Do self-reported intentions predict clinicians behaviour: a systematic review.
Background: Implementation research is the scientific study of methods to promote the systematic uptake of
clinical research findings into routine clinical practice. Several interventions have been shown to be effective in
changing health care professionals' behaviour, but heterogeneity within interventions, targeted behaviours, and
study settings make generalisation difficult. Therefore, it is necessary to identify the 'active ingredients' in
professional behaviour change strategies. Theories of human behaviour that feature an individual's "intention" to
do something as the most immediate predictor of their behaviour have proved to be useful in non-clinical
populations. As clinical practice is a form of human behaviour such theories may offer a basis for developing a
scientific rationale for the choice of intervention to use in the implementation of new practice. The aim of this
review was to explore the relationship between intention and behaviour in clinicians and how this compares to
the intention-behaviour relationship in studies of non-clinicians.
Methods: We searched: PsycINFO, MEDLINE, EMBASE, CINAHL, Cochrane Central Register of Controlled
Trials, Science/Social science citation index, Current contents (social & behavioural med/clinical med), ISI
conference proceedings, and Index to Theses. The reference lists of all included papers were checked manually.
Studies were eligible for inclusion if they had: examined a clinical behaviour within a clinical context, included
measures of both intention and behaviour, measured behaviour after intention, and explored this relationship
quantitatively. All titles and abstracts retrieved by electronic searching were screened independently by two
reviewers, with disagreements resolved by discussion.
Discussion: Ten studies were found that examined the relationship between intention and clinical behaviours in
1623 health professionals. The proportion of variance in behaviour explained by intention was of a similar
magnitude to that found in the literature relating to non-health professionals. This was more consistently the case
for studies in which intention-behaviour correspondence was good and behaviour was self-reported. Though firm
conclusions are limited by a smaller literature, our findings are consistent with that of the non-health professional
literature. This review, viewed in the context of the larger populations of studies, provides encouragement for
the contention that there is a predictable relationship between the intentions of a health professional and their
subsequent behaviour. However, there remain significant methodological challenges

Identifying muon rings in VERITAS data using convolutional neural networks trained on images classified with Muon Hunters 2
Muons from extensive air showers appear as rings in images taken with imaging atmospheric Cherenkov telescopes, such as VERITAS. These muon-ring images are used for the calibration of the VERITAS telescopes, however the calibration accuracy can be improved with a more efficient muon-identification algorithm. Convolutional neural networks (CNNs) are used in many state-of-the-art image-recognition systems and are ideal for muon image identification, once trained on a suitable dataset with labels for muon images. However, by training a CNN on a dataset labelled by existing algorithms, the performance of the CNN would be limited by the suboptimal muon-identification efficiency of the original algorithms. Muon Hunters 2 is a citizen science project that asks users to label grids of VERITAS telescope images, stating which images contain muon rings. Each image is labelled 10 times by independent volunteers, and the votes are aggregated and used to assign a 'muon' or 'non-muon' label to the corresponding image. An analysis was performed using an expert-labelled dataset in order to determine the optimal vote percentage cut-offs for assigning labels to each image for CNN training. This was optimised so as to identify as many muon images as possible while avoiding false positives. The performance of this model greatly improves on existing muon identification algorithms, identifying approximately 30 times the number of muon images identified by the current algorithm implemented in VEGAS (VERITAS Gamma-ray Analysis Suite), and roughly 2.5 times the number identified by the Hough transform method, along with significantly outperforming a CNN trained on VEGAS-labelled data

Why do women invest in pre-pregnancy health and care? A qualitative investigation with women attending maternity services
Background Despite the importance attributed to good pre-pregnancy care and its potential to improve pregnancy and child health outcomes, relatively little is known about why women invest in pre-pregnancy health and care. We sought to gain insight into why women invested in pre-pregnancy health and care. Methods We carried out 20 qualitative in-depth interviews with pregnant or recently pregnant women who were drawn from a survey of antenatal clinic attendees in London, UK. Interviewees were purposively sampled to include high and low investors in pre-pregnancy health and care, with variation in age, partnership status, ethnicity and pre-existing medical conditions. Data analysis was conducted using the Framework method. Results We identified three groups in relation to pre-pregnancy health and care: 1) The “prepared” group, who had high levels of pregnancy planning and mostly positive attitudes to micronutrient supplementation outside of pregnancy, carried out pre-pregnancy activities such as taking folic acid and making changes to diet and lifestyle. 2) The “poor knowledge” group, who also had high levels of pregnancy planning, did not carry out pre-pregnancy activities and described themselves as having poor knowledge. Elsewhere in their interviews they expressed a strong dislike of micronutrient supplementation. 3) The “absent pre-pregnancy period” group, had the lowest levels of pregnancy planning and also expressed anti-supplement views. Even discussing the pre-pregnancy period with this group was difficult as responses to questions quickly shifted to focus on pregnancy itself. Knowledge of folic acid was poor in all groups. Conclusion Different pre-pregnancy care approaches are likely to be needed for each of the groups. Among the “prepared” group, who were proactive and receptive to health messages, greater availability of information and better response from health professionals could improve the range of pre-pregnancy activities carried out. Among the “poor knowledge” group, better response from health professionals might yield greater uptake of pre-pregnancy information. A different, general health strategy might be more appropriate for the “absent pre-pregnancy period” group. The fact that general attitudes to micronutrient supplementation were closely related to whether or not women invested in pre-pregnancy health and care was an unanticipated finding and warrants further investigation.This report is independent research commissioned and funded by the Department of Health Policy Research Programme Pre-Pregnancy Health and Care in England: Exploring Implementation and Public Health Impact, 006/0068
A Search for Very High-Energy Gamma Rays from the Missing Link Binary Pulsar J1023+0038 with VERITAS
The binary millisecond radio pulsar PSR J1023+0038 exhibits many
characteristics similar to the gamma-ray binary system PSR B1259--63/LS 2883,
making it an ideal candidate for the study of high-energy non-thermal emission.
It has been the subject of multi-wavelength campaigns following the
disappearance of the pulsed radio emission in 2013 June, which revealed the
appearance of an accretion disk around the neutron star. We present the results
of very high-energy gamma-ray observations carried out by VERITAS before and
after this change of state. Searches for steady and pulsed emission of both
data sets yield no significant gamma-ray signal above 100 GeV, and upper limits
are given for both a steady and pulsed gamma-ray flux. These upper limits are
used to constrain the magnetic field strength in the shock region of the PSR
J1023+0038 system. Assuming that very high-energy gamma rays are produced via
an inverse-Compton mechanism in the shock region, we constrain the shock
magnetic field to be greater than 2 G before the disappearance of the

VERITAS and Multiwavelength Observations of the BL Lacertae Object 1ES 1741+196
We present results from multiwavelength observations of the BL Lacertae
object 1ES 1741+196, including results in the very-high-energy -ray
regime using the Very Energetic Radiation Imaging Telescope Array System
(VERITAS). The VERITAS time-averaged spectrum, measured above 180 GeV, is
well-modelled by a power law with a spectral index of
. The integral flux above 180
GeV is
m s, corresponding to 1.6% of the Crab Nebula flux on average.
The multiwavelength spectral energy distribution of the source suggests that
1ES 1741+196 is an extreme-high-frequency-peaked BL Lacertae object. The
observations analysed in this paper extend over a period of six years, during
which time no strong flares were observed in any band. This analysis is
therefore one of the few characterizations of a blazar in a non-flaring state.Comment: 8 pages, 5 figures. Accepted for publication in MNRA
Discovery of Very High Energy Gamma Rays from 1ES 1440+122
The BL Lacertae object 1ES 1440+122 was observed in the energy range from 85
GeV to 30 TeV by the VERITAS array of imaging atmospheric Cherenkov telescopes.
The observations, taken between 2008 May and 2010 June and totalling 53 hours,
resulted in the discovery of -ray emission from the blazar, which has a
redshift =0.163. 1ES 1440+122 is detected at a statistical significance of
5.5 standard deviations above the background with an integral flux of
(2.8) 10
cm s (1.2\% of the Crab Nebula's flux) above 200 GeV. The
measured spectrum is described well by a power law from 0.2 TeV to 1.3 TeV with
a photon index of 3.1 0.4 0.2.
Quasi-simultaneous multi-wavelength data from the Fermi Large Area Telescope
(0.3--300 GeV) and the Swift X-ray Telescope (0.2--10 keV) are additionally
used to model the properties of the emission region. A synchrotron self-Compton
model produces a good representation of the multi-wavelength data. Adding an
