Disfuncionalidade da lipoproteína de alta densidade e risco cardiometabólico : relação com outros biomarcadores
Tese de doutoramento em Ciências da Saúde, apresentada à Faculdade de Medicina da Universidade de CoimbraA dislipidemia é um dos factores de risco major para a DCV, que está entre as
principais causas de morbilidade e mortalidade em muitos países do Mundo, incluindo na
Europa. O arsenal terapêutico antidislipidémico actualmente disponível e em utilização
clínica, nomeadamente as estatinas, tem permitido grandes avanços no controlo dos
valores de c-Total e sobretudo de c-LDL, o que se manifestou numa redução significativa
do risco e da mortalidade por DCV. Contudo, essas terapêuticas têm um impacto muito
reduzido sobre o c-HDL, existindo um risco CV residual (mas ainda assim muito
preocupante e não negligenciável) por controlar e que deve merecer mais atenção e
investimento. Dados epidemiológicos suportam a ideia de que a redução do c-HDL é um
preditor independente de desenvolvimento de DCV, mas os ensaios clínicos recentes
envolvendo novos compostos foram incapazes de demonstrar o impacto esperado a nível
da redução da mortalidade CV, não obstante o aumento significativo dos teores de c-HDL.
Neste contexto, o conhecimento actual aponta para a necessidade de conhecer melhor as
HDL e de encontrar formas de não apenas aumentar os seus conteúdos séricos totais
mas, sobretudo, entender o conceito de HDL disfuncional, saber se tem um
relacionamento com a DCV e com outros mediadores da aterogénese em populações de
risco para que, posteriormente, seja possível modular a sua funcionalidade e assim
reduzir de forma mais abrangente a morbilidade e mortalidade por DCV.
Este trabalho pretendeu dar um contributo para a melhoria do conhecimento da
relevância das HDL e da sua funcionalidade a nível da DCV aterogénica, tendo como
principal objectivo avaliar as implicações relativas das subpopulações de HDL na
determinação do perfil cardiometabólico e a sua correlação com outros mediadores
(marcadores) do fenómeno aterogénico em populações com factores de risco para DCV.
O trabalho envolveu 4 estudos separados, em populações distintas: 1 – grupo de
voluntários controlo (sem factores de risco ou DCV diagnosticada) para avaliar o efeito do
género e menopausa; 2 – grupo de doentes diabéticos do tipo 2; 3 - grupo de doentes
jovens adultos com esclerose múltipla; 4 – grupo de doentes dislipidémicos com
concentrações séricas reduzidas de c-HDL e/ou elevadas de TGs, mas valores
normalizados de c-LDL. Os grupos de doentes foram comparados com indivíduos controlo
de idade e género comparáveis o mais possível. Foram analisados dados antropométricos
(idade, IMC e perímetro abdominal), pressão arterial, perfil glicídico e lipídico,
marcadores “não-tradicionais” de perfil lipídico (incluindo subpopulações de HDL, LDL
oxidadas e actividade paraoxonase 1), bem como outros marcadores séricos de inflamação, angionénese, oxidação e disfunção endotelial (PCRhs, TNF-α, adiponectina,
VEGF, ácido úrico e ICAM-1).
Os principais resultados obtidos são: 1) Numa população sem diagnóstico prévio
de DCV, os homens e as mulheres em pós-menopausa apresentam um perfil lipídico
indicativo de um maior risco cardiometabólico; estes resultados ficam mais patentes
quando se analisam marcadores de risco “não-tradicionais”, incluindo, entre outros, as
subpopulações de HDL (maior percentagem de pequenas e menor de grandes) e as LDLox;
2) a diabetes parece anular o efeito CV protector conferido pelo género feminino,
traduzindo-se numa degradação da qualidade das HDL, um aumento de TNF-α e de VEGF;
para além disso, as mulheres diabéticas quando entram na fase de menopausa
apresentam um perfil cardiometabólico mais nefasto, como sugerem a pior qualidade das
HDL, o agravamento da obesidade, a hipertrigliceridemia e o aumento do TNF-α; 3) numa
população jovem adulta com esclerose múltipla, existem indicações de um risco
cardiometabólico precoce, que fica realçado pelo desequilíbrio entre as subpopulações
grandes e pequenas de HDL, pelo aumento de VEGF e principalmente de LDL-ox
(relacionado de forma significativa e directa com o estadio da doença); 4) em populações
consideradas dislipidémicas, com concentrações séricas reduzidas de c-HDL e/ou elevadas
de TGs, ainda que sob medicação apropriada e com os níveis de c-LDL normalizados, o
risco CV residual parece ser melhor caracterizado com a ajuda de marcadores “nãotradicionais”,
incluindo uma vez mais as subpopulações de HDL e as LDL-ox, mas também
a adiponectina e o VEGF.
Como conclusões finais, o trabalho sugere que a medição das subpopulações das
HDL é uma informação mais relevante a nível da caracterização do perfil
cardiometabólico do que o “tradicional” doseamento do conteúdo sérico total em c-HDL.
Mais ainda, as HDL grandes relacionam-se com uma maior protecção e funcionalidade, ao
contrário das HDL pequenas que se podem considerar disfuncionais em termos de efeitos
ateroprotectores, existindo diversas correlações com outros marcadores clássicos e não
tradicionais. O trabalho realça ainda a importância de dar maior atenção a outros
biomarcadores cardiometabólicos “não-tradicionais”; no entanto, contrariamente às
subpopulações de HDL em que os resultados foram transversais a todas as populações em
estudo, estes parecem ser muito mais específicos e relacionadas com cada condição
fisiopatológica particular. A confirmação dos resultados deste trabalho poderá contribuir
no futuro para melhorar as estratégias de diagnóstico e as medidas terapêuticas de
doentes com risco cardiovascular aumentado de forma a ter implicações positivas e
significativas na morbilidade e mortalidade por DCV.Dyslipidemia is one of the major risk factors for CVD, which is among the leading
causes of morbidity and mortality in many countries in the World, including in Europe.
The antidyslipidemic therapeutic arsenal currently available and in clinical use,
particularly statins, have allowed major advances in the control of Total-c values and
above in LDL-c, which was manifested in a significant reduction in the risk and of CV
mortality. However, these therapies have little impact on HDL-c, and there is a residual CV
risk (but still very disturbing and not negligible) for controlling and that deserves more
attention and investment. Epidemiological data support the idea that the reduction of
HDL-c is an independent predictor of development of CVD, but recent clinical trials
involving new compounds were unable to demonstrate the expected impact in the
reduction of CV mortality, despite the significant increase the levels of HDL-c. In this
context, the present knowledge points the necessity to better understand the HDL and
find ways to not only increase their total serum content but, more importantly,
understand the concept of dysfunctional HDL, know if it have a relationship with CVD and
other mediators of atherogenesis in populations at risk, for, subsequently be possible to
modulate its functionality and thus reduce more comprehensively CV morbidity and
mortality.
This work aims to contribute to improving the knowledge of the relevance of HDL
and its functionality for atherogenic CVD, with the primary aim of assessing the
implications for HDL subpopulations in determining cardiometabolic profile and its
correlation with other mediators (markers) of atherogenic phenomenon in populations
with CV risk factors.
The work involved four separate studies in distinct populations: 1 - control group
of volunteers (no risk factors or diagnosed CVD) to evaluate the effect of gender and
menopause; 2 - group of type 2 diabetic patients; 3 - group of young adults patients with
multiple sclerosis, 4 - group of dyslipidemic patients with low serum HDL-c and/or high
TGs but normalized values of LDL-c. Patient groups were compared with control subjects
matched for age and gender as much as possible. Anthropometric data (age, BMI and
waist circumference), blood pressure, glucose and lipid profile, "non-traditional" markers
of lipid profile (including subpopulations of HDL, oxidized LDL and paraoxonase 1 activity)
were analyzed, as well as other serum markers of inflammation, angionenesis, oxidation
and endothelial dysfunction (hsCRP, TNF-α, adiponectin, VEGF, uric acid and ICAM-1).
The main results are: 1) a population without a previous diagnosis of CVD, men
and postmenopausal women have a lipid profile indicative of a greater cardiometabolic risk; these results are more apparent when analyzing "non-traditional" risk markers
including, among others, subpopulations of HDL (higher percentage of small and lower of
large) and ox-LDL; 2) diabetes seems to abrogate the CV protective effect conferred by
female gender, traduced into a degradation of HDL quality, increase of TNF-α and VEGF,
and moreover, diabetic women when they enter in menopause phase have a more
ominously cardiometabolic profile, as suggested by the poorer HDL quality, the worsening
of obesity, hypertriglyceridemia and increased TNF-α; 3) in a young adult population with
multiple sclerosis, there are indications of early cardiometabolic risk, which is highlighted
by the imbalance between large and small subpopulations of HDL, increased VEGF and
especially ox-LDL (significantly and directly related with the disease stage); 4) dyslipidemic
populations with reduced HDL-c and/or high TGs serum concentrations, although under
appropriate medications and LDL-c standard levels, the CV residual risk seems to be
better characterized with the aid of "non-traditional" markers, including again
subpopulations of HDL and oxidized LDL, but also adiponectin and VEGF.
As final conclusions, the study suggests that measurement of HDL subpopulations
is more relevant information of cardiometabolic profile characterization than
determination of the "traditional" total HDL-c serum level. Furthermore, large HDL are
related to better protection and functionality, as opposed to the small HDL that may be
considered dysfunctional in terms of atheroprotective effects, both correlated with other
classical and non-traditional markers. The work also highlights the importance of giving
more attention to other "non-traditional" cardiometabolic biomarkers; however, contrary
to HDL subpopulations in which the results were present in all study populations, these
biomarkers seem to be much more specific and related for each particular
pathophysiological condition. The confirmation of the results of this work can contribute
in future to improve diagnostic strategies and therapeutic measures for patients at
increased cardiovascular risk in order to have significant positive implications on CV
morbidity and mortality
Aconselhamento farmacêutico em homeopatia
Ao longo do tempo, tem-se verificado um aumento do interesse pelas Medicinas Complementares e Alternativas (MCA), sendo a homeopatia uma das mais utilizadas. Surgiu no século XVIII através do médico alemão, Samuel Hahnemann, e até hoje diferencia-se pelos princípios em que se baseia – similitude, infinitesimalidade e globalidade. Os medicamentos homeopáticos foram demonstrando vantagens em relação aos medicamentos convencionais no que diz respeito à toxicidade, aos efeitos adversos e às interações medicamentosas. Ganhando cada vez mais utilizadores em toda a Europa, foi necessária a implementação de regulamentação específica para a introdução no mercado dos medicamentos homeopáticos em qualquer Estado-Membro, de forma a promover-se a circulação segura dos mesmos. No entanto, grande parte da comunidade científica aponta falhas na demonstração de plausabilidade e de eficácia da homeopatia.Apesar da controvérsia gerada, que leva a uma divisão de opiniões bem vincada, cabe aos farmacêuticos garantir que o doente é informado de forma correta e completa sobre qualquer terapêutica ou medicamento, contribuindo para a sua saúde e bem-estar. Relativamente à homeopatia, o papel ético dos farmacêuticos passa pela precisão e imparcialidade dos aconselhamentos, que devem ser sempre acompanhados de dados atualizados correspondentes aos estudos que têm sido feitos em torno dos medicamentos homeopáticos.Para que se forneçam dados verdadeiros, atualizados e confiáveis sobre os medicamentos homeopáticos é fundamental que a homeopatia seja encarada como uma área de interesse para os farmacêuticos. Incluí-la no plano de estudos de todas as faculdades de farmácia promoveria uma atualização constante de jovens farmacêuticos, desenvolvendo os seus conhecimentos e competências, capacitando-os assim para um melhor auxílio ao paciente na decisão informada sobre a utilização de medicamentos
Pharmacist Intervention in Portuguese Older Adult Care
Healthy ageing has become one of the most significant challenges in a society with an increasing life expectancy. Older adults have a greater prevalence of chronic disease, with the need for multiple medications to appropriately control these issues. In addition to their health concerns, ageing individuals are prone to loneliness, dependence, and economic issues, which may affect their quality of life. Governments and health professionals worldwide have developed various strategies to promote active and healthy ageing to improve the quality of life of older adults. Pharmacists are highly qualified health professionals, easily accessible to the population, thus playing a pivotal role in medication management. Their proximity to the patient puts them in a unique position to provide education and training to improve therapeutic adherence and identify medication-related problems. This paper aims to address the importance of Portuguese community pharmacists in the medication management of older adults, emphasising their intervention in health promotion, patient education, medication-related problems, deprescription, dose administration aids, and medication review and reconciliation. Diabetic Nephropathy Amelioration by a Low-Dose Sitagliptin in an Animal Model of Type 2 Diabetes (Zucker Diabetic Fatty Rat)
This study was performed to assess the effect of chronic low-dose sitagliptin, a dipeptidyl peptidase 4 inhibitor, on metabolic profile and on renal lesions aggravation in a rat model of type-2 diabetic nephropathy, the Zucker diabetic fatty (ZDF) rat. Diabetic and obese ZDF (fa/fa) rats and their controls ZDF (+/+) were treated for 6 weeks with vehicle (control) or sitagliptin (10 mg/kg/bw). Blood/serum glucose, HbA1c, insulin, Total-c, TGs, urea, and creatinine were assessed, as well as kidney glomerular and tubulointerstitial lesions (interstitial fibrosis/tubular atrophy), using a semiquantitative rating from 0 (absent/normal) to 3 (severe and extensive damage). Vascular lesions were scored from 0–2. Sitagliptin in the diabetic rats promoted an amelioration of glycemia, HbA1c, Total-c, and TGs, accompanied by a partial prevention of insulinopenia. Furthermore, together with urea increment prevention, renal lesions were ameliorated in the diabetic rats, including glomerular, tubulointerstitial, and vascular lesions, accompanied by reduced lipid peroxidation. In conclusion, chronic low-dose sitagliptin treatment was able to ameliorate diabetic nephropathy, which might represent a key step forward in the management of T2DM and this serious complication
Differential Effects of Acute (Extenuating) and Chronic (Training) Exercise on Inflammation and Oxidative Stress Status in an Animal Model of Type 2 Diabetes Mellitus
This study compares the effects of a single bout of exercise (acute extenuating) with those promoted by an exercise training program (chronic), focusing on low-grade chronic inflammation profile and on oxidative stress status, using the obese ZDF rats as a model of type 2 diabetes mellitus (T2DM). Animals were sacrificed after 12 weeks of a swimming training program and after a single bout of acute extenuating exercise. Glycaemic, insulinemic, and lipidic profile (triglycerides, total-cholesterol) were evaluated, as well as inflammatory (serum CRPhs, TNF-α, adiponectin) and oxidative (lipidic peroxidation and uric acid) status. When compared to obese diabetic sedentary rats, the animals submitted to acute exercise presented significantly lower values of glycaemia and insulinaemia, with inflammatory profile and oxidative stress significantly aggravated. The trained animals showed amelioration of glycaemic and lipidic dysmetabolism, accompanied by remarkable reduction of inflammatory and oxidative markers. In conclusion, the results presented herein suggessted that exercise pathogenesis-oriented interventions should not exacerbate underlying inflammatory stress associated with T2DM
Emergent Biomarkers of Residual Cardiovascular Risk in Patients with Low HDL-c and/or High Triglycerides and Average LDL-c Concentrations: Focus on HDL Subpopulations, Oxidized LDL, Adiponectin, and Uric Acid
This study intended to determine the impact of HDL-c and/or TGs levels on patients with average LDL-c concentration, focusing on lipidic, oxidative, inflammatory, and angiogenic profiles. Patients with cardiovascular risk factors (n = 169) were divided into 4 subgroups, combining normal and low HDL-c with normal and high TGs patients. The following data was analyzed: BP, BMI, waist circumference and serum glucose, Total-c, TGs, LDL-c, oxidized-LDL, total HDL-c and HDL subpopulations, paraoxonase-1 (PON1) activity, hsCRP, uric acid, TNF- α , adiponectin, VEGF, and iCAM1. The two populations with increased TGs levels, regardless of the normal or low HDL-c, presented obesity and higher waist circumference, Total-c, LDL-c, Ox-LDL, and uric acid. Adiponectin concentration was significantly lower and VEGF was higher in the population with cumulative low values of HDL-c and high values of TGs, while HDL quality was reduced in the populations with impaired values of HDL-c and/or TGs, viewed by reduced large and increased small HDL subfractions. In conclusion, in a population with cardiovascular risk factors, low HDL-c and/or high TGs concentrations seem to be associated with a poor cardiometabolic profile, despite average LDL-c levels. This condition, often called residual risk, is better evidenced by using both traditional and nontraditional CV biomarkers, including large and small HDL subfractions, Ox-LDL, adiponectin, VEGF, and uric acid.info:eu-repo/semantics/publishedVersio
Topical delivery of nanoemulsions for skin cancer treatment
Skin cancer chemotherapeutics often lead to the development of severe cytotoxicity, compelling the development of novel delivery systems to not only enhance therapeutic efficacy but also minimize side effects and improve patient compliance. In recent years, topical nanoemulsions have emerged as powerful tools in the field of skin cancer therapeutic management. This review delves into the potential of these innovative formulations to revolutionize the treatment of skin malignancies, due to their unique properties, having relevant advantages, such as allowing high drug strength, skin drug permeation and retention enhancement, biocompatibility, and controlled release capacity. Despite the skin's formidable permeability challenges, it remains an accessible interface for the delivery of therapeutic carriers such as nanoemulsions both locally (topical and dermal) and systemically (transdermal). Nanoemulsions, once associated primarily with cosmetic applications, are now gaining prominence as essential components of skin cancer treatment strategies. This review explores the potential of topical nanoemulsions, shedding light on their ability to efficiently deliver a wide range of molecules, overcoming lipophilic barriers inherent to skin. In this comprehensive analysis of several distinct studies investigating NEs for skin cancer treatment, a diverse array of formulations and components were explored, revealing a spectrum of characteristics. The PDI spans from a minimum of 0.105 nm to a maximum of 0.421 nm, reflecting variations in droplet size distribution. Droplet sizes exhibit considerable diversity, ranging from a small 16 nm to a larger 200 nm, signifying varied potential for skin penetration. ZP values further contribute to this diversity, ranging from highly favorable (-66.6 mV) to less advantageous or near zero values, indicative of distinct surface charge characteristics. As healthcare costs continue to escalate, this nuanced overview of nanoemulsion characteristics provides valuable insights into their potential applications in the targeted treatment of melanoma and, to a lesser extent, non-melanoma skin cancers. The value of such innovative and safer drug delivery systems becomes increasingly evident. Here, we focus exclusively on the role of topical nanoemulsions in advancing skin cancer therapy
Cyclodextrin-based dermatological formulations : dermopharmaceutical and cosmetic applications
The progress in new delivery systems for active ingredients has boosted the dermopharmaceutical and cosmetic fields by allowing formulations to display enhanced skin permeation capabilities. Cyclodextrins (CDs) are cyclic oligosaccharides able to form host-guest inclusion complexes with guest active molecules, resulting in improved physicochemical properties of such molecules. The incorporation of CDs in dermopharmaceutical and cosmetics formulations has received much attention since the late 1970 s by enhancing modulation of the passage through the skin and vectorization into the target site while simultaneously offering a biocompatible delivery system. This paper features the advantages of CDs in dermopharmaceutical and cosmetic applications, such as the improvement of the apparent solubility and the stability of the active ingredients, the possibility of masking unpleasant odors, among others that are be described, emphasizing that these versatile skin active ingredient carriers are strongly promising both in the treatment of skin diseases and in the improvement of cosmetic formulations
O processo de cessação tabágica e o contributo do farmacêutico: impacto na saúde pública
O tabagismo é considerado um fator de risco de inúmeras doenças e afeta quer a população fumadora, quer a não fumadora. Este hábito é a principal causa evitável de doenças crónicas, perda de qualidade de vida e morte no mundo. Desta forma, é considerado um problema de saúde pública que afeta toda a população. O tabaco pode ser consumido de diversas formas e é causador de dependência quer física, quer psicológica, englobando diversos fatores. Todos os produtos contidos no tabaco são nocivos para a saúde, podendo comprometer todo o organismo, e não existe um limiar seguro para a sua exposição. Em Portugal, a prevenção e o controlo do tabagismo são uma prioridade. A implementação de medidas, sejam elas legislativas, educacionais, terapêuticas, económicas ou sociais, necessitam de intervenção multidisciplinar. A cessação tabágica é importante não só para o fumador em particular, mas também para a população em geral. As vantagens desta cessação são tanto maiores quanto mais cedo ela ocorrer, havendo benefícios imediatos, logo após o consumo do último cigarro. As medidas farmacológicas, importantes e muitas vezes necessárias na cessação tabágica, contribuem para a diminuição dos sintomas de abstinência e do desejo de fumar. No entanto, as medidas não farmacológicas são fundamentais para o sucesso do tratamento, quer a médio, quer a longo prazo. A falta de literacia em saúde, os interesses económicos associados ao tabaco, as estratégicas de marketing das empresas deste tipo de produtos e a aceitação social do consumo, são fatores muito relevantes na dificuldade que existe na adoção de medidas de prevenção e controlo deste problema. O presente artigo tem como objetivo contextualizar o impacto do tabagismo no âmbito da saúde pública, destacando estratégias de prevenção, controlo e tratamento do processo de cessação tabágica, e salientando aspetos em que o farmacêutico comunitário pode intervir e contribuir para a diminuição dos efeitos nefastos associados ao consumo de tabaco
