    Characterization of community-acquired Clostridioides difficile strains in Israel, 2020–2022

    BackgroundThe prevalence of community-acquired Clostridioides difficile infection (CA-CDI) has been rising, due to changes in antibiotics prescribing practices, emergence of hypervirulent strains and improved diagnostics. This study explored CA-CDI epidemiology by examining strain diversity and virulence factors of CA-CDI isolates collected across several geographical regions in Israel.MethodsStool samples of 126 CA-CDI patients were subjected to PCR and an immunoassay to identify toxin genes and proteins, respectively. Toxin loci PaLoc and PaCdt were detected by whole-genome sequencing (WGS). Biofilm production was assessed by crystal violet-based assay. Minimum inhibitory concentration was determined using the Etest technique or agar dilution. WGS and multi-locus sequence typing (MLST) were used to classify strains and investigate genetic diversity.ResultsSequence types (ST) 2 (17, 13.5%), ST42 (13, 10.3%), ST104 (10, 8%) and ST11 (9, 7.1%) were the most common. All (117, 92.8%) but ST11 belonged to Clade 1. No associations were found between ST and gender, geographic area or antibiotic susceptibility. Although all strains harbored toxins genes, 34 (27%) produced toxin A only, and 54 (42.9%) strains produced toxin B only; 38 (30.2%) produced both toxins. Most isolates were biofilm-producers (118, 93.6%), primarily weak producers (83/118, 70.3%). ST was significantly associated with both biofilm and toxin production.ConclusionC. difficile isolates in Israel community exhibit high ST diversity, with no dominant strain. Other factors may influence the clinical outcomes of CDI such as toxin production, antibiotic resistance and biofilm production. Further studies are needed to better understand the dynamics and influence of these factors on CA-CDI

    Parenting and child and adolescent mental health during the COVID-19 pandemic

    Background: Early work indicates the significant impact of the COVID-19 pandemic on the mental health of children and adolescents. Understanding which children may be more at risk for mental health problems, and which risk factors are amenable to change is crucial. The importance of studying children’s mental health within the context of the family system is recognized. Methods: The current study investigated associations between parent factors, and children’s mental health during the early phase of the COVID-19 pandemic across a number of Western countries (primarily Australia and the United Kingdom). Parents (N = 385) reported on their pandemic-related stress, mental health, and parenting behaviors, in addition to mental health changes in their 5-17 year old children (N = 481) during April/May 2020. Results: Analyses revealed significant associations between parent COVID-19 pandemic stress, parent depression, anxiety and stress symptoms, and increases in child internalizing and externalizing problems. Harsh parenting behavior was associated with trauma symptoms and increases in externalizing problems. Further, some associations were more pronounced for children with existing mental health problems, and for disadvantaged and single parent families. Limitations: The data was cross-sectional, the majority of participant parents were female, and all data were parent-report. Conclusions: Findings suggest the importance of parents in influencing children’s mental health during the acute phase of the COVID-19 pandemic. Further work is needed to investigate longer-term impacts

    The gut microbiota in anxiety and depression - A systematic review

    Growing evidence indicates the community of microorganisms throughout the gastrointestinal tract, (i.e., gut microbiota), is associated with anxiety and depressive disorders. We present the first systematic review of the gut microbiota in anxiety disorders, along with an update in depression. Consideration of shared underlying features is essential due to the high rates of comorbidity. Systematic searches, following PRISMA guidelines, identified 26 studies (two case-control comparisons of the gut microbiota in generalised anxiety disorder, 18 in depression, one incorporating both anxiety/depression, and five including symptom-only measures). Alpha and beta diversity findings were inconsistent; however, differences in bacterial taxa indicated disorders may be characterised by a higher abundance of proinflammatory species (e.g., Enterobacteriaceae and Desulfovibrio), and lower short-chain fatty acid producing-bacteria (e.g., Faecalibacterium). Several taxa, and their mechanisms of action, may relate to anxiety and depression pathophysiology via communication of peripheral inflammation to the brain. Although the gut microbiota remains a promising target for prevention and therapy, future research should assess confounders, particularly diet and psychotropic medications, and should examine microorganism function

    Observed Measures of Negative Parenting Predict Brain Development during Adolescence.

    Limited attention has been directed toward the influence of non-abusive parenting behaviour on brain structure in adolescents. It has been suggested that environmental influences during this period are likely to impact the way that the brain develops over time. The aim of this study was to investigate the association between aggressive and positive parenting behaviors on brain development from early to late adolescence, and in turn, psychological and academic functioning during late adolescence, using a multi-wave longitudinal design. Three hundred and sixty seven magnetic resonance imaging (MRI) scans were obtained over three time points from 166 adolescents (11-20 years). At the first time point, observed measures of maternal aggressive and positive behaviors were obtained. At the final time point, measures of psychological and academic functioning were obtained. Results indicated that a higher frequency of maternal aggressive behavior was associated with alterations in the development of right superior frontal and lateral parietal cortical thickness, and of nucleus accumbens volume, in males. Development of the superior frontal cortex in males mediated the relationship between maternal aggressive behaviour and measures of late adolescent functioning. We suggest that our results support an association between negative parenting and adolescent functioning, which may be mediated by immature or delayed brain maturation

    Positive parenting predicts the development of adolescent brain structure: A longitudinal study

    Little work has been conducted that examines the effects of positive environmental experiences on brain development to date. The aim of this study was to prospectively investigate the effects of positive (warm and supportive) maternal behavior on structural brain development during adolescence, using longitudinal structural MRI. Participants were 188 (92 female) adolescents, who were part of a longitudinal adolescent development study that involved mother–adolescent interactions and MRI scans at approximately 12 years old, and follow-up MRI scans approximately 4 years later. FreeSurfer software was used to estimate the volume of limbic-striatal regions (amygdala, hippocampus, caudate, putamen, pallidum, and nucleus accumbens) and the thickness of prefrontal regions (anterior cingulate and orbitofrontal cortices) across both time points. Higher frequency of positive maternal behavior during the interactions predicted attenuated volumetric growth in the right amygdala, and accelerated cortical thinning in the right anterior cingulate (males only) and left and right orbitofrontal cortices, between baseline and follow up. These results have implications for understanding the biological mediators of risk and protective factors for mental disorders that have onset during adolescence

    Associations between cognitive and affective empathy and internalizing symptoms in late childhood

    Background: Empathy is a multidimensional construct, which includes cognitive and affective components. Studies in adults have demonstrated that both cognitive and affective empathy are associated with anxious and depressive symptoms. The aim of this study was to examine these associations in childhood. Methods: Participants were 127 9- and 10-year-old children, recruited from the community. Self-report measures of cognitive and affective empathy, and internalizing symptoms were administered, as well as a task-based measure of cognitive empathy. Results: Canonical correlation analysis demonstrated that components of affective empathy, specifically affective sharing and empathic distress, were associated with internalizing (particularly social anxiety) symptoms (Rc = 0.63, non-parametric p < .001). Cognitive empathy was not associated with internalizing symptoms. Limitations: Most of our findings were based around self-report measures of empathy, which may not accurately reflect empathy ability. Conclusions: Findings suggests that children who share each other’s emotions strongly are more likely to experience anxiety, particularly of a social nature

    Dual-axis hormonal covariation in adolescence and the moderating influence of prior trauma and aversive maternal parenting

    Adversity early in life can disrupt the functioning of the hypothalamic-pituitary-adrenal and hypothalamic-pituitary-gonadal axes and increase risk for negative health outcomes. The interplay between these axes and the environment is complex, and understanding needs to be advanced by the investigation of the multiple hormonal relationships underlying these processes. The current study examined basal hormonal associations between morning levels of cortisol, testosterone, and dehydroepiandrosterone in a cohort of adolescents (mean age 15.56 years). The moderating influence of childhood adversity was also examined, as indexed by self-reported trauma (at mean age 14.91), and observed maternal aggressive parenting (at mean age 12.41). Between-person regressions revealed significant associations between hormones that were moderated by both measures of adversity. In females, all hormones positively covaried, but also interacted with adversity, such that positive covariation was typically only present when levels of trauma and/or aggressive parenting were low. In males, hormonal associations and interactions were less evident; however, interactions were detected for cortisol-testosterone - positively covarying at high levels of aggressive parenting but negatively covarying at low levels - and DHEA-cortisol - similarly positively covarying at high levels of parental aggression. These results demonstrate associations between adrenal and gonadal hormones and the moderating role of adversity, which is likely driven by feedback mechanisms, or cross-talk, between the axes. These findings suggest that hormonal changes may be the pathway through which early life adversity alters physiology and increases health risks, but does so differentially in the sexes; however further study is necessary to establish causation

    The influence of sex, temperament, risk-taking and mental health on the emergence of gambling: a longitudinal study of young people

    There are a host of complex and interlinked psychological, social and biological factors involved in the development of problem gambling (PG). While existing research, which is predominantly cross-sectional, shows that emerging adulthood is a critical period for PG, the early risk factors for PG are currently unknown. Here, we recruited a sample of 156 early adolescents with no history of PG (mean age 12.6 years) and longitudinally followed them up into late adolescence (mean age 18.9 years) to investigate the role of sex, risk-taking behaviour and changes in temperament and psychiatric symptoms in the evolution of risky gambling behaviour. There were sex-independent effects of temperament and risk-taking behaviour, with greater developmental increases in temperamental frustration (i.e. negative affectivity), greater developmental decreases in temperamental attention (i.e. effortful control) and greater involvement in risky behaviours, such as alcohol use, predicting greater likelihood of being in the risky gambling group. In addition, there were sex-dependent effects whereby higher levels of baseline aggression in females and lower levels of the same in males were more predictive of risky gambling. These findings highlight how sex-dependent and independent factors across the early- to mid-adolescent period influence the occurrence of gambling later in life

    Early onset adolescent depression is related to poor prognosis and a range of psychiatric and medical comorbidities later in life, making the identification of a priori risk factors for depression highly important. Increasingly, dysregulated levels of immune and neuroendocrine markers, such as C-reactive protein (CRP) and cortisol, have been demonstrated as both precursors to and consequences of depression.
