    Erfassung und VerĂ€nderung der allgemeinen UnterrichtsqualitĂ€t im Rahmen der Lehrerfortbildungsstudie „Lernen mit Plan“

    Die QualitĂ€t von Unterricht hat einen bedeutenden Einfluss auf das Lernen und die Motivation von SchĂŒlerinnen und SchĂŒlern. In zahlreichen Forschungsarbeiten wurden daher bereits Merkmale von erfolgreichem Unterricht identifiziert, diese zu Dimensionen zusammengefasst und Maßnahmen konzipiziert, um die UnterrichtsqualitĂ€t zu verbessern. Gleichzeitig lĂ€sst sich jedoch feststellen, dass es auch im Zusammenhang mit der Erfassung von UnterrichtsqualitĂ€t noch einige ungeklĂ€rte Fragen gibt. So ist bisher nicht untersucht, ob die in Querschnittserhebungen vielfach monierte geringe Übereinstimmung von Lehrer- und SchĂŒlerurteilen auch in LĂ€ngsschnitterhebungen bestĂ€tigt werden kann. Hierbei ist insbesondere von Interesse, ob die geringe Übereinstimmung zwischen Lehrer- und SchĂŒlerurteilen durch situative EinflĂŒsse zum jeweiligen Erhebungszeitpunkt erklĂ€rt werden kann und sich die Übereinstimmung zwischen Lehrer- und SchĂŒlereinschĂ€tzungen der UnterrichtsqualitĂ€t durch die Aggregation mehrerer Messzeitpunkte verbessert. DarĂŒber hinaus stellt sich die Frage, ob Fortbildungsmaßnahmen, die in das Unterrichtsgeschehen eingreifen, nicht nur Auswirkungen auf trainierte Aspekte des Unterrichts haben, sondern möglicherweise auch „Nebenwirkungen“ auf andere, nicht adressierte Aspekte der Unterrichtsgestaltung, die sich dann wiederum auf die Leistungsentwicklung der SchĂŒlerinnen und SchĂŒler auswirken. Die vorliegende Dissertation entstand im Kontext des Forschungsprojekts „Lernen mit Plan“, das zum Ziel hatte, mithilfe einer Lehrerfortbildung im Rahmen des Mathematikunterrichts die Selbstregulationskompetenz von SchĂŒlerinnen und SchĂŒlern der fĂŒnften Jahrgangsstufen an Haupt- und Werkrealschulen in Baden-WĂŒrttemberg zu fördern. Anhand von drei Teilstudien, die auf den Daten der Studie „Lernen mit Plan“ basieren, untersucht die vorliegende Dissertation daher drei Forschungsfragen: Teilstudie 1 untersucht die Forschungsfrage, inwieweit Lehrer- und SchĂŒlerratings der UnterrichtsqualitĂ€t zu den drei Erhebungszeitpunkten der Studie messzeitpunktspezifische und messzeitpunktĂŒbergreifende Varianzanteile aufweisen. Außerdem wird in dieser Teilstudie untersucht, ob die Übereinstimmung zwischen Lehrer- und SchĂŒlerurteilen der UnterrichtsqualitĂ€t durch Kontrolle der messzeitpunktspezifischen Varianz höher ist als die bisher nur in Querschnittstudien ermittelte Lehrer-SchĂŒlerĂŒbereinstimmung. Teilstudie 2 geht der Fragestellung nach, inwieweit sich aus Lehrer- und SchĂŒlersicht die Teilnahme der LehrkrĂ€fte an der Fortbildung „Lernen mit Plan“ nicht nur auf die Umsetzung der fokalen Trainingsinhalte (selbstregulationsspezifische UnterrichtsqualitĂ€t), sondern auch auf die nicht trainierten Aspekte der UnterrichtsqualitĂ€t (allgemeine UnterrichtsqualitĂ€t) ausgewirkt hat. In Teilstudie 3 wird aufbauend auf den Befunden von Teilstudie 2 geprĂŒft, ob die Fortbildung wie erwartet eine Verbesserung der Mathematikleistung der SchĂŒlerinnen und SchĂŒler bewirkt hat und ob dieser Effekt durch die Umsetzung der fokalen Trainingsinhalte oder durch die mögliche fortbildungsbedingte VerĂ€nderung der allgemeinen UnterrichtsqualitĂ€t vermittelt wird. Die Ergebnisse der Studien weisen zum einen darauf hin, dass die bisher in der Forschungsliteratur benannten Diskrepanzen zwischen Lehrer- und SchĂŒlerurteilen bei der Erfassung von UnterrichtsqualitĂ€t auch auf messzeitpunktspezifische EinflĂŒsse zurĂŒckzufĂŒhren sind. DarĂŒber hinaus lassen die Befunde der vorliegenden Dissertation die Annahme zu, dass eine Lehrerfortbildung zur Förderung der Selbstregulation von SchĂŒlerinnen und SchĂŒlern auch VerĂ€nderungen auf nicht trainierter Aspekte der UnterrichtsqualitĂ€t haben kann, diese nicht intendierten VerĂ€nderungen jedoch keinen Einfluss auf die als Outcome untersuchte Mathematikleistung der SchĂŒlerinnen und SchĂŒler haben. Die vorliegende Dissertation stellt damit einen wichtigen Beitrag zur weiteren Erforschung der Eigenschaften von Lehrer- und SchĂŒlerurteilen zur Erfassung der UnterrichtsqualitĂ€t dar. DarĂŒber hinaus leistet die Dissertation einen wichtigen Beitrag zur AufklĂ€rung der Wirkmechanismen von Lehrerfortbildungen als Maßnahme zur Förderung der Selbstregulation von SchĂŒlerinnen und SchĂŒlern und der Auswirkungen solcher Fortbildungen auf die Mathematikleistung dar. In der vorliegenden Dissertation werden zunĂ€chst die in den Teilstudien erarbeiteten Forschungsfragen theoretisch hergeleitet. Daran anschließend werden die drei Teilstudien vorgestellt. Die Ergebnisse der Teilstudien werden abschließend in einer Gesamtdiskussion zusammengefĂŒhrt und erörtert. Im letzten Kaptiel werden auch die Grenzen der vorliegenden Dissertation und die Implikationen fĂŒr die jeweiligen Forschungsfelder und die Praxis dargestellt

    Reliability and validity of cutaneous sarcoidosis outcome instruments among dermatologists, pulmonologists, and rheumatologists

    IMPORTANCE: Dermatologists, pulmonologists, and rheumatologists study and treat patients with sarcoidosis with cutaneous manifestations. The validity of cutaneous sarcoidosis outcome instruments for use across medical specialties remains unknown. OBJECTIVE: To assess the reliability and validity of cutaneous sarcoidosis outcome instruments for use by dermatologists and nondermatologists treating sarcoidosis. DESIGN, SETTING, AND PARTICIPANTS: We performed a cross-sectional study evaluating the use of the Cutaneous Sarcoidosis Activity and Morphology Instrument (CSAMI) and Sarcoidosis Activity and Severity Index (SASI) to assess cutaneous sarcoidosis disease severity and the Physician's Global Assessment (PGA) as a reference instrument. Four dermatologists, 3 pulmonologists, and 4 rheumatologists evaluated facial cutaneous sarcoidosis in 13 patients treated at a cutaneous sarcoidosis clinic in a 1-day study on October 24, 2014; data analysis was performed from November through December 2014. MAIN OUTCOMES AND MEASURES: Interrater and intrarater reliability and convergent validity, with correlation with quality-of-life measures as the secondary outcome. RESULTS: All instruments demonstrated excellent intrarater reliability. Interrater reliability (reported as intraclass correlation coefficient [95% CI]) was good for the CSAMI Activity scale (0.69 [0.51-0.87]) and PGA (0.66 [0.47-0.85]), weak for the CSAMI Damage scale (0.26 [0.11-0.52]), and excellent for the modified Facial SASI (0.78 [0.63-0.91]). The CSAMI Activity scale and modified Facial SASI showed moderate correlations (95% CI) with the PGA (0.67 [0.57-0.75] and 0.57 [0.45-0.66], respectively). The CSAMI Activity scale but not the modified Facial SASI showed significant correlations (95% CI) with quality-of-life instruments, such as the Dermatology Life Quality Index (Spearman rank correlation, 0.70 [0.25-0.90]) and the Skin Stigma raw score of the Sarcoidosis Assessment Tool (Pearson product moment correlation, 0.56 [0.01-0.85]). CONCLUSIONS AND RELEVANCE: The CSAMI and SASI were reliable and valid in assessing cutaneous sarcoidosis among our diverse group of specialists. The CSAMI Activity score also correlated with quality-of-life measures and suggested construct validity. These results lend credibility to expand the use of the CSAMI and SASI by dermatologists and nondermatologists in assessing cutaneous sarcoidosis disease activity

    Reliability and Convergent Validity of Two Outcome Instruments for Pemphigus

    A major obstacle in performing multicenter controlled trials for pemphigus is the lack of a validated disease activity scoring system. Here we assess the reliability and convergent validity of the PDAI (pemphigus disease area index). A group of 10 dermatologists scored 15 patients with pemphigus to estimate the inter- and intra-rater reliability of the PDAI and the recently described ABSIS (autoimmune bullous skin disorder intensity score) instrument. To assess convergent validity, these tools were also correlated with the Physician’s Global Assessment (PGA). Reliability studies demonstrated an intra-class correlation coefficient (ICC) for inter-rater reliability of 0.76 [95% CI = 0.61–0.91] for the PDAI and 0.77 [0.63–0.91] for the ABSIS. The tools differed most in reliability of assessing skin activity, with an ICC of 0.39 [0.17–0.60] for the ABSIS and 0.86 [0.76–0.95] for the PDAI. Intra-rater test-retest reliability demonstrated an ICC of 0.98 [0.96–1.0] for the PDAI and 0.80 [0.65–0.96] for the ABSIS. The PDAI also correlated more closely with the PGA. We conclude that the PDAI is more reproducible and correlates better with physician impression of extent. Subset analysis suggests that for this population of mild to moderate disease activity, the PDAI captures more variability in cutaneous disease than the ABSIS

    Heterarchy of Transcription Factors Driving Basal and Luminal Cell Phenotypes in Human Urothelium

    Cell differentiation is effected by complex networks of transcription factors that co-ordinate re-organisation of the chromatin landscape. The hierarchies of these relationships can be difficult to dissect. During in vitro differentiation of normal human uro-epithelial cells, formaldehyde-assisted isolation of regulatory elements (FAIRE-seq) and RNA-seq were used to identify alterations in chromatin accessibility and gene expression changes following activation of the nuclear receptor PPARG as a differentiation-initiating event. Regions of chromatin identified by FAIRE-seq as having altered accessibility during differentiation were found to be enriched with sequence-specific binding motifs for transcription factors predicted to be involved in driving basal and differentiated urothelial cell phenotypes, including FOXA1, P63, GRHL2, CTCF and GATA3. In addition, co-occurrence of GATA3 motifs was observed within sub-sets of differentiation-specific peaks containing P63 or FOXA1 after induction of differentiation. Changes in abundance of GRHL2, GATA3, and P63 were observed in immunoblots of chromatin-enriched extracts. Transient siRNA knockdown of P63 revealed that P63 favoured a basal-like phenotype by inhibiting differentiation and promoting expression of basal marker genes. GATA3 siRNA prevented differentiation-associated downregulation of P63 protein and transcript, and demonstrated positive feedback of GATA3 on PPARG transcript, but showed no effect on FOXA1 transcript or protein expression. This approach indicates that as a transcriptionally-regulated programme, urothelial differentiation operates as a heterarchy wherein GATA3 is able to co-operate with FOXA1 to drive expression of luminal marker genes, but that P63 has potential to transrepress expression of the same genes
