Proton Pump Inhibitors (PPIs) (Omeprazole, Lansoprazole, Pantoprazole, Rabeprazole and Esomeprazole), one of the most commonly prescribed classes of medications in the primary care setting, are considered a major advance in the treatment of acid-peptic diseases. In Italy PPIs are reimbursed by National Health Service on the basis of CUF (Commissione Unica del Farmaco) 1 and 48 Notes. In 2002 and 2003 a significant increase in PPIs consumption and expenditure have been documented, showing differences between regions. The aim of this study is to investigate and monitor, at regional level, type and entity of PPIs use through a drug utilization study, evaluating prescribing behaviour and compliance of PPIs treatments with CUF Notes indications. The study has been carried out on a sample of 436 General Practitioners belonging to 22 Piemonte's ASL (Aziende Sanitarie Locali). The data analysis shows that acid-related pathologies are significantly more common in patients with at least 50 years of age and the most frequent condition is represented by gastroesophageal reflux disease. Despite the general conditions of PPIs use by General Practitioners in terms of duration and dosage of therapy result in most cases (from 49% to 80% for duration and from 54% to 97% for dosage) compliant with what proposed by CUF Notes, in some cases the same CUF Notes indications seem to be not observed. Consequently the Piemonte Region has decided to plan a guideline on PPIs rational use. Such guideline, expected to be introduced in the regional area, may also be considered as an instrument able to lead to a more appropriate expenditure for this drug class. Moreover, in order to control PPIs expenditure, pharmacoeconomic methodologies can be applied allowing to identify the most cost - effective active substance and therapeutic scheme, overcoming CUF Notes which consider all PPIs use under the same reimbursement conditions

Ovarian Cancer in a Northern Italian Province and the Multidisciplinary Team

Ovarian cancer represents one of the most aggressive female cancers in the world, remaining a tumor with high lethality. This study aims to present how a multidisciplinary team (MDT) approach can improve the prognosis in terms of recurrence and death of patients. In total, 448 ovarian cancer cases registered in an Italian Cancer Registry between 2012 and 2020 were included. Information on age, morphology, stage, and treatment was collected. Recurrence and death rates were reported 1 and 2 years after diagnosis, comparing MDT vs. non-MDT approaches. Ninety-three percent had microscopic confirmation, and most showed cystic-mucinous morphology. In total, 50% were older than 65 years old. The distribution by stage was 17.6%, 4%, 44.9%, and 32.6% for stages I, II, III, and IV, respectively. The women followed by the MDT were 24.1%. Disease-free survival 1-year post-diagnosis, recurrences, recurrences-deaths, and deaths were 67.5%, 14.5%, 8.4%, and 9.6%, respectively, better than the non-MDT group (46.2%, 13.2%, 20.8 %, and 19.8%, respectively) (p < 0.01). The same positive results were confirmed two years after diagnosis, particularly for stages III and IV. Albeit small numbers, the study confirms a better prognosis for women managed by MDT with fewer recurrences and deaths, especially within the first 24 months of diagnosis

Study of high-lying states in 208Pb with the AGATA demonstrator

An experiment aiming at the study of the gamma decay from the Giant Resonance region in the nuclei 208Pb, 90Zr has recently been performed with the AGATA Demonstrator coupled to an array of large-volume scintillators (LaBr3:Ce, BaF2) to increase the overall gamma detection efficiency. The 208Pb and 90Zr nuclei were excited with inelastic scattering of 17O at the incident energy of 20MeV/u. The ejectiles were detected in a pair of silicon E-\u394E telescopes placed at forward angles. Preliminary results concerning the first partial analysis of the measurement with the 208Pb target are here presented. \ua92011 by Societ\ue0 Italiana di Fisica

Response of AGATA segmented HPGe detectors to gamma-rays up to 15.1 MeV

In many in-beam gamma spectroscopy experiments the detection of high-energy gamma rays in the range up to 10-20 MeV is of primary importance. New generation high-resolution gamma-ray spectrometers like AGATA and GRETA are composed of position sensitive segmented HPGe detectors and provide a reconstruction of the gamma-ray tracks. The performance of AGATA detectors in this energy range has, however, never been studied in detail. A measurement of the response to 15.1 MeV gamma rays has been performed using two HPGe triple clusters of the AGATA Demonstrator array, operating at LNL-INFN. This manuscript presents the preliminary results of a test of the AGATA detectors for the measurement of high-energy gamma rays, in terms of detection efficiency, energy resolution, tracking and performance of the pulse shape analysis (PSA) algorithms
