Atypical periprosthetic femoral fractures of the hip: A PRISMA compliant systematic review
Introduction: Long-term use of bisphosphonates (BPs) is associated with Atypical Femoral Fracture (AFF). Theoretically, periprosthetic femoral fractures (PFF) should be excluded from the diagnosis of AFF. How-ever, recently several studies reported the occurrence of PPFs around a hip arthroplasty presenting fea-tures of an AFF. The present study describes the characteristics of Atypical Periprosthetic Femoral Fracture (APFF) and evaluates the effectiveness of their management through a PRISMA compliant systematic re-view of the published case reports and series. Materials and methods: A literature search was performed using "periprosthetic fracture" and "atypical femoral fracture" as keywords. Patients demographics, drug use, clinical and imaging characteristics, stem fixation and classification, management strategies for APFF and patients' outcomes, were also collected. Results: The present review included and analysed 17 patients from 12 studies. All APFFs occurred in females with a mean age of 75.9 years of age (range 43-87). In 11 patients, APFFs occurred around an uncemented stem, and in 6 around a cemented stem. Ten fractures were incomplete, and 7 complete. Conservative management was effective in 4 of 10 patients with incomplete fracture, while all patients with complete fractures underwent open reduction and internal fixation. A fracture non-union was ob-served in 5 patients and further surgery was required. Discussions: APFFs share several clinical and imaging characteristics with AFF. An appropriate and early diagnosis may allow to improve the outcome of these fractures, the management of which should be based on the same principles of that of AFFs. Conclusions: Considering the low quality of published articles and the heterogeneity of the treatment used, a clear recommendation of the most appropriate treatment cannot be formulated. Qualitative characterisation of cultivated and wild edible plants: Mineral elements, phenols content and antioxidant capacity
This study investigated the qualitative characteristics of several edible wild herbaceous species, including those most consumed in Foggia Province (southern Italy). Analysis of qualitative characteristics was performed for the edible parts of 11 wild species (Beta vulgaris L., Foeniculum vulgare Miller, Centaurea solstitialis L., Cichorium intybus L., Scolymus hispanicus L., Sonchus oleraceus L., Borago officinalis L., Diplotaxis erucoides L., Diplotaxis tenuifolia (L.) DC, Sinapis arvensis L., Portulaca oleracea L.) and three cultivated species (C. intybus, B. officinalis, D. tenuifolia). The plants were collected from areas in the Foggia countryside, and the edible part of each species was analysed for dry matter, protein, cation and anion contents as well as total phenols and antioxidant activities. Among the cations, calcium was the most differentiated among species, ranging 784 mg kg-1 fresh weight (Fw) for B. vulgaris to 5886 mg kg-1 Fw for S. hispanicus. The nitrate contents were also highly variable, from 75 mg kg-1 Fw for C. intybus to 3874 mg kg-1 Fw for D. tenuifolia. Total polyphenols ranged from 1054 mg GAE mg kg-1 Fw for C. solstitialis to 3664 mg GAE mg kg-1 Fw for S. arvensis. Antioxidant activities ranged from 839 mg TE kg-1 Fw for B. vulgaris to 5658 mg TE kg-1 Fw for C. intybus. Significant differences were also noted between wild and cultivated plants in the qualitative parameters. Total polyphenols and antioxidant activity were higher in wild C. intybus and B. officinalis than in their cultivated counterparts. Multivariate analysis (cluster analysis and linear discriminant analysis) allowed integration of the ANOVA data to determine the qualitative characteristics of the wild species that contribute most to group differences. The results of the present study aims at improve the current knowledges about edible wild species as vegetable sources in the Mediterranean diet
Why Use Adipose-Derived Mesenchymal Stem Cells in Tendinopathic Patients: A Systematic Review
The aim of the present systematic review was to provide a clear overview of the clinical current research progress in the use of adipose-derived mesenchymal stem cells (ASCs) as an effective therapeutic option for the management of tendinopathies, pathologies clinically characterized by persistent mechanical pain and structural alteration of the tendons. The review was carried out using three databases (Scopus, ISI Web of Science and PubMed) and analyzed records from 2013 to 2021. Only English-language papers describing the isolation and manipulation of adipose tissue as source of ASCs and presenting ASCs as treatment for clinical tendinopathies were included. Overall, seven clinical studies met the inclusion criteria and met the minimum quality inclusion threshold. Data extraction and quality assessment were performed by groups of three reviewers. The available evidence showed the efficacy and safety of ASCs treatment for tendinopathies, although it lacked a clear description of the biomolecular mechanisms underlying the beneficial properties of ASCs

Liver-spleen axis, insulin-like growth factor-(IGF)-I axis and fat mass in overweight/obese females
<p>Abstract</p> <p>Background</p> <p>Fat mass (FM) in overweight/obese subjects has a primary role in determining low-grade chronic inflammation and, in turn, insulin resistance (IR) and ectopic lipid storage within the liver. Obesity, aging, and FM influence the growth hormone/insulin-like growth factor (IGF)-I axis, and chronic inflammation might reduce IGF-I signaling. Altered IGF-I axis is frequently observed in patients with Hepatic steatosis (HS). We tested the hypothesis that FM, or spleen volume and C-reactive protein (CRP)--all indexes of chronic inflammation--could affect the IGF-I axis status in overweight/obese, independently of HS.</p> <p>Methods</p> <p>The study population included 48 overweight/obese women (age 41 ± 13 years; BMI: 35.8 ± 5.8 kg/m<sup>2</sup>; range: 25.3-53.7), who underwent assessment of fasting plasma glucose and insulin, homeostasis model assessment of insulin resistance (HOMA), cholesterol and triglycerides, HDL-cholesterol, transaminases, high-sensitive CRP, uric acid, IGF-I, IGF binding protein (BP)-1, IGFBP-3, and IGF-I/IGFBP-3 ratio. Standard deviation score of IGF-I according to age (zSDS) were also calculated. FM was determined by bioelectrical impedance analysis. HS severity grading (score 0-4 according liver hyperechogenicity) and spleen longitudinal diameter (SLD) were evaluated by ultrasound.</p> <p>Results</p> <p>Metabolic syndrome (MS) and HS were present in 33% and 85% of subjects, respectively. MS prevalence was 43% in subjects with increased SLD. IGF-I values, but not IGF-I zSDS, and IGF-I/IGFBP-3 ratio were significantly lower, while FM%, FPI, HOMA, ALT, CRP, were significantly higher in patients with severe HS than in those with mild HS. IGF-I zSDS (r = -0.42, r = -0.54, respectively; p < 0.05), and IGFBP-1 (r = -0.38, r = -0.42, respectively; p < 0.05) correlated negatively with HS severity and FM%. IGF-I/IGFBP-3 ratio correlated negatively with CRP, HS severity, and SLD (r = -0.30, r = -0.33, r = -0.43, respectively; p < 0.05). At multivariate analysis the best determinants of IGF-I were FM% (β = -0.49; p = 0.001) and IGFBP-1 (β = -0.32; p = 0.05), while SLD was in the IGF-I/IGFBP-3 ratio (β = -0.43; p = 0.004).</p> <p>Conclusions</p> <p>The present study suggests that lower IGF-I status in our study population is associated with higher FM, SLD, CRP and more severe HS.</p
Management of Branched Broomrape in Field Processing Tomato Crop
In recent years, there has been a considerable increase in land area used for tomato (Lycopersicon esculentum Mill.) in many countries around the world. The essential role is played by Italy at a worldwide level as the country with the third biggest production of tomatoes for processing. Phelipanche ramosa (L) Pomel, commonly known as branched broomrape, is a root holoparasitic weed for many crops, particularly for the processing tomato. Due to its physical and metabolic overlap with the crop, its underground parasitism, and hardly destructible seed bank, the control of this parasite in the field is difficult. Results of research studies, many of them on environmental-friendly methods such as preventive, agronomic, and biological carried out in southern Italy, are discussed and summarized. The results can constitute a relevant basis for further experimental studies
Quality of life and anxious-depressive symptoms in cancer patients undergoing mindfulness-based interventions: Feasibility and preliminary outcomes on prospective single-centre case-control study (MIND4ME St.)
Background
Mindfulness based interventions (MBIs) have shown efficacy in improving psychological symptoms including depression and anxiety in cancer patients (pts). The study aimed to explore feasibility and reproducibility of MBIs in an Italian Cancer Centre measuring biochemical and psychological parameters.
Methods
In this pilot prospective case-control study, we recruited newly diagnosed pts receiving adjuvant chemotherapy (CT). A MBIs program was designed consisting of 2.5 hours weekly for 8 weeks and, including meditation, yoga and body scan. Material for 45 minutes (mn) home daily practice was provided. Primary endpoint was to evaluate feasibility. Secondary endpoints were assessment of quality of life (QoL), psychological and biochemical outcomes of stress, tested at baseline (W0), W4, W8, W24, W48. PSS (Perceived Stress Reduction), POMS (profile of mood states scores), EuroQoL (EQ-5D-3L) were administered.
Results
Ten pts underwent MBIs program arm. We present preliminary results, while data of control arm are being collected. All pts were female, two pts (20%) dropped out. Median age was 56 years. All received adjuvant CT, 5/8 received radiotherapy and hormone therapy. Mean of sessions attending was 6.8 (76%). Median daily practice was 30 mn. EQ-5D item for depression and anxiety showed decreasing trend in mean score from moderate to light (P = 0.15) and significant improvement of auto-perceived QoL was observed comparing W0 and W8 (P = 0.02)
Conclusions
In a sensitive setting such as start CT, we found high pts compliance to MBIs. Improvement in self-perceived QoL after starting program was found and comparing anxious-depressive symptoms outcomes with control arm is still needed
Long-Term Results of the FOLL05 Trial Comparing R-CVP Versus R-CHOP Versus R-FM for the Initial Treatment of Patients With Advanced-Stage Symptomatic Follicular Lymphoma
Purpose The FOLL05 trial compared R-CVP (rituximab plus cyclophosphamide, vincristine, and prednisone) with R-CHOP (rituximab plus cyclophosphamide, doxorubicin, vincristine, and prednisone) and R-FM (rituximab plus fludarabine and mitoxantrone) regimens without rituximab maintenance as initial therapy for patients with advanced-stage follicular lymphoma (FL). A previous analysis with a median follow-up of 34 months showed a superior 3-year time to treatment failure, the primary study end point, with R-CHOP and R-FM versus R-CVP and showed R-CHOP to have a better risk-benefit ratio in terms of toxicity than R-FM. We report a post hoc analysis of this trial after a median follow-up of 7 years. Patients and Methods Of the 534 enrolled patients, 504 were evaluable. At the time of analysis, the median follow-up was 84 months (range, 1 to 119 months). Results The 8-year time to treatment failure and progression-free survival rates were 44% (95% CI, 39% to 49%) and 48% (95% CI, 43% to 53%), respectively. The hazard ratio for progression-free survival adjusted by FL International Prognostic Index 2 versus R-CVP was 0.73 for R-CHOP (95% CI, 0.54 to 0.98; P = .037) and 0.67 for R-FM (95% CI, 0.50 to 0.91; P = .009). The 8-year overall survival (OS) rate was 83% (95% CI, 79% to 87%), with no significant differences among study arms. Overall, we observed a higher risk of dying as a result of causes unrelated to lymphoma progression with R-FM versus R-CVP. Conclusion With an 83% 8-year OS rate, long-term follow-up of the FOLL05 trial confirms the favorable outcome of patients with advanced-stage FL treated with immunochemotherapy. The three study arms had similar OS but different activity and toxicity profiles. Patients initially treated with R-CVP had a higher risk of lymphoma progression compared with those receiving R-CHOP, as well as a higher risk of requiring additional therapy
