The Effects of HIIT Versus MICT and Sedentary Controls on Blood Lipid Concentrations in Nondiabetic Overweight and Obese Young Adults: A Meta-analysis.

With rates of obesity and dyslipidemia rising among young adults, this meta-analysis aimed to compare the effects of high-intensity interval training (HIIT) versus moderate-intensity continuous training (MICT) and sedentary controls (CON) on low-density lipoprotein (LDL), high-density lipoprotein (HDL), triglyceride (TG), and total cholesterol (TC) in nondiabetic overweight and obese young adults to determine if HIIT or MICT is more efficacious in improving dyslipidemia. Studies included in the analysis had to be randomized controlled trials or quasi-experimental studies, comparing the effects of HIIT versus MICT or CON on at least three variables of interest: LDL, HDL, TG, and TC, in nondiabetic adults, with body mass indexes (BMIs) above 25, and average ages between 18-30. The quality of the studies was evaluated using the Physiotherapy Evidence Database (PEDro) scale. Eight studies fulfilled the selection criteria, with a mean PEDro quality score of 5.8. Compared to CON, HIIT significantly decreased the concentrations of LDL (-12.14 mg/dL, p = \u3c 0.00001) and TC (-9.27 mg/dL, p = 0.003), without significantly affecting HDL or TG. Compared to MICT, HIIT significantly decreased the concentrations of LDL (-6.23 mg/dL, p = 0.05) and TC (-7.85 mg/dL, p = 0.02), without significantly affecting HDL or TG concentrations. HIIT is superior to MICT and CON in improving the concentrations of LDL and TC in our target population. As early management of dyslipidemia improves long-term health, we recommend clinicians consider HIIT training protocols for their nondiabetic overweight and obese young adult patients
Diagnosis of bacterial vaginosis from self-obtained vaginal swabs.
OBJECTIVE: To determine the concordance between vaginal fluid Gram stains and pH obtained at speculum exam with similar stains and pH prepared from self-obtained vaginal swabs. METHODS: Using vaginal fluid Gram stain, 129 pregnant women were screened for bacterial vaginosis at 24 to 29 weeks' gestation. Two smears were collected from each woman during the same prenatal visit: the first was prepared from a self-obtained vaginal swab and the second from a physician-obtained speculum examination. Vaginal pH was recorded for each swab. Kappa coefficient was used to quantify agreement between the two sets of results. RESULTS: When compared with the physician-obtained smear, the ability of the self-obtained Gram stain to diagnose bacterial vaginosis had a sensitivity of 77%, specificity of 97%, positive predictive value of 71% and negative predictive value of 97%. There was substantial agreement (weighted kappa=0.82) between the two techniques in the ability to determine the grade of vaginal flora. CONCLUSION: When compared with physician-obtained vaginal smears, self-obtained smears have substantial agreement in the diagnosis of bacterial vaginosis
Memory Augmented Language Models through Mixture of Word Experts
Scaling up the number of parameters of language models has proven to be an
effective approach to improve performance. For dense models, increasing model
size proportionally increases the model's computation footprint. In this work,
we seek to aggressively decouple learning capacity and FLOPs through
Mixture-of-Experts (MoE) style models with large knowledge-rich vocabulary
based routing functions and experts. Our proposed approach, dubbed Mixture of
Word Experts (MoWE), can be seen as a memory augmented model, where a large set
of word-specific experts play the role of a sparse memory. We demonstrate that
MoWE performs significantly better than the T5 family of models with similar
number of FLOPs in a variety of NLP tasks. Additionally, MoWE outperforms
regular MoE models on knowledge intensive tasks and has similar performance to
more complex memory augmented approaches that often require to invoke custom
mechanisms to search the sparse memory.Comment: 14 page
Resolution of the clinical features of tyrosinemia following orthotopic liver transplantation for hepatoma
The clinical history before transplantation and subsequent clinical and biochemical course of 3 children and one adult with hereditary tyrosinemia treated by orthotopic hepatic transplantation is described. Validity of self-reported measures of workplace sitting time and breaks in sitting time

Sci. Sports Exerc., Vol. 43, No. 10, pp. 1907-1912, 2011. Purpose: To understand the prevalence and potential health effect of prolonged workplace sedentary (sitting) time, valid measures are required. Here, we examined the criterion validity of a brief self-reported measure of workplace sitting time and breaks in sitting time. Methods: An interviewer-administered questionnaire was used to assess workplace sitting time (h.d(-1)) and breaks from sitting per hour at work in a convenience sample of 121 full-time workers (36% men, mean age = 37 yr, 53% office based). These self-reported measures were compared with accelerometer-derived sedentary time (hours per day, = 100 counts per minute) during work hours. Results: Self-reported sitting time was significantly correlated with accelerometer-derived sedentary time (Pearson r = 0.39, 95% confidence interval = 0.22-0.53), with an average sitting time 0.45 h.d(-1) higher than average sedentary time. Bland-Altman plots and regression analysis showed positive associations between the difference in sitting and sedentary time and the average of sitting and sedentary time (mean difference = -2.75 h + 0.47 x average sitting and sedentary time; limits of agreement = +/- 2.25 h.d(-1)). The correlation of self-reported breaks per sitting hour with accelerometer-derived breaks per sedentary hour was also statistically significant (Spearman r(s) = 0.26, 95% confidence interval = 0.11-0.44). Conclusions: This study is the first to examine the criterion validity of an interviewer-administered questionnaire measure of workplace sitting time and breaks in sitting time using objective criterion measures. The workplace sitting measure has acceptable properties for use in observational studies concerned with sedentary behavior in groups of workers; however, the wide limits of agreement suggest caution in estimating individuals' sitting time with high precision. Simulation of Daily Flow Pathways, Tile-Drain Nitrate Concentrations, and Soil-Nitrogen Dynamics Using SWAT
Tile drainage significantly alters flow and nutrient pathways and reliable simulation at this scale is needed for effective planning of nutrient reduction strategies. The Soil and Water Assessment Tool (SWAT) has been widely utilized for prediction of flow and nutrient loads, but few applications have evaluated the model\u27s ability to simulate pathway-specific flow components or nitrate-nitrogen (NO3-N) concentrations in tile-drained watersheds at the daily time step. The objectives of this study were to develop and calibrate SWAT models for small, tile-drained watersheds, evaluate model performance for simulation of flow components and NO3-N concentration at daily intervals, and evaluate simulated soil-nitrogen dynamics. Model evaluation revealed that it is possible to meet accepted performance criteria for simulation of monthly total flow, subsurface flow (SSF), and NO3-N loads while obtaining daily surface runoff (SURQ), SSF, and NO3-N concentrations that are not satisfactory. This limits model utility for simulating best management practices (BMPs) and compliance with water quality standards. Although SWAT simulates the soil N-cycle and most predicted fluxes were within ranges reported in agronomic studies, improvements to algorithms for soil-N processes are needed. Variability in N fluxes is extreme and better parameterization and constraint, through use of more detailed agronomic data, would also improve NO3-N simulation in SWAT.
