An evaluation of the success of the ‘Heart-Fit’ phase IV cardiac rehabilitation programme in Newark, Nottinghamshire: a mixed-method approach

Cardiovascular disease (CVD) is the most common cause of death in England and Wales, with cardiac rehabilitation being one of the best examples of long term condition management for CVD patients (Pluss et al., 2011: Clinical Rehabilitation, 25, 79-87). Phase IV cardiac rehabilitation involves the long term maintenance of health in the form of exercise programmes, such as ‘Heart-Fit’. It has been recommended that mixed methods are required to evaluate CVD prevention programmes (National Institute for Health and Clinical Excellence, 2010), therefore this case study aimed to evaluate the success of a phase IV cardiac rehabilitation programme, both qualitatively and quantitatively, outlining the impact on participants. After ethical clearance from an institutional committee, eleven members (age = 71.2, stature = 1.71m, weight = 79.8kg) of the programme (male = 6, female = 5) volunteered to take part in the study. For eligibility purposes, participants attended the programme once a week. Physical measurements of blood pressure (BP), resting heart rate (RHR), Body Mass Index (BMI) were recorded before and after a twelve week intervention period. Both physical activity levels and smoking status were also recorded by a lifestyle questionnaire. Two focus groups (n = 6 & 5) were held to gain participant perceptions on their experiences relating to the performance of the programme. Paired-sample t-tests indicated statistically significant differences (P = 0.05) pre and post intervention. Using a process of thematic analysis, participants revealed six main themes as being important factors during the 12-week programme. These included the instructor’s influence, the social nature of the group, the availability of clinical observation, psychological development and the perceived beneficial physical improvements made. The CVD risk factors influenced by the programme, in accordance to past literature, were systolic and diastolic BP and BMI. Similar themes have been identified in studies that have evaluated the effectiveness of other exercise referral schemes. Qualitative data collection should be considered alongside quantitative measures in evaluations of similar interventions. Future phase IV cardiac rehabilitation programmes need to gain a greater insight into the patient experience. This will enable health planners and policy makers to generate a sense of context on how these programmes operate at local levels and develop models of best-practice

Tumors of the mammary glands developed in 37 animals, of 75 examined, between the ages of 14 and 28 months (3 adenomas and 34 carcinomas). 2. As ovarian regeneration had not taken place the probability that estrogen originated in some other organ in the absence of the ovaries is suggested. 3. The consistent nodular hyperplasia of the suprarenal cortex and close morphological similarity of cells of these nodules to lutein-like cells of the ovaries points to the abnormal suprarenals as possible sources of the estrogenic hormones

Six month durability of targeted cognitive training supplemented with social cognition exercises in schizophrenia.

Background:Deficits in cognition, social cognition, and motivation are significant predictors of poor functional outcomes in schizophrenia. Evidence of durable benefit following social cognitive training is limited. We previously reported the effects of 70 h of targeted cognitive training supplemented with social cognitive exercises (TCT + SCT) verses targeted cognitive training alone (TCT). Here, we report the effects six months after training. Methods:111 participants with schizophrenia spectrum disorders were randomly assigned to TCT + SCT or TCT-only. Six months after training, thirty-four subjects (18 TCT + SCT, 16 TCT-only) were assessed on cognition, social cognition, reward processing, symptoms, and functioning. Intent to treat analyses was used to test the durability of gains, and the association of gains with improvements in functioning and reward processing were tested. Results:Both groups showed durable improvements in multiple cognitive domains, symptoms, and functional capacity. Gains in global cognition were significantly associated with gains in functional capacity. In the TCT + SCT group, participants showed durable improvements in prosody identification and reward processing, relative to the TCT-only group. Gains in reward processing in the TCT + SCT group were significantly associated with improvements in social functioning. Conclusions:Both TCT + SCT and TCT-only result in durable improvements in cognition, symptoms, and functional capacity six months post-intervention. Supplementing TCT with social cognitive training offers greater and enduring benefits in prosody identification and reward processing. These results suggest that novel cognitive training approaches that integrate social cognitive exercises may lead to greater improvements in reward processing and functioning in individuals with schizophrenia

Association between intimate partner violence and male alcohol use and the receipt of perinatal care : evidence from Nepal demographic and health survey 2011-2016

The utilization of perinatal care services among women experiencing intimate partner violence (IPV) and male alcohol use is a major problem. Adequate and regular perinatal care is essential through the continuum of pregnancy to mitigate pregnancy and birth complications. The aim of this study is to determine the association between IPV and male alcohol use and the receipt of perinatal care in Nepal. This study used pooled data from 2011 and 2016 Nepal Demographic and Health Surveys (NDHS). A total of 3067 women who interviewed for domestic violence module and had most recent live birth 5 years prior surveys were included in the analysis. Multivariable logistic regression analysis was performed to determine the association between IPV and male alcohol use and the receipt of perinatal care. Of the total women interviewed, 22% reported physical violence, 14% emotional violence, and 11% sexual violence. Women who were exposed to physical violence were significantly more likely to report non-usage of institutional delivery [adjusted Odds Ratio (aOR) = 1.30 (95% Cl: 1.01, 1.68)] and skilled delivery assistants [aOR = 1.43 (95% Cl: 1.10, 1.88)]. Non-attendance of 4 or more skilled antenatal care visits was associated with a combination of alcohol use by male partner and exposure to emotional [aOR = 1.42 (95% Cl: 1.01, 2.00)] and physical violence [aOR = 1.39 (95% Cl: 1.03, 1.88)]. The negative association between IPV and perinatal care suggests it is essential to develop comprehensive community- based interventions which integrates IPV support services with other health services to increase the uptake of perinatal care through the continuum of pregnancy

What Should Be Done To Tackle Ghostwriting in the Medical Literature?

Background to the debate: Ghostwriting occurs when someone makes substantial contributions to a manuscript without attribution or disclosure. It is considered bad publication practice in the medical sciences, and some argue it is scientific misconduct. At its extreme, medical ghostwriting involves pharmaceutical companies hiring professional writers to produce papers promoting their products but hiding those contributions and instead naming academic physicians or scientists as the authors. To improve transparency, many editors' associations and journals allow professional medical writers to contribute to the writing of papers without being listed as authors provided their role is acknowledged. This debate examines how best to tackle ghostwriting in the medical literature from the perspectives of a researcher, an editor, and the professional medical writer
