    The Physiologic Effects of Multiple Simultaneous Electronic Control Device Discharges

    Objectives: Law enforcement and military personnel use electronic control devices to control non-compliant and actively resistive subjects. The TASER® Shockwave is a new electronic control device designed specifically as an area denial device capable of delivering multiple simultaneous discharges. This is the first study to examine the effects of multiple simultaneous device discharges in humans.Methods: Volunteers were exposed to multiple (two to three), simultaneous 5-second discharges from the Shockwave device to the chest, back, chest to abdomen, or thighs. Blood was analyzed before and after discharge for pH, lactate, potassium, creatine kinase (CK), and troponin. Continuous spirometry was performed before, during, and after the discharge. In addition, electrocardiograms (ECGs) before and after discharge were recorded, and echocardiography was used to determine the rhythm during discharge.Results: Small elevations of lactate occurred. Moderate increases in CK at 24 hours occurred and appeared to be related to the number of simultaneous discharges. There was a trend to a decrease in minute ventilation in the volunteers exposed to two simultaneous discharges, but it did not reach statistical significance. ECG changes only reflected an increase in vagal tone, and there was no evidence of capture by echocardiography. Five-second, simultaneous, multiple exposures to the TASER Shockwave device were reasonably tolerated by our human volunteers.Conclusion: Our study suggests that this device may have a reasonable risk/benefit ratio when used to protect an area from a threat.     SAM: A Tool for Measurement of Moderate to Vigorous Physical Activity (MVPA) in School Physical Education

Observational methods have been a primary methodology used by physical educators for assessing teacher and student behaviors in school physical education (PE) classes for over 30 years. Observational instruments traditionally used in PE are economical, but are time intensive and complicated. Recently national PE recommendations have been promoted to encourage practitioners to achieve ≥ 50% of PE class in moderate to vigorous physical activity (MVPA). The purpose of this preliminary study was to develop, validate, and test the reliability of the Simple Activity Measurement (SAM) instrument for assessing MVPA during school PE classes. Students (N=36, representing grades 3-10) from a convenient sample of schools in San Antonio, TX were randomly selected to wear SUUNTOTM heart rate (HR) monitors during PE classes as an MVPA intensity measure, and the SAM instrument was used to observe 6 classes (N=281 students) for MVPA each minute using the SAM 0-10 scale. The SAM instrument was found to be a significant predictor for HR (r=0.555, r2=0.308, p\u3c0.05) using linear regression, and the intra class correlation coefficient to test reliability was found to be R=0.803, p\u3c0.05. Students averaged 88.5 % of class time spent in MVPA at the elementary level and only 36.5 % of class time spent in MVPA at the high school level. The results of this preliminary study indicate that the SAM has promise to provide school PE evaluators with an effective economical observation tool to document minutes of student MVPA in PE classes, and to promote accountability with national MVPA recommendations

    Pulsar scintillation through thick and thin: Bow shocks, bubbles, and the broader interstellar medium

    Observations of pulsar scintillation are among the few astrophysical probes of very small-scale (≲ au) phenomena in the interstellar medium (ISM). In particular, characterization of scintillation arcs, including their curvature and intensity distributions, can be related to interstellar turbulence and potentially overpressurized plasma in local ISM inhomogeneities, such as supernova remnants, H II regions, and bow shocks. Here we present a survey of eight pulsars conducted at the Five-hundred-metre Aperture Spherical Telescope (FAST), revealing a diverse range of scintillation arc characteristics at high sensitivity. These observations reveal more arcs than measured previously for our sample. At least nine arcs are observed toward B1929+10 at screen distances spanning ~90 per cent of the pulsar’s 361 pc path length to the observer. Four arcs are observed toward B0355+54, with one arc yielding a screen distance as close as ∼105 au (<1 pc) from either the pulsar or the observer. Several pulsars show highly truncated, low-curvature arcs that may be attributable to scattering near the pulsar. The scattering screen constraints are synthesized with continuum maps of the local ISM and other well-characterized pulsar scintillation arcs, yielding a three-dimensional view of the scattering media in context

    Computerized assessment of vessel morphological changes during treatment of glioblastoma multiforme: Report of a case imaged serially by MRA over four years

    A patient with glioblastoma mutliforme underwent serial computerized analysis of tumor-associated vasculature defined from magnetic resonance angiographic (MRA) scans obtained over almost a four year period. The clinical course included tumor resection with subsequent radiation therapy, a long symptom-free interval, emergence of a new malignant focus, resection of that focus, a stroke, and treatment with chemotherapy and anti-angiogenic therapy. Image analysis methods included segmentation of vessels from each MRA and statistical comparison of vessel morphology over 4 regions of interest (the initial tumor site, the second tumor site, a distant control region, and the entire brain) to the same 4 regions of interest in 50 healthy volunteers (26 females and 24 males; mean age 39 years). Results suggested that following completion of focal radiation therapy (RT) vessel shape abnormalities, if elevated at the time of RT completion, may progressively normalize for months in focal regions, that progressively severe vessel shape abnormalities can precede the emergence of a gadolinium enhancing lesion by months, that lesion resection can produce a dramatic but highly transient drop in abnormal vessel tortuosity both focally and globally, and that treatment with anti-angiogenic agents does not necessarily normalize vessel shape. Quantitative measurements of vessel morphology as defined from MRA may provide useful insights into tumor development and response to therapy

    Multi-Way Multi-Group Segregation and Diversity Indices

    Background: How can we compute a segregation or diversity index from a three-way or multi-way contingency table, where each variable can take on an arbitrary finite number of values and where the index takes values between zero and one? Previous methods only exist for two-way contingency tables or dichotomous variables. A prototypical three-way case is the segregation index of a set of industries or departments given multiple explanatory variables of both sex and race. This can be further extended to other variables, such as disability, number of years of education, and former military service. Methodology/Principal Findings: We extend existing segregation indices based on Euclidean distance (square of coefficient of variation) and Boltzmann/Shannon/Theil index from two-way to multi-way contingency tables by including multiple summations. We provide several biological applications, such as indices for age polyethism and linkage disequilibrium. We also provide a new heuristic conceptualization of entropy-based indices. Higher order association measures are often independent of lower order ones, hence an overall segregation or diversity index should be the arithmetic mean of the normalized association measures at all orders. These methods are applicable when individuals selfidentify as multiple races or even multiple sexes and when individuals work part-time in multiple industries.     Two cilengitide regimens in combination with standard treatment for patients with newly diagnosed glioblastoma and unmethylated MGMT gene promoter: results of the open-label, controlled, randomized phase II CORE study

    Background Survival outcomes for patients with glioblastoma remain poor, particularly for patients with unmethylated O6-methylguanine-DNA methyltransferase (MGMT) gene promoter. This phase II, randomized, open-label, multicenter trial investigated the efficacy and safety of 2 dose regimens of the selective integrin inhibitor cilengitide combined with standard chemoradiotherapy in patients with newly diagnosed glioblastoma and an unmethylated MGMT promoter. Methods Overall, 265 patients were randomized (1:1:1) to standard cilengitide (2000 mg 2×/wk; n = 88), intensive cilengitide (2000 mg 5×/wk during wk 1−6, thereafter 2×/wk; n = 88), or a control arm (chemoradiotherapy alone; n = 89). Cilengitide was administered intravenously in combination with daily temozolomide (TMZ) and concomitant radiotherapy (RT; wk 1−6), followed by TMZ maintenance therapy (TMZ/RT→TMZ). The primary endpoint was overall survival; secondary endpoints included progression-free survival, pharmacokinetics, and safety and tolerability. Results Median overall survival was 16.3 months in the standard cilengitide arm (hazard ratio [HR], 0.686; 95% CI: 0.484, 0.972; P = .032) and 14.5 months in the intensive cilengitide arm (HR, 0.858; 95% CI: 0.612, 1.204; P = .3771) versus 13.4 months in the control arm. Median progression-free survival assessed per independent review committee was 5.6 months (HR, 0.822; 95% CI: 0.595, 1.134) and 5.9 months (HR, 0.794; 95% CI: 0.575, 1.096) in the standard and intensive cilengitide arms, respectively, versus 4.1 months in the control arm. Cilengitide was well tolerated. Conclusions Standard and intensive cilengitide dose regimens were well tolerated in combination with TMZ/RT→TMZ.     Commensal discovery of four fast radio bursts during Parkes Pulsar Timing Array observations

    The Parkes Pulsar Timing Array (PPTA) project monitors two dozen millisecond pulsars (MSPs) in order to undertake a variety of fundamental physics experiments using the Parkes 64-m radio telescope. Since 2017 June, we have been undertaking commensal searches for fast radio bursts (FRBs) during the MSP observations. Here, we report the discovery of four FRBs (171209, 180309, 180311, and 180714). The detected events include an FRB with the highest signal-to-noise ratio ever detected at the Parkes Observatory, which exhibits unusual spectral properties. All four FRBs are highly polarized.
