    Case report: Tackling the complexities of an extremely premature newborn with intrauterine growth restriction and congenital metabolic disorders through a multidisciplinary approach

    Background and objectivesThe premature birth of a newborn can present a complex challenge for healthcare providers, particularly in cases of extreme prematurity combined with intrauterine growth restriction and multiple metabolic deficiencies. In this report, we aim to shed light on the difficulties and considerations involved in the management of such a case. In addition, our study is aimed to raise awareness of the importance of a multidisciplinary team in managing an extreme premature case with multiple comorbidities.Case presentation and main findingsWe present the case of a 28-week premature female newborn with very low birth weight (660 g, percentile <10%) and intrauterine growth restriction. She was born through emergency cesarean delivery due to maternal Hemolysis, Elevated Liver enzymes, and Low Platelet count (HELLP) syndrome and had a high-risk pregnancy (spontaneous twin pregnancy, with one fetus stopping development at 16 weeks and maternal hypertension). In the first hours of life, she presented with persistent hypoglycemia requiring progressive glucose supplementation up to 16 g/kg/day to maintain normal blood glucose levels. The baby then showed favorable progress. However, from days 24 to 25, hypoglycemia recurred and did not respond to glucose boluses or supplementation in both intravenous and oral feeds, leading to the suspicion of a congenital metabolic disorder. Endocrine and metabolic screenings led to suspicion of primary carnitine deficiency and a deficiency in hepatic form of carnitine-palmitoyltransferase type I (CPT1) on the second screening.Conclusion and clinical implicationsThe study highlights rare metabolic anomalies that can be due to both organ and system immaturity and delayed enteral feeding and excessive use of antibiotics. The clinical implications of this study emphasize the need for careful monitoring and comprehensive care of premature infants to prevent and manage potential metabolic abnormalities by neonatal metabolic screening

    Chromium Substituted Cobalt Ferrites by Glycine-Nitrates Process

    Chromium substituted cobalt ferrites (CoFe2–xCrxO4, 0 ≀ x ≀ 2) were synthesized through solution combustion method using glycine as fuel, named glycine-nitrates process (GNP). As evidenced by X-ray diffraction data (XRD), single cubic spinel phase was formed for all CoFe2–xCrxO4 (0 ≀ x ≀ 2) series. The cubic lattice parameter (a) decreases with increasing chromium content. Room temperature 57Fe Mössbauer spectra revealed the Fe3+ and Cr3+ site occupancy, the local hyperfine magnetic fields and the substitution of Fe3+ by Cr3+ in the lattice. Scanning electron microscopy (SEM) showed a refinement of particle size with the increase of Cr3+ content. Magnetic measurements from 5 K to 120 K have shown a dropping in the saturation magnetization as the chromium content increases. This behaviour has been explained in terms of substitution of Fe3+ by Cr3+ in the cubic lattice of cobalt ferrite

    LURK: Server-Controlled TLS Delegation

    By design, TLS (Transport Layer Security) is a 2-party, end-to-end protocol. Yet, in practice, TLS delegation is often deployed: that is, middlebox proxies inspect and even modify TLS traffic between the endpoints. Recently, industry-leaders (e.g., Akamai, Cloudflare, Telefonica, Ericcson), standardization bodies (e.g., IETF, ETSI), and academic researchers have proposed numerous ways of achieving safer TLS delegation. We present LURK the LURK (Limited Use of Remote Keys) extension for TLS~1.2, a suite of designs for TLS delegation, where the TLS-server is aware of the middlebox. We implement and test LURK. We also cryptographically prove and formally verify, in Proverif, the security of LURK. Finally, we comprehensively analyze how our designs balance (provable) security and competitive performance

    The Effect of Clay Type on the Physicochemical Properties of New Hydrogel Clay Nanocomposites

    This study focuses on the investigation of clay type effect on the final properties of semi-interpenetrated Salecan/poly(methacrylic acid)/clay hydrogel nanocomposites. Previous studies have indicated that the presence of clay in polymer composites leads to better swelling capacity and mechanical properties as functions of clay type. On the other hand, Salecan, which is a water soluble extracellular polysaccharide, was proved to assure greater flexibility to hydrogels. These properties recommend clay and Salecan for semi-interpenetrated hydrogels preparation with specific application in biomedicine. The purpose was to determine the most suitable type of clay as well as Salecan influence for developing the desired water retention/delivery ability and mechanically enhanced semi-interpenetrating polymer network (SIPN) nanocomposites. For our investigations, we have chosen commercially available montmorillonite (ClNa) and different commercial organomodified clay (Cl30B, Cl20A and Cl15A). Several analyses results (FTIR, TGA, DMA, XRD, microscopy and swelling studies) demonstrated that not only the presence of Salecan but also the clay type influenced the structure and properties of the final nanocomposites

Although globally the proportion of children who receive recommended vaccines has remained steady for the past few years, we observed during our clinical practice that the vaccination rate in Romania might be lower than the needed vaccination rate. The objective of this study is to test this hypothesis and to highlight the causes of low vaccination rates in Romania

    Significant benefits of AIP testing and clinical screening in familial isolated and young-onset pituitary tumors

    Context Germline mutations in the aryl hydrocarbon receptor-interacting protein (AIP) gene are responsible for a subset of familial isolated pituitary adenoma (FIPA) cases and sporadic pituitary neuroendocrine tumors (PitNETs). Objective To compare prospectively diagnosed AIP mutation-positive (AIPmut) PitNET patients with clinically presenting patients and to compare the clinical characteristics of AIPmut and AIPneg PitNET patients. Design 12-year prospective, observational study. Participants & Setting We studied probands and family members of FIPA kindreds and sporadic patients with disease onset ≀18 years or macroadenomas with onset ≀30 years (n = 1477). This was a collaborative study conducted at referral centers for pituitary diseases. Interventions & Outcome AIP testing and clinical screening for pituitary disease. Comparison of characteristics of prospectively diagnosed (n = 22) vs clinically presenting AIPmut PitNET patients (n = 145), and AIPmut (n = 167) vs AIPneg PitNET patients (n = 1310). Results Prospectively diagnosed AIPmut PitNET patients had smaller lesions with less suprasellar extension or cavernous sinus invasion and required fewer treatments with fewer operations and no radiotherapy compared with clinically presenting cases; there were fewer cases with active disease and hypopituitarism at last follow-up. When comparing AIPmut and AIPneg cases, AIPmut patients were more often males, younger, more often had GH excess, pituitary apoplexy, suprasellar extension, and more patients required multimodal therapy, including radiotherapy. AIPmut patients (n = 136) with GH excess were taller than AIPneg counterparts (n = 650). Conclusions Prospectively diagnosed AIPmut patients show better outcomes than clinically presenting cases, demonstrating the benefits of genetic and clinical screening. AIP-related pituitary disease has a wide spectrum ranging from aggressively growing lesions to stable or indolent disease course
