    特別支援学校(肢体不自由部門)教諭の児童生徒の健康状態に関する認識状況

    本研究は、特別支援学校教諭(肢体不自由部門)の児童生徒の健康状態に関する認識状況を明らかにするために、特別支援学校教諭に質問紙調査を行った。その結果、特別支援学校(肢体不自由部門)の教諭は、受け持ちの児童生徒の健康状態を認識するためにバイタルサイン、一般状態、随伴症状、発育経過といった健康観察指標を把握している者が多かった。しかし、肢体不自由児の健康異常を見極めることは難しく、看護師等専門職のサポートが必要と考えられた。また、肢体不自由児の教職経験による分析からは、教職経験が長くなるに従って、健康観察指標を把握している教諭が多くなり、重視する項目も画一的な項目から受け持ち児の障害の実態に応じた項目へと発展させ、健康異常を見極めることの困難度も低下していくことが明らかになった。また、バイタルサインの把握状況、随伴症状の把握状況、健康異常の見極め困難度については、教職経験「1年未満」「1~3年」と「4~9年」「10年以上」に差が見られ、特に教職経験3年までの教諭に対するサポートの必要性が示唆された

    Importance: Although research has shown that centenarians tend to experience shorter periods of serious illness compared with other age groups, few studies have focused on the medical expenditures of centenarians as a potential indicator of the scale of medical resources used in their last year of life. Objective: To compare Japanese centenarians' and noncentenarians' monthly medical expenditures during the year before death according to age and sex. Design, setting, and participants: This retrospective cohort study used linked national health and long-term care insurance data collected from April 2013 to March 2018 in Nara Prefecture, Japan, for residents aged 75 years or older who were insured under the Medical Care System for older adults and died between April 2014 and March 2018. Data were analyzed from April 2013 to March 2018. Exposures: Age of 100 years or older (centenarians) vs 75 to 99 years (noncentenarians). Main outcomes and measures: The numbers of unique inpatients and outpatients and medical expenditures related to decedents' hospitalization and outpatient care were extracted and analyzed based on sex and age group. The Jonckheere-Terpstra test was used to identify trends in unadjusted medical expenditures by age group, and generalized estimating equations were used to estimate monthly median expenditures by age group with adjustment for comorbidity burden and functional status. Results: Of 34 317 patients aged 75 to 109 years (16 202 men [47.2%] and 18 115 women [52.8%]) who died between April 2014 and March 2018, 872 (2.5%) were aged 100 to 104 years (131 men [15.0%] and 741 women [85.0%]) and 78 (0.2%) were aged 105 to 109 years (fewer than 10 were men). The analysis of unadjusted medical expenditures in the last year of life showed a significant trend of lower expenditures for the older age groups; the median adjusted total expenditures during the 30 days before death by age group were 6784(IQR,6784 (IQR, 4884-9703)forages75to79years,9703) for ages 75 to 79 years, 5894 (IQR, 42924292-8536) for 80 to 84 years, 5069(IQR,5069 (IQR, 3676-7150)for85to89years,7150) for 85 to 89 years, 4205 (IQR, 30853085-5914) for 90 to 94 years, 3522(IQR,3522 (IQR, 2626-4861)for95to99years,4861) for 95 to 99 years, 2898 (IQR, 22412241-3835) for 100 to 104 years, and 2626(IQR,2626 (IQR, 1938-$3527) for 105 to 109 years. The proportion of inpatients among all patients in the year before death also decreased with increasing age: 4311 of all 4551 patients aged 75 to 79 years (94.7%); 43 of all 78 patients aged 105 to 109 years (55.1%); 2831 of 2956 men aged 75 to 79 years (95.8%); 50.0% of men aged 105 to 109 years (the number is not reported owing to the small sample size); 1480 of 1595 women aged 75 to 79 years (92.8%); and 55.7% of women aged 105 to 109 years (the number of women is not reported to prevent back-calculation of the number of men). Specifically, 274 of 872 patients aged 100 to 104 years (31.4%) and 35 of 78 patients aged 105 to 109 years (44.9%) had not been admitted to a hospital in the year before death. Conclusions and relevance: This cohort study found that medical expenditures in the last year of life tended to be lower for centenarians than for noncentenarians aged 75 years or older in Japan. The proportion of inpatients also decreased with increasing age.     Up-regulation of ras-GAP genes is reversed by a MEK inhibitor and doxorubicin in v-Ki-ras-transformed NIH/3T3 fibroblasts

    金沢大学大学院医学系研究科脳細胞分子学金沢大学薬学部Ras-GTPase-activating proteins (Ras-GAPs) have been implicated both as suppressors of Ras and as effectors in regulating cellular activities. To study whether Ras-GAPs have roles in tumor cell survival or not, mRNA levels of ras-related genes were measured in v-Ki-ras-transformed (DT) and the parental NIH/3T3 cells, using real-time PCR. mRNA levels of p120-Gap, Gap1m, and PIK3CA were increased in DT cells compared with NIH/3T3 cells. p120-Gap and PIK3CA genes were induced by addition of serum or epidermal growth factor to serum-starved DT cells. Three anti-cancer drugs, an ERK kinase (MEK) inhibitor PD98059, a topoisomerase II poison doxorubicin (adriamycin), and a histone deacetylase inhibitor trichostatin A, selectively blocked the overexpression of p120-Gap and Gap1m genes in DT cells. These drugs also caused reversion of DT cells to the adherent shape associated with growth arrest. Our results suggest that p120-Gap and Gap1m genes provide important biomarkers for cancer therapies. © 2007 Elsevier Inc. All rights reserved

    教員養成大学において、教員に求められる資質能力である実践的指導力や協働性の向上に向けて、PBLの取組が取り入れられつつある。特別支援学校教員養成においては、PBLの観点での授業の取組の有効性について、必ずしも十分に検討されていない。筆者らは、地域の知的障害特別支援学校と連携を図り、特別支援教育専門科目である「知的障害教育論Ⅱ」の科目において、PBLの枠組みを用いた実践を試みた。支援学校生徒と大学生の交流活動を学生らが計画した。履修者を対象とし、教員の資質能力に関する学びと課題について事後アンケートを実施した。その結果、交流活動に参加した学生と参加しなかった学生のいずれにおいても、実践的指導力および協働性に関して肯定的な結果が得られた。特別支援学校の教員を対象とした事後アンケートからは、参加生徒の学習意欲の向上や教員による生徒の実態把握の再確認の機会となるといった有効性が示された

    The Quiescent Intracluster Medium in the Core of the Perseus Cluster

    Clusters of galaxies are the most massive gravitationally-bound objects in the Universe and are still forming. They are thus important probes of cosmological parameters and a host of astrophysical processes. Knowledge of the dynamics of the pervasive hot gas, which dominates in mass over stars in a cluster, is a crucial missing ingredient. It can enable new insights into mechanical energy injection by the central supermassive black hole and the use of hydrostatic equilibrium for the determination of cluster masses. X-rays from the core of the Perseus cluster are emitted by the 50 million K diffuse hot plasma filling its gravitational potential well. The Active Galactic Nucleus of the central galaxy NGC1275 is pumping jetted energy into the surrounding intracluster medium, creating buoyant bubbles filled with relativistic plasma. These likely induce motions in the intracluster medium and heat the inner gas preventing runaway radiative cooling; a process known as Active Galactic Nucleus Feedback. Here we report on Hitomi X-ray observations of the Perseus cluster core, which reveal a remarkably quiescent atmosphere where the gas has a line-of-sight velocity dispersion of 164+/-10 km/s in a region 30-60 kpc from the central nucleus. A gradient in the line-of-sight velocity of 150+/-70 km/s is found across the 60 kpc image of the cluster core. Turbulent pressure support in the gas is 4% or less of the thermodynamic pressure, with large scale shear at most doubling that estimate. We infer that total cluster masses determined from hydrostatic equilibrium in the central regions need little correction for turbulent pressure.Comment: 31 pages, 11 Figs, published in Nature July

    Genetic analysis of typical wet-type age-related macular degeneration and polypoidal choroidal vasculopathy in Japanese population

    Age-related macular degeneration (AMD) is a common cause of blindness in the elderly. Caucasian patients are predominantly affected by the dry form of AMD, whereas Japanese patients have predominantly the wet form of AMD and/or polypoidal choroidal vasculopathy (PCV). Although genetic association in the 10q26 (ARMS2/HTRA1) region has been established in many ethnic groups for dry-type AMD, typical wet-type AMD, and PCV, the contribution of the 1q32 (CFH) region seem to differ among these groups.     Recent Results from LHD Experiment with Emphasis on Relation to Theory from Experimentalist’s View

    he Large Helical Device (LHD) has been extending an operational regime of net-current free plasmas towardsthe fusion relevant condition with taking advantage of a net current-free heliotron concept and employing a superconducting coil system. Heating capability has exceeded 10 MW and the central ion and electron temperatureshave reached 7 and 10 keV, respectively. The maximum value of β and pulse length have been extended to 3.2% and 150 s, respectively. Many encouraging physical findings have been obtained. Topics from recent experiments, which should be emphasized from the aspect of theoretical approaches, are reviewed. Those are (1) Prominent features in the inward shifted configuration, i.e., mitigation of an ideal interchange mode in the configuration with magnetic hill, and confinement improvement due to suppression of both anomalous and neoclassical transport, (2) Demonstration ofbifurcation of radial electric field and associated formation of an internal transport barrier, and (3) Dynamics of magnetic islands and clarification of the role of separatrix
