Ankle-brachial index, high-sensitivity C-reactive protein and endothelial function in a cardiovascular risk population
Atherosclerotic vascular disease is the leading cause of death in the Western world.
Its main three manifestations are coronary heart disease, cerebrovascular disease, and
peripheral arterial disease. Asymptomatic peripheral arterial disease is usually diagnosed
using the ankle brachial index, and values ≤ 0.90 are used to determine the diagnosis.
The classical risk factors of peripheral arterial disease, such as smoking and diabetes,
are well known and early interventions are mandatory to improve the prognosis. What
is not well known is the role of inflammation as a risk factor. Yet, a novel approach to
cardiovascular diseases is the measurement of endothelial function.
In this thesis, we studied the ankle-brachial index, C-reactive protein and endothelial
function in a cardiovascular risk population. A total of 2856 subjects were invited to the
study and 2085 (73%) responded. From these subjects, a cohort of 1756 risk persons
was screened. We excluded the subjects with previously known cardiovascular disease
or diabetes, because they were already under systematic follow-up. Out of the study
subjects, 983 (56%) were women and 773 (44%) men. The ankle brachial index and
high-sensitivity C-reactive protein were measured from 1047 subjects. Endothelial
function was assessed by measuring reactive hyperemia pulse amplitude tonometry from
66 subjects with borderline peripheral arterial disease.
In this study, smoking was a crucial risk factor for peripheral arterial disease. Subclinical
peripheral arterial disease seems to be more common in hypertensive patients even without
comorbidities. The measurement of the ankle brachial index is an efficient method to
identify patients at an increased cardiovascular risk. High-sensitivity C-reactive protein
did not correlate with the ankle brachial index or peripheral arterial disease. Instead, it
correlated with measures of obesity. In a cardiovascular risk population with borderline
peripheral arterial disease, nearly every fourth subject had endothelial dysfunction. This
might point out a subgroup of individuals in need of more intensive treatment for their

Traditionally ABI values > 1.00–1.40 have been considered normal and ABI ≤ 0.90 defines PAD. Recent studies, however, have shown that individuals with ABI values between 0.90–1.00 are also at risk of cardiovascular events. We studied this cardiovascular risk population subgroup in order to determine their endothelial function using peripheral arterial tonometry (PAT).Methods: We selected 66 individuals with cardiovascular risk and borderline ABI. They all had hypertension, newly diagnosed glucose disorder, metabolic syndrome, obesity, or a ten year risk of cardiovascular disease death of 5% or more according to the Systematic Coronary Risk Evaluation System (SCORE). Subjects with previously diagnosed diabetes or cardiovascular disease were excluded. Endothelial function was assessed by measuring the reactive hyperemia index (RHI) from fingertips using an Endo-PAT device.Results: The mean ABI was 0.95 and mean RHI 2.11. Endothelial dysfunction, defined as RHI < 1.67, was detected in 15/66 (23%) of the subjects. There were no statistically significant differences in RHI values between subjects with different cardiovascular risk factors. The only exception was that subjects with impaired fasting glucose (IFG) had slightly lower RHI values (mean RHI 1.91) than subjects without IFG (mean RHI 2.24) (P = 0.02).Conclusions: In a cardiovascular risk population with borderline ABI nearly every fourth subject had endothelial dysfunction, indicating an elevated risk of cardiovascular events. Kyrönjoen vesistön tulvantorjunnan toimintasuunnitelma
Kyrönjoen vesistössä sattui 1970- ja 1980-luvuilla sekä viimeksi keväällä 2006 useita vaikeita kevättulvatilanteita.
Ne aiheutuivat suurista kevättulvavirtaamista ja niiden yhteydessä sattuneista äkillisistä jääpato-ongelmista.
Tulvan kannalta ongelmallisimmat alueet Kyrönjoen ylä- ja alaosilla on suojattu tulvapenkereillä 1/20 vuoden toistuvuutta
pienemmiltä tulvilta. Asutuksen suojaamiseksi penkereitä on rakennettu myös Ilmajoelle ja Jalasjärvelle.
Ylempänä vesistössä sijaitsee neljä tekojärveä ja yksi säännöstelty järvi, jotka helpottavat tilannetta tulvaongelmien
osalta ja kompensoivat tulvien siirtymistä alajuoksulle. Tulvantorjuntarakenteista huolimatta lähes koko matkaltaan
asutussa jokivarressa veden äkillinen nousu jääpadon seurauksena saattaa aiheuttaa merkittäviä paikallisia
vahinkoja, kuten tapahtui keväällä 2006 Isossakyrössä.
Kyrönjoen vesistön tulvantorjunnan toimintasuunnitelmassa on kuvailtu vesistön nykyisiä säännöstelymääräyksiä
ja arvioitu merkittävimpien jokiosuuksien osalta tulvavahinkojen suuruus 1/250 tulvalla. Suunnitelmassa on esitelty
lyhyesti nykyisin käytössä oleva vesistömalli ennusteiden laatimiseksi. Vesistöjen osalta on selvitetty tulvantorjuntatoimenpiteet,
joilla vahinkoja pystytään ennaltaehkäisemään. Lisäksi on kuvattu käytännön tulvantorjunnan organisointi
ja eri viranomaistahojen tehtävät tulvien torjuntatyössä. Tulvantorjuntamahdollisuuksien kehittämiseksi on
esitetty suosituksia, joita tulevaisuudessa olisi hyödynnettävä
Ablaatiohoidot - sädehoidon jälkeen paikallisesti uusiutuneen eturauhassyövän hoitovaihtoehto
Uusien kuvantamismenetelmien myötä eturauhassyövän paikantaminen ja riskiluokitus ovat tarkentuneet. Magneettikuvauksella pystytään usein luotettavasti visualisoimaan ennusteen kannalta merkittävin syöpäkasvain. Modernit kuvantamisohjatut ablatiiviset eli kudosta tuhoavat hoitomenetelmät kohdistuvat hoidettavaan elimeen niin, että pyritään välttämään ympäröivien kudosten vaurioita. Koko rauhasen käsittäviin ablaatiohoitoihin vaikuttaa liittyvän suotuisampi haittavaikutusprofiili kuin leikkaukseen ja sädehoitoon. Primaarikasvaimeen kohdistuvassa hoidossa eli rauhasen osan hoitostrategiassa (fokaaliterapia) elämänlaadulliset haitat näyttävät edelleen vähentyvän ilman, että tehossa olisi merkittävästi eroa koko rauhasen hoitoihin verrattuna. Tieteellinen näyttö koko rauhasen ja rauhasen osan ablaatiohoitojen pitkäaikaistehosta paikallisen eturauhassyövän hoidossa on toistaiseksi niukkaa, ja ablaatio- ja standardihoitoja vertailevat tutkimukset puuttuvat. Sen sijaan sädehoidon jälkeen paikallisesti uusiutuneen eturauhassyövän ablatiiviset hoitomenetelmät tuovat uuden tehokkaan hoidon tähän vaativaan kliiniseen ongelmaan
Kivessyövän hoito
Kivessyöpä on tavallisin nuorten miesten syöpä. Sen ilmaantuvuus lisääntyy. Taudin varhainen toteaminen perusterveydenhuollossa on tärkeää, vaikka levinneenkin syövän ennuste on hyvä. Hoidot on keskitetty yliopistosairaaloihin. Seuranta jatkuu erikoissairaanhoidossa yleensä viiden vuoden ajan. Myöhäisuusiutumat ovat erittäin harvinaisia. Hoidoilla on myöhäishaittoja, esimerkiksi sydän- ja verisuonitauteja sekä sekundaarisia syöpiä, joten terveet elämäntavat ovat tärkeitä
Perusopetuksen oppimisympäristöjen digitalisaation nykytilanne ja opettajien valmiudet hyödyntää digitaalisia oppimisympäristöjä
Tällä selvityksellä esitetään yleiskuva siitä, millainen on oppimisympäristöjen digitalisaation nykytilanne ja minkälaiset ovat opettajien valmiudet niiden hyödyntämiseen perusasteella. Selvitys toteutettiin verkkopohjaista Opeka-työkalua käyttäen ja saatua aineistoa täydennettiin kohderyhmien haastatteluilla. Kohdennettu haastattelu suoritettiin valituille sivistystoimenjohtajille, rehtoreille ja opettajille.
Koulujen digitaalisuudessa ei ole suuria alueellisia eroja eri puolilla Suomea ja opettajat suhtautuvat pääosin (75 %) myönteisesti tieto- ja viestintäteknologian (TVT) käytön lisäämiseen omassa työssään. Opettajista noin puolet arvioi omaavansa perustason TVT:n käyttötaidot, viidenneksen kokiessa taitojensa olevan perustasoa paremmat. Selvityksen mukaan omien taitojen kehittämistäkin tarvitaan, noin 20 % vastaajista kokee osaamisessaan merkittäviä puutteita. Käytössä olevien laitteistojen tai välineistön määrä koetaan riittämättömäksi ja käytössä olevien laitteistojen laadussa nähdään olevan puutteita (60 %). Puolet vastaajista koki käytössään olevan internetyhteyden riittäväksi. Joka kolmannes vastaajista koki tyytymättömyyttä käytössään olevaan langattomaan verkkoon, osasta kouluja langaton verkko
puuttuu edelleen. Uusi teknologia aiheuttaa myös stressiä lähes puolelle opettajista.
Joustavat, innovatiiviset täydennyskoulutusratkaisut sekä teknisen ja pedagogisen tuen kehittäminen nousevat esiin tässäkin selvityksessä. Digitaalisuus ei ole itseisarvo, vaan väline kehittää koulutusta ja tarjota lapsille ja nuorille taitoja hyödyntää digitaalisuuden suomia mahdollisuuksia niin opiskelussa, työssä kuin vapaa-ajalla.

Individualised non-contrast MRI-based risk estimation and shared decision-making in men with a suspicion of prostate cancer : Protocol for multicentre randomised controlled trial (multi-IMPROD V.2.0)
Introduction European Association of Urology and UK National Institute for Health and Care Excellence guidelines recommend that all men with suspicions of prostate cancer should undergo prebiopsy contrast enhanced, that is, multiparametric prostate MRI. Subsequent prostate biopsies should also be performed if MRI is positive, that is, Prostate Imaging-Reporting and Data System (PI-RADS) scores 3-5. However, several retrospective post hoc analyses have shown that this approach still leads to many unnecessary biopsy procedures. For example, 88%-96% of men with PI-RADS, three findings are still diagnosed with clinically non-significant prostate cancer or no cancer at all. Methods and analysis This is a prospective, randomised, controlled, multicentre trial, being conducted in Finland, to demonstrate non-inferiority in clinically significant cancer detection rates among men undergoing prostate biopsies post-MRI and men undergoing prostate biopsies post-MRI only after a shared decision based on individualised risk estimation. Men without previous diagnosis of prostate cancer and with abnormal digital rectal examination findings and/or prostate-specific antigen between 2.5 ug/L and 20.0 ug/L are included. We aim to recruit 830 men who are randomised at a 1:1 ratio into control (all undergo biopsies after MRI) and intervention arms (the decision to perform biopsies is based on risk estimation and shared decision-making). The primary outcome of the study is the proportion of men with clinically significant prostate cancer (Gleason 4+3 prostate cancer or higher). We will also compare the overall biopsy rate, benign biopsy rate and the detection of non-significant prostate cancer between the two study groups. Ethics and dissemination The study (protocol V.2.0, 4 January 2021) was approved by the Ethics Committee of the Hospital District of Southwest Finland (IORG number: 0001744, IBR number: 00002216; trial number: 99/1801/2019). Participants are required to provide written informed consent. Full reports of this study will be submitted to peer-reviewed journals, mainly urology and radiology. How to read biparametric MRI in men with a clinical suspicious of prostate cancer: Pictorial review for beginners with public access to imaging, clinical and histopathological database
Prostate Magnetic Resonance Imaging (MRI) is increasingly being used in men with a clinical suspicion of prostate cancer (PCa). Performing prostate MRI without the use of an intravenous contrast (IV) agent in men with a clinical suspicion of PCa can lead to reduced MRI scan time. Enabling a large array of different medical providers (from mid-level to specialized radiologists) to evaluate and potentially report prostate MRI in men with a clinical suspicion of PCa with a high accuracy could be one way to enable wide adoption of prostate MRI in men with a clinical suspicion of PCa. The aim of this pictorial review is to provide an insight into acquisition, quality control and reporting of prostate MRI performed without IV contrast agent in men with a clinical suspicion of PCa, aimed specifically at radiologists starting reporting prostate MRI, urologists, urology/radiology residents and mid-level medical providers without experience in reporting prostate MRI. Free public access (http://petiv.utu.fi/improd/and http://petiv.utu.fi/multiimprod/) to complete datasets of 161 and 338 men is provided. The imaging datasets are accompanied by clinical, laboratory and histopathological findings. Several topics are simplified in order to provide a solid base for the development of skills needed for an unsupervised review and potential reporting of prostate MRI in men with a clinical suspicion of PCa. The current review represents the first step towards enabling a large array of different medical providers to review and report accurately prostate MRI performed without IV contrast agent in men with a clinical suspicion of PCa
