    Identifying Second Language Speech Tasks and Ability Levels for Successful Nurse Oral Interaction with Patients in a Linguistic Minority Setting::An Instrument Development Project

    One of the most demanding situations for members of linguistic minorities is a conversation between a health professional and a patient, a situation that frequently arises for linguistic minority groups in North America, Europe, and elsewhere. The present study reports on the construction of an oral interaction scale for nurses serving linguistic minorities in their second language (L2). A mixed methods approach was used to identify and validate a set of speech activities relating to nurse interactions with patients and to derive the L2 ability required to carry out those tasks. The research included an extensive literature review, the development of an initial list of speech tasks, and validation of this list with a nurse focus group. The retained speech tasks were then developed into a questionnaire and administered to 133 Quebec nurses who assessed each speech task for difficulty in an L2 context. Results were submitted to Rasch analysis and calibrated with reference to the Canadian Language Benchmarks, and the constructs underlying the speech tasks were identified through exploratory and confirmatory factor analyses. Results showed that speech tasks dealing with emotional aspects of caregiving and conveying health-specific information were reported as being the most demanding in terms of L2 ability, and the most strongly associated with L2 ability required for nurse-patient interactions. Implications are discussed with respect to the development and use of assessment instruments to facilitate L2 workplace training for health care professionals

    Farmer-herder relations and conflict management in agro-pastoral zone of Niger

    Farmer-herder conflicts are enduring features of social life in the Sudano-Sahelian zone. A survey was carried out between August and December 2004 in four sites in Niger, namely Bokki, Katanga, Sabon Gida and Tountoubé to determine the proximate and long-term causes of conflict over natural resource use, to evaluate the appropriateness of existing institutional arrangements for managing conflicts and identify innovative options and incentives to reduce the incidence and severity of conflicts. The research was implemented in three phases: (1) collection of village and household level socioeconomic information, (2) social network mapping, and (3) collection of conflict history and conflict management strategies. Additionally, governmental and NGO agencies in Niamey that address conflict management and/or resolution at the regional and national levels were interviewed. The research employed both quantitative and qualitative survey instruments. Surveys collected information on: historical micro-geographies of cropping and herding in the area encompassing village territory; local day-to-day relationships between transhumance herders, settled herders, and farming households at the study site; nodes of communication under different types of disagreements and negotiative settings; documentation of past conflicts and role of government officials, customary authorities and NGOs in conflict management. Results from this study showed that in all sites, damage to crops was the first reported cause of conflict between farmers and herders. Crop damage is not limited to damage to growing crops on the field but also included unauthorized grazing of crop residues after harvest. Other causes of conflict reported were access to watering points, expansion of crop fields across corridors for animal passage and thefts of animal. The ability of rural communities to prevent and manage conflict is largely based on the strength of networks of communication between herding and farming interests, respected community leaders, and leaders in neighboring communities. Overall, the local institutional arrangements are functional and a high percentage of conflicts are effectively managed at local levels.     Clinical Science Postoperative fluid collections after colon resection: the utility of clinical assessment

    Abstract BACKGROUND: Computed tomography (CT) scans often identify postoperative fluid collections of uncertain clinical relevance. METHODS: Consecutive adult patients undergoing colorectal resection and postoperative CT scan from January 1, 2000 to December 31, 2008, at a university teaching hospital were identified from a prospective database. A host of clinical and CT findings were recorded. Fisher's exact test and logistic regression with univariate and multivariate analysis were used to assess the predictive value of clinical and radiologic variables. RESULTS: Nine hundred six patients had a colon resection during the study period. Fifty-four patients had a postoperative fluid collection, of which 36 were found to be abscesses. Only high clinical suspicion of an abscess predicted the presence of an abscess (P 5 .009); of the radiologic criteria, only proximity to the anastomosis was predictive (P 5 .05). CONCLUSIONS: Clinical judgment is superior to radiologic and individual clinical parameters. This finding has the potential to prevent many unnecessary procedures

    Chiral photonic crystals with four-fold symmetry: Band structure and S-parameters of eight-fold intergrown Gyroid nets

    The Single Gyroid, or srs, nanostructure has attracted interest as a circular-polarisation sensitive photonic material. We develop a group theoretical and scattering matrix method, applicable to any photonic crystal with symmetry I432, to demonstrate the remarkable chiral-optical properties of a generalised structure called 8-srs, obtained by intergrowth of eight equal-handed srs nets. Exploiting the presence of four-fold rotations, Bloch modes corresponding to the irreducible representations E- and E+ are identified as the sole and non-interacting transmission channels for right- and left-circularly polarised light, respectively. For plane waves incident on a finite slab of the 8-srs, the reflection rates for both circular polarisations are identical for all frequencies and transmission rates are identical up to a critical frequency below which scattering in the far field is restricted to zero grating order. Simulations show the optical activity of the lossless dielectric 8-srs to be large, comparable to metallic metamaterials, demonstrating its potential as a nanofabricated photonic material

    Emergence of a New Highly Successful Acapsular Group A Streptococcus Clade of Genotype emm89 in the United Kingdom

    UNLABELLED: Group A Streptococcus (GAS) genotype emm89 is increasingly recognized as a leading cause of disease worldwide, yet factors that underlie the success of this emm type are unknown. Surveillance identified a sustained nationwide increase in emm89 invasive GAS disease in the United Kingdom, prompting longitudinal investigation of this genotype. Whole-genome sequencing revealed a recent dramatic shift in the emm89 population with the emergence of a new clade that increased to dominance over previous emm89 variants. Temporal analysis indicated that the clade arose in the early 1990s but abruptly increased in prevalence in 2008, coinciding with an increased incidence of emm89 infections. Although standard variable typing regions (emm subtype, tee type, sof type, and multilocus sequence typing [MLST]) remained unchanged, uniquely the emergent clade had undergone six distinct regions of homologous recombination across the genome compared to the rest of the sequenced emm89 population. Two of these regions affected known virulence factors, the hyaluronic acid capsule and the toxins NADase and streptolysin O. Unexpectedly, and in contrast to the rest of the sequenced emm89 population, the emergent clade-associated strains were genetically acapsular, rendering them unable to produce the hyaluronic acid capsule. The emergent clade-associated strains had also acquired an NADase/streptolysin O locus nearly identical to that found in emm12 and modern emm1 strains but different from the rest of the sequenced emm89 population. The emergent clade-associated strains had enhanced expression of NADase and streptolysin O. The genome remodeling in the new clade variant and the resultant altered phenotype appear to have conferred a selective advantage over other emm89 variants and may explain the changes observed in emm89 GAS epidemiology. IMPORTANCE: Sudden upsurges or epidemic waves are common features of group A streptococcal disease. Although the mechanisms behind such changes are largely unknown, they are often associated with an expansion of a single genotype within the population. Using whole-genome sequencing, we investigated a nationwide increase in invasive disease caused by the genotype emm89 in the United Kingdom. We identified a new clade variant that had recently emerged in the emm89 population after having undergone several core genomic recombination-related changes, two of which affected known virulence factors. An unusual finding of the new variant was the loss of the hyaluronic acid capsule, previously thought to be essential for causing invasive disease. A further genomic adaptation in the NADase/streptolysin O locus resulted in enhanced production of these toxins. Recombination-related genome remodeling is clearly an important mechanism in group A Streptococcus that can give rise to more successful and potentially more pathogenic variants

    We study the induced 4-dimensional linearized Einstein field equations in an m-dimensional bulk space by means of a confining potential. It is shown that in this approach the mass of graviton is quantized. The cosmological constant problem is also addressed within the context of this approach.     Effect of logic family on radiated emissions from digital circuits

    Radiated emissions were measured for simple digital circuits designed to operate with various logic families. Emissions in the near and far field were found to depend both on the circuit layout and the choice of logic family. However, the difference in peak emissions between any two logic families was found to be independent of circuit layout. The greatest difference in peak emissions was between high-speed 74ACT logic and low-speed 4000 CMOS logic devices, with a mean value of approximately 20 dB. Emissions from a more complex circuit were compared with the measurements on simple loop circuits. Test circuits were used to measure the propagation delay, the rise and fall times, the maximum operating frequency and the transient switching currents between two successive logic gates for each logic family. Empirical formulas have been derived that relate relative peak emissions to these switching parameters. It is hoped that these will assist designers to assess the effect of choice of logic family on electromagnetic compatibility

    An automotive engine charge-air intake conditioner system: thermodynamic analysis of performance characteristics

    A first law thermodynamic model has been developed and used to characterize the performance of an automotive engine charge-air intake conditioner system. This system employs a compressor, intercooler, and expander to provide increased charge density with the possibility of reducing, the charge-air temperature below the sink temperature. The model was validated against experimental measurements. The variation of system effectiveness with compressor, intercooler, and expander efficiency was quantified and system operating limits were identified. While the expander was found to have a greater effect than the compressor, the performance of the system was shown to be most dependent upon intercooler effectiveness
