    Staphylococcus aureus typing by digestion of protein A coding gene using Bsp143I

    Background: Protein A is the virulence factors of Staphylococcus aureus rolling in its pathogenesis, and its gene is used for typing. Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) with different enzymes has been used for this action. Objectives: In this study, we used Bsp143I enzyme for digestion of the gene, coding protein A (spa gene) in S. aureus. The bacteria were isolated from patients and healthy carriers in Gorgan, north of Iran. Patients and Methods: DNAs of 128 S. aureus subjects (53 from healthy carriers and 75 from patients) were extracted and amplified using specific primers of the spa gene. The product was digested by Bsp143I enzyme and its pattern was assessed by gel electrophoresis. Results: There were seven spa types among the tested S. aureus samples, among which six types differed in the repeated X region of the spa gene, but the seventh type had a deletion on one of BSP143I restriction sites. The frequency of spa types among isolated S. aureus samples as well as healthy carriers was six and five, respectively. S. aureus isolated from wounds showed the most diverse spa types (five) among clinical samples. Types 1, 2 and 4 were observed in all clinical samples, while only one case of type 3 was identified among patients, whereas this type constituted over 32% of the isolates among carriers. We found seven and four spa types among methicillin-resistant S. aureus (MRSA) and methicillin-sensitive S. aureus (MSSA) isolates, respectively. Conclusions: Our results showed that typing the spa gene using PCR-RFLP using Bsp143I was an acceptable method for typing S. aureus. Furthermore, this survey showed that the types in healthy carriers and MSSA were more variable than patient and MRSA isolates, respectively. We used the Bsp143I enzyme, which was not used in any previous studies on the spa gene. The results of this study suggested that we can use PCR-RFLP of spa gene by Bsp143I for molecular typing and sequencing of S. aureus, instead of relatively expensive methods. This method is relatively rapid and inexpensive, and can be accomplished in centers with conventional molecular facilities. © 2014, Ahvaz Jundishapur University of Medical Sciences

    Solar extreme ultraviolet variability of the quiet Sun

    The last solar minimum has been unusually quiet compared to the previous minima (since space-based radiometric measurements are available). The Sun's magnetic flux was substantially lower during this minimum. Some studies also show that the total solar irradiance during the minimum after cycle 23 may have dropped below the values known from the two minima prior to that. For chromospheric and coronal radiation, the situation is less clear-cut. The Sun's 10.7\,cm flux shows a decrease of ∼4%\sim4\% during the solar minimum in 2008 compared to the previous minimum, but \ion{Ca}{II} K does not. Here we consider additional wavelengths in the extreme ultraviolet (EUV), specifically transitions of \ion{He}{I} at 584.3\,\AA\ and \ion{O}{V} at 629.7\,\AA , of which the CDS spectrometer aboard SOHO has been taking regular scans along the solar central meridian since 1996. We analysed this unique dataset to verify if and how the radiance distribution undergoes measurable variations between cycle minima. To achieve this aim we determined the radiance distribution of quiet areas around the Sun centre. Concentrating on the last two solar minima, we found out that there is very little variation in the radiance distribution of the chromospheric spectral line \ion{He}{I} between these minima. The same analysis shows a modest, although significant, 4\% variation in the radiance distribution of the transition region spectral line \ion{O}{V}. These results are comparable to those obtained by earlier studies employing other spectral features, and they confirm that chromospheric indices display a small variation, whereas in the TR a more significant reduction of the brighter features is visible

    Accessory gene regulator types of Staphylococcus aureus isolated in Gorgan, North of Iran

    Background: Staphylococcus aureus is a gram-positive bacterium that has remained a persistent pathogen, causing infections such as endocarditis, meningitis, and toxic shock syndrome in humans. The accessory gene regulator (agr) system of Staphylococcus aureus is responsible for controlling the expression of many genes that code for virulence factors. In this study, we assessed the S.aureus agr Group, based on their source of isolation, in Gorgan, North of Iran. Materials and Methods: DNA of 194 S. aureus isolates was extracted by lysozyme-phenol chloroform method, which included 85 clinical samples, 58 samples which were isolated from noses of health care workers and 51 cases which were obtained from food products in Gorgan, northern Iran. PCR-based assays were used to evaluate agr locus nucleotide polymorphism for the identification of agr specificity Group. Distributions of each agr Group were determined and comparison between different sources was assessed by X2. A p-value of <0.05 was considered as significant. Results: The majority of isolates belonged to agr Group I (43.3%), followed by agr Group III (28.87%), agr Group II (22.68%), and agr Group IV (5.15%). In our study, a majority of S. aureus isolates were recovered from health care workers and food product specimens were of agr Group I and isolates which were recovered from patients were of agr Group III. These differences were statistically significant (P=0.005). There was no statistical difference between the source of isolation of clinical samples of S.aureus and agr type. Conclusion: Agr Group I was predominant among health care workers and food product specimens in Gorgan, North of Iran, but in strains which were isolated from patients, agr Group III was predominant. Investigating the possible role of agr Group III in Staphylococcus aureus infection in future studies is recommended

    S100B Protein as a Post-traumatic Biomarker for Prediction of Brain Death in Association With Patient Outcomes

    Background: S100B is a calcium-binding protein, belonging to the S100 family proteins which are characterized by their high solubility and, currently, comprises 21 members which are expressed in a cell-specific manner. If we can predict the possibility of definite brain death after brain injury, we will rescue some organs of body to transplant proposes. Objectives: In this regard our study focused on the S100B protein value in predicting brain death after head trauma. In this study, the use of serum level of protein S100, 24 hours after trauma has been considered as a reliable index for predicting brain death. Patients and Methods: 72 patients (50 male and 22 female) aged 5 - 80 years old (median 40 ± 17.72 years) with severe head traumas (GCS ≤ 8) were recruited in this cross-sectional study. Glasgow Coma Scale (GCS) and computed tomography (CT) scan findings were recorded for all patients, and then a single 5mL blood sample was obtained from each patient on admission, after 48 hours and a week later or after brain death to determine the level of S100B protein. Results: Primary and the last GCS of patients had a predictive value in determining brain death (P < 0.0005), also there was a significant correlation between GCS and level of S100B protein. There was a significant correlation between CT scan findings and S100B protein only after 48 hours of trauma. Conclusions: Changes in S100B protein, especially the levels of this dimer 48 hours after trauma can be used as marker to predict brain death. Alongside other known prognostic factors such as age, GCS and diameters of the pupils, however, this factor individually can not conclusive predict the patient's clinical course and incidence of brain death. However, it is suitable to use GCS, CT scan, clinical symptoms and biomarkers together for a perfect prediction of brain death

    Marked increase in the incidence rate of esophageal adenocarcinoma in a high-risk area for esophageal cancer

    BACKGROUND: Esophageal cancer (EC) is the eighth common cancer worldwide. Esophageal squamous cell carcinoma (ESCC) and adenocarcinoma (EAD) are the most common histologic types of EC. Many recent reports showed an increasing trend in EAD and a decreasing trend in ESCC in many Western countries. Golestan Province in northeastern Iran has been known as a high-risk area for EC. The aim of this study was to describe the time trend of EAD in this area between 2000-2009. METHODS: Data on cancer cases were obtained from Golestan Population-based Cancer Registry. Analysis was done using Joinpoint software. To examine the incidence trends, the annual percent change was calculated. The possibilities of anatomic and histologic misclassification were considered by assessing the trend of ESCC and gastric adenocarcinoma. RESULTS: A total number of 1186 histologically-confirmed EC cases were recruited. The incidence rate of EAD showed a significant increasing trend. There was no significant trend in the incidence of ESCC during the study period. A significant increase in the incidence rate of gastric adenocarcinoma was observed during the period of 2000-2005, followed by a plateau during the period of 2005-2009. CONCLUSIONS: We found a significant increasing trend in the incidence rate of EAD. We find no evidence to support an alternative explanation including anatomic and histologic misclassification. So, the observed rise in the incidence of EAD seems to be real. Therefore, designing and implementation of control programs, including control of preventable risk factors of EAD, should be considered in this high- risk area

    Effect of Matricaria Camomilla Aroma on Severity of first stage Labor Pain

    Abstract Background and Aims: Labor pain relief is one of the challenges in health care systems in the most countries and is the main goal in midwifery cares. This study aimed to investigate the Effect of Matricaria Camomilla Aroma on Severity of first stage Labor Pain in primiparous women in Abhar Emdadi hospital, 2013. Methods: This study was a randomized clinical trial carried out on 130 eligible primiparous women (65 in intervention and 65 in control group) at Emdadi hospital of Abhar. In the aroma therapy group gauze were soaked in 2 drop of Matrica Camomilla water and in the control group gauze were soaked in 2 drop of distilled water and attached to the women's collars. Pain severity was measured in first and after intervention in 3-4, 5-7, and 8-10 centimeter dilatations. Data were collected by demographic and obstetrics questionnaire, observation and exam checklist and the numeral scale of pain measurement. Results: The mean pain severity score after the intervention in 3-4 cm (5/75 1/99), 5-7cm (7/84 1/52) and 8-10cm (8/55 1/22) decreased significantly compared with control group in dilatation 3-4 cm (6/93 1/96), 5-7 cm (9/41 0/63) and 8-10 cm (9/8 0/40). (p<0/005). Conclusion:The findings of the study revealed that aroma therapy using Matrica Camomilla water alleviates the labor pain. More research for uses Matrica Camomilla to reduce the labor pain is recommended

    Rapid DNA extraction of bacterial genome using laundry detergents and assessment of the efficiency of DNA in downstream process using polymerase chain reaction

    Genomic DNA extraction from bacterial cells involves processes normally performed in most biological laboratories. Therefore, various methods have been offered, manually and kit, but these methods may be time consuming and costly. In this paper, genomic DNA extraction of Pseudomonas aeruginosa was investigated using some laundry detergent brands available in Iran. Afterwards, efficiency of the detergents was compared with manually standard methods and kits. To evaluate the efficiency of the genomic DNA in the processes in which DNA is used as a template, the polymerase chain reaction (PCR) tests and enzyme digestion of PCR product were used. The results show that the detergents could be used to extract genomic DNA. Among the brands studied, five-enzyme Taj and three-enzyme Saftlan had the best performance compared to standard methods.Key words: Bacterial genome, DNA extraction, laundry powder, detergent

    Prevalence of chronic kidney disease and its associated risk factors: The first report from Iran using both microalbuminuria and urine sediment

    Background: The incidence of major risk factors of chronic kidney disease (CKD) in the world is on the rise, and it is expected that this incidence and prevalence, particularly in developing countries, will continue to increase. Using data on urinary sediment and microalbuminuria, we aimed to estimate the prevalence of CKD in northeast Iran. Methods: In a cross-sectional study, the prevalence of CKD in a sample of 1557 regionally representative people, aged � 18 years, was analyzed. CKD was determined based on glomerular filtration rate (GFR) and microalbuminuria. Life style data, urine and blood samples were collected. Urine samples without any proteinuria in the initial dipstick test were checked for qualitative microalbuminuria. If the latter was positive, quantitative microalbuminuria was evaluated. Results: 1557 subjects with a mean age of 56.76 ± 12.04 years were enrolled in this study. Based on the modifcation of diet in renal disease (MDRD) equation, 137 subjects (8.89%) were categorized as CKD stages III-V. Based on urine abnormalities, the prevalence of combined CKD stages I and II was 10.63%, and based on macro- and microalbuminuria it was 14.53%. The prevalence of CKD was significantly associated with sex, age, marital status, education, diabetes mellitus (DM), hypertension (HTN), ischemic heart disease (IHD), waist to hip ratio, myocardial infarction (MI), and cerebrovascular accident (CVA). Conclusion: CKD and its main risk factors are common and represent a definite health threat in this region of Iran. Using and standardizing less expensive screening tests in low resource countries could be a good alternative that may improve the outcome through early detection of CKD

    Southeast of Iran is an endemic area for Malaria and Crimean-Congo hemorrhagic fever (CCHF). In 1999, we faced with an outbreak of CCHF in Sistan and Baluchistan Province, in the border of Pakistan and Afghanistan. The most cases of Malaria in Iran are also reported from this area. This article presents a 17-year- old woman who admitted to our hospital because of acute fever, headache, epistaxis, hemorrhagic lesions on the skin and vaginal bleeding.
