Soft Computing methodologies represents the amalgamation of promising problem - solving technologies such as Neural Networks (NNs), Fuzzy Logic (FL), Probabilistic Reasoning (PR) and Genetic Algorithms (GAs) . We present a collection of tools and methods that can be used to perform diagnostics, estimation, and control. These tools are an enormous match for real - world applications that are characterized by imprecise, uncertain data and incomplete domain knowledge. Each of these technologies offer us with complementary reasoning and sear ching methods to solve complex problems. In this paper after a brief description of each of these technologies, we will analyze some of their most useful combinations, such as the use of FL to control GAs and NNs parameters; the application of GAs to evolv e NNs or to tune FL controllers; and the implementation of FL controllers as NNs tuned by back propagation - type algorithms . We summarize the recompense of SC techniques and in particular the synergy derived from the use of hybrid SC system

Cigarette smoke is a source of free radicals that cause health problems throughout the world. The study aimed to determine the effects of hydro- ethanolic extract of Euphorbia hirta leaves extract (EHLE) on hemato-biochemical changes in cigarette smoke exposed (CSE) rats. The study using 24 male Wistar rats consisted of Control (not exposed to cigarette smoke and given distilled water), group II Diseased group (cigarette smoke and given distilled water), group III treated group (Cigarette smoke and Euphorbia hirta leaves extract 200 mg/kg BW) and group IV plant alone (Euphorbia hirta leaves extract 200 mg/kg BW alone). Exposure to cigarette smoke and administration of extracts was carried out for 12 weeks. End of the experimental period, the rats were euthanized and dissected to collect the blood for hemato-biochemical studies. The results of the study showed that treatment with Euphorbia hirta leaves extract to CSE rats restored the hematological parameters, liver and kidney markers

    Primary intraocular lymphoma.

Primary intraocular lymphoma (PIOL) is an ocular malignancy that is a subset of primary central system lymphoma (PCNSL). Approximately one-third of PIOL patients will have concurrent PCNSL at presentation, and 42-92% will develop PCNSL within a mean of 8-29 months. Although rare, the incidence has been rising in both immunocompromised and immunocompetent populations. The majority of PIOL is diffuse large B-cell lymphoma, though rare T-cell variants are described. Recently, PIOL has been classified by main site of involvement in the eye, with vitreoretinal lymphoma as the most common type of ocular lymphoma related to PCNSL. Diagnosis remains challenging for ophthalmologists and pathologists. PIOL can masquerade as noninfectious or infectious uveitis, white dot syndromes, or occasionally as other neoplasms such as metastatic cancers. Laboratory diagnosis by cytology has been much aided by the use of immunocytochemistry, flow cytometry, biochemical finding of interleukin changes (IL10:IL6 ratio > 1), and cellular microdissection with polymerase chain reaction amplification for clonality. Use of several tests improves the diagnostic yield. Approaches to treatment have centered on systemic methotrexate-based chemotherapy, often with cytarabine (Ara-C) and radiotherapy. Use of intravitreal chemotherapy with methotrexate (0.4 mg/0.1 mL) is promising in controlling ocular disease, and intravitreal rituximab (anti-CD20 monoclonal antibody) has also been tried. Despite these advances, prognosis remains poor

Giant Cell Arteritis and COVID-19: Similarities and Discriminators. A Systematic Literature Review

    Get PDF
OBJECTIVE: To identify shared and distinct features of giant cell arteritis (GCA) and Coronavirus disease 2019 (COVID-19) to reduce diagnostic error that could cause delays in correct treatment. METHODS: Two systematic literature reviews determined the frequency of clinical features of GCA and COVID-19 in published reports. Frequencies in each disease were summarised using median and range. RESULTS: Headache was common in GCA but was also observed in COVID-19 (66% for GCA, 10% for COVID-19). Jaw claudication or visual loss (43% and 26% in GCA, respectively) were not reported in COVID-19. Both diseases featured fatigue (38% for GCA, 43% for COVID-19) and elevated inflammatory markers (CRP elevated in 100% of GCA, 66% of COVID-19), but platelet count was elevated in 47% of GCA but 4% of COVID-19. Cough and fever were commonly reported in COVID-19 and less frequently in GCA (cough, 63% for COVID-19 versus 12% for GCA; fever, 83% for COVID-19 versus 27% for GCA). Gastrointestinal upset was occasionally reported in COVID-19 (8%), rarely in GCA (4%). Lymphopenia was more common in COVID-19 than GCA (53% in COVID-19, 2% in GCA). Alteration of smell and taste been described in GCA but their frequency is unclear. CONCLUSION: Overlapping features of GCA and COVID-19 include headache, fever, elevated CRP and cough. Jaw claudication, visual loss, platelet count and lymphocyte count may be more discriminatory. Physicians should be aware of the possibility of diagnostic confusion. We have designed a simple checklist to aid evidence-based evaluation of patients with suspected GCA

The new paradigm of hepatitis C therapy: integration of oral therapies into best practices.

Emerging data indicate that all-oral antiviral treatments for chronic hepatitis C virus (HCV) will become a reality in the near future. In replacing interferon-based therapies, all-oral regimens are expected to be more tolerable, more effective, shorter in duration and simpler to administer. Coinciding with new treatment options are novel methodologies for disease screening and staging, which create the possibility of more timely care and treatment. Assessments of histologic damage typically are performed using liver biopsy, yet noninvasive assessments of histologic damage have become the norm in some European countries and are becoming more widespread in the United States. Also in place are new Centers for Disease Control and Prevention (CDC) initiatives to simplify testing, improve provider and patient awareness and expand recommendations for HCV screening beyond risk-based strategies. Issued in 2012, the CDC recommendations aim to increase HCV testing among those with the greatest HCV burden in the United States by recommending one-time testing for all persons born during 1945-1965. In 2013, the United States Preventive Services Task Force adopted similar recommendations for risk-based and birth-cohort-based testing. Taken together, the developments in screening, diagnosis and treatment will likely increase demand for therapy and stimulate a shift in delivery of care related to chronic HCV, with increased involvement of primary care and infectious disease specialists. Yet even in this new era of therapy, barriers to curing patients of HCV will exist. Overcoming such barriers will require novel, integrative strategies and investment of resources at local, regional and national levels

DISPATCH: An Optimally-Competitive Algorithm for Maximum Online Perfect Bipartite Matching with i.i.d. Arrivals

This work presents an optimally-competitive algorithm for the problem of maximum weighted online perfect bipartite matching with i.i.d. arrivals. In this problem, we are given a known set of workers, a distribution over job types, and non-negative utility weights for each pair of worker and job types. At each time step, a job is drawn i.i.d. from the distribution over job types. Upon arrival, the job must be irrevocably assigned to a worker and cannot be dropped. The goal is to maximize the expected sum of utilities after all jobs are assigned. We introduce DISPATCH, a 0.5-competitive, randomized algorithm. We also prove that 0.5-competitive is the best possible. DISPATCH first selects a "preferred worker" and assigns the job to this worker if it is available. The preferred worker is determined based on an optimal solution to a fractional transportation problem. If the preferred worker is not available, DISPATCH randomly selects a worker from the available workers. We show that DISPATCH maintains a uniform distribution over the workers even when the distribution over the job types is non-uniform
