness of treatment of demodicosis based on the application of albendazole in the complex treatment along with immune-modulating therapy and hepatoprotection increased significantly. There has been a reliable rapid regression of clinical symptoms in most patients with demodicosis

    Clinical and biochemical correlates of serum L-ergothioneine concentrations in community-dwelling middle-aged and older adults

    Background: Despite the increasing interest towards the biological role of L-ergothioneine, little is known about the serum concentrations of this unusual aminothiol in older adults. We addressed this issue in a representative sample of communitydwelling middle-aged and older adults. Methods: Body mass index, estimated glomerular filtration rate, serum concentrations of L-ergothioneine, taurine, homocysteine, cysteine, glutathione, cysteinylglycine, and glutamylcysteine were evaluated in 439 subjects (age 55–85 years) randomly selected from the Hunter Community Study. Results: Median L-ergothioneine concentration in the entire cohort was 1.01 IQR 0.78–1.33 mmol/L. Concentrations were not affected by gender (P = 0.41) or by presence of chronic medical conditions (P = 0.15). By considering only healthy subjects, we defined a reference interval for L-ergothioneine serum concentrations from 0.36 (90% CI 0.31–0.44) to 3.08 (90% CI 2.45–3.76) mmol/L. Using stepwise multiple linear regression analysis L-ergothioneine was negatively correlated with age (rpartial =20.15; P = 0.0018) and with glutamylcysteine concentrations (rpartial =20.13; P = 0.0063). Conclusions: A thorough analysis of serum L-ergothioneine concentrations was performed in a large group of communitydwelling middle-aged and older adults. Reference intervals were established. Age and glutamylcysteine were independently negatively associated with L-ergothioneine serum concentration.</br
