    IMPLEMENTACIÓN DE UN DETECTOR DE CAÍDAS PARA SU APLICACIÓN EN PACIENTES HOSPITALIZADOS Y PERSONAS DE LA TERCERA EDAD

    ResumenEn este trabajo se presenta la implementación de un detector de caídas para aplicarlo en pacientes hospitalizados y de la tercera edad. Una unidad de medición inercial MPU-6050 es utilizada para registrar los cambios en la aceleración de los movimientos de los individuos que lo porten. Se utiliza un Raspberry Pi 3 para procesar los datos adquiridos mediante la unidad de medición inercial. Se utilizó Python para la implementación de nuestros algoritmos. Los datos se analizaron a través de un algoritmo, que al detectar un cambio drástico en la aceleración genera una alarma, la cual es trasmitida a una central de alarma remota utilizando WiFi. Los resultados muestran que un umbral de aceleración de 2g es suficiente para detectar caídas.     CONSTRUCCIÓN DE UN SISTEMA DE ADQUISICIÓN DE TEMPERATURA DE SUELO Y NIVEL DE MANTO FREÁTICO PARA APLICACIONES EN BOMBAS DE CALOR DE FUENTES TERRESTRES (CONSTRUCTION OF A SYSTEM FOR THE ACQUISITION OF SOIL TEMPERATURE AND GROUND FLOOR LEVEL FOR APPLICATIONS IN HEAT PUMPS FROM GROUND SOURCES)

    Resumen Las bombas de calor de fuente terrestre son sistemas renovables y amigables con el medio ambiente que utilizan la energía subterránea superficial para lograr la calefacción y refrigeración de espacios con el fin de brindar comodidad térmica. En el presente trabajo se presenta la implementación de un sistema de adquisición de datos de nivel freático y temperatura del subsuelo para aplicaciones en bombas de calor de fuentes terrestres. Se han hecho investigaciones sobre el suelo para conocer sus características térmicas en diferentes ciudades de México, sin embargo, no se han hecho estas investigaciones en la zona de Ciudad del Carmen Campeche. Se utilizaron sondas de temperatura Dallas ds18b20 y un sensor de distancia ultrasónico HC-SR04. Para la transmisión y recepción inalámbrica se utilizó el transceptor NRF24L01. El sistema de lectura de sensores-transmisión y el sistema de recepción-adquisición de datos fue implementado con el microcontrolador Atmega328p en una tarjeta de desarrollo de prototipos Arduino uno. El sistema de lectura y transmisión de datos fue del tipo autónomo alimentado por un panel solar de 12 voltios y 450 mah de tipo mono cristalino. Se utilizó una batería de plomo-ácido de 12 voltios. La adquisición de datos del receptor a una computadora personal fue a través del puerto USB, utilizando el software Processing, el cual almacena los datos cada hora en una hoja de cálculo. Los datos fueron recibidos y almacenados adecuadamente, de igual forma el sistema de energía fotovoltaica funcionó ininterrumpidamente. Abstract Ground Source Heat Pumps are environmentally friendly, renewable systems that use surface underground energy to achieve space heating and cooling for thermal comfort. In this work, the implementation of a temperature and water table data acquisition system for applications in Ground Source Heat Pump is presented. Investigations have been made on the soil to know its thermal characteristics in different cities of Mexico; however these investigations have not been done in the Ciudad del Carmen Campeche area. Dallas ds18b20 temperature probes and an HC-SR04 ultrasonic distance sensor were used. For wireless transmission and reception, the NRF24L01 transceiver was used. The sensor reading-transmission system and the data reception-acquisition system were implemented with the Atmega328p microcontroller on an Arduino Uno prototype development board. The data transmission and reading system was of the autonomous type, powered by a 12 V, 450 mah mono-crystalline type solar cell. A 12 volts lead-acid battery was used. The acquisition of data from the receiver to the pc was through the USB port, using the software processing, which stores the data every hour in a spreadsheet.     CONTROL NUMÉRICO COMPUTARIZADO UTILIZANDO INTERPOLACIÓN LINEAL PARA AUTONIVELAR LA SUPERFICIE DE TRABAJO EN UNA CNC (COMPUTERIZED NUMERICAL CONTROL USING LINEAR INTERPOLATION TO SELF-LEVEL THE WORKING SURFACE IN A CNC)

    En este trabajo se presenta la implementación de una máquina CNC (Control Numérico Computarizado, por sus siglas en español) de bajo costo que utiliza un algoritmo de autonivelación para mejorar el desempeño del maquinado. Mediante el algoritmo se puede corregir por software el desnivel de la cama de fresado independientemente de las imperfecciones del material que se va a maquinar. Básicamente lo que hace el software es deformar el dibujo que se imprimirá de tal manera que este compense la deformación del material. Se utiliza una tarjeta de desarrollo “Arduino UNO” para capturar los datos de una sonda de contacto, posteriormente la información obtenida será procesada por una computadora, que a su vez enviará los datos procesados al Arduino para que este accione los motores. Se utilizó NetBeans para el desarrollo de la plataforma del usuario y el firmware GRBL (para Arduino) como lenguaje de programación, ambos de código libre, además el software Matlab es utilizado para realizar las simulaciones del código. Los resultados muestran que el uso del algoritmo de autonivelación efectivamente mejora el proceso de maquinado.This paper presents the implementation of a CNC machine (Computerized Numerical Control, for its acronym in Spanish) of low cost that uses a self-leveling algorithm to improve the machining performance. By means of the algorithm, the unevenness of the milling bed can be corrected, by software, independently of the imperfections of the material to be machined. Basically, what the software does is to deform the drawing that will be printed in such a way that it compensates for the deformation of the material. An "Arduino UNO" board is used to capture the data from a contact probe, then the information obtained will be processed by a computer, which in turn will send the processed data to the Arduino, so that it drives the motors. NetBeans was used for the development of the user platform and the GRBL firmware (for Arduino) as programming language, both of them are free code, in addition the Matlab software is used to perform the code simulations. The results show that the use of the self-leveling algorithm improves the machining process

El Congreso ofreció un espacio para la presentación, difusión e intercambio de importantes investigaciones nacionales e internacionales ante la comunidad universitaria que se dio cita en el encuentro. El uso de herramientas tecnológicas para la gestión de los trabajos de investigación como la plataforma Open Conference Systems y la web de presentación del Congreso http://citis.blog.ups.edu.ec/, hicieron de CITIS 2017 un verdadero referente entre los congresos que se desarrollaron en el país. La preocupación de nuestra Universidad, de presentar espacios que ayuden a generar nuevos y mejores cambios en la dimensión humana y social de nuestro entorno, hace que se persiga en cada edición del evento la presentación de trabajos con calidad creciente en cuanto a su producción científica.     Executive Summary of ARIA 2019: Integrated care pathways for allergic rhinitis in Argentina, Spain and Mexico

    El impacto sanitario y económico de las enfermedades alérgicas está aumentando rápidamente y se necesitan cambios en las estrategias para su manejo. Su influencia reduce al menos en un tercio la capacidad de desempeño laboral y escolar. Los ICP (Vías Integradas de Atención) de las enfermedades de las vías respiratorias son planes de atención estructurados y multidisciplinarios, que promueven las recomendaciones de las guías en protocolos locales y su aplicación a la práctica clínica. En este documento se presenta un resumen ejecutivo para Argentina, México y España. Se desarrollan las guías ARIA de próxima generación para el tratamiento farmacológico de la rinitis alérgica (RA) utilizando las pautas basadas en GRADE para RA, probadas con evidencia de la vida real proporcionada por tecnología móvil basada en escalas visuales analógicas. Se concluye que en el tratamiento de la RA, los antihistamínicos anti-H1 son menos efectivos que los corticoides intranasales (CINS), que en la rinitis grave los CINS representan la primera línea de tratamiento, y que la combinación intranasal de CINS + anti-H1 es más eficaz que la monoterapia. Sin embargo, según el estudio MASK observacional en vida real, los pacientes tienen pobre adherencia al tratamiento y frecuentemente se automedican de acuerdo con sus necesidades.The health and economic impact of allergic diseases are increasing rapidly, and changes in management strategies are required. Its influence reduces the capacity of work and school performance by at least a third. The ICPs of the airways (integrated care pathways for respiratory diseases) are structured multidisciplinary healthcare plans, promoting the recommendations of the guidelines in local protocols and their application to clinical practice. This document presents an executive summary for Argentina, Mexico, and Spain. Next-generation ARIA guidelines are being developed for the pharmacological treatment of allergic rhinitis (AR), using the GRADE-based guidelines for AR, tested with real-life evidence provided by mobile technology with visual analogue scales. It is concluded that in the AR treatment, H1-antihistamines are less effective than intranasal corticosteroids (INCS), in severe AR the INCS represent the first line of treatment, and intranasal combination INCS + anti-H1 is more effective than monotherapy. However, according to the MASK real-life observational study, patients have poor adherence to treatment and often self-medicate, according to their needs

    Evolution over Time of Ventilatory Management and Outcome of Patients with Neurologic Disease∗

    OBJECTIVES: To describe the changes in ventilator management over time in patients with neurologic disease at ICU admission and to estimate factors associated with 28-day hospital mortality. DESIGN: Secondary analysis of three prospective, observational, multicenter studies. SETTING: Cohort studies conducted in 2004, 2010, and 2016. PATIENTS: Adult patients who received mechanical ventilation for more than 12 hours. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Among the 20,929 patients enrolled, we included 4,152 (20%) mechanically ventilated patients due to different neurologic diseases. Hemorrhagic stroke and brain trauma were the most common pathologies associated with the need for mechanical ventilation. Although volume-cycled ventilation remained the preferred ventilation mode, there was a significant (p < 0.001) increment in the use of pressure support ventilation. The proportion of patients receiving a protective lung ventilation strategy was increased over time: 47% in 2004, 63% in 2010, and 65% in 2016 (p < 0.001), as well as the duration of protective ventilation strategies: 406 days per 1,000 mechanical ventilation days in 2004, 523 days per 1,000 mechanical ventilation days in 2010, and 585 days per 1,000 mechanical ventilation days in 2016 (p < 0.001). There were no differences in the length of stay in the ICU, mortality in the ICU, and mortality in hospital from 2004 to 2016. Independent risk factors for 28-day mortality were age greater than 75 years, Simplified Acute Physiology Score II greater than 50, the occurrence of organ dysfunction within first 48 hours after brain injury, and specific neurologic diseases such as hemorrhagic stroke, ischemic stroke, and brain trauma. CONCLUSIONS: More lung-protective ventilatory strategies have been implemented over years in neurologic patients with no effect on pulmonary complications or on survival. We found several prognostic factors on mortality such as advanced age, the severity of the disease, organ dysfunctions, and the etiology of neurologic disease

    Relation of Lipoprotein(a) Levels to Incident Type 2 Diabetes and Modification by Alirocumab Treatment

    none1691siOBJECTIVE: In observational data, lower levels of lipoprotein(a) have been associated with greater prevalence of type 2 diabetes. Whether pharmacologic lowering of lipoprotein(a) influences incident type 2 diabetes is unknown. We determined the relationship of lipoprotein(a) concentration with incident type 2 diabetes and effects of treatment with alirocumab, a PCSK9 inhibitor. RESEARCH DESIGN AND METHODS: In the ODYSSEY OUTCOMES trial alirocumab was compared with placebo in patients with acute coronary syndrome. Incident diabetes was determined from laboratory, medication, and adverse event data. RESULTS: Among 13,480 patients without diabetes at baseline, 1,324 developed type 2 diabetes over a median 2.7 years. Median baseline lipoprotein(a) was 21.9 mg/dL. With placebo, 10 mg/dL lower baseline lipoprotein(a) was associated with hazard ratio 1.04 (95% CI 1.02-1.06, P < 0.001) for incident type 2 diabetes. Alirocumab reduced lipoprotein(a) by a median 23.2% with greater absolute reductions from higher baseline levels and no overall effect on incident type 2 diabetes (hazard ratio 0.95, 95% CI 0.85-1.05). At low baseline lipoprotein(a) levels, alirocumab tended to reduce incident type 2 diabetes, while at high baseline lipoprotein(a) alirocumab tended to increase incident type 2 diabetes compared with placebo (treatment-baseline lipoprotein(a) interaction P = 0.006). In the alirocumab group, a 10 mg/dL decrease in lipoprotein(a) from baseline was associated with hazard ratio 1.07 (95% CI 1.03-1.12; P = 0.0002) for incident type 2 diabetes. CONCLUSIONS: In patients with acute coronary syndrome, baseline lipoprotein(a) concentration associated inversely with incident type 2 diabetes. Alirocumab had neutral overall effect on incident type 2 diabetes. However, treatment-related reductions in lipoprotein(a), more pronounced from high baseline levels, were associated with increased risk of incident type 2 diabetes.     Lipoprotein(a) and Benefit of PCSK9 Inhibition in Patients With Nominally Controlled LDL Cholesterol

    International audienceBackground: Guidelines recommend nonstatin lipid-lowering agents in patients at very high risk for major adverse cardiovascular events (MACE) if low-density lipoprotein cholesterol (LDL-C) remains ≥70 mg/dL on maximum tolerated statin treatment. It is uncertain if this approach benefits patients with LDL-C near 70 mg/dL. Lipoprotein(a) levels may influence residual risk.Objectives: In a post hoc analysis of the ODYSSEY Outcomes (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) trial, the authors evaluated the benefit of adding the proprotein subtilisin/kexin type 9 inhibitor alirocumab to optimized statin treatment in patients with LDL-C levels near 70 mg/dL. Effects were evaluated according to concurrent lipoprotein(a) levels.Methods: ODYSSEY Outcomes compared alirocumab with placebo in 18,924 patients with recent acute coronary syndromes receiving optimized statin treatment. In 4,351 patients (23.0%), screening or randomization LDL-C was 13.7 mg/dL or ≤13.7 mg/dL; corresponding adjusted treatment hazard ratios were 0.82 (95% CI: 0.72-0.92) and 0.89 (95% CI: 0.75-1.06), with Pinteraction = 0.43.Conclusions: In patients with recent acute coronary syndromes and LDL-C near 70 mg/dL on optimized statin therapy, proprotein subtilisin/kexin type 9 inhibition provides incremental clinical benefit only when lipoprotein(a) concentration is at least mildly elevated.     Highly-parallelized simulation of a pixelated LArTPC on a GPU

    The rapid development of general-purpose computing on graphics processing units (GPGPU) is allowing the implementation of highly-parallelized Monte Carlo simulation chains for particle physics experiments. This technique is particularly suitable for the simulation of a pixelated charge readout for time projection chambers, given the large number of channels that this technology employs. Here we present the first implementation of a full microphysical simulator of a liquid argon time projection chamber (LArTPC) equipped with light readout and pixelated charge readout, developed for the DUNE Near Detector. The software is implemented with an end-to-end set of GPU-optimized algorithms. The algorithms have been written in Python and translated into CUDA kernels using Numba, a just-in-time compiler for a subset of Python and NumPy instructions. The GPU implementation achieves a speed up of four orders of magnitude compared with the equivalent CPU version. The simulation of the current induced on 10310^3 pixels takes around 1 ms on the GPU, compared with approximately 10 s on the CPU. The results of the simulation are compared against data from a pixel-readout LArTPC prototype

    The DUNE Far Detector Vertical Drift Technology, Technical Design Report

    International audienceDUNE is an international experiment dedicated to addressing some of the questions at the forefront of particle physics and astrophysics, including the mystifying preponderance of matter over antimatter in the early universe. The dual-site experiment will employ an intense neutrino beam focused on a near and a far detector as it aims to determine the neutrino mass hierarchy and to make high-precision measurements of the PMNS matrix parameters, including the CP-violating phase. It will also stand ready to observe supernova neutrino bursts, and seeks to observe nucleon decay as a signature of a grand unified theory underlying the standard model. The DUNE far detector implements liquid argon time-projection chamber (LArTPC) technology, and combines the many tens-of-kiloton fiducial mass necessary for rare event searches with the sub-centimeter spatial resolution required to image those events with high precision. The addition of a photon detection system enhances physics capabilities for all DUNE physics drivers and opens prospects for further physics explorations. Given its size, the far detector will be implemented as a set of modules, with LArTPC designs that differ from one another as newer technologies arise. In the vertical drift LArTPC design, a horizontal cathode bisects the detector, creating two stacked drift volumes in which ionization charges drift towards anodes at either the top or bottom. The anodes are composed of perforated PCB layers with conductive strips, enabling reconstruction in 3D. Light-trap-style photon detection modules are placed both on the cryostat's side walls and on the central cathode where they are optically powered. This Technical Design Report describes in detail the technical implementations of each subsystem of this LArTPC that, together with the other far detector modules and the near detector, will enable DUNE to achieve its physics goals
