    Propulsion simulator for magnetically-suspended wind tunnel models

    The objective of phase two of a current investigation sponsored by NASA Langley Research Center is to demonstrate the measurement of aerodynamic forces/moments, including the effects of exhaust gases, in magnetic suspension and balance system (MSBS) wind tunnels. Two propulsion simulator models are being developed: a small-scale and a large-scale unit, both employing compressed, liquified carbon dioxide as propellant. The small-scale unit was designed, fabricated, and statically-tested at Physical Sciences Inc. (PSI). The large-scale simulator is currently in the preliminary design stage. The small-scale simulator design/development is presented, and the data from its static firing on a thrust stand are discussed. The analysis of this data provides important information for the design of the large-scale unit. A description of the preliminary design of the device is also presented

    Disassembly of interchromatin granule clusters alters the coordination of transcription and pre-mRNA splicing

    To examine the involvement of interchromatin granule clusters (IGCs) in transcription and pre-mRNA splicing in mammalian cell nuclei, the serine-arginine (SR) protein kinase cdc2-like kinase (Clk)/STY was used as a tool to manipulate IGC integrity in vivo. Both immunofluorescence and transmission electron microscopy analyses of cells overexpressing Clk/STY indicate that IGC components are completely redistributed to a diffuse nuclear localization, leaving no residual structure. Conversely, overexpression of a catalytically inactive mutant, Clk/STY(K190R), causes retention of hypophosphorylated SR proteins in nuclear speckles. Our data suggest that the protein-protein interactions responsible for the clustering of interchromatin granules are disrupted when SR proteins are hyperphosphorylated and stabilized when SR proteins are hypophosphorylated. Interestingly, cells without intact IGCs continue to synthesize nascent transcripts. However, both the accumulation of splicing factors at sites of pre-mRNA synthesis as well as pre-mRNA splicing are dramatically reduced, demonstrating that IGC disassembly perturbs coordination between transcription and pre-mRNA splicing in mammalian cell nuclei

    Intrapancreatic accessory spleen false positive to 68Ga-Dotatoc: case report and literature review

    Background: Intrapancreatic accessory spleen (IPAS) is an uncommon finding of pancreatic mass. Differential diagnosis with pancreatic tumor, especially with non-functional neuroendocrine tumor (NF-NET), may be very hard and sometimes it entails unnecessary surgery. A combination of CT scan, MRI, and nuclear medicine can confirm the diagnosis of IPAS. 68-Ga-Dotatoc PET/CT is the gold standard in NET diagnosis and it can allow to distinguish between IPAS and NET. Case presentation: A 69-year-old man was admitted to our hospital for an incidental nodule in the tail of the pancreas with focal uptake of 68-Ga-dotatate at PET/CT. NET was suspected and open distal splenopancreatectomy was performed. Pathologic examination revealed an IPAS. Conclusion: This is the second IPAS case in which a positive 68Ga-Dotatoc uptake led to a false diagnosis of pancreatic NET. Here is a proposal of a literature review

    Reduced Admissions for Cerebrovascular Events during COVID-19 Outbreak in Italy

    Background and Purpose: We aimed to investigate the rate of hospital admissions for cerebrovascular events and of revascularization treatments for acute ischemic stroke in Italy during the coronavirus disease 2019 (COVID-19) outbreak. Methods: The Italian Stroke Organization performed a multicenter study involving 93 Italian Stroke Units. We collected information on hospital admissions for cerebrovascular events from March 1 to March 31, 2020 (study period), and from March 1 to March 31, 2019 (control period). Results: Ischemic strokes decreased from 2399 in 2019 to 1810 in 2020, with a corresponding hospitalization rate ratio (RR) of 0.75 ([95% CI, 0.71-0.80] P<0.001); intracerebral hemorrhages decreased from 400 to 322 (hospitalization RR, 0.81 [95% CI, 0.69-0.93]; P=0.004), and transient ischemic attacks decreased from 322 to 196 (hospitalization RR, 0.61 [95% CI, 0.51-0.73]; P<0.001). Hospitalizations decreased in Northern, Central, and Southern Italy. Intravenous thrombolyses decreased from 531 (22.1%) in 2019 to 345 in 2020 (19.1%; RR, 0.86 [95% CI, 0.75-0.99]; P=0.032), while primary endovascular procedures increased in Northern Italy (RR, 1.61 [95% CI, 1.13-2.32]; P=0.008). We found no correlation (P=0.517) between the hospitalization RRs for all strokes or transient ischemic attack and COVID-19 incidence in the different areas. Conclusions: Hospitalizations for stroke or transient ischemic attacks across Italy were reduced during the worst period of the COVID-19 outbreak. Intravenous thrombolytic treatments also decreased, while endovascular treatments remained unchanged and even increased in the area of maximum expression of the outbreak. Limited hospitalization of the less severe patients and delays in hospital admission, due to overcharge of the emergency system by COVID-19 patients, may explain these data

    Relationship of national institutes of health stroke scale to 30-day mortality in medicare beneficiaries with acute ischemic stroke.

    BackgroundThe National Institutes of Health Stroke Scale (NIHSS), a well-validated tool for assessing initial stroke severity, has previously been shown to be associated with mortality in acute ischemic stroke. However, the relationship, optimal categorization, and risk discrimination with the NIHSS for predicting 30-day mortality among Medicare beneficiaries with acute ischemic stroke has not been well studied.Methods and resultsWe analyzed data from 33102 fee-for-service Medicare beneficiaries treated at 404 Get With The Guidelines-Stroke hospitals between April 2003 and December 2006 with NIHSS documented. The 30-day mortality rate by NIHSS as a continuous variable and by risk-tree determined or prespecified categories were analyzed, with discrimination of risk quantified by the c-statistic. In this cohort, mean age was 79.0 years and 58% were female. The median NIHSS score was 5 (25th to 75th percentile 2 to 12). There were 4496 deaths in the first 30 days (13.6%). There was a strong graded relation between increasing NIHSS score and higher 30-day mortality. The 30-day mortality rates for acute ischemic stroke by NIHSS categories were as follows: 0 to 7, 4.2%; 8 to 13, 13.9%; 14 to 21, 31.6%; 22 to 42, 53.5%. A model with NIHSS alone provided excellent discrimination whether included as a continuous variable (c-statistic 0.82 [0.81 to 0.83]), 4 categories (c-statistic 0.80 [0.79 to 0.80]), or 3 categories (c-statistic 0.79 [0.78 to 0.79]).ConclusionsThe NIHSS provides substantial prognostic information regarding 30-day mortality risk in Medicare beneficiaries with acute ischemic stroke. This index of stroke severity is a very strong discriminator of mortality risk, even in the absence of other clinical information, whether used as a continuous or categorical risk determinant.     Preferred music listening is associated with perceptual learning enhancement at the expense of self-focused attention

    Can preferred music listening improve following attentional and learning performances? Here we suggest that this may be the case. In Experiment 1, following preferred and non-preferred musical-piece listening, we recorded electrophysiological responses to an auditory roving-paradigm. We computed the mismatch negativity (MMN – the difference between responses to novel and repeated stimulation), as an index of perceptual learning, and we measured the correlation between trial-by-trial EEG responses and the fluctuations in Bayesian Surprise, as a quantification of the neural attunement with stimulus informational value. Furthermore, during music listening, we recorded oscillatory cortical activity. MMN and trial-by-trial correlation with Bayesian surprise were significantly larger after subjectively preferred versus non-preferred music, indicating the enhancement of perceptual learning. The analysis on oscillatory activity during music listening showed a selective alpha power increased in response to preferred music, an effect often related to cognitive enhancements. In Experiment 2, we explored whether this learning improvement was realized at the expense of self-focused attention. Therefore, after preferred versus non-preferred music listening, we collected Heart-Beat Detection (HBD) accuracy, as a measure of the attentional focus toward the self. HBD was significantly lowered following preferred music listening. Overall, our results suggest the presence of a specific neural mechanism that, in response to aesthetically pleasing stimuli, and through the modulation of alpha oscillatory activity, redirects neural resources away from the self and toward the environment. This attentional up-weighting of external stimuli might be fruitfully exploited in a wide area of human learning activities, including education, neurorehabilitation and therapy.     Determinants of Early Recurrence of Cerebral Infarction: The Stroke Data Bank

    We Studied 1,273 Patients with Ischemic Cerebral Infarction Who Were Entered into the Stroke Data Bank, a Prospective, Observational Study Involving Four University Hospitals and the Biometry and Field Studies Branch of the National Institute of Neurological Disorders and Stroke. Forty Patients Had Non-Iatrogenic Recurrent Stroke within 30 Days after the Index Cerebral Infarction. using Life Tables, the 30-Day Cumulative± SE Risk of Early Recurrence for All Infarctions Was 3.3±0.4%. the Risk of Early Recurrence Was Greatest for Atherothrombotic Infarction (7.9±2.2%, Eight of 113 Patients) and Least for Lacunar Infarction (2.2±1.2%, Eight of 337 Patients). Both Cardioembolic Infarction (4.3±0.9%, 10 of 246 Patients) and Infarction of Undetermined Cause (3.0±0.5%, 14 of 508 Patients) Had Intermediate Risks. History of Hypertension and Diabetes Mellitus, as Well as Diastolic Hypertension and Elevated Blood Sugar Concentration at Admission, Were Associated with Early Recurrence. Logistic Regression Analysis Estimated the Risk of Early Recurrence to Be 8.56% in Those with Coexisting Hypertension and a Glucose Concentration of 300 Mg/dl Versus 0.77% in the Absence of These Two Abnormalities. Early Recurrence Was Associated with Longer Median Duration of Initial Hospital Stay (27 vs.. 14 Days) and a Higher 30-Day Case—fatality Rate (20% vs.. 7.4%). Increased Weakness Scores Were Associated with Early Recurrent Stroke.     Propulsion simulator for magnetically-suspended wind tunnel models

    In order to demonstrate the measurement of aerodynamic forces/moments, including the effects of exhaust jets in Magnetic Suspension and Balance System (MSBS) wind tunnels, two propulsion simulator models were developed at Physical Sciences Inc. (PSI). Both the small-scale model (1 in. diameter X 8 in. long) and the large-scale model (2.5 in. diameter X 15 in. long) employed compressed, liquefied carbon dioxide as a propellant. The small-scale simulator, made from a highly magnetizable iron alloy, was demonstrated in the 7 in. MSBS wind tunnel at the University of Southampton. It developed a maximum thrust of approximate 1.3 lbf with a 0.098 in. diameter nozzle and 0.7 lbf with a 0.295 in. diameter nozzle. The Southampton MSBS was able to control the simulator at angles-of attack up to 20 deg. The large-scale simulator was demonstrated to operate in both a steady-state and a pulse mode via a miniaturized solinoid valve. It developed a stable and repeatable thrust of 2.75 lbf over a period of 4s and a nozzle pressure ratio (NPR) of 5

    Detection of 33.8 ms X-ray pulsations in SAX J0635+0533

    We revisited the BeppoSAX observation of SAX J0635+0533, suggested as a possible counterpart to the gamma-ray source 2EG J0635+0521. We have discovered a 33.8 ms pulsation from the source and derived an improved position, consistent with the location of the Be star proposed as a binary companion.
