    Exotic see-saw mechanism for neutrinos and leptogenesis in a Pati-Salam model

    We discuss non-perturbative corrections to the neutrino sector, in the context of a D-brane Pati-Salam-like model, that can be obtained as a simple alternative to SO(10)SO(10) GUT's in theories with open and unoriented strings. In such D-brane models, exotic stringy instantons can correct the right-handed neutrino mass matrix in a calculable way, thus affecting mass hierarchies and modifying the see-saw mechanism to what we name exotic see-saw. For a wide range of parameters, a compact spectrum of right-handed neutrino masses can occur that gives rise to a predictive scenario for low energy observables. This model also provides a viable mechanism for Baryon Asymmetry in the Universe (BAU) through leptogenesis. Finally, a Majorana mass for the neutron is naturally predicted in the model, leading to potentially testable neutron-antineutron oscillations. Combined measurements in neutrino and neutron-antineutron sectors could provide precious informations on physics at the quantum gravity scale.Comment: 35 pages, 2 figures, matches the journal versio

    Recent advances in cancer photo-theranostics: the synergistic combination of transition metal complexes and gold nanostructures

    AbstractIn this mini review, we highlight advances in the last five years in light-activated cancer theranostics by using hybrid systems consisting of transition metal complexes (TMCs) and plasmonic gold nanostructures (AuNPs). TMCs are molecules with attractive properties and high potential in biomedical application. Due to their antiproliferative abilities, platinum-based compounds are currently first-choice drugs for the treatment of several solid tumors. Moreover, ruthenium, iridium and platinum complexes are well-known for their ability to photogenerate singlet oxygen, a highly cytotoxic reactive species with a key role in photodynamic therapy. Their potential is further extended by the unique photophysical properties, which make TMCs particularly suitable for bioimaging. Recently, gold nanoparticles (AuNPs) have been widely investigated as one of the leading nanomaterials in cancer theranostics. AuNPs—being an inert and highly biocompatible material—represent excellent drug delivery systems, overcoming most of the side effects associated with the systemic administration of anticancer drugs. Furthermore, due to the thermoplasmonic properties, AuNPs proved to be efficient nano-sources of heat for photothermal therapy application. Therefore, the hybrid combination TMC/AuNPs could represent a synergistic merger of multiple functionalities for combinatorial cancer therapy strategies. Herein, we report the most recent examples of TMC/AuNPs systems in in-vitro in-vivo cancer tharanostics application whose effects are triggered by light-exposure in the Vis–NIR region, leading to a spatial and temporal control of the TMC/AuNPs activation for light-mediated precision therapeutics

    Vaginal treatment of endometrial cancer: role in the elderly

    BACKGROUND: To compare abdominal hysterectomy, the most currently used for treating cancer of the endometrium, to the vaginal hysterectomy in term of survival, morbidity and failure rates. METHODS: We retrospectively analyzed 68 cases divided into two sub-groups. A study group of 31 cases received vaginal surgery; a control group of 37 cases was treated with a laparotomy. Mean operative time, median hospital stay, intra- and post-operative complications, DFS and OS time as well as occurrence of local or distant recurrences have been evaluated and reported. Cases included patients with a higher rate of medical morbidities (p = 0.01) than controls. RESULTS: Mean age was 76.2 and 70.4 years in the vaginal (V) group and abdominal (A) group respectively. Mean operative time was longer for the group A. Group V patients had a lower mean post-operative hospital stay (p < 0.05). Differences in the two groups regarding intra- and post-operative complications, occurrence of local or distant recurrences and DFS time were not statistically significant. Disease specific survival time at 5 years scored 97% for group V, and 97% for group A. CONCLUSIONS: Results show how vaginal approach had a similar outcome in selected patients. Vaginal surgery could therefore be the proper choice in patients with early stages and lower surgical risk, in addition to elderly patients exposed to a higher surgical risk

    Stand-alone oblique lumbar interbody fusion (OLIF) for the treatment of adjacent segment disease (ASD) after previous posterior lumbar fusion: clinical and radiological outcomes and comparison with posterior revision surgery

    Background: Radiological evidence of adjacent segment disease (ASD) has been reported to have a prevalence of more than 30% and several risk factors have been reported. The aim of this study is to evaluate the clinical and radiological outcomes of patients with symptomatic ASD treated with stand-alone OLIF and compare results with a posterior revision surgery cohort. Methods: This is a retrospective case-control study. Clinical-patient-reported outcomes were obtained at preoperative, postoperative and final follow-up visits using the Short Form (SF-36) scale, the Oswestry Disability Index (ODI) and the visual analog scale (VAS). Radiological measures include lumbar lordosis (LL), segmental lordosis (SL), pelvic incidence-lumbar lordosis (PI-LL) mismatch, segmental coronal Cobb angle and intervertebral disc height (DH). The data are compared with a retrospective series of patients that underwent a posterior revision surgery for ASD. Results: Twenty-eight patients in the OLIF group and 25 patients in the posterior group meet inclusion criteria. The mean ages at the time of the surgery are 65.1 years and 67.5, respectively. The mean follow-up time is 36.1 months (range of 14-56). The clinical outcomes significantly improve from preoperative values from the surgery in both groups. The radiological parameters are significantly improved postoperatively and were maintained at the last follow-up in both groups. A statistically significant difference is observed between the two groups for minor complication rate, length of surgery, blood loss and DH restoration. Conclusions: Stand-alone OLIF is an effective and safe technique with low morbidity and complication rates for the treatment of selected patients with symptomatic ASD following a previous lumbar fusion

    Carmustine wafers implantation in patients with newly diagnosed high grade glioma: is it still an option?

    Background: The implantation protocol for Carmustine Wafers (CWs) in high grade glioma (HGG) was developed to offer a bridge between surgical resection and adjuvant treatments, such as radio- and chemotherapy. In the last years, however, a widespread use of CWs has been limited due to uncertainties regarding efficacy, in addition to increased risk of infection and elevated costs of treatment. Objective: The aims of our study were to investigate the epidemiology of patients that underwent surgery for HGG with CW implantation, in addition to the assessment of related complications, long-term overall survival (OS), and associated prognostic factors. Methods: Three different medical databases were screened for conducting a systematic review of the literature, according to the PRISMA statement guidelines, evaluating the role of BCNU wafer implantation in patients with newly diagnosed HGG. The search query was based on a combination of medical subject headings (MeSH): "high grade glioma " [MeSH] AND "Carmustine " [MeSH] and free text terms: "surgery " OR "BCNU wafer " OR "Gliadel " OR "systemic treatment options " OR "overall survival. " Results: The analysis of the meta-data demonstrated that there was a significant advantage in using CWs in newly diagnosed GBM in terms of OS, and a very low heterogeneity among the included studies [mean difference 2.64 (95% CI 0.85, 4.44); p = 0.004; I2149 = 0%]. Conversely, no significant difference between the two treatment groups in terms of PFS wad detected (p = 0.55). The analysis of complications showed a relatively higher rate in Carmustine implanted patients, although this difference was not significant (p = 0.53). Conclusions: This meta-analysis seems to suggest that CWs implantation plays a significant role in improving the OS, when used in patients with newly diagnosed HGG. To minimize the risk of side effects, however, a carful patient selection based mainly on patient age and tumor volume should be desirable

    Energy-oriented denial of service attacks: an emerging menace for large cloud infrastructures

    This work analyzes a new and very subtle kind of security threat that can affect large-scale cloud-based IT service infrastructures, by exploiting the computational resources of their component data center to waste as much energy as possible. The consequence of these threats ranges from increased costs in the energy bill, to penalization for exceeding the agreed quantity of greenhouse gases (GHG) emissions, up to complete denial of service caused by electrical outages due to power budget exhaustion. We analyzed different types of such attacks with their potential impacts on the energy consumption, modeled their behavior and quantified how current energy-proportional technologies may provide attackers with great opportunities for raising the target facility emissions and costs. These efforts resulted in a simple model with some parametric reference values that can be used to estimate the impact of such attacks also in presence of very large infrastructures containing thousands or millions of servers.Peer ReviewedPostprint (author's final draft
