    Lenalidomide belongs to a novel class of drugs called Immunomodulators which are now being used for the treatment of plasma cell dyscrasias with variable degrees of efficacy and toxicity. Though Second Primary Malignancies (SPM) have been a concern with its use, the benefits of the treatment outweigh the risks. The leukemogenic risk seems to be potentiated especially when combined with alkylating agents and the SPMs documented are predominantly myeloblastic. To date there are no reported cases of new lymphocytic leukemias in AL amyloidosis, regardless of whether undergone treatment or not. We present a case of AL amylodosis who was treated with lenalidomide and subsequently developed acute lymphoblastic leukemia

    Interpretation of 2D resistivity with engineering characterization of subsurface exploration in Nusajaya Johor, Malaysia

    2-D resistivity technique and pole-dipole array with spacing of 2 m electrode and total spacing of 80 m were adopted to map and characterize the shallow subsurface in a sedimentary area at Nusajaya, Johor. Geological field mapping and laboratory testing were conducted to determine weathering grades.Res2Dinv software was used to generate the inversion model resistivity. The result shows sandstone contains iron mineral (30-1000ohm-m) and weathered sandstone (500-1000 ohm-m). The lowest layer represents sandstone and siltstone with the highest range from 1500 through 5000 ohm-m. The weathering grade IV and V of sandstone in the actual profile indicates the range from 30 to 1000 ohm-m, whereas grade II and III in ground mass matched the higest range. Overall, the increase of weathering grade influenced both the physical properties and strength of rocks

    Correlation of resistivity value with geotechnical N-Value of sedimentary area in Nusajaya, Johor, Malaysia

    Electrical resistivity survey and the geotechnical SPT blow counts (N-value) were carried out simultaneously on the tropically weathered sedimentary rock mass for an excavation project at Nusajaya, Johor, Malaysia. This study aims to determine subsurface profile by using 2D-resistivity methods and correlate with N-value derived from boring works. Four boreholes were investigated in five survey lines that revealed the site is underlain by moderately to completely weathered sandstone, clay, silt and shale. Data analysis from 2Dresistivity image shows that zones with high resistivity value generally have high N-value, and vice versa. Five zones have inversed the proportional relation between N-value and resistivity Ωm value due to different types of soil lithology. It indicates that 2D-resistivity is significance to detect bodies of anomalous materials or estimating the depth of bedrock. As a conclusion, the integration of geophysical and geotechnical analysis provides a promise approach to understand some relationship concerning the subsurface subsurface ground through the combination of 2D-resistivity and N-value

    Rapidly Decaying Supernova 2010X: A Candidate ".Ia" Explosion

    We present the discovery, photometric and spectroscopic follow-up observations of SN 2010X (PTF 10bhp). This supernova decays exponentially with tau_d=5 days, and rivals the current recordholder in speed, SN 2002bj. SN 2010X peaks at M_r=-17mag and has mean velocities of 10,000 km/s. Our light curve modeling suggests a radioactivity powered event and an ejecta mass of 0.16 Msun. If powered by Nickel, we show that the Nickel mass must be very small (0.02 Msun) and that the supernova quickly becomes optically thin to gamma-rays. Our spectral modeling suggests that SN 2010X and SN 2002bj have similar chemical compositions and that one of Aluminum or Helium is present. If Aluminum is present, we speculate that this may be an accretion induced collapse of an O-Ne-Mg white dwarf. If Helium is present, all observables of SN 2010X are consistent with being a thermonuclear Helium shell detonation on a white dwarf, a ".Ia" explosion.     Physico-mechanical characteristics of tropical granite boulders in weathered heterogeneous zones for geotechnical design purposes

    The presence of isolated or clustered granite boulders in tropical weathered masses commonly formed a very thick heterogeneous zone in weathering Zone 3 to 5. The boulders with various characteristics are always misinterpreted during the geotechnical design process due to being poorly understood and studied. This study aims to determine the physico-mechanical characteristics of boulders in different weathering zones for design purposes. The parameters studied include boulder types, porosity, density, durability, point load and uniaxial compressive strength. Results revealed boulders in weathering Zone 3, 4 and 5 can be classified into Type A, B and C, respectively. These boulders have significant differences in physical and mechanical properties. The boulders in Zone 4 and 5 consists of corestone, ringlets and saprolites and the boulders in Zone 3 have no ringlets. Corestones have the highest durability, point load and compressibility strength with a median of 94.5%, 7.80 MPa and 187.07 MPa, respectively. The ringlets possess the highest porosity range of 23.3%–31.3% compared with saprolite. Saprolite in Zone 5 has the lowest durability, point load and compressive strength with respectively less than 7%, 0.22 MPa and 1 MPa. The various characteristics of the boulders in different weathering zones could influence the geotechnical design model

    Mapping on slope seepage problem using Electrical Resistivity Imaging (ERI)

    The stability of slope may influenced by several factors such as its geomaterial properties, geometry and environmental factors. Problematic slope due to seepage phenomenon will influenced the slope strength thus promoting to its failure. In the past, slope seepage mapping suffer from several limitation due to cost, time and data coverage. Conventional engineering tools to detect or mapped the seepage on slope experienced those problems involving large and high elevation of slope design. As a result, this study introduced geophysical tools for slope seepage mapping based on electrical resistivity method. Two spread lines of electrical resistivity imaging were performed on the slope crest using ABEM SAS 4000 equipment. Data acquisition configuration was based on long and short arrangement, schlumberger array and 2.5 m of equal electrode spacing interval. Raw data obtained from data acquisition was analyzed using RES2DINV software. Both of the resistivity results show that the slope studied consists of three different anomalies representing top soil (200 – 1000 Ωm), perched water (10 – 100 Ωm) and hard/dry layer (> 200 Ωm). It was found that seepage problem on slope studied was derived from perched water zones with electrical resistivity value of 10 – 100 Ωm. Perched water zone has been detected at 6 m depth from the ground level with varying thickness at 5 m and over. Resistivity results have shown some good similarity output with reference to borehole data, geological map and site observation thus verified the resistivity results interpretation. Hence, this study has shown that the electrical resistivity imaging was applicable in slope seepage mapping which consider efficient in term of cost, time, data coverage and sustainability

    Intraoperative monitoring study of ipsilateral motor evoked potentials in scoliosis surgery

    Ipsilateral motor evoked potentials (MEPs) in spinal cord surgery intraoperative monitoring is not well studied. We show that ipsilateral MEPs have significantly larger amplitudes and were elicited with lower stimulation intensities than contralateral MEPs. The possible underlying mechanisms are discussed based on current knowledge of corticospinal pathways. Ipsilateral MEPs may provide additional information on the integrity of descending motor tracts during spinal surgery monitoring

    Monoclonal antibody ONS-M21 recognizes integrin α3 in gliomas and medulloblastomas

    The monoclonal antibody ONS-M21 recognizes an antigen found on the surface of glioma and medulloblastoma cells but does not react with the antigens of normal brain tissue. We purified and identified the 140-kDa protein by means of an antibody-binding affinity column. This 140-kDa antigen has sequences homologous to the amino-terminal region and five parts of the internal domain of integrin α3. When the integrin α3-related sequences was amplified and used to analyse the mRNA of glioma and medulloblastoma surgical specimens, the transcription level of integrin α3 mRNA appeared to be quantitatively correlated with the grade of malignancy. These findings suggest that the ONS-M21 antibody, which reacts with integrin α3, might be useful in the diagnosis of gliomas and medulloblastomas. © 1999 Cancer Research Campaig

    Helping someone with problem drinking: Mental health first aid guidelines - a Delphi expert consensus study

    Background Alcohol is a leading risk factor for avoidable disease burden. Research suggests that a drinker's social network can play an integral role in addressing hazardous (i.e., high-risk) or problem drinking. Often however, social networks do not have adequate mental health literacy (i.e., knowledge about mental health problems, like problem drinking, or how to treat them). This is a concern as the response that a drinker receives from their social network can have a substantial impact on their willingness to seek help. This paper describes the development of mental health first aid guidelines that inform community members on how to help someone who may have, or may be developing, a drinking problem (i.e., alcohol abuse or dependence). Methods A systematic review of the research and lay literature was conducted to develop a 285-item survey containing strategies on how to help someone who may have, or may be developing, a drinking problem. Two panels of experts (consumers/carers and clinicians) individually rated survey items, using a Delphi process. Surveys were completed online or via postal mail. Participants were 99 consumers, carers and clinicians with experience or expertise in problem drinking from Australia, Canada, Ireland, New Zealand, the United Kingdom, and the United States. Items that reached consensus on importance were retained and written into guidelines. Results The overall response rate across all three rounds was 68.7% (67.6% consumers/carers, 69.2% clinicians), with 184 first aid strategies rated as essential or important by ≥80% of panel members. The endorsed guidelines provide guidance on how to: recognize problem drinking; approach someone if there is concern about their drinking; support the person to change their drinking; respond if they are unwilling to change their drinking; facilitate professional help seeking and respond if professional help is refused; and manage an alcohol-related medical emergency. Conclusion The guidelines provide a consensus-based resource for community members seeking to help someone with a drinking problem. Improving community awareness and understanding of how to identify and support someone with a drinking problem may lead to earlier recognition of problem drinking and greater facilitation of professional help seeking
